Most PIs are associated with important drug-drug interactions so they must be used with caution in patients on other medications. There are numerous resources available to patients on these medications to make sure that they do not adversely interact with other HIV or non HIV-related drugs.

Although epidemics are public crises, they begin with individuals. The rights of people who have AIDS and those who do not are often in contention and seldom more so than in private life. It is no surprise that people with HIV continue having sex, nor is it a surprise that this behavior is, usually, legal. Unfortunately, some do so without knowing they have the virus. Even more unfortunately, others do so in full knowledge that they are HIV-positive but without informing their partners. This dangerous behavior has opened one area of AIDS law that affects individuals: the legal duty to warn a partner before engaging in behavior that can transmit the infection. A similar duty was recognized by courts long before AIDS ever appeared, with regard to other sexually transmitted diseases.

Acquired Immune Deficiency Syndrome (AIDS) is a collection of infections and cancers that people with HIV develop. Human Immuno deficiency virus (HIV) is a retrovirus which takes over the body cells and produces new HIV retrovirus. When someone becomes infected with the HIV virus it begins to attack their immune system. The body’s immune system cells are destroyed, allowing pathogens and cancers which the body might have fought off normally to pose a serious threat to infected individuals due to a significant drop in their resistance levels. This process is not visible and a person who is infected can look and feel perfectly well for many years and they may not know that they are infected. As their immune system weakens they become more vulnerable to illnesses that their immune system would normally have fought off. As time goes by they are likely to become ill more often.

Blood transmission — the risk of transmitting HIV through blood transfusion is extremely low in developed countries, thanks to meticulous screening and precautions. However, among people who inject drugs, sharing and reusing syringes contaminated with HIV-infected blood is extremely hazardous.

Shortly after primary infection, most HIV-positive individuals enter a period of many years where they have no symptoms at all. During this time, CD4 cells may gradually decline, and with this decline in the immune system, patients may develop the mild HIV symptoms and signs such as vaginal or oral candidiasis thrush (a fungal infection), fungal infections of the nails, a white brush-like border on the sides of tongue called hairy leukoplakia, chronic rashes, diarrhea, fatigue, and weight loss. Any of these symptoms should prompt HIV testing if it is not being done for other reasons. With a further decline in function of the immune system, patients are at increasing risk of developing more severe of HIV, including more serious infections (opportunistic infections), malignancies, severe weight loss, and decline in mental function. From a practical perspective, most physicians think about patients with HIV diseases as having no symptoms, mild symptoms, or being severely symptomatic. In addition, many would characterize a patient’s level of immunosuppression by the degree and type of symptoms they have as well as the CD4 cell count. The Centers for Disease Control and Prevention have defined the presence of a long list of specific diseases or the presence of less than 200 CD4 cells per mm3 as meeting a somewhat arbitrary definition of AIDS. It is important to note that with effective antiretroviral therapy many of the signs and symptoms of HIV as well as severity of immunosuppression can be completely reversed, restoring even the most symptomatic patients to a state of excellent health.

For people infected with HIV, the risk of progression to AIDS increases with the number of years the person has been infected. The risk of progression to AIDS is decreased by using highly effective antiretroviral therapy (ART) regimens.

This has the most rapid onset of the hepatitis viruses affecting humans; transmission is easier than for the hepatitis B and C viruses, but infection generally does not persist. Other measures can help. For men, circumcision, an inexpensive, safe procedure, reduces the risk of becoming infected during vaginal intercourse with an infected woman by about half. Because circumcision provides only partial protection against HIV infection, people should also use other measures to prevent HIV infection . At present, there is no effective HIV vaccine to prevent HIV infection or slow the progression of AIDS in people who are already infected. However, treating people who have HIV infection reduces the risk of their transmitting the infection to other people.

There is less evidence that treatment of HIV-2 infection slows progression, and certain antiretroviral medications (specifically the non-nucleoside–analogue reverse-transcriptase inhibitors) are not effective against HIV-2. Tissue biopsies, samples of cerebrospinal fluid, and sophisticated imaging techniques, such as magnetic resonance imaging (MRI) and computed tomography scans (CT) are used to diagnose AIDS-related cancers, some opportunistic infections, damage to the central nervous system, and wasting of the muscles. Urine and stool samples are used to diagnose infections caused by parasites. AIDS patients are also given blood tests for syphilis and other sexually transmitted diseases.

The idea of combining medications into a “cocktail” came in the mid-nineteen-nineties, mirroring the way oncologists treated cancer. Cancer cells, like H.I.V. particles, can mutate quickly enough to escape a single targeted drug. The treatment regimen—HAART, for highly active antiretroviral therapy—was put through clinical trials by prominent researchers such as David Ho, of the Aaron Diamond Institute, in New York. I gave the cocktail to one of my patients, David Sanford, and less than a month after beginning treatment his fever fell, his infections disappeared, his energy returned, and he started to gain weight. The H.I.V. in his bloodstream plummeted to an undetectable level, where it has remained. Later, in a Pulitzer Prize-winning article, Sanford wrote, “I am probably more likely to be hit by a truck than to die of AIDS.” That now holds true for a great majority of people with H.I.V. in the United States. In the past five years, not one of the dozens of H.I.V. patients I’ve cared for has died of the disease.

If the CD4 count falls below about 200 cells per microliter of blood, the immune system becomes less able to fight certain infections (such as Pneumocystis jirovecii pneumonia). Most of these infections are rare in healthy people. However, they are common among people with a weakened immune system. Such infections are called opportunistic infections because they take advantage of a weakened immune system.

He told me, “I’m no longer that concerned about the virus itself. I’m more concerned about my internal organs and premature aging.” In 1999, at fifty, he learned that fatty deposits had substantially constricted the blood flow in a major artery that supplies the heart’s left ventricle. He began to experience crippling pain when he walked, because the blood supply to his bone tissue had diminished—a condition called avascular necrosis. In 2002, he had his first hip replacement, and the second in 2010. His muscles have shrunk, and sitting can be uncomfortable, so he sometimes wears special foam-padded underwear. Every other year, he has his face injected with poly-L-lactic acid, which replaces lost connective tissue.

Because HIV is not transmitted through the air or by casual contact (such as touching, holding, or dry kissing), hospitals and clinics do not isolate HIV-infected people unless they have another contagious infection.

Because the recommended population for HIV testing includes adolescents, it also is important to have practices in place to assist young patients. This includes a process of discussing safe-sex practices, risk factors, and behavior that may lead to HIV exposure. Currently, some states allow minors to access HIV testing in a confidential fashion without disclosing testing or results to a parent or guardian (9, 10). However, there are others that require some degree of notification or consent from a parent before testing. It is important for Fellows to be aware of the local policies in place and to fulfill the legal and ethical obligations to their adolescent patients who seek HIV testing as part of their reproductive health care. This period is sometimes called asymptomatic HIV infection or chronic HIV infection. During this phase, HIV is still active but reproduces at very low levels. People may not have any symptoms or get sick during this time. For people who aren’t taking medicine to treat HIV, this period can last a decade or longer, but some may progress through this phase faster. People who are taking medicine to treat HIV (ART) the right way, every day may be in this stage for several decades. It’s important to remember that people can still transmit HIV to others during this phase, although people who are on ART and stay virally suppressed (having a very low level of virus in their blood) are much less likely to transmit HIV than those who are not virally suppressed. At the end of this phase, a person’s viral load starts to go up and the CD4 cell count begins to go down. As this happens, the person may begin to have symptoms as the virus levels increase in the body, and the person moves into Stage 3.

Testing for HIV and other STIs is strongly advised for all people exposed to any of the risk factors. This way people learn of their own infection status and access necessary prevention and treatment services without delay. WHO also recommends offering testing for partners or couples. Additionally, WHO is recommending assisted partner notification approaches so that people with HIV receive support to inform their partners either on their own, or with the help of health care providers.

Untreated HIV destroys certain cells within the immune system (CD4+ or helper T cells) from the time of infection onwards, causing more and more damage. Eventually the damage to the immune system is so great the body can no longer stop some infections or cancers it normally fights successfully. Infections not usually seen in healthy people, called opportunistic infections, and certain unusual tumours such as Kaposi’s sarcoma, may occur. Women with untreated HIV infection are at increased risk of developing cervical cancer and both men and women are at increased risk of anal cancer. Untreated HIV can cause infection in the brain, which can lead to nervous system disorders or dementia in some people with HIV infection.

During the latent period, the virus continues to multiply actively. It infects and kills critical infection fighting cells, a type of white blood cell called CD4 cells or T helper cells (T cells). Even though the person has no symptoms, he or she is contagious and can pass HIV to others through the routes described above. At the end of this phase, as the virus overwhelms the CD4 cells, the HIV viral load starts to rise, and the CD4 cell count begins to drop. As this happens, the person may begin to have symptoms as the virus levels increase in the body. This is stage 3.

Screening test. There are several kinds of tests. Some are blood tests, others are mouth fluid tests. They check for antibodies to the HIV virus, HIV antigen, or both. Some screening tests can give results in 30 minutes or less.

Other actions during the 1990s have relied upon the ADA. In 1994, the U.S. Justice Department reached a settlement in a lawsuit with the city of Philadelphia that ensures that city employees will treat patients with AIDS. The first settlement in a health care–related ADA suit, the case grew out of an incident in 1993: when an HIV-positive man collapsed on a Philadelphia street, emergency medical workers not only refused to touch him but told him to get on a stretcher by himself. The man sued. In settling the case, the city agreed to begin an extensive training program for its 900 emergency medical technicians and 1,400 firefighters. In addition, officials paid the man $10,000 in Compensatory Damages and apologized. The Justice Department viewed the suit as an important test of the ADA. Most AIDS patients require complex long-term treatment with medications for infectious diseases. This treatment is often complicated by the development of resistance in the disease organisms. AIDS-related malignancies in the central nervous system are usually treated with radiation therapy. Cancers elsewhere in the body are treated with chemotherapy.

The ward occupies the sixth floor of an Art Deco building on the north side of campus. I found Deeks in his office, wearing a flannel shirt and New Balance sneakers. He explained his concerns about the drug cocktail. “Antiretroviral drugs are designed to block H.I.V. replication, and they do that quite well,” he said. But they don’t enable many patients to recover fully. The immune system improves enough to prevent AIDS, but, because the virus persists, the immune system must mount a continuous low-level response. That creates chronic inflammation, which injures tissues.

Each virus can be contracted individually, or they can be contracted together in what is referred to as co-infection. HIV-2 seems to have lower mortality rates, less severe symptoms and slower progression to AIDS than HIV-1 alone or the co-infection. In co-infection, however, this is largely dependent on which virus was contracted first. HIV-1 tends to out compete HIV-2 for disease progression. Co-infection seems to be a growing problem globally as time progresses, with most cases being identified in West African countries, as well as some cases in the US.[24]

Macrophages. Tissue macrophages are one of the target cells for HIV. These macrophages harbour the virus and are known to be the source of viral proteins. However, the infected macrophages are shown to lose their ability to ingest and kill foreign microbes and present antigen to T cells. This could have a major contribution in overall immune dysfunction caused by HIV infection.

In 1985, a blood test became available that measures antibodies to HIV that are the body’s immune response to the HIV. The test that for decades had been most commonly used for diagnosing infection with HIV was referred to as an ELISA. If the ELISA found HIV antibodies, the results needed to be confirmed, typically by a test called a Western blot. Recently, tests have become available to look for these same antibodies in saliva, some providing results within one to 20 minutes of testing. As a result, the FDA has approved home HIV antibody testing that is self-administered using saliva. Antibodies to HIV typically develop within several weeks of infection. During this interval, patients have virus in their body but will test negative by the standard antibody test, the so-called “window period.” In this setting, the diagnosis can be made if a test is used that actually detects the presence of virus in the blood rather than the antibodies, such as tests for HIV RNA or p24 antigen. A relatively new test has been approved that measures both HIV antibodies and p24 antigen, shrinking the duration of the window period from infection to diagnosis. New federal guidelines now recommend that HIV screening tests be performed with these assays and, if they are positive, that a confirmatory antibody test be performed that will determine if the patient has HIV-1, the most common form of HIV circulating around the world, or HIV-2, a related virus that occurs most frequently in Western Africa. If the confirmatory antibody test is negative, then there remains the possibility that the original test detected viral p24 antigen and not antibodies. Therefore, the recommendations are that if the confirmatory antibody test is negative a test for HIV RNA, a test for the presence of virus be performed. If the antibody is negative and the viral test is positive, the patient is diagnosed with acute or primary HIV infection and will develop a positive antibody test over the ensuing weeks.

The search for a cure for HIV began as soon as the virus was identified. HIV is probably one of the most studied viruses in history. Scientists have a detailed knowledge of the virus’ genes, proteins, and understand how it functions. In fact, the combinations of drugs that make up ART therapy were chosen because they attack different parts of the virus life cycle, causing it to malfunction. However, ART is not a cure and the drugs must be taken for life. Even when viral levels are low, the virus is still present in the body.

Usually, HIV infection does not directly cause death. Instead, HIV infection leads to a substantial loss of weight (wasting), opportunistic infections, cancers, and other disorders, which then lead to death.

Once infection has progressed to AIDS, the survival period is usually less than 2 years in untreated patients. Persons in whom the infection does not progress long-term may not develop AIDS for 15 years or longer, although many still exhibit laboratory evidence of CD4 T-cell decline or dysfunction. Once HIV is in the immune system, it multiplies inside the CD4 cells, disabling and killing them in the course of the infection, and thus interfering with their normal function. The immune system gradually deteriorates until it reaches a point where it can no longer fight off any infection.

§ Social-structural variables were used to identify a representative sample for NHBS of heterosexual persons at increased risk of HIV infection. Heterosexual persons at increased risk were defined as male or female (not transgender) in a metropolitan statistical area with high AIDS prevalence, who had sex with a member of the opposite sex in the past 12 months, never injected drugs, and met low income or low education criteria. Low income was defined as not exceeding U.S. Department of Health and Human Services poverty guidelines and low education as having a high school education or less.

Analysis of reported AIDS cases shows that 51% had Pneumocystis carinii pneumonia (PCP) without Kaposi’s sarcoma (KS) (with or without other “opportunistic” infections (OOI) predictive of cellular immunodeficiency); 30% had KS without PCP (with or without OOI); 7% had both PCP and KS (with or without OOI); and 12% had OOI with neither PCP nor KS. The overall mortality rate for cases of PCP without KS (47%) was more than twice that for cases of KS without PCP (21%), while the rate for cases of both PCP and KS (68%) was more than three times as great. The mortality rate for OOI with neither KS nor PCP was 48%.

In general, the higher the level of HIV in the blood (the viral load), the more likely that person is to transmit HIV. People who have HIV but have a very low or undetectable viral load (because they are on HIV medicines) are much less likely to transmit HIV. So taking HIV medication is one way to reduce the risk of infecting others. Still, HIV may be present in genital fluids in levels enough to transmit.

Each set of guidelines is developed by a panel of experts in HIV care and research that includes health professionals, researchers, and community members. PEP is short for post-exposure prophylaxis and refers to preventive treatment after occupational exposure to HIV. Occupational transmission of HIV to health-care workers is extremely rare, and the proper use of safety devices minimizes the risk of exposure while caring for patients with HIV. A health-care worker who has a possible exposure should see a doctor immediately. PEP must be started within 72 hours after a recent possible exposure to HIV. HIV continues to be a major public health crisis both in the United States and around the world. ABSTRACT: Early diagnosis and treatment of human immunodeficiency virus (HIV) can improve survival and reduce morbidity. The Centers for Disease Control and Prevention and the American College of Obstetricians and Gynecologists recommend that females aged 13–64 years be tested at least once in their lifetime and annually thereafter based on factors related to risk. In addition, obstetrician–gynecologists should annually review patients’ risk factors for HIV and assess the need for retesting. The opportunity for repeat testing should be made available to all women even in the absence of identified risk factors. Women who are infected with HIV should receive or be referred for appropriate clinical and supportive care. Obstetrician–gynecologists who use rapid tests must be prepared to provide counseling to women who receive positive test results the same day that the specimen is collected. Obstetrician–gynecologists should be aware of and comply with legal requirements regarding HIV testing in their jurisdictions and institutions.

Transmission in pregnancy. High-risk mothers include women sexually active with bisexual men, intravenous drug users, and women living in neighborhoods with a high rate of HIV infection among heterosexuals. The chances of transmitting the disease to the child are higher in women in advanced stages of the disease. Breast feeding increases the risk of HIV transmission as HIV passes into breast milk. The rate of pediatric HIV transmission in the United States had decreased substantially because of HIV testing and improved drug treatment for infected mothers, so fewer than 1% of AIDS cases now occur in children under age 15. In the developing world, mother to infant transmission remains epidemic. In 2006, AIDS was the single most common cause of death in children under age 5 in South Africa, while worldwide children account for about 10% of all AIDS cases.

Risk of HIV transmission after skin penetration with a medical instrument contaminated with infected blood is on average about 1/300 without postexposure antiretroviral prophylaxis. Immediate prophylaxis probably reduces risk to < 1/1500. Risk appears to be higher if the wound is deep or if blood is inoculated (eg, with a contaminated hollow-bore needle). Risk is also increased with hollow-bore needles and with punctures of arteries or veins compared with solid needles or other penetrating objects coated with blood because larger volumes of blood may be transferred. Thus, sharing needles that have entered the veins of other injection drug users is a very high risk activity. AIDS is not a virus but a set of symptoms (or syndrome) caused by the HIV virus. A person is said to have AIDS when their immune system is too weak to fight off infection, and they develop certain defining symptoms and illnesses. This is the last stage of HIV, when the infection is very advanced, and if left untreated will lead to death. There is a specific decline in the CD4+ helper T cells, resulting in inversion of the normal CD4/CD8 T-cell ratio and dysregulation of B-cell antibody production. [26, 27] Immune responses to certain antigens begin to decline, and the host fails to adequately respond to opportunistic infections and normally harmless commensal organisms. Because the defect preferentially affects cellular immunity, the infections tend to be nonbacterial (fungal, viral). HIV transmission is also possible when sperm from an infected donor is used to inseminate a woman. In the United States, measures have been taken to reduce this risk. Fresh semen samples are no longer used. Sperm from donors is frozen for 6 months or more. Then the donors are retested for HIV infection before the sperm is used. There has been a great deal of attention given to the more recently identified problem of "lipodystrophy." Individuals suffering from this syndrome can be categorized as having lipohypertrophy (fat accumulation) syndromes, such as the "buffalo hump" on the back of the neck, breast enlargement, or increased abdominal girth. Others primarily suffer from lipoatrophy with fat loss under the skin with complaints of prominent veins on the arms and legs, sunken cheeks, and decreased gluteal (buttock) size. These syndromes appear to be related to multiple factors, including, but not limited to, drug therapy. The NRTIs appear to be most closely linked to lipoatrophy, in particular D4T and to a lesser extent In fact, some studies have suggested slow accumulation of fat in those who modify the NRTI component of their regimen. Some NRTIs also have been linked to elevation in lipid (fat) levels in the blood. While switching therapy is always a consideration in those experiencing potential drug-related toxicity, this should only be done under the careful supervision of an experienced HIV provider. Such rampant levels of anti-LGBTQ bias is particularly worrisome when so few PLWH in the U.S. seem to have the virus under control. Of the 1.2 million people living with HIV in the U.S. in 2011, only 30% of them had consistently taken their medication and were able to lower the amount of HIV in their bodies to undetectable levels. While undetectable, a person living with HIV remains in good health, and it is virtually impossible transmit the virus to a partner. Prevention options (e.g., condoms, Pre-Exposure Prophylaxis) exist for those in relationships where one partner is not yet undetectable. About 70 percent of all infections occur in people living in sub-Saharan Africa, and in some countries of the region the prevalence of HIV infection of inhabitants exceeds 10 percent of the population. Rates of infection are lower in other parts of the world, but different subtypes of the virus have spread to Europe, India, South and Southeast Asia, Latin America, and the Caribbean. Rates of infection have leveled off somewhat in the United States and Europe. In the United States more than 1.2 million people are living with HIV/AIDS, and about 44 percent of all new infections are among African Americans. In Asia sharp increases in HIV infection have occurred in China and Indonesia. Access to antiretroviral treatment for AIDS remains limited in some areas of the world, although more people are receiving treatment today than in the past. Human immunodeficiency virus infection and AIDs can cause a plethora of hematologic problems. Early on during HIV infection, immune thrombocytopenia is common as is the development of antiphospholipid antibodies. Anemia is the most common manifestation of HIV infection and is multifactorial due to both direct and indirect effects of the virus.12 Anemia is most often a hypoproliferative, low reticulocyte anemia due to anemia of chronic disease. Often, there is a blunted erythropoietin response. Coombs-positive autoimmune hemolytic anemia also occurs with increased frequency in HIV infection. Antiretroviral therapy often causes macrocytosis. Since AIDS can be transmitted from an infected mother to a fetus during pregnancy or to an infant during the birth process or through breastfeeding, all infants born to HIV-positive mothers are considered a high-risk group. However, prenatal drug treatment of HIV-positive mothers in developed countries has reduced the number of children born infected with HIV. In the developing world, drug treatment is either not available or not affordable. According to the United Nations Children's Fund (UNICEF) worldwide 2.3 million children under age 13 were living with HIV in 2006. The previous year, about 380,000 children died of AIDS and more than half a million children were newly infected. UNICEF estimates that at least 15 million children have lost at least one parent to AIDS. He told me, “I’m no longer that concerned about the virus itself. I’m more concerned about my internal organs and premature aging.” In 1999, at fifty, he learned that fatty deposits had substantially constricted the blood flow in a major artery that supplies the heart’s left ventricle. He began to experience crippling pain when he walked, because the blood supply to his bone tissue had diminished—a condition called avascular necrosis. In 2002, he had his first hip replacement, and the second in 2010. His muscles have shrunk, and sitting can be uncomfortable, so he sometimes wears special foam-padded underwear. Every other year, he has his face injected with poly-L-lactic acid, which replaces lost connective tissue. Persons unaware of their human immunodeficiency virus (HIV) infection are estimated to account for approximately 40% of ongoing transmissions in the United States (1). As a result of increased testing, the percentage of persons living with HIV who are aware of their infection has steadily increased; at the end of 2014, an estimated 85% of persons living with HIV were aware of their infection, approaching the national goal of 90% by 2020 (2). Persons aware of their HIV infection reduce their transmission risk behaviors and can enter HIV care and take antiretroviral treatment to achieve viral suppression (a viral load result of <200 copies/mL, or undetectable levels) (3). Viral suppression not only preserves immune function, decreasing a person’s risk for morbidity and mortality, but also profoundly reduces risk for sexual transmission to others (4–6). One of the first high-profile cases of AIDS was the American Rock Hudson, a gay actor who had married and divorced earlier in life, who died on October 2, 1985 having announced that he was suffering from the virus on July 25 that year. He had been diagnosed during 1984.[264] A notable British casualty of AIDS that year was Nicholas Eden, a gay politician and son of the late prime minister Anthony Eden.[265] On November 24, 1991, the virus claimed the life of British rock star Freddie Mercury, lead singer of the band Queen, who died from an AIDS-related illness having only revealed the diagnosis on the previous day.[266] However, he had been diagnosed as HIV positive in 1987.[267] One of the first high-profile heterosexual cases of the virus was Arthur Ashe, the American tennis player. He was diagnosed as HIV positive on August 31, 1988, having contracted the virus from blood transfusions during heart surgery earlier in the 1980s. Further tests within 24 hours of the initial diagnosis revealed that Ashe had AIDS, but he did not tell the public about his diagnosis until April 1992.[268] He died as a result on February 6, 1993 at age 49.[269]

Virions have a plasma half-life of about 6 h. In moderate to heavy HIV infection, about 108 to 109 virions are created and removed daily. The high volume of HIV replication and high frequency of transcription errors by HIV reverse transcriptase result in many mutations, increasing the chance of producing strains resistant to host immunity and drugs.

AIDS is an advanced stage of HIV infection. Because the CD4 cells in the immune system have been largely destroyed, people with AIDS often develop symptoms and signs of unusual infections or cancers. When a person with HIV infection gets one of these infections or cancers, it is referred to as an “AIDS-defining condition.” Examples of AIDS-defining conditions are listed in Table 1. Significant, unexplained weight loss also is an AIDS-defining condition. Because common conditions like cancer or other viral conditions like infectious mononucleosis also can cause weight loss and fatigue, it is sometimes easy for a physician to overlook the possibility of HIV/AIDS. It is possible for people without AIDS to get some of these conditions, especially the more common infections like tuberculosis.

If an exposure occurs, the exposed person can reduce the risk of getting HIV by taking antiretroviral medications. Current recommendations suggest two or more antiretroviral medications, depending on the risk of transmission and type of exposure. Medications should be started as soon as possible, preferably within hours of exposure and should be continued for four weeks, if tolerated. People who have been exposed should be tested for HIV at the time of the injury and again at six weeks, 12 weeks, and six months after exposure.

Although malaria is not typically considered an opportunistic infection, its incidence was found to be significantly higher among children in Tanzania that were perinatally infected with HIV than those without HIV infection. [69] This was true for physician-diagnosed clinical malaria, probable malaria involving laboratory testing for parasitemia as well as malaria that was confirmed by blood smear.

HIV invariably develops resistance to any of these drugs if they are used alone. Resistance develops after a few days to several months of use, depending on the drug and the virus. HIV becomes resistant to drugs because of mutations that occur when it replicates.

But these measures have not extended to most black gay and bisexual men. A C.D.C. report in February noted that only 48 percent of black gay and bisexual men effectively suppress the virus with consistent medication, and the numbers are even lower for these men in their late teens and 20s. In 2014, nearly one in five black gay men who had received a diagnosis of H.I.V. had progressed to AIDS by the time they learned of their infection — which meant that they were generally very ill by the time they began treatment. Only a small percentage of black people use PrEP to prevent contracting the virus, accounting for only 10 percent of prescriptions; the vast majority of users are white. Many black gay and bisexual men either can’t afford PrEP or don’t know about it — they may not see a doctor regularly at all, and many medical providers haven’t even heard of PrEP.

Interruption of ART is usually safe if all drugs are stopped simultaneously, but levels of slowly metabolized drugs (eg, nevirapine) may remain high and thus increase the risk of resistance. Interruption may be necessary if intervening illnesses require treatment or if drug toxicity is intolerable or needs to be evaluated. After interruption to determine which drug is responsible for toxicity, clinicians can safely restart most drugs as monotherapy for up to a few days. Note: The most important exception is abacavir; patients who had fever or rash during previous exposure to abacavir may develop severe, potentially fatal hypersensitivity reactions with reexposure. Risk of an adverse reaction to abacavir is 100-fold higher in patients with HLA-B*57:01, which can be detected by genetic testing.

In making decisions about patient care, health care professionals who are infected with HIV should adhere to the fundamental professional obligation to avoid harm to patients. Physicians who have reason to believe that they have been at significant risk of being infected should be tested voluntarily for HIV for the protection of their patients as well as for their own benefit. The physician as a patient is entitled to the same rights to privacy and confidentiality as any other patient.

Cure of HIV infection has not been thought possible, and thus lifelong drug treatment is considered necessary. Patients living with HIV infection should be urged to take their antiretroviral drugs consistently. An instance of a possible cure was widely reported in an infant with transient eradication of replication-competent HIV after about 15 mo of antiretroviral therapy. However, HIV replication subsequently resumed. In a large international clinical trial, risk of opportunistic infection or death from any cause, particularly from premature coronary artery disease, cerebrovascular events, or liver and kidney disorders, was significantly higher when antiretroviral therapy was taken episodically (guided by the CD4 count) than when it was taken continuously (1).

Antiviral treatment options have primarily included combinations of two NRTIs, often referred to as “nucs,” and a third drug, typically being a boosted PI, a NNRTI, often called “non-nucs, and InSTIs such as RAL, or dolutegravir (Tivicay, DTG). Many of these drugs are available in fixed-dose combinations as well as increasing numbers of drugs as single-tablet regimens.

HIV/Aids is caused by the Human Immunodeficiency Virus (HIV). HIV is mainly transmitted through sexual intercourse, but can also be passed down from mother to child, acquired via blood transfusion with infected blood, or other methods. Once a person is infected, the virus remains in the body for life. There is no cure for HIV/Aids, but there are drugs that help control the virus, enabling people with symptoms of HIV to live full and healthy lives. There are also various methods to help prevent the spread of the disease.

Early diagnosis of HIV infection is important because it enables doctors to identify people with HIV infection before their CD4 cell count decreases too much. The sooner people start taking antiretroviral drugs, the more quickly their CD4 count is likely to increase and the higher the count is likely to become.

The initial infection with HIV generally occurs after transfer of body fluids from an infected person to an uninfected one. The virus is carried in infected CD4 T cells, dendritic cells, and macrophages, and as a free virus in blood, semen, vaginal fluid, or milk. It is most commonly spread by sexual intercourse, contaminated needles used for intravenous drug delivery, and the therapeutic use of infected blood or blood products, although this last route of transmission has largely been eliminated in the developed world where blood products are screened routinely for the presence of HIV. An important route of virus transmission is from an infected mother to her baby at birth or through breast milk. In Africa, the perinatal transmission rate is approximately 25%, but this can largely be prevented by treating infected pregnant women with the drug zidovudine (AZT) (see Section 11-23). Mothers who are newly infected and breastfeed their infants transmit HIV 40% of the time, showing that HIV can also be transmitted in breast milk, but this is less common after the mother produces antibodies to HIV. If infected people are not treated, AIDS develops in most of them. How quickly the number of CD4 cells decreases and HIV infection progresses toward AIDS varies greatly from person to person. Generally, experts estimate that people develop AIDS at the following rates:

More than one million people in the United States are living with HIV. It’s different for everybody, but many enjoy a good quality of life and can expect a longer lifespan than those diagnosed before today’s treatments were available.

In May 2007, the WHO and UNAIDS issued new guidance recommending “provider-initiated” HIV testing in healthcare settings. This aimed to widen knowledge of HIV status and greatly increase access to HIV treatment and prevention.83

Developing AIDS requires that the person acquire HIV infection. Risks for acquiring HIV infection include behaviors that result in contact with infected blood or sexual secretions, which pose the main risk of HIV transmission. These behaviors include sexual intercourse and injection drug use. The presence of sores in the genital area, like those caused by herpes, makes it easier for the virus to pass from person to person during intercourse. HIV also has been spread to health care workers through accidental sticks with needles contaminated with blood from HIV-infected people, or when broken skin has come into contact with infected blood or secretions. Blood products used for transfusions or injections also may spread infection, although this has become extremely rare (less than one in 2 million transfusions in the U.S.) due to testing of blood donors and blood supplies for HIV. Finally, infants may acquire HIV from an infected mother either while they are in the womb, during birth, or by breastfeeding after birth.

A small but vocal minority of people, including some scientists, continue to argue that HIV does not exist, or does not cause AIDS, and that the HIV tests are unreliable or that the therapies are toxic. Such misinformation is usually based on a lack of understanding of the scientific literature, deliberate misrepresentation, or logical fallacies based on pseudoscientific arguments.

In the United States, the face of the HIV/AIDS epidemic has changed dramatically. Adolescents and young adults less than 25 years of age now account for half the new HIV infections reported annually to the CDC and for most perinatally acquired infections. As a result, strategies to prevent new infection and manage the long-term effects of past infection have focused increasingly on the second and third decades of life.

As of early 2009, there was no cure for AIDS and no vaccine to prevent infection. Treatment stresses aggressive combination drug therapy for those patients with access to the expensive medications and who tolerate them adequately. Some enveloped RNA viruses can be produced in infected cells that continue growing and dividing without being killed. This probably involves some sort of intracellular regulation of viral growth. It is also possible for the DNA of some viruses to be incorporated into the host cell DNA, producing a carrier state. These are almost always retroviruses, which are called proviruses before and after integration of viral DNA into the host genome.

Therapy is initiated and individualized under the supervision of a physician who is an expert in the care of HIV-infected patients. A combination of at least three ART drugs is needed to suppress the virus from replicating and boost the immune system. How these drugs are combined depends on the most current treatment guidelines, individual patient preferences, other medical conditions, past treatment history, and any resistance mutations in the individual’s virus. Resistance mutations may already be present at the time of infection, thus most clinicians will test the patient’s virus for resistance mutations prior to starting or changing a regimen.

Contributing to the increased cross-prevalence were persons with hemophilia who had been infected with HIV from contaminated factor VIII concentrate and persons who used intravenous drugs, an activity that transcends all sexual preferences. For nearly two decades, the United States has focused money and attention on the H.I.V./AIDS epidemic elsewhere. Barbara Lee, the longtime United States representative from Northern California, has signed her name as a sponsor to every piece of major federal H.I.V./AIDS legislation since she was first elected in 1998. Plasma HIV virion levels, expressed as number of HIV RNA copies/mL, stabilize after about 6 mo at a level (set point) that varies widely among patients but averages 30,000 to 100,000/mL (4.2 to 5 log10/mL). The higher this set point, the more quickly the CD4 count decreases to a level that seriously impairs immunity (< 200/μL) and results in the opportunistic infections and cancers that define AIDS. The human immunodeficiency virus (HIV) which causes acquired immune deficiency syndrome (AIDS) has brought about a global epidemic of massive proportions. Rash and liver inflammation can occur with both EFV and DLV, and these drugs may also be linked to abnormalities of lipids in the blood.

The crisis is most acute in Southern states, which hold 37 percent of the country’s population and as of 2014 accounted for 54 percent of all new H.I.V. diagnoses. The South is also home to 21 of the 25 metropolitan areas with the highest H.I.V. prevalence among gay and bisexual men. Jackson, capital of Mississippi, the country’s poorest state, is best known for blues, barbecue and “The Help.” It also has the nation’s highest rate — 40 percent — of gay and bisexual men living with H.I.V., followed by Columbia, S.C.; El Paso; Augusta, Ga.; and Baton Rouge, La. In Jackson, a small city of just over 170,000, half a dozen black gay or bisexual men receive the shock of a diagnosis every month, and more than 3,600 people, the majority of them black men, live with the virus.

As the men settled into their seats, Sturdevant asked them to go around and “check in.” Jermerious Buckley, watchful behind black rectangular glasses, with no sign of the makeup and colorful pumps he wore on weekends at Metro, told the group, “I’m doing a whole lot better.” Last year, he said, “Daddy,” as he called Sturdevant, had pulled him back from the dead, after he had shrunk to 85 pounds, his arms covered with Kaposi’s sarcoma lesions, his kidneys failing. He felt like a “zombie,” he said, too weak and hopeless to bother with his meds. Now Buckley thought he was finally strong enough to get back onto the pageant circuit where he competed. From his phone, he pulled up a picture of himself as “Akeelah,” unrecognizable in a shimmery white body-hugging gown and towering wig. “November in New Orleans — y’all wish me luck,” he said.

Medications that fight HIV are called antiretroviral medications. Different antiretroviral medications target the virus in different ways. When used in combination with each other, they are very effective at suppressing the virus. It is important to note that there is no cure for HIV. ART only suppresses reproduction of the virus and stops or delays the disease from progressing to AIDS. Most guidelines currently recommend that all HIV-infected people who are willing to take medications should have them initiated shortly after being diagnosed with the infection. This delays or prevents disease progression, improves overall health of an infected person, and makes it less likely that they will transmit the virus to their partners.

These organs make and release lymphocytes. These are white blood cells classified as B cells and T cells. B and T cells fight invaders called antigens. B cells release antibodies specific to the disease your body detects. T cells destroy foreign or abnormal cells.

Scientists identified a type of chimpanzee in Central Africa as the source of HIV infection in humans. They believe that the chimpanzee version of the immunodeficiency virus (called simian immunodeficiency virus, or SIV) most likely was transmitted to humans and mutated into HIV when humans hunted these chimpanzees for meat and came into contact with their infected blood. Studies show that HIV may have jumped from apes to humans as far back as the late 1800s. Over decades, the virus slowly spread across Africa and later into other parts of the world. We know that the virus has existed in the United States since at least the mid to late 1970s. HIV Encephalopathy is a severe condition usually seen in end-stage disease. Milder cognitive impairments may exist with less advanced disease. For example, one study found significant deficits in cognition, planning, coordination and reaction times in HIV-infected compared to uninfected children, effects that were more pronounced in those with higher viral loads. [71]

Other drugs can prevent or treat opportunistic infections (OIs). ART has also reduced the rate of most OIs. In most cases, these drugs work very well. The newer, stronger ARVs have helped reduce the rates of most OIs.

For this strategy to work, however, one must be able to test people at risk of infection with HIV periodically, so that they can take the steps necessary to avoid passing the virus to others. This, in turn, requires strict confidentiality and mutual trust. A barrier to the control of HIV is the reluctance of individuals to find out whether they are infected, especially as one of the consequences of a positive HIV test is stigmatization by society. As a result, infected individuals can unwittingly infect many others. Balanced against this is the success of therapy with combinations of the new protease inhibitors and reverse transcriptase inhibitors, which provides an incentive for potentially infected people to identify the presence of infection and gain the benefits of treatment. Responsibility is at the heart of AIDS prevention, and a law guaranteeing the rights of people infected with HIV might go a long way to encouraging responsible behavior. The rights of HIV-infected people are protected in the Netherlands and Sweden. The problem in the less-developed nations, where elementary health precautions are extremely difficult to establish, is more profound.

In June, the first reports of AIDS in children hinted that it could be passed via casual contact but this was later ruled out and it was concluded that they had probably directly acquired AIDS from their mothers before, during or shortly after birth.17

Preexposure prophylaxis with antiretrovirals (PrEP): In PrEP, people who are not infected with HIV but are at high risk (eg, by having an HIV-infected sexual partner) take an antiretroviral drug daily to reduce their risk of infection. The combination of tenofovir disoproxil fumarate plus emtricitabine (TDF/FTC) can be used. Use of PrEP does not eliminate the need to use other methods of reducing risk of HIV infection, including using condoms and avoiding high-risk behaviors (eg, needle sharing). Data concerning infants of HIV-negative mothers taking TDF/FTC PrEP during pregnancy are incomplete, but currently, no adverse effects have been reported in children born to HIV-infected women treated with TDF/FTC. Use of PrEP to reduce the risk of HIV infection in injection drug users is being studied. For the current CDC recommendations, see Pre-Exposure Prophylaxis (PrEP).

A safe and effective vaccine for the prevention of HIV infection and AIDS is an attractive goal, but its achievement is fraught with difficulties that have not been faced in developing vaccines against other diseases. The first problem is the nature of the infection itself, featuring a virus that proliferates extremely rapidly and causes sustained infection in the face of strong cytotoxic T-cell and antibody responses. As we discussed in Section 11-25, HIV evolves in individual patients by the selective proliferative advantage of mutant virions encoding peptide sequence changes that escape recognition by antibodies and by cytotoxic T lymphocytes. This evolution means that the development of therapeutic vaccination strategies to block the development of AIDS in HIV-infected patients will be extremely difficult. Even after the viremia has been largely cleared by drug therapy, immune responses to HIV fail to prevent drug-resistant virus from rebounding and replicating at pretreatment levels.

On 7 November, professional basketball player Earvin (Magic) Johnson announced he had HIV and retired from the sport, planning to educate young people about the virus. This announcement helped begin to dispel the stereotype, still widely held in the US and elsewhere, of HIV as a ‘gay’ disease.52

Cross-sectional data reported in this analysis are from MSM, persons who inject drugs, and heterosexual persons at increased risk for HIV infection recruited for face-to-face interviews and HIV testing through venue-based sampling (MSM) and respondent-driven sampling (persons who inject drugs and heterosexual persons) in NHBS surveys from 2008 to 2016. NHBS sampling procedures have been previously described (10). Persons were eligible to participate if they resided in a participating city, could complete the survey in English or Spanish, and met cycle-specific inclusion criteria (MSM: born male, aged ≥18 years, identified as male, and had oral or anal sex with another man; persons who inject drugs: aged ≥18 years, injected drugs in the past 12 months; and heterosexual persons: male or female [not transgender], aged 18–60 years, had sex with a member of the opposite sex in the past 12 months, never injected drugs, and met low income or low education criteria).§ For inclusion in current analyses, participants must have tested negative during the NHBS cycle, MSM must have had sex with another man in the past 12 months, and persons who inject drugs must have been male or female (not transgender). Data were analyzed by sex, age, and race/ethnicity (American Indian or Alaska Native; Asian; black or African American [blacks]; Hispanic or Latino; Native Hawaiian or Other Pacific Islander; white; and multiple race).

Some viruses have only a few genes coding for capsid proteins. Other more complex ones may have a few hundred genes. But no virus has the thousands of genes required by even the simplest cells. Although in general viruses “steal” their lipid envelope from the host cell, virtually all of them produce “envelope proteins” that penetrate the envelope and serve as receptors. Some envelope proteins facilitate viral entry into the cell, and others have directly pathogenic effects.

AIDS is caused by a virus called HIV, the Human Immunodeficiency Virus. If you get infected with HIV, your body will try to fight the infection. It will make “antibodies,” special molecules to fight HIV.

Gordon’s longevity, and the dozens of drugs he has taken to stay alive, exemplifies the experience of millions of infected AIDS patients. His state-of-the-art treatment costs almost a hundred thousand dollars a year. Although it’s covered by his insurance and by the State of California, he calls it “a ransom: your money or your life.” For Deeks, the question is “Can the world find the resources to build a system to deliver, on a daily basis, antiretroviral drugs to some thirty-five million people, many in very poor regions?” He is doubtful, which is why he is focussed on helping to find a cure. “Our philosophy is that in order to cure H.I.V., we need to know where and why it persists,” he said.

In 2009 a new strain of HIV-1 was discovered in a woman from Cameroon. The virus was closely related to a strain of SIV found in wild gorillas. Researchers placed the new virus into its own group, HIV-1 group P, because it was unique from all other types of HIV-1. It was unclear whether the newly identified virus causes disease in humans.

HIV-positive women who might become pregnant should talk to their provider about the risk to their unborn child. They should also discuss methods to prevent their baby from becoming infected, such as taking antiretroviral medicines during pregnancy.

Epidemics have no single answer beyond a cure. Since no cure for AIDS existed as of the early 2000s, the law continued to grapple with a vast number of problems. The federal government has addressed AIDS in two broad ways: by spending money on research and treatment of the disease and by prohibiting unfairness to people with HIV or AIDS. It has funded medical treatment, research, and public education, and it has passed laws prohibiting discrimination against people who are HIV-positive or who have developed AIDS. States and local municipalities have joined in these efforts, sometimes with federal help. In addition, states have criminalized the act of knowingly transmitting the virus through sexual behavior or blood donation. The courts, of course, are the decision makers in AIDS law. They have heard a number of cases in areas that range from employment to education and from crimes to torts. Although a body of case law has developed, it remains relatively new with respect to most issues and controversial in all.

When HIV infection destroys CD4+ lymphocytes, it weakens the body’s immune system, which protects against many infections and cancers. This weakening is part of the reason that the body is unable to eliminate HIV infection once it has started. However, the immune system is able to mount some response. Within a month or two after infection, the body produces lymphocytes and antibodies that help lower the amount of HIV in the blood and keep the infection under control. For this reason, untreated HIV infection may cause no symptoms or only a few mild symptoms for an average of about 10 years (ranging from 2 to more than 15 years).

Major advancements in HIV prevention, treatment, and care have put an AIDS-free generation squarely within reach. HIV tests are faster and more reliable than ever before. HIV medications are safer and more effective, and there are now several ways to prevent the spread of HIV, including condoms and Pre-Exposure Prophylaxis (PrEP). PrEP is an HIV prevention strategy that currently involves taking a once daily-pill called Truvada ®.
