HIV-1 testing is initially done using an enzyme-linked immunosorbent assay (ELISA) to detect antibodies to HIV-1. Specimens with a non-reactive result from the initial ELISA are considered HIV-negative unless new exposure to an infected partner or partner of unknown HIV status has occurred. Specimens with a reactive ELISA result are retested in duplicate.[105] If the result of either duplicate test is reactive, the specimen is reported as repeatedly reactive and undergoes confirmatory testing with a more specific supplemental test (e.g., a polymerase chain reaction (PCR), western blot or, less commonly, an immunofluorescence assay (IFA)). Only specimens that are repeatedly reactive by ELISA and positive by IFA or PCR or reactive by western blot are considered HIV-positive and indicative of HIV infection. Without treatment, it usually takes about 10 years for someone with HIV to develop AIDS. Treatment slows down the damage the virus causes and can help people stay healthy for several decades before developing AIDS.

Macrophage-tropic (M-tropic) strains of HIV-1, or non-syncytia-inducing strains (NSI; now called R5 viruses[41]) use the β-chemokine receptor CCR5 for entry and are, thus, able to replicate in both macrophages and CD4+ T cells.[42] This CCR5 co-receptor is used by almost all primary HIV-1 isolates regardless of viral genetic subtype. Indeed, macrophages play a key role in several critical aspects of HIV infection. They appear to be the first cells infected by HIV and perhaps the source of HIV production when CD4+ cells become depleted in the patient. Macrophages and microglial cells are the cells infected by HIV in the central nervous system. In tonsils and adenoids of HIV-infected patients, macrophages fuse into multinucleated giant cells that produce huge amounts of virus.

Risk of transmission from infected health care practitioners who take appropriate precautions is unclear but appears minimal. In the 1980s, one dentist transmitted HIV to ≥ 6 of his patients by unknown means. However, extensive investigations of patients cared for by other HIV-infected physicians, including surgeons, have uncovered few other cases.

The course of HIV infection involves three stages: primary HIV infection, the asymptomatic phase, and AIDS. The number of persons with undiagnosed HIV infection was estimated by subtracting the number of reported cumulative diagnoses from the number of estimated cumulative infections. The percentage of undiagnosed infections was determined by dividing the number of undiagnosed infections by the total HIV prevalence.

Due to their nonspecific character, these symptoms are not often recognized as signs of HIV infection. Even cases that do get seen by a family doctor or a hospital are often misdiagnosed as one of the many common infectious diseases with overlapping symptoms. Thus, it is recommended that HIV be considered in people presenting an unexplained fever who may have risk factors for the infection.[28]

Once introduced into humans, HIV was spread through sexual intercourse from person to person. As infected people moved around, the virus spread from Africa to other areas of the world. In 1981, U.S. physicians noticed that a large number of young men were dying of unusual infections and cancers. Initially, U.S. victims were predominately gay men, probably because the virus inadvertently entered this population first in this country and because the virus is transmitted easily during anal intercourse. However, it is important to note that the virus also is efficiently transmitted through heterosexual activity and contact with infected blood or secretions. In Africa, which remains the center of the AIDS pandemic, most cases are heterosexually transmitted. Twenty years ago, the news that Magic Johnson had acquired HIV heterosexually helped the country realize that the infection was not limited to men who had sex with men. Currently in the U.S., approximately 27% of new HIV infections are a result of heterosexual transmission.

Human immunodeficiency virus (HIV) is the virus that causes AIDS. When a person becomes infected with HIV, the virus attacks and weakens the immune system. As the immune system weakens, the person is at risk of getting life-threatening infections and cancers. When that happens, the illness is called AIDS. Once a person has the virus, it stays inside the body for life.

Such rampant levels of anti-LGBTQ bias is particularly worrisome when so few PLWH in the U.S. seem to have the virus under control. Of the 1.2 million people living with HIV in the U.S. in 2011, only 30% of them had consistently taken their medication and were able to lower the amount of HIV in their bodies to undetectable levels. While undetectable, a person living with HIV remains in good health, and it is virtually impossible transmit the virus to a partner. Prevention options (e.g., condoms, Pre-Exposure Prophylaxis) exist for those in relationships where one partner is not yet undetectable.

In June 1982, a group of cases among gay men in Southern California suggested that the cause of the immune deficiency was sexual and the syndrome was initially called gay-related immune deficiency (or GRID).6

Acute retroviral syndrome describes a group of symptoms that can resemble mononucleosis and that may be the first sign of HIV infection in 50-70% of all patients and 45-90% of women. Most patients are not recognized as HIV positive during this phase and may not seek medical attention. The symptoms are similar to those of many other diseases and may include fever, fatigue, muscle aches, loss of appetite, digestive disturbances, weight loss, skin rashes, headache, and chronically swollen lymph nodes (lymphadenopathy). Some patients will experience a form of meningitis during this phase in which the membranes that cover the brain and spinal cord become inflamed. Acute retroviral syndrome develops between one and six weeks after infection and lasts for two to three weeks. Blood tests during this period will indicate the presence of HIV virus and the appearance of the viral p24 antigen in the blood.

In June, the 6th International AIDS Conference in San Francisco protested against the USA’s immigration policy which stopped people with HIV from entering the country. NGOs boycotted the conference.47   

Cultural factors (e.g., stigma, fear, discrimination, and homophobia) might contribute to longer diagnosis delays in some populations (12). Asians accounted for the highest percentage of persons living with undiagnosed HIV infection compared with all other race/ethnicity groups (13). Although blacks were more likely than whites to report testing in the past 12 months across all groups at risk, the median diagnosis delay was 1 year longer for blacks (median = 3.3 years) than for whites (median = 2.2 years). The testing results might reflect national efforts to improve access to testing among blacks, and black MSM in particular, through prevention programs and media campaigns. The medical facts about HIV and AIDS are especially relevant to the law. Unless exposed in one of a few very specific ways, most people have nothing to fear. Casual contact with people who are infected is safe. Current medical knowledge is quite strong on this point: no one is known to have caught the virus by sitting next to, shaking the hand of, or breathing the same air as an infected person. For this reason, U.S. law has moved to protect the Civil Rights of HIV-positive and AIDS-symptomatic persons. Section 504 of the Rehabilitation Act of 1973, 29 U.S.C. § 794 (1994) prohibits discrimination against otherwise qualified disabled individuals, including individuals with a contagious disease or an infection such as HIV or AIDS. The AIDS quilt, on display in Washington, D.C., has become a well-known symbol of support for victims of AIDS and their families. Families and supporters of victims of AIDS create a panel to commemorate that person’s life and that panel is joined with others from around the country to create the quilt.

A fusion inhibitor blocks an early step in the viral life cycle. Enfuvirtide (Fuzeon, T-20) attaches to the envelope surrounding the virus and prevents it from entering the CD4 cells. This prevents the infection of CD4 cells by HIV. T-20 is the first approved drug in this class. It is given as a twice-daily subcutaneous injection (90 mg). It is used primarily in individuals who have developed resistance to other classes of drugs order to create a new potent combination. Like all other antivirals, it is most useful in those taking other active drugs at the same time in order to optimize the chance of getting viral loads to undetectable levels and to prevent the development of drug resistance.

Talk to your sexual partner(s). Get tested for other sexually transmitted diseases (STDs), and use protection every time you have sex. Talk to your doctor about pre-exposure prophylaxis (PrEP). When used consistently, this daily medication can lower the chances of transmission.

Dendritic cells (DCs). DCs are large cells with dendritic cytoplasmic extensions. These cells present processed antigens to T lymphocytes in lymph nodes. Epidermal DCs, expressing CD1a and Birbeck granules, are probably among the first immune cells to combat HIV at the mucosal surfaces. These cells transport HIV from the site of infection to lymphoid tissue. The follicular DCs, found in lymphoid tissue, are also key antigen-presenting cells that trap and present antigens on their cell surfaces. In the lymph node follicles, DCs provide signals for the activation of B lymphocytes.

Improving access to quality health care for populations disproportionately affected by HIV, such as people of color and gay and bisexual men, is a fundamental public health strategy for HIV prevention. People getting care for HIV can receive:

Acute retroviral syndrome usually begins within 1 to 4 wk of infection and usually lasts 3 to 14 days. Symptoms and signs are often mistaken for infectious mononucleosis or benign, nonspecific viral syndromes and may include fever, malaise, fatigue, several types of dermatitis, sore throat, arthralgias, generalized lymphadenopathy, and septic meningitis.

In people with AIDS, HIV itself may cause symptoms. Some people experience relentless fatigue and weight loss, known as “wasting syndrome.” Others may develop confusion or sleepiness due to infection of the brain with HIV, known as HIV encephalopathy. Both wasting syndrome and HIV encephalopathy are AIDS-defining illnesses.

AIDS was first clinically observed in 1981 in the United States.[120] The initial cases were a cluster of injection drug users and gay men with no known cause of impaired immunity who showed symptoms of Pneumocystis carinii pneumonia (PCP), a rare opportunistic infection that was known to occur in people with very compromised immune systems.[121] Soon thereafter, additional gay men developed a previously rare skin cancer called Kaposi’s sarcoma (KS).[122][123] Many more cases of PCP and KS emerged, alerting U.S. Centers for Disease Control and Prevention (CDC) and a CDC task force was formed to monitor the outbreak.[124] The earliest retrospectively described case of AIDS is believed to have been in Norway beginning in 1966.[125]

Acute HIV infection progresses over a few weeks to months to become an asymptomatic HIV infection (no symptoms). This stage can last 10 years or longer. During this period, the person might have no reason to suspect they have HIV, but they can spread the virus to others.

Rapid test results usually will be available during the same clinical visit that the specimen (eg, blood or oral swab sample) is collected. Obstetrician–gynecologists who use these tests must be prepared to provide counseling to women who receive positive test results the same day that the specimen is collected. Women with positive test results should be counseled regarding the meaning of these preliminarily positive test results and the need for confirmatory testing (11). Obstetrician–gynecologists should develop collaborative care plans with health care professionals who can provide these counseling services on an emergent basis or train their own staff to handle the initial encounter and, thereafter, transition infected individuals to professionals who can serve as ongoing resources to them. Women whose confirmatory testing yields positive results and, therefore, are infected with HIV should receive or be referred for appropriate clinical and supportive care.

The earliest unambiguously identified HIV-antibody positive serum stems from Kinhasa, Zaire dating back to 1959. HIV infection spread unrecognized in the 1960s and 1970s before it was finally recognized in 1981. The spread of the virus has been phenomenal thereafter, and close to 40 million people are estimated to be infected with the virus.

Few believe there is the kind of energy, leadership, money and political will in the current political climate to fix the situation in the community that has fallen through the cracks for so long. And experts in the field have grown increasingly worried about the new administration’s commitment to fighting the disease. Soon after President Trump’s inauguration, the web page of the Office of National AIDS Policy, the architect of the National H.I.V./AIDS Strategy, was disabled on the White House website. The president’s proposed budget includes a $186 million cut in the C.D.C.’s funding for H.I.V./AIDS prevention, testing and support services. The congressional fight over the repeal of the Affordable Care Act, and the president’s declarations that “Obamacare is dead,” have conjured a disastrous return to even more alarming conditions, like waiting lists for medication. As recently as 2011, the AIDS Drug Assistance Program state-by-state list of people waiting for H.I.V. medication ballooned to over 9,000 people, mostly poor black and brown men in Southern states.

Young adults tend to be at higher risk of acquiring HIV, typically through high-risk activities such as unprotected sexual intercourse or intravenous drug use. In 2009 in the US, the largest percentage (15% of all diagnoses) and the highest rate (36.9 per 100,000 population) were in persons aged 20–24 years. [72]

One way to measure the damage to your immune system is to count your CD4 cells you have. These cells, also called “T-helper” cells, are an important part of the immune system. Greg Millett, a senior scientist for the C.D.C. for 14 years and a senior policy adviser for the Obama administration’s White House Office of National AIDS Policy, put it more candidly. “During the Bush years, the administration dropped all pretense that they cared about AIDS in this country,” said Millett, who is now the vice president and director of public policy at amfAR, the Foundation for AIDS Research. “The White House said H.I.V. is only a problem in sub-Saharan Africa, and that message filtered down to the public. The classical process of infection of a cell by a virion can be called “cell-free spread” to distinguish it from a more recently-recognized process called “cell-to-cell spread”.[81] In cell-free spread (see figure), virus particles bud from an infected T cell, enter the blood or extracellular fluid and then infect another T cell following a chance encounter.[81] HIV can also disseminate by direct transmission from one cell to another by a process of cell-to-cell spread, for which two pathways have been described. Firstly, an infected T cell can transmit virus directly to a target T cell via a virological synapse.[57][82] Secondly, an antigen-presenting cell (APC), such as a macrophage or dendritic cell, can transmit HIV to T cells by a process that either involves productive infection (in the case of macrophages) or capture and transfer of virions in trans (in the case of dendritic cells).[83] Whichever pathway is used, infection by cell-to-cell transfer is reported to be much more efficient than cell-free virus spread.[84] A number of factors contribute to this increased efficiency, including polarised virus budding towards the site of cell-to-cell contact, close apposition of cells, which minimizes fluid-phase diffusion of virions, and clustering of HIV entry receptors on the target cell to the contact zone.[82] Cell-to-cell spread is thought to be particularly important in lymphoid tissues where CD4+ T cells are densely packed and likely to interact frequently.[81] Intravital imaging studies have supported the concept of the HIV virological synapse in vivo.[85] The hybrid spreading mechanisms of HIV contribute to the virus’ ongoing replication in spite of anti-retroviral therapies.[81][86]

It is now established that, given the right treatment, someone living with HIV can reduce his or her viral load to such a degree that it is no longer detectable. After assessing a number of large studies, the CDC concluded that individuals who have no detectable viral load “have effectively no risk of sexually transmitting the virus to an HIV-negative partner.”

Antenatal testing and the availability of drugs to reduce mother-to-child transmission has resulted in a mother-to-child transmission rate of just 1%. In 2011, the number of infections resulting from mother-to-child transmission was 95. Increasing numbers of HIV-positive women are becoming pregnant and choosing not to have terminations. It is thought this is due to the increasing availability of drugs to prevent mother-to-child transmission.

Before starting treatment, patients must be aware of the short- and long-term side effects of the drugs, including the fact that some long-term complications may not be known. Patients also need to realize that therapy is a long-term commitment and requires consistent adherence to the drugs. In addition, clinicians and patients should recognize that depression, feelings of isolation, substance abuse, and side effects of the antiviral drugs can all be associated with the failure to follow the treatment program.

The most important thing you can do is start antiretroviral treatment as soon as possible. And it’s important to follow up with your doctor regularly. By taking your medications exactly as prescribed, you can keep your viral count low and your immune system strong.

The largest Collaboratory, with more than twenty members, is led by David Margolis, at the University of North Carolina. Margolis, an infectious-disease expert, is targeting the reservoirs directly. The idea, which has come to be known as “shock and kill,” is to reactivate the dormant virus, unmasking the cells that carry it, so that they can be destroyed. In 2012, he published the results of a clinical trial of the drug Vorinostat, which was originally developed for blood cancers of T cells, as a shock treatment. This October, “shock and kill” was widely discussed when the Collaboratory teams convened at the N.I.H., along with hundreds of other researchers, assorted academics, and interested laypeople. Margolis and his group explored in their talk new ways to shock the virus out of dormancy.

On a late, lazy Sunday afternoon in early April, Sturdevant, in cutoff fatigues and a white tank top stained with barbecue sauce stretched over his generous belly, was flipping chicken and rib tips on his grill. He had gathered his family — nearly two dozen sons and daughters, some related by blood, most not — to his house in South Jackson for a family barbecue. His daughter Tenisha, who had moved in with her two children in November, handed off 6-month-old Kory Cedric to her father. Sturdevant nuzzled his grandson’s chubby cheek before passing him to one of his unrelated “sons,” Cord, who lifted the laughing baby high over his head.

The Ethics Committee of the American Society for Reproductive Medicine has said, “Health care workers who are willing to provide reproductive assistance to couples whose offspring are irreducibly at risk for a serious genetic disease should find it ethically acceptable to treat HIV-positive individuals or couples who are willing to take reasonable steps to minimize the risks of transmission.” (20).

It takes about 8 to 10 years from initial infection and symptom manifestation to the development of AIDS. Once AIDS has developed, untreated disease results in death in about 20 months. Treatment with HAART can prolong life and delay disease progression, and improve quality of life.

HIV infections in the United States continue to be a major public health crisis. An estimated 1.2 million Americans are living with HIV, and 1 out of 8 people with HIV do not know they have it.1 Although recent data show that annual HIV infections declined 18% in the U.S. from 2008 to 2014, HIV continues to spread.2

The human immunodeficiency virus-1 envelope protein gp120 was shown to induce apoptosis in hippocampal neurons, thus perhaps causing directly the acquired immunodeficiency syndrome dementia syndrome (for references, see Meucci et al., 1998). However, in the presence of either ABCD-1 or ABCD-3, human immunodeficiency virus-1 gp120-induced neuronal death was considerably slowed (Meucci et al., 1998).

Entry of HIV into the host cell also requires the participation of a set of cell-surface proteins that normally serve as receptors for chemokines (hormonelike mediators that attract immune system cells to particular sites in the body). Those receptors, which occur on T cells, are often described as coreceptors, since they work in tandem with CD4 to permit HIV entry into the cells. Chemokine receptors that are known to act as HIV coreceptors include CCR5 (chemokine [C-C motif] receptor 5) and CXCR4 (chemokine [C-X-C motif] receptor 4), both of which are classified as G protein-coupled receptors. The binding of gp120 to CD4 exposes a region of gp120 that interacts with the chemokine receptors. That interaction triggers a conformational change that exposes a region of the viral envelope protein gp41, which inserts itself into the membrane of the host cell so that it bridges the viral envelope and the cell membrane. An additional conformational change in gp41 pulls those two membranes together, allowing fusion to occur. After fusion the viral genetic information can enter the host cell. Both CCR5 and CXCR4 have generated significant interest as targets for drug development; agents that bind to and block those receptors could inhibit HIV entry into cells.

Some viruses have only a few genes coding for capsid proteins. Other more complex ones may have a few hundred genes. But no virus has the thousands of genes required by even the simplest cells. Although in general viruses “steal” their lipid envelope from the host cell, virtually all of them produce “envelope proteins” that penetrate the envelope and serve as receptors. Some envelope proteins facilitate viral entry into the cell, and others have directly pathogenic effects.

HIV-1 has 6 additional accessory genes: tat, rev, nef, vif, vpu, and vpr. HIV-2 does not have vpu instead has the unique gene vpx. The only other virus known to contain the vpu gene is simian immunodeficiency virus in chimpanzees (SIVcpz), which is the simian equivalent of HIV. [10] Interestingly, chimpanzees with active HIV-1 infection are resistant to disease. [20]

Abstinence-only adherents think being less frank is being more responsible. They view sexuality as a moral issue properly left for parents to discuss with their children and one that lies beyond the responsibilities of schools. The conservative columnist Cal Thomas spoke for this viewpoint when he argued that parents “have lost a significant right to rear their children according to their own moral standards.” Other objections come from religious conservatives who oppose any neutral or positive discussion of homosexuality. Koop, for example, was blasted for allegedly “sponsoring homosexually oriented curricula” and “teaching buggery in the 3rd grade.” In addition to voicing moral objections, critics say comprehensive sex ed. is generally a failure because it encourages a false sense of security among teens that leads to experimentation with sex or drugs. “We have given children more information presumably because we think it will change their behavior, and yet the behavior has gotten worse, not better,” said Gary Bauer, president of the Family Research Council.

Because death rarely occurs suddenly in people with AIDS, people usually have time to make plans for the kind of their health care they want if their condition worsens. Nonetheless, people should record such plans in a legal document early and should include clear instructions about the kind of care they want (called advance directives). Other legal documents, including powers of attorney and wills, should be prepared. These documents are particularly important for same-sex couples because they may wish to protect the assets and rights (including visitation and decision-making) of their partners.

Human immunodeficiency virus (HIV) is a blood-borne, sexually transmissible virus (see the image below.) The virus is typically transmitted via sexual intercourse, shared intravenous drug paraphernalia, and mother-to-child transmission (MTCT), which can occur during the birth process or during breastfeeding.

The Centers for Disease Control and Prevention (CDC) recommends that everyone ages 15 to 65 have a screening test for HIV. People with risky behaviors should be tested regularly. Pregnant women should also have a screening test.

AIDS is the most severe form of HIV infection. HIV infection is considered to be AIDS when at least one serious complicating illness develops or the number (count) of CD4+ lymphocytes decreases substantially.

The opportunity for repeat testing should be made available to all women even in the absence of identified risk factors. Repeat screening after age 64 years is indicated if there is ongoing risk of HIV infection, as indicated by an individualized risk assessment. Obstetrician–gynecologists also should encourage women and their prospective sex partners to be tested before initiating a new sexual relationship. The benefits of periodic retesting should be discussed with patients and provided if requested, regardless of risk factors. Patients may be concerned about their status and do not know about or want to disclose risk-taking behavior to their health care providers.

Side effects associated with EFV are mostly dizziness, confusion, fatigue, and vivid dreams. These tend to be most prominent during the first weeks of therapy and then often decrease in severity. It is generally recommended that EFV be taken at bedtime so that the patient is asleep during the time dizziness and confusion may be most severe. It is also noteworthy that there may be an increased risk of depression associated with the use of this drug, and it should be used with caution in those with poorly managed depression. Rash and liver inflammation can occur with both EFV and DLV, and these drugs may also be linked to abnormalities of lipids in the blood.

If a person gets an “AIDS-defining illness,” this is usually a sign that the person has AIDS. Healthy people do not get these illnesses, because a healthy immune system is strong enough to fight off these diseases. Because of this, getting an AIDS-defining illness is a sign that a person’s immune system is seriously damaged. The classical process of infection of a cell by a virion can be called “cell-free spread” to distinguish it from a more recently-recognized process called “cell-to-cell spread”.[81] In cell-free spread (see figure), virus particles bud from an infected T cell, enter the blood or extracellular fluid and then infect another T cell following a chance encounter.[81] HIV can also disseminate by direct transmission from one cell to another by a process of cell-to-cell spread, for which two pathways have been described. Firstly, an infected T cell can transmit virus directly to a target T cell via a virological synapse.[57][82] Secondly, an antigen-presenting cell (APC), such as a macrophage or dendritic cell, can transmit HIV to T cells by a process that either involves productive infection (in the case of macrophages) or capture and transfer of virions in trans (in the case of dendritic cells).[83] Whichever pathway is used, infection by cell-to-cell transfer is reported to be much more efficient than cell-free virus spread.[84] A number of factors contribute to this increased efficiency, including polarised virus budding towards the site of cell-to-cell contact, close apposition of cells, which minimizes fluid-phase diffusion of virions, and clustering of HIV entry receptors on the target cell to the contact zone.[82] Cell-to-cell spread is thought to be particularly important in lymphoid tissues where CD4+ T cells are densely packed and likely to interact frequently.[81] Intravital imaging studies have supported the concept of the HIV virological synapse in vivo.[85] The hybrid spreading mechanisms of HIV contribute to the virus’ ongoing replication in spite of anti-retroviral therapies.[81][86]

Other information on sexual risk reduction: The riskiest sexual behavior is unprotected receptive anal intercourse — the least risky sexual behavior is receiving oral sex. Performing oral sex on a man is associated with some risk of HIV transmission, but this is less risky than unprotected vaginal intercourse.

About 70 percent of all infections occur in people living in sub-Saharan Africa, and in some countries of the region the prevalence of HIV infection of inhabitants exceeds 10 percent of the population. Rates of infection are lower in other parts of the world, but different subtypes of the virus have spread to Europe, India, South and Southeast Asia, Latin America, and the Caribbean. Rates of infection have leveled off somewhat in the United States and Europe. In the United States more than 1.2 million people are living with HIV/AIDS, and about 44 percent of all new infections are among African Americans. In Asia sharp increases in HIV infection have occurred in China and Indonesia. Access to antiretroviral treatment for AIDS remains limited in some areas of the world, although more people are receiving treatment today than in the past.

When HIV infection is advanced, either through treatment failure or in untreated infection, and has caused immune system destruction, secondary infections (opportunistic infections) can occur. Using other antiviral drugs and antibiotics to prevent secondary infection may prevent severe illness and premature (early) death.

HIV is a virus that attacks the immune system, which is our body’s natural defence against illness. The virus destroys a type of white blood cell in the immune system called a T-helper cell, and makes copies of itself inside these cells. T-helper cells are also referred to as CD4 cells.

Treatment recommendations for children are somewhat different from those for adults. The World Health Organization recommends treating all children less than 5 years of age; children above 5 are treated like adults.[163] The United States guidelines recommend treating all children less than 12 months of age and all those with HIV RNA counts greater than 100,000 copies/mL between one year and five years of age.[164]

As a consequence of its high variability, HIV rapidly develops resistance to antiviral drugs. When antiviral drugs are administered, variants of the virus that carry mutations conferring resistance to their effects emerge and expand until former levels of plasma virus are regained. Resistance to some of the protease inhibitors appears after only a few days (Fig. 11.27). Resistance to some of the nucleoside analogues that are potent inhibitors of reverse transcriptase develops in a similarly short time. By contrast, resistance to the nucleoside zidovudine (AZT), the first drug to be widely used for treating AIDS, takes months to develop. This is not because AZT is a more powerful inhibitor, but because resistance to zidovudine requires three or four mutations in the viral reverse transcriptase, whereas a single mutation can confer resistance to the protease inhibitors and other reverse-transcriptase inhibitors. As a result of the relatively rapid appearance of resistance to all known anti-HIV drugs, successful drug treatment might depend on the development of a range of antiviral drugs that can be used in combination. It might also be important to treat early in the course of an infection, thereby reducing the chances that a variant virus has accumulated all the necessary to resist the entire cocktail. Current treatments follow this strategy and use combinations of viral protease inhibitors together with nucleoside analogues (see Fig. 11.26).

Restricting sexual activity to a single partner and practicing safer sex (i.e., always using a condom). There are complete copies of HIV genetic material among the strands of mRNAs produced by the cell. These gather together with newly made HIV proteins and enzymes to form new viral particles, which are then released from the cell. The enzyme protease plays a vitla role at this stage of HIV’s life cycle by cutting down long strands of protein into smaller pieces, which are used to construct mature viral cores.

HIV is an enveloped retrovirus whose structure is shown in Fig. 11.22. Each virus particle, or virion, contains two copies of an RNA genome, which are transcribed into DNA in the infected cell and integrated into the host cell chromosome. The RNA transcripts produced from the integrated viral DNA serve both as mRNA to direct the synthesis of the viral proteins and later as the RNA genomes of new viral particles, which escape from the cell by budding from the plasma membrane, each in a membrane envelope. HIV belongs to a group of retroviruses called the lentiviruses, from the Latin lentus, meaning slow, because of the gradual course of the diseases that they cause. These viruses persist and continue to replicate for many years before causing overt signs of disease.

When HIV infection is advanced, either through treatment failure or in untreated infection, and has caused immune system destruction, secondary infections (opportunistic infections) can occur. Using other antiviral drugs and antibiotics to prevent secondary infection may prevent severe illness and premature (early) death.

Two RNA genomes are encapsidated in each HIV-1 particle (see Structure and genome of HIV). Upon infection and replication catalyzed by reverse transcriptase, recombination between the two genomes can occur.[72][73] Recombination occurs as the single-strand (+)RNA genomes are reverse transcribed to form DNA. During reverse transcription, the nascent DNA can switch multiple times between the two copies of the viral RNA. This form of recombination is known as copy-choice. Recombination events occur throughout the genome. Anywhere from two to 20 recombination events per genome may occur at each replication cycle, and these events can rapidly shuffle the genetic information that is transmitted from parental to progeny genomes.[73]

Mutations that occur as HIV replicates can allow variants of the virus to escape recognition by antibody or cytotoxic T cells and can contribute to the failure of the immune system to contain the infection in the long term. Direct escape of virus-infected cells from killing by cytotoxic T lymphocytes has been shown by the occurrence of mutations of immunodominant viral peptides presented by MHC class I molecules. In other cases, variant peptides produced by the virus have been found to act as antagonists (see Section 6-12) for T cells responsive to the wild-type epitope, thus allowing both mutant and wild-type viruses to survive. Mutant peptides acting as antagonists have also been reported in hepatitis B virus infections, and similar mutant peptides might contribute to the persistence of some viral infections, especially when, as often happens, the immune response of an individual is dominated by T cells specific for a particular epitope.

The infection rates in many developed countries remain stable, and some developing have achieved significant gains in controlling and even reversing the effects of the HIV epidemic. However, this is partially due to deaths in HIV-infected people, together with simultaneous prevention of new infections. India, for example, has used a national prevention campaign focusing on high-risk populations that may have prevented 100,000 new HIV infections over the 5 years it has been implemented, with increasing results seen in areas with higher levels of investment. In June 1995, the FDA approved the first protease inhibitor beginning a new era of highly active antiretroviral treatment (HAART). Once incorporated into clinical practice HAART brought about an immediate decline of between 60% and 80% in rates of AIDS-related deaths and hospitalisation in those countries which could afford it.62

In Australia it is now recommended that HIV treatment starts as soon as possible after diagnosis. Whilst it is not a cure, treatment is known to slow or even halt the disease progression that would otherwise have led to AIDS.

Sexual practices such as fellatio and cunnilingus appear to be relatively low risk but not absolutely safe (see Table: HIV Transmission Risk for Several Sexual Activities). Risk does not increase significantly if semen or vaginal secretions are swallowed. However, open sores in the mouth may increase risk.

Exposure to HIV does not always lead to infection, and some people who have had repeated exposures over many years remain uninfected. Moreover, many HIV-infected people remain well for more than a decade. A very few HIV-infected, untreated people have remained well for over 20 years. Why some people become ill so much sooner than others is not fully understood, but a number of genetic factors appear to influence both susceptibility to infection and progression to AIDS after infection.

As of 2010, there are 8 known HIV-2 groups (A to H). Of these, only groups A and B are pandemic. Group A is found mainly in West Africa, but has also spread globally to Angola, Mozambique, Brazil, India, Europe, and the US. If the CD4 count drops below 200 cells per microliter of blood, the antibiotic trimethoprim-sulfamethoxazole is given to prevent Pneumocystis jirovecii pneumonia. This antibiotic also prevents toxoplasmosis, which can damage the brain.

HIV-2 has been found to be less pathogenic than HIV-1. The mechanism of HIV-2 is not clearly defined, nor the difference from HIV-1, however the transmission rate is much lower in HIV-2 than HIV-1. Both infections can lead to AIDS in affected individuals and both can mutate to develop drug resistance.[22] Disease Monitoring in patients with HIV-2 includes clinical evaluation and CD4 cell counts, while treatment includes Anti-Retroviral Therapy (ART), Nucleoside Reverse Transcriptase Inhibitors (NRTIs), Protease Inhibitors (PI), and Non-Nucleoside Reverse Transcriptase Inhibitors (NNRTIs) with the addition of CCR5 co-receptor antagonists and fusion inhibitors.[24]
