HIV has been found in saliva, tears, nervous system tissue, blood, semen (including pre-seminal fluid, or “pre-cum”), vaginal fluid, and breast milk. However, only blood, semen, vaginal secretions, and breast milk have been proven to transmit infection to others.

Currently, the risk of infection with HIV in the United States through receiving a blood transfusion or blood products is extremely low and has become progressively lower, even in geographic areas with high HIV prevalence.  

Death is rarely sudden; thus, patients usually have time to make plans. Nonetheless, patients should record their plans for health care early, with clear instructions for end-of-life care. Other legal documents, including powers of attorney and wills, should be in place. These documents are particularly important for homosexual patients because protection of assets and rights (including visitation and decision-making) for their partners may be problems.

One way to measure the damage to your immune system is to count your CD4 cells you have. These cells, also called “T-helper” cells, are an important part of the immune system. Healthy people have between 500 and 1,500 CD4 cells in a milliliter of blood. Fact Sheet 124 has has more information on CD4 cells.

If you have these symptoms, that doesn’t mean you have HIV. Each of these symptoms can be caused by other illnesses. But if you have these symptoms after a potential exposure to HIV, see a health care provider and tell them about your risk. The only way to determine whether you are infected is to be tested for HIV infection.

Ideally, prior to initiating treatment, the viral load and the CD4 cell count should be checked and the viral load test then repeated after approximately four weeks of treatment. If the patient is beginning a regimen that includes two to three drugs for which the patient’s virus does not appear to be resistant, it is expected that the amount of virus should decrease by at least a hundredfold during this interval. The ultimate goal is for the viral load to decrease to undetectable levels which should occur by approximately 12-24 weeks. There are some individuals that despite taking all of their medications correctly will suppress their viral load to less than 200 copies/mL but not consistently undetectable levels. is not completely known how to optimally manage this situation but many experts would continue to monitor on current therapy as long as viral load remains below 200 copies/mL. Those who are not having an appropriate response to therapy need to be questioned to make sure that they are taking their medications correctly, and if not, why. If the viral load is not going to undetectable levels and the patient is taking the medications correctly, then it is likely that there is a resistant virus to some of the medications. Drug-resistance testing then should be performed and the patient managed as described in the next section. Once the patient’s viral load is suppressed, they can often have viral load and CD4 cell counts performed less frequently (for example, every three to four months and in select cases every six months or possibly even less).

Human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS) has affected people on a global basis. It has been shown that dietary fats may play a role in the parthenogenesis of the infection and disease progression. By examining the effects of saturated, unsaturated, and omega-3 fatty acids on HIV infection, it was found that HIV infection could be halted with the consumption of these dietary fats. The virus can be then further immobilized with prolonged antiretroviral therapy and clinical sessions. Dietary fats have the ability to reduce problems related to body composition and health in persons with HIV.

HIV-1 originated in Central Africa during the first half of the 20th century when a closely related chimpanzee virus first infected people. The global spread of HIV-1 began in the late 1970s, and AIDS was first recognized in 1981. In 2015, about 36.7 million people were living with HIV infection worldwide, there were 1.1 million AIDS-related deaths, and 2.1 million people were newly infected.

Screening test. There are several kinds of tests. Some are blood tests, others are mouth fluid tests. They check for antibodies to the HIV virus, HIV antigen, or both. Some screening tests can give results in 30 minutes or less.

Regular blood tests are needed to make sure the virus level in the blood (viral load) is kept low, or suppressed. The goal of treatment is to lower the HIV virus in the blood to a level that is so low that the test can’t detect it. This is called an undetectable viral load.

Universal precautions: Medical and dental health care practitioners should wear gloves in situations that may involve contact with any patient’s mucous membranes or body fluids and should be taught how to avoid needlestick accidents. Home caregivers of patients with HIV infection should wear gloves if their hands may be exposed to body fluids. Surfaces or instruments contaminated by blood or other body fluids should be cleaned and disinfected. Effective disinfectants include heat, peroxide, alcohols, phenolics, and hypochlorite (bleach). Isolation of HIV-infected patients is unnecessary unless indicated by an opportunistic infection (eg, TB). Guidelines to prevent transmission from infected practitioners to patients have not been established. The typical course of an infection with HIV is illustrated in Fig. 11.21. However, it has become increasingly clear that the course of the disease can vary widely. Thus, although most people infected with HIV go on to develop AIDS and ultimately to die of opportunistic infection or cancer, this is not true of all individuals. A small percentage of people seroconvert, making antibodies against many HIV proteins, but do not seem to have progressive disease, in that their CD4 T-cell counts and other measures of immune competence are maintained. These long-term nonprogressors have unusually low levels of circulating virus and are being studied intensively to determine how they are able to control their HIV infection. A second group consists of seronegative people who have been highly exposed to HIV yet remain disease-free and virus-negative. Some of these people have specific cytotoxic lymphocytes and TH1 lymphocytes directed against infected cells, which confirms that they have been exposed to HIV or possibly noninfectious HIV antigens. It is not clear whether this immune response accounts for clearing the infection, but it is a focus of considerable interest for the development and design of vaccines, which we will discuss later. There is a small group of people who are resistant to HIV infection because they carry mutations in a cell-surface receptor that is used as a co-receptor for viral entry, as we will see below.

HIV-positive patients who are taking anti-retroviral medications are less likely to transmit the virus. For example, pregnant women who are on treatment at the time of delivery transmit HIV to the infant about 5% of the time, compared to approximately 20% if medications are not used.

Epidemiologic studies have shown that the risk of HIV transmission from patient to health care professional is exceedingly low and is related to needle stick or intraoperative injury or to potentially infectious fluid that comes in contact with a mucous membrane (17). Most contacts between health care professionals and women who are infected with HIV occur, however, during routine obstetric and gynecologic care. Siliciano told me about the first time he saw the latent virus emerge in the memory T cells of an H.I.V. patient on HAART. The patient was thought to be cured. “He had been biopsied in every imaginable place, and nobody could find any virus,” Siliciano said. Researchers took twenty tubes of the patient’s blood, isolated the T cells, and divided them into multiple wells. The specimen was then intermixed with cells from uninfected people. If the healthy T cells became infected, the virus would reproduce and be released. Detection of the virus would be signalled by a color change to blue. Siliciano remembers sitting at his desk, talking with a visitor, when a graduate student burst in: “The wells are turning blue!” He said, “It was a very strange moment, because it was a confirmation of this hypothesis—so it was exciting—but it was also a disaster. Everybody came to the same conclusion: that these cells persisted despite the antiretroviral therapy.”

For people without a history of drug resistance, there are now two effective fixed-dose combination pills that include TDF plus FTC with either EFV (Sustiva) or RPV (Complera), both as a single pill that can be taken once per day. There is also a formulation of TAF plus FTC with RPV (Odefsey). The combination with RPV (Complera) was shown to be very effective and well tolerated but not as good at suppressing the viral load as the combination with EFV (Atripla), particularly amongst those who started therapy with higher viral loads and lower CD4 cell counts (for example, >100,000 copies/mL and <200 cells/mm3, respectively). It is currently recommended only for those that have viral load levels of <100,000 copies/mL and CD4 cell counts greater than 200 cells/mm3. As he stepped into Jordon’s stuffy bedroom, Sturdevant’s eyes scanned from a wheelchair leaning against the wall to a can of Ensure on the bedside table before settling on the young man. He was rubbing his feet, wincing from H.I.V.-related neuropathy that caused what he described as “ungodly pain.” Jordon’s round, hooded eyes were sunk deep into his face. Gray sweatpants pooled around his stick-thin legs, so fragile they looked as if you could snap them in two. His arms were marked with scars from hospital visits and IVs. Over six feet tall, he weighed barely 100 pounds. He smiled slightly when he saw Sturdevant, dimples folding into his hollow cheeks. “Hey, Mr. Ced,” he said, his voice raspy.

With the use of antiretroviral therapy, chronic HIV can last several decades. Without treatment, HIV can be expected to progress to AIDS sooner. By that time, the immune system is quite damaged and has a hard time fighting off infection and disease.

Three groups of HIV-1 have been identified on the basis of differences in the envelope (env) region: M, N, and O.[97] Group M is the most prevalent and is subdivided into eight subtypes (or clades), based on the whole genome, which are geographically distinct.[98] The most prevalent are subtypes B (found mainly in North America and Europe), A and D (found mainly in Africa), and C (found mainly in Africa and Asia); these subtypes form branches in the phylogenetic tree representing the lineage of the M group of HIV-1. Co-infection with distinct subtypes gives rise to circulating recombinant forms (CRFs). In 2000, the last year in which an analysis of global subtype prevalence was made, 47.2% of infections worldwide were of subtype C, 26.7% were of subtype A/CRF02_AG, 12.3% were of subtype B, 5.3% were of subtype D, 3.2% were of CRF_AE, and the remaining 5.3% were composed of other subtypes and CRFs.[99] Most HIV-1 research is focused on subtype B; few laboratories focus on the other subtypes.[100] The existence of a fourth group, “P”, has been hypothesised based on a virus isolated in 2009.[101] The strain is apparently derived from gorilla SIV (SIVgor), first isolated from western lowland gorillas in 2006.[101]

When HIV infection destroys CD4+ lymphocytes, it weakens the body’s immune system, which protects against many infections and cancers. This weakening is part of the reason that the body is unable to eliminate HIV infection once it has started. However, the immune system is able to mount some response. Within a month or two after infection, the body produces lymphocytes and antibodies that help lower the amount of HIV in the blood and keep the infection under control. For this reason, untreated HIV infection may cause no symptoms or only a few mild symptoms for an average of about 10 years (ranging from 2 to more than 15 years).

In addition to thrush and painful ulcers in the mouth, patients may develop a condition called hairy leukoplakia. The CDC also regards this condition as an indicator of full-blown AIDS. Hairy leukoplakia is a white area of diseased tissue on the tongue that may be flat or slightly raised. It is associated with infection by the Epstein-Barr virus.

Without treatment, risk of progression to AIDS is about 1 to 2%/yr in the first 2 to 3 yr of infection and about 5 to 6%/yr thereafter. Eventually, AIDS almost invariably develops in untreated patients.

According to the Joint United Nations Programme on HIV/AIDS (UNAIDS), [76] worldwide in 2015, approximately 36.7 million people (1% of the global adult population aged 15-49 years) were infected with HIV, a decline from 2006 (39.5 million reported at that time). UNAIDS estimates that approximately 2.1 million people were newly infected with HIV and that 1.1 million people died of AIDS in 2015, both statistics showing a decline over time.

In 1981, cases of a rare lung infection called Pneumocystis carinii pneumonia (PCP) were found in five young, previously healthy gay men in Los Angeles.2 At the same time, were reports of a group of men in New York and California with an unusually aggressive cancer named Kaposi’s Sarcoma.3

People who are likely to come into contact with blood or other body fluids at their job should wear protective latex gloves, masks, and eye shields. These precautions apply to body fluids from all people, not just those from people with HIV, and are thus called universal precautions. Universal precautions are taken for two reasons:

Persistent generalized lymphadenopathy, or PGL, is a condition in which HIV continues to produce chronic, painless swellings in the lymph nodes during the latent period. The lymph nodes that are most frequently affected by PGL are those in the areas of the neck, jaw, groin, and armpits. PGL affects between 50-70% of patients during latency.

Despite the persistent anti-L.G.B.T. stigma and entrenched social and economic issues that cling to the South, Sturdevant feels a complicated, bone-deep tie to the people and the place. When he encourages his “sons” and “daughters” to take care of themselves and others, he is echoing the love and acceptance he received from his own large family. After years of hiding, when he came out to his mother in his 20s, she told him, “I love you regardless.” When his family eventually found out that he was sick, his mother and sister drove up to where he was living in Memphis, along with six carloads of aunts, uncles, nieces, nephews and cousins. They tried to serve him plates laden with down-home food that he was too ill to eat and did their best to love him back to health. In the hospital, he finally admitted to his mother he had AIDS. “She told me, ‘Boy, you gonna be all right; God got you,’ ” he recalls, tearing up. In the end, they took him home. He moved back to his mother’s house in Metcalfe, with somebody from the sprawling network of nearly 100 family members always close by, until he recovered. “They saved my life, and I’ll never forget that,” he said.

As patients near the end of life, clinicians may need to prescribe drugs to relieve pain, anorexia, agitation, and other distressing symptoms. The profound weight loss in many people during the last stages of AIDS makes good skin care difficult. The comprehensive support provided by hospice programs helps many patients because hospice providers are unusually skilled at symptom management, and they support caregivers and patient autonomy.

A few exceptional patients can control their HIV strain without treatment; they maintain normal CD4 counts and very low blood levels of HIV (long-term nonprogressors) or normal CD4 counts and undetectable blood levels of HIV (elite controllers). These patients may not require ART, but studies to determine whether treating them is helpful have not been done and would be difficult because there are few of these patients and they would likely do well not taking ART for long periods. The largest Collaboratory, with more than twenty members, is led by David Margolis, at the University of North Carolina. Margolis, an infectious-disease expert, is targeting the reservoirs directly. The idea, which has come to be known as “shock and kill,” is to reactivate the dormant virus, unmasking the cells that carry it, so that they can be destroyed. In 2012, he published the results of a clinical trial of the drug Vorinostat, which was originally developed for blood cancers of T cells, as a shock treatment. This October, “shock and kill” was widely discussed when the Collaboratory teams convened at the N.I.H., along with hundreds of other researchers, assorted academics, and interested laypeople. Margolis and his group explored in their talk new ways to shock the virus out of dormancy.

Although there is currently no cure for HIV with the right treatment and support, people with HIV can live long and healthy lives. To do this, it is especially important to take treatment correctly and deal with any possible side-effects.

If an exposure occurs, the exposed person can reduce the risk of getting HIV by taking antiretroviral medications. Current recommendations suggest two or more antiretroviral medications, depending on the risk of transmission and type of exposure. Medications should be started as soon as possible, preferably within hours of exposure and should be continued for four weeks, if tolerated. People who have been exposed should be tested for HIV at the time of the injury and again at six weeks, 12 weeks, and six months after exposure.

The assumption was that since students will have sex anyway—despite warnings not to—they had better be protected. Conservatives see this position as a cop-out in two ways: it sells values short and it undermines parental authority. In 1992, in Washington, D.C., critics erupted over a decision by the Public Health Commission to hand out condoms in junior and senior high schools without parental consent. William Brown, president of the D.C. Congress of Parents and Teachers, complained: “We are looking to build and reinforce and establish family values where they have been lost, and here we have an agency of our government that totally ignores those things we are working for.” Dr. Mary Ellen Bradshaw, the commission’s chief, replied: “Our whole focus is to save the lives of these children, stressing abstinence as the only sure way to avoid [AIDS] and making condoms available only after intensive education.” In other cities, upset parents simply sued. By 1992, Class Action lawsuits had been brought against school districts in New York City, Seattle, and Falmouth, Massachusetts, arguing that condom distribution violated parents’ right to privacy.

HIV/Aids is caused by the Human Immunodeficiency Virus (HIV). HIV is mainly transmitted through sexual intercourse, but can also be passed down from mother to child, acquired via blood transfusion with infected blood, or other methods. Once a person is infected, the virus remains in the body for life. There is no cure for HIV/Aids, but there are drugs that help control the virus, enabling people with symptoms of HIV live full and healthy lives. There are also various methods to help prevent the spread of the disease.

One of the obstacles to treatment of the human immunodeficiency virus is its high genetic variability.[1] HIV can be divided into two major types, HIV type 1 (HIV-1) and HIV type 2 (HIV-2). HIV-1 is related to viruses found in chimpanzees and gorillas living in western Africa, while HIV-2 viruses are related to viruses found in the endangered west African primate sooty mangabey.[2] HIV-1 viruses may be further divided into groups. The HIV-1 group M viruses predominate and are responsible for the AIDS pandemic. Group M can be further subdivided into subtypes based on genetic sequence data. Some of the subtypes are known to be more virulent or are resistant to different medications. Likewise, HIV-2 viruses are thought to be less virulent and transmissible than HIV-1 M group viruses, although HIV-2 is known to cause AIDS.

Being HIV-positive, or having HIV disease, is not the same as having AIDS. Many people are HIV-positive but don’t get sick for many years. As HIV disease continues, it slowly wears down the immune system. Viruses, parasites, fungi and bacteria that usually don’t cause any problems can make you very sick if your immune system is damaged. These are called “opportunistic infections.” See Fact Sheet 500 for an overview of opportunistic infections.

The only available drug in this class is called maraviroc (Selzentry, MVC), which is now approved for use in combination therapy in treatment-experienced and naïve patients who do not have detectable CXCR4-using virus as determined by a tropism assay. This is a unique drug in a new class that blocks viral entry by interacting with the CCR5 molecule on the surface of the CD4 cell. It is known that HIV first binds to the CD4 molecule on the surface of CD4 cells and then connects with the CCR5 or CXCR4 molecule. Only after this second step is the virus able to enter the cell. The CCR5 antagonist prevents viruses that use CCR5 from getting into the cell. What is unique about this drug compared to others is that 20%-50% of patients have viruses that are able to use the CXCR4 receptor. In these cases, CCR5 antagonists do not appear to be active at suppressing virus. Therefore, in order to know if the drug will work for a given patient, a new test needs to be performed, the so-called tropism assays. This test will tell the provider and patient whether there is virus that uses CXCR4, in which case the patient would not be a candidate for MVC, or if they only have viruses that use CCR5, in which case MVC should be an active drug. Without tropism results, it is impossible to know whether MVC will be an active drug for a given patient.

The most important thing you can do is start antiretroviral treatment as soon as possible. And it’s important to follow up with your doctor regularly. By taking your medications exactly as prescribed, you can keep your viral count low and your immune system strong.

Between 1 million and 1.2 million individuals in the United States are estimated to be living with human immunodeficiency virus (HIV) or acquired immunodeficiency syndrome (AIDS) (1). Soon thereafter, the same condition was seen in IV drug abusers, haemophilliacs and babies of IV drug abusing mothers. These patients had profound immunosuppression due to the depletion of T4 helper lymphocytes and the name ‘acquired immunodeficiency’ was coined for this syndrome. Epidemiological studies have now established that the disease is infectious and can be transmitted by sexual intercourse, blood or blood products. The lymphocytes of patients died early, creating a difficulty in isolating the virus. Montagnier and Gallo eventually isolated the virus in 1984 and HIV-2 was isolated in 1986 from West Africa. If treatment fails, drug susceptibility (resistance) assays can determine the susceptibility of the dominant HIV strain to all available drugs. Genotypic and phenotypic assays are available and can help clinicians select a new regimen that should contain at least 2 and preferably 3 drugs to which the HIV strain is more susceptible. The dominant HIV strain in the blood of patients who are taken off antiretroviral therapy may revert over months to years to the wild-type (ie, susceptible) strain because the resistant mutants replicate more slowly and are replaced by the wild type. Thus, if patients have not been treated recently, the full extent of resistance may not be apparent through resistance testing, but when treatment resumes, strains with resistance mutations often reemerge from latency and again replace the wild-type HIV strain.

In the developed world, HIV infection is much more common in males. In 2015, males accounted for 81% of all diagnoses of HIV infection among adults and adolescents in the United States. [72] Among heterosexuals, females are more likely to acquire HIV infection from an infected male than a male is from an infected female, but a large proportion of infections in males are due to homosexual contact, with or without injection drug use. Males are also more likely to acquire HIV infection from injection drug use alone.

During Millett’s decades in government and nonprofit organizations, he has combed through mounds of data about H.I.V./AIDS and black gay and bisexual men. Two years ago, he and his amfAR colleagues published a comprehensive report titled “H.I.V. and the Black Community: Do #Black(Gay)Lives Matter?” When the calm, usually sunny Millett, known for his bookish blue glasses and ready smile, talks about what he calls this “perfect storm,” his voice takes on a harder edge. “We are going to eventually end AIDS in the United States, but I fear it’s not going to happen for black M.S.M.,” he said, referring to men who have sex with men. “We have waited too long. With so many black gay men already infected, the horse is already out of the barn.”

If a person gets an “AIDS-defining illness,” this is usually a sign that the person has AIDS. Healthy people do not get these illnesses, because a healthy immune system is strong enough to fight off these diseases. Because of this, getting an AIDS-defining illness is a sign that a person’s immune system is seriously damaged. In a person with HIV, getting an AIDS-defining illness signals that the HIV has damaged the immune system badly enough that the person now has AIDS.

Epidemics have no single answer beyond a cure. Since no cure for AIDS existed as of the early 2000s, the law continued to grapple with a vast number of problems. The federal government has addressed AIDS in two broad ways: by spending money on research and treatment of the disease and by prohibiting unfairness to people with HIV or AIDS. It has funded medical treatment, research, and public education, and it has passed laws prohibiting discrimination against people who are HIV-positive or who have developed AIDS. States and local municipalities have joined in these efforts, sometimes with federal help. In addition, states have criminalized the act of knowingly transmitting the virus through sexual behavior or blood donation. The courts, of course, are the decision makers in AIDS law. They have heard a number of cases in areas that range from employment to education and from crimes to torts. Although a body of case law has developed, it remains relatively new with respect to most issues and controversial in all.

Treatment with antiretroviral drugs is recommended for almost all people with HIV infection because without treatment, HIV infection can lead to serious complications and because newer, less toxic drugs have been developed. For most people, early treatment has the best results.

Cure of HIV infection has not been thought possible, and thus lifelong drug treatment is considered necessary. Patients living with HIV infection should be urged to take their antiretroviral drugs consistently. An instance of a possible cure was widely reported in an infant with transient eradication of replication-competent HIV after about 15 mo of antiretroviral therapy. However, HIV replication subsequently resumed. In a large international clinical trial, risk of opportunistic infection or death from any cause, particularly from premature coronary artery disease, cerebrovascular events, or liver and kidney disorders, was significantly higher when antiretroviral therapy was taken episodically (guided by the CD4 count) than when it was taken continuously (1).

Over the past 3 decades the world has witnessed the evolution of the HIV pandemic. The impact of this infection continues to devastate much of Africa and many other poor communities throughout the world. The immunosuppression and immune dysregulation that typifies this disease is triggered by the human immunodeficiency virus (HIV), of which there are two subtypes: HIV-1 and HIV-2. HIV-1 has been responsible for the majority of infections worldwide, whilst HIV-2 causes a milder disease and has affected predominantly those in West Africa.

These results provide a dramatic confirmation of experimental work suggesting that CCR5 is the major macrophage and T-lymphocyte co-receptor used by HIV to establish primary infection in vivo, and offers the possibility that primary infection might be blocked by therapeutic antagonists of the CCR5 receptor. Indeed, there is preliminary evidence that low molecular weight inhibitors of this receptor can block infection of macrophages by HIV in vitro. Such low molecular weight inhibitors might be the precursors of useful drugs that could be taken by mouth. Such drugs are very unlikely to provide complete protection against infection, as a very small number of individuals who are homozygous for the nonfunctional variant of CCR5 are infected with HIV. These individuals seem to have suffered from primary infection by CXCR4-using strains of the virus.

In May 2007, the WHO and UNAIDS issued new guidance recommending “provider-initiated” HIV testing in healthcare settings. This aimed to widen knowledge of HIV status and greatly increase access to HIV treatment and prevention.83

As HIV destroys more CD4 cells and makes more copies of itself, it gradually breaks down a person’s immune system. This means someone living with HIV, who is not receiving treatment, will find it harder and harder to fight off infections and diseases.

Exclusion from childcare, preschool, school and work is not necessary. Children with HIV infection may be advised to stay away from school during outbreaks of infectious disease (for example, chickenpox) to prevent them getting the infection.

There is however a Home Access test approved which can be found at most drugstores. In this test blood from a finger prick is placed on a card and sent to a licensed lab. Consumers are given an identification number to use when phoning for results and have the opportunity to speak with a counselor if desired.

Seroconversion may take a few weeks, up to several months. Symptoms during this time may include fever, flulike illness, lymphadenopathy, and rash. These manifestations develop in approximately half of all people infected with HIV.

Other actions during the 1990s have relied upon the ADA. In 1994, the U.S. Justice Department reached a settlement in a lawsuit with the city of Philadelphia that ensures that city employees will treat patients with AIDS. The first settlement in a health care–related ADA suit, the case grew out of an incident in 1993: when an HIV-positive man collapsed on a Philadelphia street, emergency medical workers not only refused to touch him but told him to get on a stretcher by himself. The man sued. In settling the case, the city agreed to begin an extensive training program for its 900 emergency medical technicians and 1,400 firefighters. In addition, officials paid the man $10,000 in Compensatory Damages and apologized. The Justice Department viewed the suit as an important test of the ADA. Assistant Attorney General James Turner said the settlement would “send a clear message to all cities across the nation that we will not tolerate discrimination against persons with AIDS.”

Abstinence-only adherents think being less frank is being more responsible. They view sexuality as a moral issue properly left for parents to discuss with their children and one that lies beyond the responsibilities of schools. The conservative columnist Cal Thomas spoke for this viewpoint when he argued that parents “have lost a significant right to rear their children according to their own moral standards.” Other objections come from religious conservatives who oppose any neutral or positive discussion of homosexuality. Koop, for example, was blasted for allegedly “sponsoring homosexually oriented curricula” and “teaching buggery in the 3rd grade.” In addition to voicing moral objections, critics say comprehensive sex ed. is generally a failure because it encourages a false sense of security among teens that leads to experimentation with sex or drugs. “We have given children more information presumably because we think it will change their behavior, and yet the behavior has gotten worse, not better,” said Gary Bauer, president of the Family Research Council.

People who already have a sexually transmitted infection, such as syphilis, genital herpes, chlamydia, human papillomavirus (HPV), gonorrhea, or bacterial vaginosis, are more likely to acquire HIV infection during sex with an infected partner.

According to the 2006 report on the Global AIDS Epidemic by the Joint United Nations Programme, approximately 37.2 million adults and 2.3 million children were living with HIV at the end of 2006. During 2006, some 4.3 million people became infected with HIV, and approximately 2.9 million deaths resulted from HIV/AIDS.

Some people may develop a flu-like illness within a month or two after exposure to the HIV virus, although many people do not develop any symptoms at all when they first become infected. Many people mistake this flu-like illness as being caused by something else. Persistent or severe symptoms may not surface for 10 years or more, after HIV first enters the body in adults, or within two years in children born with an HIV infection.

Although a fever technically is any body temperature above the normal of 98.6 F (37 C), in practice, a person is usually not considered to have a significant fever until the temperature is above 100.4 F (38 C). Fever is part of the body’s own disease-fighting arsenal; rising body temperatures apparently are capable of killing off many disease-producing organisms.

Many patients develop low-grade fevers, chronic fatigue, and general weakness. HIV also may cause a combination of food malabsorption, loss of appetite, and increased metabolism that contribute to AIDS wasting syndrome.

HIV/AIDS has had a great impact on society, both as an illness and as a source of discrimination.[22] The disease also has large economic impacts.[22] There are many misconceptions about HIV/AIDS such as the belief that it can be transmitted by casual non-sexual contact.[23] The disease has become subject to many controversies involving religion including the Catholic Church’s position not to support condom use as prevention.[24] It has attracted international medical and political attention as well as large-scale funding since it was identified in the 1980s.[25]

In the early days, the CDC did not have an official name for the disease, often referring to it by way of the diseases that were associated with it, for example, lymphadenopathy, the disease after which the discoverers of HIV originally named the virus.[222][223] They also used Kaposi’s sarcoma and opportunistic infections, the name by which a task force had been set up in 1981.[224] At one point, the CDC coined the phrase “the 4H disease”, since the syndrome seemed to affect heroin users, homosexuals, hemophiliacs, and Haitians.[225][226] In the general press, the term “GRID”, which stood for gay-related immune deficiency, had been coined.[227] However, after determining that AIDS was not isolated to the gay community,[224] it was realized that the term GRID was misleading and the term AIDS was introduced at a meeting in July 1982.[228] By September 1982 the CDC started referring to the disease as AIDS.[229]

The ‘N’ stands for “non-M, non-O”. This group was discovered by a Franco-Cameroonia team in 1998, when they identified and isolated the HIV-1 variant strain, YBF380, from a Cameroonian woman who died of AIDS in 1995. When tested, the YBF380 variant reacted with an envelope antigen from SIVcpz rather than with those of Group M or Group O, indicating it was indeed a novel strain of HIV-1.[11] As of 2015, less than 20 Group N infections have been recorded.[12]

Because HIV is not transmitted through the air or by casual contact (such as touching, holding, or dry kissing), hospitals and clinics do not isolate HIV-infected people unless they have another contagious infection.

Although the risk of clinician-to-patient transmission is extremely low, all infected physicians must make a decision as to which procedures they can continue to perform safely. This decision primarily will depend on the particular surgical involved and also on the physician’s level of expertise and medical condition, including mental status. Greg Millett, a senior scientist for the C.D.C. for 14 years and a senior policy adviser for the Obama administration’s White House Office of National AIDS Policy, put it more candidly. “During the Bush years, the administration dropped all pretense that they cared about AIDS in this country,” said Millett, who is now the vice president and director of public policy at amfAR, the Foundation for AIDS Research. “The White House said H.I.V. is only a problem in sub-Saharan Africa, and that message filtered down to the public. The community’s awakening came in 1991, when Magic Johnson tearfully announced, “Because of the H.I.V. virus I have obtained, I will have to retire from the Lakers today,” and warned, “It can happen to anyone.” By 1994, AIDS had become the No. 1 killer of all African-Americans ages 25 to 44. The virus was 16 times as common in black women as in their white counterparts — and the gap would widen over the next few years. I was an editor at Essence in 1994 when the magazine’s editor in chief, Susan L. Taylor, insisted that we shine a light on the disturbing increase of H.I.V. among African-American women by putting Rae Lewis Thornton, a Chicago woman who described herself as “young, educated, drug-free and dying of AIDS,” on the cover.

Opt-out testing removes the requirement for pretest counseling and detailed, testing-related informed consent. Under the opt-out strategy, physicians must inform patients that routine blood work will include HIV testing and that they have the right to refuse this test. The goal of this strategy is to make HIV testing less cumbersome and more likely to be performed by incorporating it into the routine battery of tests (eg, the first-trimester prenatal panel or blood counts and cholesterol screening for annual examinations). In theory, if testing barriers are reduced, more physicians may offer testing, which may lead to the identification and treatment of more women who are infected with HIV and, if pregnant, to the prevention of mother-to-infant transmission of HIV. This testing strategy aims to balance competing ethical considerations. On the one hand, personal freedom (autonomy) is diminished. On the other hand, there are medical and social benefits for the woman and, if she is pregnant, her newborn from identifying HIV infection. Although many welcome the now widely endorsed opt-out testing policy for the potential benefits it confers, others have raised concerns about the possibility that the requirement for notification before testing will be ignored, particularly in today’s busy practice environment. Indeed, the opt-out strategy is an ethically acceptable testing strategy only if the patient is given the option to refuse testing. In the absence of that notification, this approach is merely mandatory testing in disguise. If opt-out testing is elected as a testing strategy, a clinician must notify the patient that HIV testing is to be performed. Refusal of testing should not have an adverse effect on the care the patient receives or lead to denial of health care. This guarantee of a right to refuse testing ensures that respect for a woman’s autonomy is not completely abridged in the quest to achieve a difficult-to-reach public health goal.

The Centers for Disease Control has defined AIDS as beginning when a person with HIV infection has a CD4 cell (also called “t-cell”, a type of immune cell) count below 200. It is also defined by numerous opportunistic infections and cancers that occur in the presence of HIV infection.

Although epidemics are public crises, they begin with individuals. The rights of people who have AIDS and those who do not are often in contention and seldom more so than in private life. It is no surprise that people with HIV continue having sex, nor is it a surprise that this behavior is, usually, legal. Unfortunately, some do so without knowing they have the virus. Even more unfortunately, others do so in full knowledge that they are HIV-positive but without informing their partners. This dangerous behavior has opened one area of AIDS law that affects individuals: the legal duty to warn a partner before engaging in behavior that can transmit the infection. A similar duty was recognized by courts long before AIDS ever appeared, with regard to other sexually transmitted diseases.

Women exposed to HIV infection through heterosexual contact are the most rapidly growing risk group in the United States. The percentage of AIDS cases diagnosed in American women has risen from 7% in 1985 to about 25% in 2006. According to the CDC, in 2006 approximately 278,400 women in the United States were living with HIV/AIDS. The rate was highest among black women and lowest among white women. About 75% of these women contracted HIV through high-risk heterosexual activity; almost all of the remainder acquired the infection through needle sharing.

On June 5, 1981, the U.S. Centers for Disease Control and Prevention (CDC) published a report describing a rare lung infection known as Pneumocystis carinii pneumonia in five homosexual men in Los Angeles. Expert review of the cases suggested that the disease likely was acquired through sexual contact and that it appeared to be associated with immune dysfunction caused by exposure to some factor that predisposed the affected individuals to opportunistic infection. The following month the CDC published a report describing an outbreak of cases of a rare cancer called Kaposi sarcoma in homosexual men in New York City and San Francisco. The report noted that in many instances the cancers were accompanied by opportunistic infections, such as P. carinii pneumonia. Complementary and alternative medicine, including Chinese medicine (CM), has been used to treat acquired immune deficiency syndrome (AIDS) for almost 30 years. We aimed to compare the main differences between AIDS treatment and evaluation strategies between CM and Western Medicine (WM), and analyze advantages and disadvantages. The characteristics of integrative medicine (IM), based on CM and WM, include a patient-centered mode of medicine based on evidence. IM focuses on complex intervention and management with systemic and individual treatment. The evaluation indexes of IM might consist of objective indicators and subjective indexes. IM might be a more valuable method for treating AIDS in the future instead of WM or CM alone.

Sex is only one kind of behavior that has prompted criminal prosecution related to AIDS. Commonly, defendants in cases have been prosecuted for assault. In United States v. Moor, 846 F.2d 1163 (8th Cir., 1988), the Eighth Circuit upheld the conviction of an HIV-infected prisoner found guilty of assault with a deadly weapon—his teeth—for biting two prison guards during a struggle. Teeth were also on trial in Brock v. State, 555 So. 2d 285 (1989), but the Alabama Court of Criminal Appeals refused to regard them as a dangerous weapon. In State v. Haines, 545 N.E.2d 834 (2d Dist. 1989), the Indiana Court of Appeals affirmed a conviction of attempted murder against a man with AIDS who had slashed his wrists to commit suicide; when police officers and paramedics refused to let him die, he began to spit, bite, scratch, and throw blood.

However, clear clinical implications arose before society became aware of the disease; for example, prior to the recognition of HIV, only one case of Pneumocystis pneumonia not clearly associated with immune suppression was diagnosed in the United States between January 1976 and June 1980. In 1981 alone, 42 similar diagnoses were made, and by December 1994, 127,626 cases of Pneumocystis pneumonia with HIV infection as the only identified cause of immune suppression had been reported to the Centers for Disease Control and Prevention (CDC). Also, Kaposi sarcoma is up to 30,000 times more likely to develop in persons with HIV infection than in immunocompetent persons.

HIV is a member of the genus Lentivirus,[12] part of the family Retroviridae.[13] Lentiviruses have many morphologies and biological properties in common. Many species are infected by lentiviruses, which are characteristically responsible for long-duration illnesses with a long incubation period.[14] Lentiviruses are transmitted as single-stranded, positive-sense, enveloped RNA viruses. Upon entry into the target cell, the viral RNA genome is converted (reverse transcribed) into double-stranded DNA by a virally encoded enzyme, reverse transcriptase, that is transported along with the viral genome in the virus particle. The resulting viral DNA is then imported into the cell nucleus and integrated into the cellular DNA by a virally encoded enzyme, integrase, and host co-factors.[15] Once integrated, the virus may become latent, allowing the virus and its host cell to avoid detection by the immune system, for an indiscriminate amount of time.[16] The HIV virus can remain dormant in the human body for up to ten years after primary infection; during this period the virus does not cause symptoms. Alternatively, the integrated viral DNA may be transcribed, producing new RNA genomes and viral proteins, using host cell resources, that are packaged and released from the cell as new virus particles that will begin the replication cycle anew.

Regardless of the cause for the disruption, a loss of thymic replacements in the face of an induced state of immune activation and T-cell loss seems to be a key component of the mechanism by which HIV narrows the T-cell repertoire and progresses to AIDS. [51, 52, 53]

If the CD4 count is low, people are more likely to develop serious infections and other complications of HIV such as certain cancers. Viral load helps predict how fast the CD4 count is likely to decrease over the next few years.

In Seattle, a group headed by Hans-Peter Kiem and Keith Jerome is taking a more futuristic approach. Using an enzyme called Zinc Finger Nuclease, they are genetically altering blood and marrow stem cells so as to disable CCR5, the doorway for infection in T cells. Researchers will modify the stem cells outside the body, so that when the cells are returned some portion of the T cells in the bloodstream will be resistant to H.I.V. infection. Over time, they hope, those cells will propagate, and the patient will slowly build an immune system that is resistant to the virus. Those patients might still have a small reservoir of H.I.V., but their bodies would be able to regulate the infection.

In the United States, HIV disease was first described in 1981 among 2 groups, one in San Francisco and the other in New York City. Numerous young homosexual men presented with opportunistic infections that, at the time, were typically associated with severe immune deficiency: Pneumocystis pneumonia (PCP) and aggressive Kaposi sarcoma. HIV differs from many viruses in that it has very high genetic variability. This diversity is a result of its fast replication cycle, with the generation of about 1010 virions every day, coupled with a high mutation rate of approximately 3 x 10−5 per nucleotide base per cycle of replication and recombinogenic properties of reverse transcriptase.[87][88][89]

Acquired immune deficiency syndrome (AIDS) is an infectious disease caused by the human immunodeficiency virus (HIV). There are two variants of the HIV virus, HIV-1 and HIV-2, both of which ultimately cause AIDS.

Current treatments do not cure the infection. The medicines only work as long as they are taken every day. If the medicines are stopped, the viral load will go up and the CD4 count will drop. If the medicines are not taken regularly, the virus can become resistant to one or more of the drugs, and the treatment will stop working.

Without treatment, risk of progression to AIDS is about 1 to 2%/yr in the first 2 to 3 yr of infection and about 5 to 6%/yr thereafter. Eventually, AIDS almost invariably develops in untreated patients.

Rapid test results usually will be available during the same clinical visit that the specimen (eg, blood or oral swab sample) is collected. Obstetrician–gynecologists who use these tests must be prepared to provide counseling to women who receive positive test results the same day that the specimen is collected. Women with positive test results should be counseled regarding the meaning of these preliminarily positive test results and the need for confirmatory testing (11). Obstetrician–gynecologists should develop collaborative care plans with health care professionals who can provide these counseling services on an emergent basis or train their own staff to handle the initial encounter and, thereafter, transition infected individuals to professionals who can serve as ongoing resources to them. Women whose confirmatory testing yields positive results and, therefore, are infected with HIV should receive or be referred for appropriate clinical and supportive care.

Recently, the CDC changed testing recommendations. All adults should be screened at least once. People who are considered high risk (needle drug users, multiple sex partners, for example) should be tested more often. All pregnant women should be tested. Anyone who has sustained a needle stick or significant blood exposure from a person known to have HIV or from an unknown source should be tested, too.

In 2016, about 36.7 million people, including about 2.1 million children (< 15 yr), were living with HIV worldwide, according to the World Health Organization (WHO [1]). Since 2010, new infections among children have decreased by 47%, from about 300,000 to 160,000 (in 2016). Retroviruses produce the enzyme reverse transcriptase, which allows the viral RNA genome to be transcribed into DNA inside the host cell. HIV is transmitted through contact with an infected individual’s blood, semen, breast milk, cervical secretions, cerebrospinal fluid, or synovial fluid. It infects CD4-positive helper T cells of the immune system and causes infection with an incubation period that averages 10 years. With the immune system destroyed, AIDS develops as opportunistic infections such as candidiasis, Kaposi’s sarcoma, Pneumocystis pneumonia, and tuberculosis attack organ systems throughout the body. Aside from the initial antibody tests (enzyme-linked immunosorbent assay and Western blot) that establish the diagnosis for HIV infection, the most important laboratory test for monitoring the level of infection is the CD4 lymphocyte test, which determines the percentage of T lymphocytes that are CD4 positive. Patients with CD4 cell counts greater than 500/mm3 are considered most likely to respond to treatment with alpha-interferon and/or zidovudine. A significant drop in the CD4 cell count is a signal for therapeutic intervention with antiretroviral therapy. An immune deficiency disease occurs when the immune system is not working properly. If you are born with a deficiency or if there is a genetic cause, it is called primary immunodeficiency disease. There are more than 100 primary immunodeficiency disorders.

HIV, or human immunodeficiency virus, attacks the immune system by destroying white blood cells, which are vital to fighting infection. Once enough of these cells have been destroyed and the person has another “opportunistic” infection like pneumonia or tuberculosis, the diagnosis moves to the final stage of the infection, called AIDS.

These symptoms can be so mild that you might not even notice them. However, the amount of virus in your bloodstream (viral load) is quite high at this time. As a result, the infection spreads more easily during primary infection than during the next stage.

Specific adverse events are related to the antiretroviral agent taken.[160] Some relatively common adverse events include: lipodystrophy syndrome, dyslipidemia, and diabetes mellitus, especially with protease inhibitors.[2] Other common symptoms include diarrhea,[160][161] and an increased risk of cardiovascular disease.[162] Newer recommended treatments are associated with fewer adverse effects.[29] Certain medications may be associated with birth defects and therefore may be unsuitable for women hoping to have children.[29]

Because disease-related complications can occur in untreated patients with high CD4 counts and because less toxic drugs have been developed, treatment with ART is now recommended for nearly all patients. The benefits of ART outweigh the risks in every patient group and setting that has been carefully studied. In the Strategic Timing of AntiRetroviral Treatment (START) study, 5472 treatment-naïve patients with HIV infection and CD4 counts > 350 cells/mL were randomized to start ART immediately (immediate initiation) or to defer ART until their CD4 count decreased to < 250 cells/mL(deferred initiation). Risk of AIDS-related events (eg, TB, Kaposi sarcoma, malignant lymphomas) and non-AIDS–related events (eg, non-AIDS cancer, cardiovascular disease) was lower in the immediate-initiation group (1). There are still tremendous hurdles. Thirty-five million people in the world are living with the virus. In sub-Saharan Africa, where most new cases are reported, sixty-three per cent of those eligible for the drug regimen do not receive it; those who do often fail to receive it in full. There are two goals of treatment for pregnant women with HIV infection: to treat maternal infection and to reduce the risk of HIV transmission from mother to child. Women can pass HIV to their babies during pregnancy, during delivery, or after delivery by breastfeeding. Without treatment of the mother and without breastfeeding, the risk of transmission to the baby is about 25%. With treatment of the mother before and during birth and with treatment of the baby after birth, the risk decreases to less than 2%. Because of this benefit, it is recommended that all pregnant women be routinely tested for HIV as part of their prenatal care. Once diagnosed, there are several options for treatment, although some antiretroviral medications cannot be used in pregnancy and others have not been studied in pregnancy. For example, the medication efavirenz (Sustiva) is usually avoided in early pregnancy or in women who are likely to become pregnant. Fortunately, there are treatment regimens that have been shown to be well-tolerated by most pregnant women, significantly improving the outcome for mother and child. The same principles of testing for drug resistance and combining antiretrovirals that are used for nonpregnant patients are used for pregnant patients. All pregnant women with HIV should be treated with ART regardless of their CD4 cell count, although the choice of drugs may differ slightly from nonpregnant women. Screening of blood donors with tests for both antibody to HIV and HIV RNA has minimized risk of transmission via transfusion. Seroconversion is the clearest evidence for an adaptive immune response to infection with HIV, but the generation of T lymphocytes responding to infected cells is thought by most workers in the field to be central in controlling the infection. Both CD8 cytotoxic T cells and TH1 cells specifically responsive to infected cells are associated with the decline in detectable virus after the initial infection. These T-cell responses are unable to clear the infection completely and can cause some pathology. Nevertheless, there is evidence that the virus itself is cytopathic, and T-cell responses that reduce viral spread should therefore, on balance, reduce the pathology of the disease.

The pattern of opportunistic infections in a geographic region reflects the pathogens that are common in that area. For example, persons with AIDS in the United States tend to present with commensal organisms such as Pneumocystis and Candida species, homosexual men are more likely to develop Kaposi sarcoma because of co-infection with HHV8, and tuberculosis is common in developing countries.

A course of antiretrovirals administered within 48 to 72 hours after exposure to HIV-positive blood or genital secretions is referred to as post-exposure prophylaxis (PEP).[136] The use of the single agent zidovudine reduces the risk of a HIV infection five-fold following a needle-stick injury.[136] As of 2013, the prevention regimen recommended in the United States consists of three medications—tenofovir, emtricitabine and raltegravir—as this may reduce the risk further.[137]

It is also important to foster wider availability of comprehensive services for people living with HIV and their partners through partnerships among health departments, community-based organizations, and health care and social service providers.

This complex scenario leads to the generation of many variants of HIV in a single infected patient in the course of one day.[87] This variability is compounded when a single cell is simultaneously infected by two or more different strains of HIV. When simultaneous infection occurs, the genome of progeny virions may be composed of RNA strands from two different strains. This hybrid virion then infects a new cell where it undergoes replication. As this happens, the reverse transcriptase, by jumping back and forth between the two different RNA templates, will generate a newly synthesized retroviral DNA sequence that is a recombinant between the two parental genomes.[87] This recombination is most obvious when it occurs between subtypes.[87]

There is good evidence that if the levels of HIV remain suppressed and the CD4 count remains high (>200), that life and quality of life can be significantly prolonged and improved. However, HIV tends to become resistant in patients who do not take their medications every day. Also, certain strains of HIV mutate easily and may become resistant to HAART especially quickly.

The impact of AIDS in southern Africa has been devastating. Some communities have been very hard hit with many deaths and economic hardship related to loss of the workforce of young adults. However, significant progress has been made in the last decade. South Africa has the largest antiviral roll-out programme in the world. Campaigns to reduce homophobia are encouraging MSM to declare their sexuality and come forward for testing and treatment. Innovative work with sex workers and injectable drug users, antiretroviral treatment of children, condom distribution programmes and mother-to-child transmission prevention services are all beginning to bear fruit. Life expectancy has increased by five years since the height of the epidemic.[21]With a prevalence of 17.9% and a population of 6.1 million, South Africa has the largest HIV epidemic of any country. In neighbouring countries in southern Africa, the prevalance ranges from 10-15%.[2]

In 2010, the iPrEx study reported the results of the first large study testing the effectiveness of PrEP using orally administered therapy, as opposed to topical agents as in the vaginal PrEP studies. In this study, HIV-uninfected men who had sex with men who took TDF/FTC once daily along with a comprehensive program to promote safe-sex practices and early treatment of sexually transmitted diseases experienced a markedly reduced risk of acquiring HIV compared with those receiving similar prevention practice without TDF/FTC. There are several other studies that have shown that once daily TDF or TDF/FTC have been effective for PrEP in heterosexual men, women, and intravenous drug users. Nevertheless, there are other studies of high-risk HIV-uninfected women that have shown no benefit, with convincing data in both studies demonstrating extremely low levels of treatment adherence with study medications. Based upon the data available, the United States FDA has approved TDF/FTC for use in high-risk HIV-uninfected individuals. When this therapy is utilized, it is clear that people need to be extensively counseled regarding the importance of continued use of condoms as well as diligent screening for HIV infection, acquisition of sexually transmitted diseases, as well as treatment adherence. Treated individuals also need to be made aware of potential side effects of treatment, including gastrointestinal symptoms, kidney damage, and decreases in bone mineral density.

Those at highest risk include homosexual or bisexual men engaging in unprotected sex, intravenous drug users who share needles, the sexual partners of those who participate in high-risk activities, infants born to mothers with HIV, and people who received blood transfusions or clotting products between 1977 and 1985 (prior to standard screening for the virus in the blood). The O (“Outlier”) group is not usually seen outside of West-central Africa. It is reportedly most common in Cameroon, where a 1997 survey found that about 2% of HIV-positive samples were from Group O.[13] The group caused some concern because it could not be detected by early versions of the HIV-1 test kits. A person is considered to have wasting syndrome if they lose 10% or more of their body weight and have had diarrhea or weakness and fever for more than 30 days, according to the U.S. Department of Health and Human Services.

This stage of HIV infection generally lasts around 10 years if you’re not receiving antiretroviral therapy. But sometimes, even with this treatment, it lasts for decades. Some people develop more severe disease much sooner.

Fungi. The most common fungal disease associated with AIDS is Pneumocystis carinii pneumonia (PCP). PCP is the immediate cause of death in 15-20% of AIDS patients. It is an important measure of a patient’s prognosis. Other fungal infections include a yeast infection of the mouth (candidiasis or thrush) and cryptococcal meningitis.

Survival rates have dramatically improved for those individuals using protease inhibitors, but other problems have also arisen. Some persons do not respond to these medications or the side effects from taking the drugs diminish the quality of life. Protease inhibitors, for many people, are intolerable because of nausea, diarrhea, vomiting, headache, kidney stones, and serious drug interactions with other medications. By 2003 researchers had found that serious side effects include increased risk of heart attack, abnormalities in fat distribution, an increased propensity toward diabetes, and abnormalities in cholesterol metabolism.

Kidney disease, which is a common complication of HIV infection and its treatment, may shorten the lifespan of affected patients. This review considers the breadth of conditions that may affect the kidneys in persons with HIV infection.

HIV is a retrovirus. That is, it stores its genetic information as ribonucleic acid (RNA). Once inside a CD4+ lymphocyte, the virus uses an enzyme called reverse transcriptase to make a copy of its RNA, but the copy is made as deoxyribonucleic acid (DNA). HIV mutates easily at this point because reverse transcriptase is prone to making errors during the conversion of HIV RNA to DNA. These mutations make HIV more difficult to control because the many mutations increase the chance of producing HIV that can resist attacks by the person’s immune system and/or antiretroviral drugs.

HIV is transmitted when the virus enters the body, usually by infected immune cells in blood, vaginal fluids, or semen. Having the following risk factors increases the chance a person may become infected with HIV.

HIV/AIDS is diagnosed via laboratory testing and then staged based on the presence of certain signs or symptoms.[26] HIV screening is recommended by the United States Preventive Services Task Force for all people 15 years to 65 years of age including all pregnant women.[105] Additionally, testing is recommended for those at high risk, which includes anyone diagnosed with a sexually transmitted illness.[29] In many areas of the world, a third of HIV carriers only discover they are infected at an advanced stage of the disease when AIDS or severe immunodeficiency has become apparent.[29]

Other antiviral agents are in investigational stages and many new drugs are in the pipeline. Growth factors that stimulate cell growth, such as Epogen (erthythropoetin) and G-CSF are sometimes used to treat anemia and low white blood cell counts associated with AIDS.

HIV antibody tests detect antibodies the body produces to neutralize the virus. HIV RNA testing uses polymerase chain reaction to detect HIV RNA in a person’s blood. It usually takes one to three days to get results.

Acute HIV infection may be associated with symptoms resembling mononucleosis or the flu within 2 to 4 weeks of exposure. HIV seroconversion (converting from HIV negative to HIV positive) usually occurs within 3 months of exposure.

Animal models show that Langerhans cells are the first cellular targets of HIV, which fuse with CD4+ lymphocytes and spread into deeper tissues. Patients with most acute opportunistic infections benefit from early ART (initiated during the management of the opportunistic infection). However, for some opportunistic infections, such as tuberculous meningitis or cryptococcal meningitis, the evidence suggests that ART should be delayed until the first phase of antimicrobial therapy for these infections is finished.

The transmission of HIV requires contact with a body fluid that contains the virus or cells infected with the virus. HIV can appear in nearly any body fluid, but transmission occurs mainly through blood, semen, vaginal fluids, and breast milk. Although tears, urine, and saliva may contain low concentrations of HIV, transmission through these fluids is extremely rare, if it occurs at all. HIV is not transmitted by casual contact (such as touching, holding, or dry kissing) or by close, nonsexual contact at work, school, or home. No case of HIV transmission has been traced to the coughing or sneezing of an infected person or to a mosquito bite. Transmission from an infected doctor or dentist to a patient is extremely rare.

Cryptococcal meningitis. Meningitis is an inflammation of the membranes and fluid surrounding your brain and spinal cord (meninges). Cryptococcal meningitis is a common central nervous system infection associated with HIV, caused by a fungus found in soil.

HIV seeks out and destroys CCR5 expressing CD4+ T cells during acute infection.[102] A vigorous immune response eventually controls the infection and initiates the clinically latent phase. CD4+ T cells in mucosal tissues remain particularly affected.[102] Continuous HIV replication causes a state of generalized immune activation persisting throughout the chronic phase.[103] Immune activation, which is reflected by the increased activation state of immune cells and release of pro-inflammatory cytokines, results from the activity of several HIV gene products and the immune response to ongoing HIV replication. It is also linked to the breakdown of the immune surveillance system of the gastrointestinal mucosal barrier caused by the depletion of mucosal CD4+ T cells during the acute phase of disease.[104]

According to the August 2008 report issued by the Joint United Nations Programme on HIV/AIDS (UNAIDS), as of 2007, approximately 33 million people worldwide are HIV positive. Over half of the 33 million are women and this statistic has remained stable for several years. The highest number of cases is found in sub-Saharan Africa and Southeast Asia.

The main treatment for HIV is antiretroviral therapy (ART), a combination of daily medications that stop the virus from reproducing. This helps protect your CD4 cells, keeping your immune system strong enough to fight off disease.

The stage of symptomatic infection, in which the body’s immune (or defense) system has been suppressed and complications have developed, is called the acquired immunodeficiency syndrome (AIDS). The symptoms are caused by the complications of AIDS, which include one or more unusual infections or cancers, severe loss of weight, and intellectual deterioration (called dementia).

Without treatment, HIV is likely to advance to AIDS. At that point, the immune system is too weak to fight off life-threatening disease and infection. Untreated, life expectancy with AIDS is about three years.

In communities with a relatively low prevalence of HIV, rapid testing can present certain logistic difficulties. With the traditional approach, testing would occur during an initial visit, and results would be provided during a follow-up encounter. That would give the health care professional an opportunity to arrange for an individual with expertise in posttest counseling to be available in a circumstance in which the health care professional knew that a patient was returning to receive a positive result. A program of testing and notification at the same visit does not allow the health care professional the luxury of notifying a counselor before a patient who is infected with HIV returns for a visit or of steering an individual who is infected with HIV to a certain session at which the counselor is routinely available. However, the obligation to make sure that appropriate counseling and support services are available still holds. Health care professionals should develop links with individuals who can provide those services on an emergent basis or train their own staff to handle the initial encounter and thereafter transition infected individuals to professionals who can serve as ongoing resources to them.

HIV produces cellular immune deficiency characterized by the depletion of helper T lymphocytes (CD4+ cells). The loss of CD4+ cells results in the development of opportunistic infections and neoplastic processes.

HIV provirus may lie dormant within a cell for a long time but when the cell becomes activated, it treats HIV genes in much the same way as human genes. First, it converts them into mRNAs using human enzymes. The mRNA is then transported outside the nucleus and is used as a blueprint for producing new HIV proteins and enzymes.

It is important to remember that sometimes, for reasons not entirely understood, the viral load can briefly increase. Unexpected increases, therefore, necessitate repeated testing of the viral load before any clinical decisions are made. If, however, the viral load is continually detected despite proper adherence to the prescribed therapy, serious consideration must be given to the possibility that the virus has become resistant to one or more of the medications being given, especially if viral load is greater than 200 copies/mL. There is now an abundance of data showing that the use of drug-resistance tests can improve the response to a follow-up regimen. Testing can be used to determine if an individual’s HIV has become resistant to one or more of the drugs that are being taken. There are currently two main types of resistance tests available in the clinic: one that is called a genotype and the other a phenotype assay. The former looks for mutations in the virus and the latter the actual amount of drug it takes to block infection by the patient’s virus. The genotype test is very helpful in those being screened for the presence of resistant virus prior to initiating treatment and those experiencing viral rebound on one of their first treatment regimens. The phenotype test is particularly useful in those who are highly treatment experienced and have substantial amounts of drug resistance, especially to the protease class. The information derived from these tests, along with a tropism test will ultimately tell the provider which of the many approved drugs are likely to be fully active against the specific patient’s virus. Using this information, the goal is to include at least two and at times preferably three fully active drugs in the next regimen in order to optimize the chances of suppressing the viral load to undetectable levels. It is often useful to seek expert consultation in managing those with multidrug resistant virus.

Human immunodeficiency virus (HIV) infection results from 1 of 2 similar retroviruses (HIV-1 and HIV-2) that destroy CD4+ lymphocytes and impair cell-mediated immunity, increasing risk of certain infections and cancers. Initial infection may cause nonspecific febrile illness. Risk of subsequent manifestations—related to immunodeficiency—is proportional to the level of CD4+ lymphocyte depletion. HIV can directly damage the brain, gonads, kidneys, and heart, causing cognitive impairment, hypogonadism, renal insufficiency, and cardiomyopathy. Manifestations range from asymptomatic carriage to acquired immune deficiency syndrome (AIDS), which is defined by serious opportunistic infections or cancers or a CD4 count of < 200/μL. HIV infection can be diagnosed by antibody, nucleic acid (HIV RNA), or antigen (p24) testing. Screening should be routinely offered to all adults and adolescents. Treatment aims to suppress HIV replication by using combinations of ≥ 3 drugs that inhibit HIV enzymes; treatment can restore immune function in most patients if suppression of replication is sustained. A few exceptional patients can control their HIV strain without treatment; they maintain normal CD4 counts and very low blood levels of HIV (long-term nonprogressors) or normal CD4 counts and undetectable blood levels of HIV (elite controllers). These patients may not require ART, but studies to determine whether treating them is helpful have not been done and would be difficult because there are few of these patients and they would likely do well not taking ART for long periods.
