On Wednesday evenings once a month, Sturdevant runs an H.I.V./AIDS support group in a stark conference room near the State Capitol in Jackson. The meetings end promptly at 7:30 p.m., so the dozen or so young men can race home to watch “Empire.” Sturdevant began October’s gathering with a prayer. “Hold hands and bow your heads — and take off that hat,” he said to Tommy Brown, who had rushed in from his job at Popeyes. The willowy young man snatched off his baseball cap, embroidered with the fast-food chain’s red-and-orange logo, and lowered his head. “Gracious God, we want to thank you once again for the unity that we have here, Lord,” Sturdevant intoned in his gravelly baritone. “Thank you for showing us how to love each other and love ourselves. We ask that you bring more people in that need somebody to talk to. That need the laughter. That need the understanding.”

In addition to diagnostic blood tests, other blood tests are used to track the course of AIDS in patients that have already been diagnosed. These include blood counts, viral load tests, p24 antigen assays, and measurements of 2-microglobulin (2M).

Reported AIDS cases may be separated into groups based on these risk factors: homosexual or bisexual males–75%, intravenous drug abusers with no history of male homosexual activity–13%, Haitians with neither a history of homosexuality nor a history of intravenous drug abuse–6%, persons with hemophilia A who were not Haitians, homosexuals, or intravenous drug abusers–0.3%, and persons in none of the other groups–5%. Most PIs are associated with important drug-drug interactions so they must be used with caution in patients on other medications. There are numerous resources available to patients on these medications to make sure that they do not adversely interact with other HIV or non HIV-related drugs.

Brown is known as the Berlin patient, after the city where he became the only person ever to have been cured of H.I.V. In 2006, more than a decade after he discovered he was H.I.V.-positive, he was given an unrelated diagnosis of acute myelogenous leukemia, a cancer of the bone marrow. After initial treatment, the leukemia returned. Brown needed a bone-marrow transplant. His hematologist, Gero Huetter, made the imaginative suggestion that they use a donor with a genetic mutation that shuts down the protein CCR5, a doorway for H.I.V. into helper T cells. On February 7, 2007, Brown received the transplant. One year later, he underwent the procedure again, and by 2009 biopsies of Brown’s brain, lymph nodes, and bowel showed that the virus had not returned, and his T-cell count was back to normal.

Approximately 20% of new diagnoses are in women. In the United States, heterosexual transmission accounts for approximately one-quarter of new diagnoses, with intravenous drug use contributing to the remaining cases in the U.S.

AIDS is usually marked by a very low number of CD4+ lymphocytes, followed by a rise in the frequency of opportunistic infections and cancers. Doctors monitor the number and proportion of CD4+ lymphocytes in the patient’s blood in order to assess the progression of the disease and the effectiveness of different medications. About 10% of infected individuals never progress to this overt stage of the disease.

After the virus enters the body there is a period of rapid viral replication, leading to an abundance of virus in the peripheral blood. During primary infection, the level of HIV may reach several million virus particles per milliliter of blood.[95] This response is accompanied by a marked drop in the number of circulating CD4+ T cells. The acute viremia is almost invariably associated with activation of CD8+ T cells, which kill HIV-infected cells, and subsequently with antibody production, or seroconversion. The CD8+ T cell response is thought to be important in controlling virus levels, which peak and then decline, as the CD4+ T cell counts recover. A good CD8+ T cell response has been linked to slower disease progression and a better prognosis, though it does not eliminate the virus.[96]

HIV continues to be a major public health crisis both in the United States and around the world. While major scientific advances have made it easier than ever to prevent and treat HIV, there remains no vaccine or cure, and tens of thousands of people continue to contract HIV every year. Insufficient funding for public health programs, ideological opposition to common sense prevention policies, and societal barriers like stigma and discrimination, have made it especially difficult for us to turn the tide against the epidemic. South & South East Asia is the second most affected; in 2010 this region contained an estimated 4 million cases or 12% of all people living with HIV resulting in approximately 250,000 deaths.[210] Approximately 2.4 million of these cases are in India.[209]

Abstinence-only adherents think being less frank is being more responsible. They view sexuality as a moral issue properly left for parents to discuss with their children and one that lies beyond the responsibilities of schools. The conservative columnist Cal Thomas spoke for this viewpoint when he argued that parents “have lost a significant right to rear their children according to their own moral standards.” Other objections come from religious conservatives who oppose any neutral or positive discussion of homosexuality. Koop, for example, was blasted for allegedly “sponsoring homosexually oriented curricula” and “teaching buggery in the 3rd grade.” In addition to voicing moral objections, critics say comprehensive sex ed. is generally a failure because it encourages a false sense of security among teens that leads to experimentation with sex or drugs. “We have given children more information presumably because we think it will change their behavior, and yet the behavior has gotten worse, not better,” said Gary Bauer, president of the Family Research Council.

HIV infection is commonly diagnosed by blood tests. Testing for HIV is usually a two-step process. First, a screening test is done. If that test is positive, a second test (Western blot) is done to confirm the result.

In June, first reports of AIDS in children hinted that it could be passed via casual contact but this was later ruled out and it was concluded that they had probably directly acquired AIDS from their mothers before, during or shortly after birth.17

With effort, Jordon sat up slightly, untangling himself from a jumble of sheets. Sturdevant asked how he was doing, and he cataloged a laundry list of what he called his “old man” ailments. “I’ve had everything — diarrhea, hemorrhoids, now this neuropathy,” he said. “My body hates me.” Once a month, his mother or grandmother drove him to medical appointments in Jackson, to receive care from providers experienced in treating people living with H.I.V. and to avoid the small-town gaze at the local facilities; there is no Gay Men’s Health Crisis for him to visit in his small town, as there would be if he lived in New York. “Everybody knows everybody here,” Jordon said. “At the hospital, they know my mom and my brother and my grandmother. I would rather be around people who don’t know me.” Too ashamed to admit that he had the virus, Jordon had told few friends about his diagnosis.

Detection of antibodies to HIV is sensitive and specific except during the first few weeks after infection. Currently, a 4th-generation combination immunoassay is recommended; it detects antibodies to both HIV-1 and HIV-2 as well as the p24 HIV antigen (p24 is a core protein of the virus). The laboratory version is probably preferred over the point-of-care one for diagnosing early infection, but both can be done quickly (within 30 min). If the test result is positive, an assay to differentiate HIV-1 and HIV-2 and an HIV RNA assay are done.

If a woman is untreated, two years of breastfeeding results in an HIV/AIDS risk in her baby of about 17%. Treatment decreases this risk to 1 to 2% per year. Due to the increased risk of death without breastfeeding in many areas in the developing world, the World Health Organization recommends either: (1) the mother and baby being treated with antiretroviral medication while breastfeeding being continued (2) the provision of safe formula.[80] Infection with HIV during pregnancy is also associated with miscarriage.[81]

Ideally, prior to initiating treatment, the viral load and the CD4 cell count should be checked and the viral load test then repeated after approximately four weeks of treatment. If the patient is beginning a regimen that includes two to three drugs for which the patient’s virus does not appear to be resistant, it is expected that the amount of virus should decrease by at least a hundredfold during this interval. The ultimate goal is for the viral load to decrease to undetectable levels which should occur by approximately 12-24 weeks. There are some individuals that despite taking all of their medications correctly will suppress their viral load to less than 200 copies/mL but not consistently undetectable levels. It is not completely known how to optimally manage this situation but many experts would continue to monitor on current therapy as long as viral load remains below 200 copies/mL. Those who are not having an appropriate response to therapy need to be questioned to make sure that they are taking their medications correctly, and if not, why. If the viral load is not going to undetectable levels and the patient is taking the medications correctly, then it is likely that there is a resistant virus to some of the medications. Drug-resistance testing then should be performed and the patient managed as described in the next section. Once the patient’s viral load is suppressed, they can often have viral load and CD4 cell counts performed less frequently (for example, every three to four months and in select cases every six months or possibly even less).

Needle sticks or body fluid splashes among health care professionals. Transmission through theses sources accounts for fewer than 0.3% of all HIV infections in the United States. This rate reflects the emphasis on universal safety precautions (e.g., use of gloves, face shields, proper disposal of needles) among health care professionals and first responders.

¶ Data include all participants with complete, valid survey data from 17 cities who reported male or female gender, who ever had sex with a member of the opposite sex, never injected drugs, and who had negative HIV test results.

Medical male circumcision, reduces the risk of heterosexually acquired HIV infection in men by approximately 60%. This is a key prevention intervention supported in 15 countries in Eastern and Southern Africa (ESA) with high HIV prevalence and low male circumcision rates. VMMC is also regarded as a good approach to reach men and adolescent boys who do not often seek health care services. Since the 2007 WHO recommendation for VMMC as an additional prevention strategy, nearly 15 million adolescent boys and men in ESA were provided a package of services including HIV testing and education on safer sex and condom use.

HIV is a sexually transmitted infection (STI). It can also be spread by contact with infected blood or from mother to child during pregnancy, childbirth or breast-feeding. Without medication, it may take years before HIV weakens your immune system to the point that you have AIDS.

There may be some value in providing prophylactic treatment. A Cochrane review found some benefit in treating latent tuberculosis.[17]Another review found only one trial that examined the benefit of prophylactic co-trimoxazole in children. It was from Zambia and the result was positive.[18]Prophylactic co-trimoxazole was subsequently endorsed as official WHO policy for exposed infants. However, this guidance has been the subject of controversy and its benefits have been questioned by several subsequent trials.[19]The value of prophylaxis against oropharyngeal candidiasis is uncertain, especially in children. There may be some benefit but at a risk of resistance developing and for poorer countries the cheaper options should be examined.[20]

The US blood supply is among the safest in the world. Nearly all people infected with HIV through blood transfusions received those transfusions before 1985, the year HIV testing began for all donated blood.

Another, less well-understood prognostic factor is the level of immune activation as determined by evaluating the expression of activation markers on CD4 and CD8 lymphocytes. Activation, which may be caused by leakage of bacteria across the HIV-damaged colonic mucosa, is a strong prognostic predictor but is not used clinically because this test is not widely available and antiretroviral therapy changes the prognosis, making this test less important.

In January 1995, the settlement in a lawsuit brought by a Philadelphia construction worker with AIDS illustrated that the ADA could be used to fight caps on coverage. In 1992, the joint union-management fund for the Laborers’ District Council placed a $10,000 limit on AIDS benefits, in stark contrast to the $100,000 allowed for other catastrophic illnesses. At that time, the fund said the cap on AIDS benefits was designed to curb all health costs. In 1993, the EEOC ruled that the fund violated the ADA, and, backed by the AIDS Law Project of Philadelphia, the worker sued. Rather than fight an expensive lawsuit, the insurance fund settled: under the agreement, it extended coverage for all catastrophic illnesses to $100,000. Hailing the settlement as a major blow against widespread discrimination in insurance coverage, the law project’s executive director, Nan Feyler, told the Philadelphia Inquirer, “You can’t single out someone based on a stereotype.”

HIV may be the human version of simian immunodeficiency virus (SIV), known to infect African chimpanzees. It may have crossed over and mutated in humans who ate infected chimpanzee meat as long ago as the late 1800s.

One of the proteins that enters the cell with the viral genome is the viral reverse transcriptase, which transcribes the viral RNA into a complementary DNA (cDNA) copy. The viral cDNA is then integrated into the host cell genome by the viral integrase, which also enters the cell with the viral RNA. The integrated cDNA copy is known as the provirus. The cycle up to the integration of the provirus is shown in Fig. 11.23. In activated CD4 T cells, virus replication is initiated by transcription of the provirus, as we will see in the next section. However, HIV can, like other retroviruses, establish a latent infection in which the provirus remains quiescent. This seems to occur in memory CD4 T cells and in dormant macrophages, and these cells are thought to be an important reservoir of infection.

Cellular immune response to HIV. The cellular immune response is induced upon the entry of HIV into the target cells (e.g., T cells) and synthesis of viral proteins (Figure 1). MHC class I on the cell surface displays the intracellularly degraded HIV peptide fragments for recognition by T-cell receptors (TCR) on CD8+ T cells (Figure 3). CD8+ T cells lyse HIV infected cells and secrete cytokines, i.e. interferon-γ (IFN-γ), tumor necrosis factor α (TNF-α), and chemokines, i.e. MIP-1 α, MIP β and RANTES, that inhibit virus replication and block  viral entry into CD4+ T cells. Development of CD8+ T cells is crucial for control of HIV replication. This results in declining viraemia after primary infection. In the early stages of infection, CD4+ T cells lose their proliferative capacity and therefore their contribution to viral control is minor. However, during chronic infection CD4+T cells are present and secrete interleukin-2 (IL-2) or cytokines, such as IFN-γ, to control viraemia.

In July 2015, UNAIDS announced that the Millennium Development Goal (MDG) relating to HIV and AIDS had been reached six months ahead of schedule. The target of MDG 6 – halting and reversing the spread of HIV – saw 15 million people receive treatment.95

The CDC and the College recommend that females aged 13–64 years be tested at least once in their lifetime and annually thereafter based on factors related to risk. Obstetrician–gynecologists should annually review patients’ risk factors for HIV and assess the need for retesting. Repeat HIV testing should be offered at least annually to women who

HIV can infect dendritic cells (DCs) by this CD4-CCR5 route, but another route using mannose-specific C-type lectin receptors such as DC-SIGN can also be used.[58] DCs are one of the first cells encountered by the virus during sexual transmission. They are currently thought to play an important role by transmitting HIV to T-cells when the virus is captured in the mucosa by DCs.[58] The presence of FEZ-1, which occurs naturally in neurons, is believed to prevent the infection of cells by HIV.[59]

The ward occupies the sixth floor of an Art Deco building on the north side of campus. I found Deeks in his office, wearing a flannel shirt and New Balance sneakers. He explained his concerns about the drug cocktail. “Antiretroviral drugs are designed to block H.I.V. replication, and they do that quite well,” he said. But they don’t enable many patients to recover fully. The immune system improves enough to prevent AIDS, but, because the virus persists, the immune system must mount a continuous low-level response. That creates chronic inflammation, which injures tissues.

Taking the drugs as directed for a life time is demanding. Some people skip doses or stop taking the drugs for a time (called a drug holiday). These practices are dangerous because they enable HIV to develop resistance to the drugs. Because taking HIV drugs irregularly often leads to drug resistance, health care practitioners try to make sure that people are both willing and able to adhere to the treatment regimen. To simplify the drug schedule and to help people take the drugs as directed, doctors often prescribe treatment that combines two or more drugs in one tablet that can be taken only once a day.

Plasma HIV virion levels, expressed as number of HIV RNA copies/mL, stabilize after about 6 mo at a level (set point) that varies widely among patients but averages 30,000 to 100,000/mL (4.2 to 5 log10/mL). The higher this set point, the more quickly the CD4 count decreases to a level that seriously impairs immunity (< 200/μL) and results in the opportunistic infections and cancers that define AIDS. In May of 1986, after much dispute about credit for the discovery (the French finally won the Nobel, in 2008), an international committee of scientists agreed on the name H.I.V., or human immunodeficiency virus. By the end of that year, about twenty-five thousand of the nearly twenty-nine thousand Americans with reported AIDS diagnoses had died. Supporters of a comprehensive approach say AIDS demands frankness. Originating in comprehensive sex ed. theory, their ideas also came from pacesetting health authorities such as former surgeon general c. everett koop. Arguing in the mid-1980s that AIDS classes should be specific and detailed and taught as early as kindergarten, Koop countered conservative arguments by saying, "Those who say 'I don't want my child sexually educated' are hiding their heads in the sand." This position holds that educators are obligated to teach kids everything that can stop the spread of the disease. "What is the moral responsibility?" Jerald Newberry, a health coordinator of Virginia schools, asked the Washington Times in 1992. "I think it's gigantic." Abstinence is a part of this approach, but expecting teens to refrain from having sex was considered by many to be unrealistic given some studies that show that nearly three out of four high school students have had sex before graduation. Thus, the comprehensive curriculum might well include explaining the proper use of condoms, discussing homosexual practices, describing the sterilization of drug needles, and so on. Shortly after the viral capsid enters the cell, an enzyme called reverse transcriptase liberates the positive-sense single-stranded RNA genome from the attached viral proteins and copies it into a complementary DNA (cDNA) molecule.[65] The process of reverse transcription is extremely error-prone, and the resulting mutations may cause drug resistance or allow the virus to evade the body’s immune system. The reverse transcriptase also has ribonuclease activity that degrades the viral RNA during the synthesis of cDNA, as well as DNA-dependent DNA polymerase activity that creates a sense DNA from the antisense cDNA.[66] Together, the cDNA and its complement form a double-stranded viral DNA that is then transported into the cell nucleus. Trends continue toward simplifying drug regimens to improve adherence and decrease side effects. In addition, the availability of multiple new drugs in new classes has made it possible to suppress viral load to undetectable levels even in many of the most treatment-experienced patients. Moreover, many are virologically suppressed taking a single well-tolerated pill per day. As noted in the section on new therapies in development, another major advance could emerge with the availability of every one to two month injections of long-acting therapies. With great success in treatment, the field has increasingly considered strategies that may someday allow patients to control viral replication without the use of antiretrovirals. This could be in the form of a true cure with complete eradication of HIV from the body or a functional cure where the virus persists but is unable to replicated, a situation analogous to what happens when patients are on effective antiretroviral therapy. Research is in the very earliest stages with regard to development of strategies for viral eradication. Studies to control viral replication in the absence of antiretroviral therapy are actively being pursued, although thus far with limited success. One strategy has been to use immune-based therapies to boost the natural immune response to HIV and allow for complete or partial control. Another area of research is to purge infected cells, so-called “latent reservoir,” with various agents to facilitate eradication from the body. While research in these areas is under way, it has met with limited success.

In considering disclosure, clinicians may have competing obligations: protecting the patient’s confidentiality, on the one hand, and disclosing test results to prevent substantial harm to a third party, on the other. In some jurisdictions, a breach of confidentiality may be required by mandatory reporting regulations. Even absent legal requirements, in some situations the need to protect potentially exposed third parties may seem compelling. In these situations, the clinician first should educate the patient about her rights and responsibilities and encourage her to inform any third parties involved. If she remains reluctant to voluntarily share information regarding her infection, consultation with an institutional ethics committee, a medical ethics specialist, or an attorney may be helpful in deciding whether to disclose her HIV status. In general, a breach of confidentiality may be ethically justified for purposes of partner notification when all of the following four conditions are met:

HIV is a lifelong infection, but it is treatable and can be controlled with medications. With consistent treatment using highly specialized antiviral medications, a person with HIV may live about as long as an uninfected person.

HIV is a virus spread through certain body fluids that attacks the body’s immune system, specifically the CD4 cells, often called T cells. Over time, HIV can destroy so many of these cells that the body can’t fight off infections and disease. These special cells help the immune system fight off infections. Untreated, HIV reduces the number of CD4 cells (T cells) in the body. This damage to the immune system makes it harder and harder for the body to fight off infections and some other diseases. The only way to know if you have HIV is to take an HIV test. Most tests looks for signs of HIV in your blood. A small sample of blood is taken from your arm. The blood is sent to a lab and tested for HIV. There are other tests available that check for HIV in the urine and oral fluid. The urine test is not very sensitive. There are currently two FDA-approved oral fluid tests. They are OraSure and OraQuick Advance.

HIV attacks and destroys a type of white blood cell called a CD4 cell, commonly called the T-cell. This cell’s main function is to fight disease. When a person’s CD4 cell count gets low, they are more susceptible to illnesses.

Between 2000 and 2016, new HIV infections fell by 39%, and HIV-related deaths fell by one third with 13.1 million lives saved due to ART in the same period. This achievement was the result of great efforts by national HIV programmes supported by civil society and a range of development partners.

Other antiviral agents are in investigational stages and many new drugs are in the pipeline. Growth factors that stimulate cell growth, such as Epogen (erthythropoetin) and G-CSF are sometimes used to treat anemia and low white blood cell counts associated with AIDS.

A deficiency of cellular immunity induced by infection with the human immunodeficiency virus (HIV-1) and characterized by opportunistic diseases, including Pneumocystis jiroveci (formerly carinii) pneumonia, Kaposi sarcoma, oral hairy leukoplakia, cytomegalovirus disease, tuberculosis, Mycobacterium avium complex (MAC) disease, candidal esophagitis, cryptosporidiosis, isoporiasis, cryptococcosis, non-Hodgkin lymphoma, progressive multifocal leukoencephalopathy (PML), herpes zoster, and lymphoma. HIV is transmitted from person to person in cell-rich body fluids (notably blood and semen) through sexual contact, sharing of contaminated needles (as by IV drug abusers), or other contact with infected blood (as in accidental needlesticks among health care workers). Maternal-fetal transmission also occurs. The primary targets of HIV are cells with the CD4 surface protein, including principally helper T lymphocytes. Antibody to HIV, which appears in the serum 6 weeks to 6 months after infection, serves as a reliable diagnostic marker but does not bind or inactivate HIV. Gradual decline in the CD4 lymphocyte count, typically occurring over a period of 10-12 years, culminates in loss of ability to resist opportunistic infections. The appearance of one or more of these infections defines the onset of AIDS. In some patients, generalized lymphadenopathy, fever, weight loss, dementia, or chronic diarrhea occurs much earlier in the course of the infection. Untreated AIDS is uniformly lethal within 2-5 years after the first appearance of an opportunistic infection. Besides prophylaxis against opportunistic infection, standard therapy of HIV infection includes use of nucleoside analogues (for example, didanosine, lamivudine, ribavirin, stavudine, zipovudine), nonnucleoside reverse transcriptase inhibitors (for example, delavirine, efavirenz, nevirapine) and protease inhibitors (for example, atazanavir, crixivan, indinavir, ritonavir, saquinavir).

Political attitudes toward AIDS have gone through dramatically different phases. In the early 1980s, it was dubbed the gay disease and as such was easy for lawmakers to ignore. No one hurried to fund research into a disease that seemed to be killing only members of a historically unpopular group. When it was not being ignored, some groups dismissed AIDS as a problem that homosexuals deserved, perhaps brought on them by divine intervention. Discriminatory action matched this talk as gay men lost jobs, housing, and medical care. AIDS activists complained bitterly about the failure of most U.S. citizens to be concerned. Public opinion only began to shift in the late 1980s, largely through awareness of highly publicized cases. As soon as AIDS had a familiar or more mainstream face, it became harder to ignore; when it became clear that heterosexuals were also contracting the disease, the epidemic acquired higher priority.

Without treatment, human immunodeficiency virus (HIV) infection will usually result in acquired immune deficiency syndrome (AIDS). However, in Australia the HIV therapies introduced in the mid-1990s, which are available to all Australians living with HIV, have resulted in fewer AIDS related illnesses and deaths. Therefore, whilst a cure is yet to be found for HIV and it remains a lifelong infection, HIV in Australia is now considered a chronic manageable condition.

The infection rates in many developed countries remain stable, and some developing countries have achieved significant gains in controlling and even reversing the effects of the HIV epidemic. However, this is partially due to deaths in HIV-infected people, together with simultaneous prevention of new infections. India, for example, has used a national prevention campaign focusing on high-risk populations that may have prevented 100,000 new HIV infections over the 5 years it has been implemented, with increasing results seen in areas with higher levels of investment. HIV is a preventable disease. Effective HIV prevention interventions have been proven to reduce HIV transmission. People who get tested for HIV and learn that they are infected can make significant behavior changes to improve their health and reduce the risk of transmitting HIV to their sex or drug-using partners. Recent scientific advances have demonstrated that early initiation of antiretroviral therapy (ART) not only preserves the health of people living with HIV but also reduces their risk of transmitting HIV to others by 93%.3

You don’t actually “get” AIDS. You might get infected with HIV, and later you might develop AIDS. In addition to sexual behavior, only a few other means of HIV transmission exist. Sharing unsterilized needles used in drug injections is one way, owing to the exchange of blood on the needle, and thus intravenous drug users are an extremely high-risk group. Several cities have experimented with programs that offer free, clean needles. These programs have seen up to a 75 percent reduction in new HIV cases. Receipt of donations of blood, semen, organs, and other human tissue can also transmit HIV, although here, at least, screening methods have proved largely successful. Childbirth and breast feeding are also avenues of transmission, and thus children of HIV-positive mothers may be at risk.

These organs make and release lymphocytes. These are white blood cells classified as B cells and T cells. B and T cells fight invaders called antigens. B cells release antibodies specific to the disease your body detects. T cells destroy foreign or abnormal cells.

The World Health Organization (WHO) has issued recommendations regarding nutrient requirements in HIV/AIDS.[173] A generally healthy diet is promoted. Dietary intake of micronutrients at RDA levels by HIV-infected adults is recommended by the WHO; higher intake of vitamin A, zinc, and iron can produce adverse effects in HIV positive adults, and is not recommended unless there is documented deficiency.[173][174][175][176] Dietary supplementation for people who are infected with HIV and who have inadequate nutrition or dietary deficiencies may strengthen their immune systems or help them recover from infections, however evidence indicating an overall benefit in morbidity or reduction in mortality is not consistent.[177]

Consistent condom use reduces the risk of HIV transmission by approximately 80% over the long term.[110] When condoms are used consistently by a couple in which one person is infected, the rate of HIV infection is less than 1% per year.[111] There is some evidence to suggest that female condoms may provide an equivalent level of protection.[112] Application of a vaginal gel containing tenofovir (a reverse transcriptase inhibitor) immediately before sex seems to reduce infection rates by approximately 40% among African women.[113] By contrast, use of the spermicide nonoxynol-9 may increase the risk of transmission due to its tendency to cause vaginal and rectal irritation.[114]

Needles. HIV is frequently spread by sharing needles, syringes, or drug use equipment with someone who is infected with the virus. Transmission from patient to healthcare worker, or vice-versa, through accidental sticks with contaminated needles or other medical instruments, is rare.

Paradoxical IRIS typically occurs during the first few months of treatment and usually resolves on its own. If it does not, corticosteroids, given for a short time, are often effective. Paradoxical IRIS is more likely to cause symptoms and symptoms are more likely to be severe when ART is started soon after treatment of an opportunistic infection is started. Thus, for some opportunistic infections, ART is delayed until treatment of the opportunistic infection has reduced or eliminated the infection.

HIV is the virus that’s passed from person to person. Over time, HIV destroys an important kind of the cell in your immune system (called CD4 cells or T cells) that helps protect you from infections. Fungi. The most common fungal disease associated with AIDS is Pneumocystis carinii pneumonia (PCP). PCP is the immediate cause of death in 15-20% of AIDS patients. It is an important measure of a patient’s prognosis. Other fungal infections include a yeast infection of the mouth (candidiasis or thrush) and cryptococcal meningitis.

If the CD4 count is low, people are more likely to develop serious infections and other complications of HIV such as certain cancers. Viral load helps predict how fast the CD4 count is likely to decrease over the next few years.

The PrEP Heroes campaign aims to increase awareness of drugs that prevent HIV from establishing itself if a person is exposed. “Being a part of the PrEP Hero campaign was important because it was an opportunity to show diversity in communities where HIV and LGBT intersect,” Franco De Marco said.

This past July, results came in on the third case. In 2010, a girl known as the Mississippi baby was born to an H.I.V.-positive mother who had taken no antiretrovirals, and the baby had the virus in her blood. Thirty hours after delivery, the newborn started on antiretroviral therapy. Within weeks, the viral count fell below the limit of detection. The baby was eighteen months old when the treatment was interrupted, against medical advice. For two years, the girl’s blood showed no trace of the virus, and researchers speculated that very early HAART might prevent the virus from forming a dormant reservoir. Twenty-seven months after going off the drugs, however, the child tested positive for the virus. Though researchers were impressed that early intervention had temporarily banished H.I.V., she was not cured.

Researchers are also trying to switch off a molecule called PD-1, which the body uses to restrain the immune system. Deactivating PD-1 has worked in clinical studies with melanoma and lung-cancer patients, and one patient seems to have been cured of hepatitis C by a single infusion of a PD-1 blocker from Bristol-Myers Squibb.

After the virus enters the body there is a period of rapid viral replication, leading to an abundance of virus in the peripheral blood. During primary infection, the level of may reach several million virus particles per milliliter of blood.[95] This response is accompanied by a marked drop in the number of circulating CD4+ T cells. The acute viremia is almost invariably associated with activation of CD8+ T cells, which kill HIV-infected cells, and subsequently with antibody production, or seroconversion. The CD8+ T cell response is thought to be important in controlling virus levels, which peak and then decline, as the CD4+ T cell counts recover. A good CD8+ T cell response has been linked to slower disease progression and a better prognosis, though it does not eliminate the virus.[96]

HIV attacks and destroys a type of white blood cell called a CD4 cell, commonly called the T-cell. This cell’s main function is to fight disease. When a person’s CD4 cell count gets low, they are more susceptible to illnesses.

It’s important to know whether you will breastfeed or bottle-feed your baby prior to delivery, as the breasts’ ability to produce milk diminishes soon after childbirth without the stimulation of breastfeeding. Breast milk is easily digested by babies and contains infection-fighting antibodies and cholesterol, which promotes brain growth. Formula-fed babies actually need to eat somewhat less often since formula is less readily digested by the baby than human milk. Stevens-Johnson syndrome (SJS) is a rare allergic reaction to HIV medication. Symptoms include fever and swelling of the face and tongue. Rash, which can involve the skin and mucous membranes, appears and spreads quickly.

A disease caused by the human immunodeficiency virus (HIV) and transmitted by sexual contact or by blood spread on infected needles and other implements. AIDS is not a specifically homosexual disorder. Rather it is a disease of sexually promiscuous populations that harbour large numbers of HIV. The virus attacks a particular group of white cells of the immune system (helper T lymphocytes) causing a severe reduction in the ability of the body to resist infection and certain forms of cancer. The resulting recurrent infections, often with organisms not normally causing disease (opportunistic infectors), can usually be treated, but, to date, no wholly effective treatment for the underlying HIV infection has been developed. Combinations of drugs, including protease inhibitors, reverse transcriptase inhibitors, fusion inhibitors and DNA polymerase inhibitors, can, however, greatly prolong life and have virtually converted AIDS from an inevitably fatal, to a potentially serious chronic disease. The condition may involve many different disorders including a form of pneumonia caused by Pneumocystis carinii , CYTOMEGALOVIRUS infections, widespread herpes simplex infections, widespread thrush (CANDIDIASIS), KAPOSI’S SARCOMA and other malignancies, and brain damage from direct infection of neurons by HIV. The presence of the AIDS virus can be detected by the ELISA and other tests.

Estimation of current incidence of HIV is difficult. A back-calculation analysis (a statistical method using incubation period to project future distribution of infection) suggests there has been little change in HIV incidence in MSM over recent years. If there has been a decrease in transmissibility associated with antiretroviral treatment in those diagnosed it may have been offset by an increase in risky behaviours. In 2012, there were 2,300-2,500 new infections annually and 7,200 MSM undiagnosed.[5]London has been the main focus of the HIV epidemic in the UK. Of those MSM receiving HIV care in 2012, 50% lived in London.[7]

Condoms made of latex provide good protection HIV (as well as other common sexually transmitted diseases), but they are not foolproof. Oil-based lubricants (such as petroleum jelly) should not be used because they may dissolve latex, reducing the condom’s effectiveness.

Paradoxical IRIS typically occurs during the first few months of treatment and usually resolves on its own. If it does not, corticosteroids, given for a short time, are often effective. Paradoxical IRIS is more likely to cause symptoms and symptoms are more likely to be severe when ART is started soon after treatment of an opportunistic infection is started. Thus, for some opportunistic infections, ART is delayed until treatment of the opportunistic infection has reduced or eliminated the infection.

HIV/AIDS has had a great impact on society, both as an illness and as a source of discrimination.[22] The disease also has large economic impacts.[22] There are many misconceptions about HIV/AIDS such as the belief that it can be transmitted by casual non-sexual contact.[23] The disease has become subject to many controversies involving religion including the Catholic Church’s position not to support condom use as prevention.[24] It has attracted international medical and political attention as well as large-scale funding since it was identified in the 1980s.[25]

HIV-1 infection causes chronic ongoing inflammation and production of reactive oxygen species.[77] Thus, the HIV genome may be vulnerable to oxidative damages, including breaks in the single-stranded RNA. For HIV, as well as for viruses generally, successful infection depends on overcoming host defensive strategies that often include production of genome-damaging reactive oxygen. Thus, Michod et al.[78] suggested that recombination by viruses is an adaptation for repair of genome damages, and that recombinational variation is a byproduct that may provide a separate benefit.

During successful treatment, the viral load decreases to very low or undetectable levels (less than about 20 to 40 copies per microliter of blood). However, inactive (latent) HIV is still present within cells, and if treatment is stopped, HIV starts replicating and the viral load increases.

The Centers for Disease Control and Prevention (CDC) estimates that approximately 40,000–50,000 new human immunodeficiency virus (HIV) infections occurred annually in the United States from 2006 to 2009 (1). Almost 1 in 5 (18.1%) of all individuals infected with HIV are unaware of their HIV status (2). In order to identify individuals with undiagnosed HIV infection, the CDC recommends HIV screening for all patients aged 13–64 years in health care settings (3). Because obstetrician–gynecologists provide primary and preventive care for adolescents and women, they are ideally suited to play an important role in promoting HIV screening for their patients. The American College of Obstetricians and Gynecologists (the College) recommends routine HIV screening for females aged 13–64 years and older women with risk factors. Screening after age 64 years is indicated if there is ongoing risk of HIV infection, as indicated by risk assessment (eg, new sexual partners).

Production of the clotting factor concentrates, mainly to treat patients with haemophilia A and haemophilia B (Christmas disease), involves the pooling of very many donations and a single donation could contaminate a batch of concentrate used to treat many patients. There have been no recorded transmissions of HIV by this route in the UK since the introduction of heat inactivation of concentrates and donor screening in 1985.

There are currently nine approved PIs that all have distinct toxicities. The most common side effects associated with these drugs are nausea and diarrhea, which occur more often with some PIs than others. For example, diarrhea is more common with NFV than other PIs but can occur with any and all drugs in this class. Many of the drugs in this class also increase blood lipid levels, some more than others with ATV and DRV appearing to have less effect on lipids than other drugs in the class. Other unique toxicities associated with various PIs are kidney stones, kidney damage, and increases in blood bilirubin levels and potentially jaundice with IDV and ATV. Some of these drugs also have been associated with elevations in blood sugar levels and bleeding in hemophiliacs. Finally, little is known regarding the role these drugs may play in the development of lipodystrophy. There is also some data suggesting that LPV/RTV and DRV may be associated with an increased risk of cardiovascular events.

Full blood count: This is a test to check on the levels of white blood cells, red blood cells, platelets and haemoglobins in your blood. This test needs to be done before and regularly after treatment to check for anaemia (reduced blood haemoglobin) and reduction of other blood cells.

Risk of transmission from infected health care practitioners who take appropriate precautions is unclear but appears minimal. In the 1980s, one dentist transmitted HIV to ≥ 6 of his patients by unknown means. However, extensive investigations of patients cared for by other HIV-infected physicians, including surgeons, have uncovered few other cases.

Use a clean needle. If you use a needle to inject drugs, make sure it’s sterile and don’t share it. Take advantage of needle-exchange programs in your community and consider seeking help for your drug use.

If you believe you have been exposed to HIV, seek medical attention right away. DO NOT delay. Starting antiviral medicines right after the exposure (up to 3 days after) can reduce the chance that you will be infected. This is called post-exposure prophylaxis (PEP). It has been used to prevent transmission in health care workers injured by needlesticks.

Taking an antiretroviral drug beforebeing exposed to HIV can reduce the risk of HIV infection. Such preventive treatment is called preexposure prophylaxis (PrEP). However, PrEP is expensive and is effective only if people take the drug every day. Thus, PrEP is recommended only for people who have a very high risk of becoming infected, such as people who have a partner who is infected with HIV.

There has been a great deal of attention given to the more recently identified problem of “lipodystrophy.” Individuals suffering from this syndrome can be categorized as having lipohypertrophy (fat accumulation) syndromes, such as the “buffalo hump” on the back of the neck, breast enlargement, or increased abdominal girth. Others primarily suffer from lipoatrophy with fat loss under the skin with complaints of prominent veins on the arms and legs, sunken cheeks, and decreased gluteal (buttock) size. These syndromes appear to be related to multiple factors, including, but not limited to, drug therapy. The NRTIs appear to be most closely linked to lipoatrophy, in particular D4T and to a lesser extent ZDV. In fact, some studies have suggested slow accumulation of fat in those who modify the NRTI component of their regimen. Some NRTIs also have been linked to elevation in lipid (fat) levels in the blood. While switching therapy is always a consideration in those experiencing potential drug-related toxicity, this should only be done under the careful supervision of an experienced HIV provider.

Although the symptoms of immune deficiency characteristic of AIDS do not appear for years after a person is infected, the bulk of CD4+ T cell loss occurs during the first weeks of infection, especially in the intestinal mucosa, which harbors the majority of the lymphocytes found in the body.[99] The reason for the preferential loss of mucosal CD4+ T cells is that the majority of mucosal CD4+ T cells express the CCR5 protein which HIV uses as a co-receptor to gain access to the cells, whereas only a small fraction of CD4+ T cells in the bloodstream do so.[100] A specific genetic change that alters the CCR5 protein when present in both chromosomes very effectively prevents HIV-1 infection.[101]

One morning in the winter of 1981, my wife came home after her on-call shift at the U.C.L.A. Medical Center and told me about a baffling new case. Queenie was an eighteen-year-old prostitute, his hair dyed the color of brass. He had arrived at the emergency room with a high fever and a cough, and appeared to have a routine kind of pneumonia, readily treated with antibiotics. But the medical team retrieved a microbe from his lungs called Pneumocystis carinii. The microbe was known for causing a rare fungal pneumonia that had been seen in severely malnourished children and in adults undergoing organ transplants or chemotherapy.

In 1983, two separate research groups led by Robert Gallo and Luc Montagnier declared that a novel retrovirus may have been infecting people with AIDS, and published their findings in the same issue of the journal Science.[230][231] Gallo claimed that a virus his group had isolated from a person with AIDS was strikingly similar in shape to other human T-lymphotropic viruses (HTLVs) his group had been the first to isolate. Gallo’s group called their newly isolated virus HTLV-III. At the same time, Montagnier’s group isolated a virus from a person presenting with swelling of the lymph nodes of the neck and physical weakness, two characteristic symptoms of AIDS. Contradicting the report from Gallo’s group, Montagnier and his colleagues showed that core proteins of this virus were immunologically different from those of HTLV-I. Montagnier’s group named their isolated virus lymphadenopathy-associated virus (LAV).[221] As these two viruses turned out to be the same, in 1986, LAV and HTLV-III were renamed HIV.[232]

Until recently, Justin Huff, a former Jackson State student, shared a room on the second floor of Grace House’s main facility. He was infected with H.I.V. a year and a half ago, when a man he met on Jack’d sexually assaulted him. He received his diagnosis just after his 21st-birthday celebration. “I was throwing up and couldn’t eat anything for a few days; I thought it was from the drinking,” Huff said. “When I went to the doctor, he was like, if I hadn’t made it in the next two days, I would’ve been dead.”

HIV is spread through contact with infected blood or fluids such as sexual secretions. Over time, the virus attacks the immune system, focusing on special cells called “CD4 cells” which are important in protecting the body from infections and cancers, and the number of these cells starts to fall. Eventually, the CD4 cells fall to a critical level and/or the immune system is weakened so much that it can no longer fight off certain types of infections and cancers. This advanced stage of HIV infection is called AIDS.

Human immunodeficiency virus (HIV) infection results from 1 of 2 similar retroviruses (HIV-1 and HIV-2) that destroy CD4+ lymphocytes and impair cell-mediated immunity, increasing risk of certain infections and cancers. Initial infection may cause nonspecific febrile illness. Risk of subsequent manifestations—related to immunodeficiency—is proportional to the level of CD4+ lymphocyte depletion. Two types of HIV have been characterized: HIV-1 and HIV-2. HIV-1 is the virus that was originally discovered (and initially referred to also as LAV or HTLV-III). It is more virulent, more infective,[93] and is the cause of the majority of HIV infections globally. The lower infectivity of HIV-2 as compared with HIV-1 implies that fewer people exposed to HIV-2 will be infected per exposure. Because of its relatively poor capacity for transmission, HIV-2 is largely confined to West Africa.[94] Turning things around would mean expanding testing and providing affordable treatment for those who are positive — to stop sickness and dying and also to block transmission of the virus. It would also require getting information and medication, including PrEP, to those most at risk. Even more challenging would be reducing the stigma, discrimination and shame that drive gay and bisexual men to hide their sexuality and avoid the health care system — and making sure providers have adequate resources and understand how to care for H.I.V. patients. In making decisions about patient care, health care professionals who are infected with HIV should adhere to the fundamental professional obligation to avoid harm to patients. Physicians who have reason to believe that they have been at significant risk of being infected should be tested voluntarily for HIV for the protection of their patients as well as for their own benefit. The physician as a patient is entitled to the same rights to privacy and confidentiality as any other patient. Treatments with HAART have shown considerable progress since the first antiretroviral was approved for use by the FDA in 1987. Impressive improvements in life expectancy and quality of life have ensued. There are, however, still many problems. In 1997, amid euphoria about HAART, people first started thinking seriously about a cure. Sooner or later, all infected cells die on their own. Could the right drugs in the right combination rout the virus for good? That year, David Ho published a paper in Nature in which he mathematically predicted that an H.I.V. patient on the HAART regimen should be able to conquer the detectable virus in twenty-eight to thirty-seven months. That issue also contained a very different report from Robert Siliciano, currently a Howard Hughes investigator at the Johns Hopkins School of Medicine. Using an uncommonly sensitive measurement technique that he’d invented, Siliciano located H.I.V. in a type of helper T cell that provides memory to our immune system and normally survives for decades. Memory T cells are uniquely important: they recognize the antigens in infections and orchestrate speedy responses. But the virus proved to be even cleverer. It lay dormant in strands of host DNA, untouched by the drug cocktail, later springing back to life and degrading the immune system.

Healthcare visits in the preceding year were associated with a lower rate of unawareness (37% vs 81%) but a higher rate of HIV-positivity (21% vs 12%). Because this study targeted a high-risk group and may involve participation bias, the overall rate of HIV infection (19%) cannot be easily extrapolated to the overall population. [73]

The second phase of HIV infection, the asymptomatic period, lasts an average of 10 years. During that period the virus continues to replicate, and there is a slow decrease in the CD4 count (the number of helper T cells). When the CD4 count falls to about 200 cells per microlitre of blood (in an uninfected adult it is typically about 1,000 cells per microlitre), patients begin to experience opportunistic infections—i.e., infections that arise only in individuals with a defective immune system. That is AIDS, the final stage of HIV infection. The most-common opportunistic infections are Pneumocystis carinii pneumonia, tuberculosis, Mycobacterium avium infection, herpes simplex infection, bacterial pneumonia, toxoplasmosis, and cytomegalovirus infection. In addition, patients can develop dementia and certain cancers, including Kaposi sarcoma and lymphomas. Death ultimately results from the relentless attack of opportunistic pathogens or from the body’s inability to fight off malignancies.

Condoms made of latex provide good protection against HIV (as well as other common sexually transmitted diseases), but they are not foolproof. Oil-based lubricants (such as petroleum jelly) should not be used because they may dissolve latex, reducing the condom’s effectiveness.

HIV/AIDS has become a chronic rather than an acutely fatal disease in many areas of the world.[185] Prognosis varies between people, and both the CD4 count and viral load are useful for predicted outcomes.[28] Without treatment, average survival time after infection with HIV is estimated to be 9 to 11 years, depending on the HIV subtype.[15] After the diagnosis of AIDS, if treatment is not available, survival ranges between 6 and 19 months.[186][187] HAART and appropriate prevention of opportunistic infections reduces the death rate by 80%, and raises the life expectancy for a newly diagnosed young adult to 20–50 years.[185][188][189] This is between two thirds[188] and nearly that of the general population.[29][190] If treatment is started late in the infection, prognosis is not as good:[29] for example, if treatment is begun following the diagnosis of AIDS, life expectancy is ~10–40 years.[29][185] Half of infants born with HIV die before two years of age without treatment.[167]

Non-nucleoside reverse transcriptase inhibitors (NNRTIs) are commonly used in combination with NRTIs to help keep the virus from multiplying. Examples of NNRTIs are efavirenz (Sustiva), nevirapine (Viramune), delavirdine (Rescriptor), etravirine (Intelence), and rilpivirine (Edurant). Complete HIV treatment regimens that combine two NRTIs and one NNRTI in one pill taken once a day are available for convenience; these include Atripla (efavirenz/emtricitabine/tenofovir) and Complera (rilpivirine/emtricitabine/tenofovir).

The community’s awakening came in 1991, when Magic Johnson tearfully announced, “Because of the H.I.V. virus I have obtained, I will have to retire from the Lakers today,” and warned, “It can happen to anyone.” By 1994, AIDS had become the No. 1 killer of all African-Americans ages 25 to 44. The virus was 16 times as common in black women as in their white counterparts — and the gap would widen over the next few years. I was an editor at Essence in 1994 when the magazine’s editor in chief, Susan L. Taylor, insisted that we shine a light on the disturbing increase of H.I.V. among African-American women by putting Rae Lewis Thornton, a Chicago woman who described herself as “young, educated, drug-free and dying of AIDS,” on the cover.

HIV is a retrovirus, one of a unique family of viruses that consist of genetic material in the form of RNA (instead of DNA) surrounded by a lipoprotein envelope. HIV cannot replicate on its own and instead relies on the mechanisms of the host cell to produce new viral particles. HIV infects helper T cells by means of a protein embedded in its envelope called gp120. The gp120 protein binds to a molecule called CD4 on the surface of the helper T cell, an event that initiates a complex set of reactions that allow the HIV genetic information into the cell.

Antiviral treatment options have primarily included combinations of two NRTIs, often referred to as “nucs,” and a third drug, typically being a boosted PI, a NNRTI, often called “non-nucs, and InSTIs such as RAL, EVG or dolutegravir (Tivicay, DTG). Many of these drugs are available in fixed-dose combinations as well as increasing numbers of drugs as single-tablet regimens.

Federal and state programs are also hampered by policy decisions grounded in ideology rather than science such as the allocation of more than $1 billion to failed abstinence-only sex education programs or the enactment of outdated HIV criminalization statutes. In more than 30 states, people living with HIV can be tried and imprisoned simply because a partner accuses them of withholding their HIV status. There’s no proof these laws work, and they run counter to public health by perpetuating stigma and subsequently deterring people from getting tested or treated for HIV.

Developing AIDS requires that the person acquire HIV infection. Risks for acquiring HIV infection include behaviors that result in contact with infected blood or sexual secretions, which pose the main risk of HIV transmission. These behaviors include sexual intercourse and injection drug use. The presence of sores in the genital area, like those caused by herpes, makes it easier for the virus to pass from person to person during intercourse. HIV also has been spread to health care through accidental sticks with needles contaminated with blood from HIV-infected people, or when broken skin has come into contact with infected blood or secretions. Blood products used for transfusions or injections also may spread infection, although this has become extremely rare (less than one in 2 million transfusions in the U.S.) due to testing of blood donors and blood supplies for HIV. Finally, infants may acquire HIV from an infected mother either while they are in the womb, during birth, or by breastfeeding after birth.

Brown’s cure was spectacular, but difficult to repeat. His doctor had twice destroyed all his native blood cells, with radiation and chemotherapy, and twice rebuilt his immune system with transplanted stem cells. It had been very dangerous and costly. Researchers wondered if they could create a scaled-down version. In 2013, physicians at Brigham and Women’s Hospital, in Boston, reported on the outcome of a study in which two H.I.V.-positive men on HAART had received bone-marrow transplants for lymphoma. Their marrow donors, unlike Brown’s, did not have the CCR5 mutation, and their chemotherapy regimen was less intensive. HAART was stopped a few years after the transplants, and the virus remained undetectable for months, but then resurfaced.

HIV is the causative agent of Acquired Immunodeficiency Syndrome (AIDS). AIDS is a severe, life-threatening disease that represents the late clinical stage of infection with the HIV. 2.5 million people died of AIDS in 2005 alone, and estimates place the number of people living with HIV/AIDS at 38.6 million. HIV/AIDS has claimed more than 25 million lives since 1981.

In contrast, ‘lymphocyte-tropic’ variants of HIV infect only CD4 T cells in vivo and use CXCR4, which binds the CXC chemokine stromal-derived factor-1 (SDF-1), as a co-receptor. The lymphocyte-tropic variants of HIV can grow in vitro in T-cell lines, and require high levels of CD4 on the cells that they infect.

The specific opportunistic infections and cancers that develop cause many of the symptoms. These infections occur more frequently or are more severe in people with HIV infection than in those without the infection. For example, an infection with the fungus Candida may cause white patches in the mouth and sometimes pain when swallowing (called thrush) or a thick, white discharge from the vagina that resembles cottage cheese (a vaginal yeast infection). Shingles (herpes zoster) may cause pain and a rash.

International Issues By 2003 the international AIDS problem had become a crisis in Africa and parts of Asia. The United Nations(UN) and the World Health Organization (WHO) have worked together to address the issues of prevention and treatment, but the statistics reveal grim conditions. In December 2002 a joint UN-WHO report disclosed that 42 million people in the world are living with HIV and AIDS. In 2002 five million people contracted HIV and over three million people died of AIDS. The situation is gravest in sub-Saharan Africa, where over 29 million adults and children are living with HIV and AIDS, contracted mainly through heterosexual contact. These figures stand in stark contrast to North America, where less than one million people are living with HIV and AIDS.

Negotiating a maze of unpaved roads in Jackson in the company car, a 13-year-old Ford Expedition with cracked seats and chipped paint, he stopped to drop off H.I.V. medication at a couple’s home. One of the men was H.I.V.-positive, the other negative; they lived in the neighborhood locals call the Bottom, where every fifth or sixth home is abandoned, with broken windows, doors hanging off hinges, downed limbs and dry leaves blanketing front yards. Sturdevant banged on the door of a small house, its yard overgrown with weeds; he knew not to leave the package on the doorstep, where it could be stolen. After a while a young man emerged, shirtless, shrugging off sleep. He had just gotten out of jail. Sturdevant handed him the package, shook his hand and told him to “stay out of trouble.”

HIV-2 is closely related to simian immunodeficiency virus endemic in sooty mangabeys (Cercocebus atys atys) (SIVsmm), a monkey species inhabiting the forests of Littoral West Africa. Phylogenetic analyses show that the virus most closely related to the two strains of HIV-2 which spread considerably in humans (HIV-2 groups A and B) is the SIVsmm found in the sooty mangabeys of the Tai forest, in western Ivory Coast.[20]

A type of protein molecule in human blood, sometimes called the T4 antigen, that is present on the surface of 65% of immune cells. The HIV virus infects cells with CD4 surface proteins, and as a result, depletes the number of immune system cells (T cells, B cells, natural killer cells, monocytes) in the individual’s blood. Most of the damage to an AIDS patient’s immune system is done by the virus’ destruction of CD4+ lymphocytes.

ABSTRACT: Because human immunodeficiency virus (HIV) infection often is detected through prenatal and sexually transmitted disease testing, an obstetrician–gynecologist may be the first health professional to provide care for a woman infected with HIV. Universal testing with patient notification and right of refusal (“opt-out” testing) is recommended by most national organizations and federal agencies. Although opt-out and “opt-in” testing (but not mandatory testing) are both ethically acceptable, the former approach may identify more women who are eligible for therapy and may have public health advantages. It is unethical for an obstetrician–gynecologist to refuse to accept a patient or to refuse to continue providing health care for a patient solely because she is, or is thought to be, seropositive for HIV. Health care professionals who are infected with HIV should adhere to the fundamental professional obligation to avoid harm to patients. Physicians who believe that they have been at significant risk of being infected should be tested voluntarily for HIV.

During this phase, the infection is established and a proviral reservoir is created. [60, 61] This reservoir consists of persistently infected cells, typically macrophages, and appears to steadily release virus. Some of the viral release replenishes the reservoir, and some goes on to produce more active infection.

Sturdevant, born and raised in Metcalfe, a tiny Mississippi Delta town of about 1,000, understands all too well the fear, stigma and isolation that can come with being a black gay man in the South. “Growing up, I was taught that God was not fixing to forgive a person who was homosexual,” Sturdevant said. “The Bible supposedly said you’re going straight to hell, automatically, there’s no forgiveness. There were several times I thought about suicide. There were several times I wanted to get sick and die. Finally, my thought was, I just want to get out of here.” He moved to Dallas, and then to Memphis.

Sturdevant moved his seat back, preparing for a long drive, and adjusted the radio to 107.5, the local R.&B. oldies station. Toni Braxton’s wail — “I wish you’d hold me in your arms like that Spanish guitar” — filled the car. He was headed to a small town 90 miles east of the city to visit Jordon, an H.I.V.-positive 24-year-old. When Sturdevant himself was at his lowest point, he said, “I looked something like this boy we’re going to see.”

Key populations are groups who are at increased risk of HIV irrespective of epidemic type or local context. They include: men who have sex with men, people who inject drugs, people in prisons and other closed settings, sex workers and their clients, and transgender people.

The appropriate use of combination antiretroviral therapies and prophylaxis for opportunistic infections dramatically improves survival and greatly decreases the risk of secondary opportunistic infections. [83, 84, 85] The risk of AIDS-associated lymphoma is not altered by antiviral therapy and, as such, has grown in prevalence among overall AIDS-defining conditions.

In the U.S., more than 1 million people are currently infected with HIV, and approximately 50,000 are newly infected each year. Over the years, more than 600,000 people in the U.S. have died from AIDS, many of them during what should have been their most productive years of life.

In 2015, among 1,122,900 persons living with HIV infection, 162,500 (14.5%) were unaware of their infection. The percentage of undiagnosed HIV infections ranged from 5.7% to 18.5% across states (Figure 1); 50.5% of undiagnosed infections were in the South. Among 39,720 persons with HIV infection diagnosed in 2015, 21.6% had stage 3 infection (AIDS) at the time of diagnosis, and the estimated median interval from HIV infection to diagnosis was 3.0 years (Table 1). Diagnosis delays were longer among persons who were older at diagnosis than among those who were younger (median = 4.5 years among persons aged ≥55 years compared with 2.4 years among persons aged 13–24 years) (p<0.01). By race/ethnicity, median diagnosis delay ranged from 2.2 years among whites to 4.2 years among Asians (p<0.01). Transmission of HIV requires contact with body fluids—specifically blood, semen, vaginal secretions, breast milk, saliva, or exudates from wounds or skin and mucosal lesions—that contain free HIV virions or infected cells. Transmission is more likely with the high levels of virions that are typical during primary infection, even when such infections are asymptomatic. Transmission by saliva or droplets produced by coughing or sneezing, although conceivable, is extremely unlikely. Merely having HIV does not mean a person has AIDS. AIDS is an advanced stage of HIV infection and requires that the person have evidence of a damaged immune system. That evidence comes from at least one of the following: The use of mother-to-child transmission prevention strategies is another important strand of AIDS prevention programmes. In South Africa, for example, expansion of the strategy has resulted in the mother-to-child transmission rate falling to 3.5%.[21] A transmissible retrovirus that causes AIDS in humans. Two forms of HIV are now recognized: HIV-1, which causes most cases of AIDS in Europe, North and South America, and most parts of Africa; and HIV-2, which is chiefly found in West African patients. The resistance of these rare individuals to HIV infection has now been explained by the discovery that they are homozygous for an allelic, nonfunctional variant of CCR5 caused by a 32-base-pair deletion from the coding region that leads to a frameshift and truncation of the translated protein. The gene frequency of this mutant allele in Caucasoid populations is quite high at 0.09 (meaning that about 10% of the Caucasoid population are heterozygous carriers of the allele and about 1% are homozygous). The mutant allele has not been found in Japanese or black Africans from Western or Central Africa. Heterozygous deficiency of CCR5 might provide some protection against sexual transmission of HIV infection and a modest reduction in the rate of progression of the disease. In addition to the structural polymorphism of the gene, variation of the promoter region of the CCR5 gene has been found in both Caucasian and African Americans.
