Drug treatment guidelines for HIV/AIDS change frequently as new drugs are approved and new drug regimens developed. After the first month or so, HIV enters the clinical latency stage. This stage can last from a few years to a few decades. Progression can be slowed with antiretroviral therapy. Some people have symptoms. Many people do not, but it’s still contagious.

In people with unmasked IRIS, doctors treat the newly identified opportunistic infection with antimicrobial drugs. Occasionally, when the symptoms are severe, corticosteroids are also used. Usually, when unmasked IRIS occurs, cART is continued. An exception is when a cryptococcal infection affects the brain. Then cART is temporarily interrupted until the infection is controlled.

If the CD4 count is low, people are more likely to develop serious infections and other complications of HIV such as certain cancers. Viral load helps predict how fast the CD4 count is likely to decrease over the next few years.

The goal is to start PEP as soon after exposure as possible if prophylaxis is warranted. CDC recommends providing PEP within 24 to 36 h after a longer interval after exposure requires the advice of an expert.

Some people die a few months after getting infected, while others live fairly normal lives for many years, even after they “officially” have AIDS. Diagnostic blood tests for AIDS are given to individuals in high-risk populations, pregnant women, health care and public service workers who have been exposed to HIV, those who have symptoms associated with AIDS, or others who fear they may have been exposed to the virus. The first blood test for AIDS was developed in 1985. Patients who are being tested for HIV infection are usually given an enzyme-linked immunosorbent assay (ELISA) test for the presence of HIV antibody in their blood. Positive ELISA results are then tested with a Western blot or immunofluorescence (IFA) assay for confirmation. The combination of the ELISA and Western blot tests is more than 99.9% accurate in detecting HIV infection within four to eight weeks following exposure. The polymerase chain reaction (PCR) test can be used to detect the presence of viral nucleic acids in the very small number of HIV patients who have false-negative results on the ELISA and Western blot tests. These tests are also used to detect viruses and bacteria other than HIV and AIDS.

If HIV is left untreated, it may take up to 10 or 15 years for the immune system to be so severely damaged it can no longer defend itself at all. However, the speed HIV progresses will vary depending on age, health and background.

The prognosis in patients with untreated HIV infection is poor, with an overall mortality rate of more than 90%. HIV infection takes different forms within different cells. As we have seen, more than 95% of the virus that can be detected in the plasma is derived from productively infected cells, which have a very short half-life of about 2 days. Productively infected CD4 lymphocytes are found in the T-cell areas of lymphoid tissue, and these are thought to succumb to infection in the course of being activated in an immune response. Latently infected memory CD4 cells that are activated in response to antigen presentation also produce virus. Such cells have a longer half-life of 2 to 3 weeks from the time that they are infected. Once activated, HIV can spread from these cells by rounds of replication in other activated CD4 T cells. In addition to the cells that are infected productively or latently, there is a further large population of cells infected by defective proviruses; such cells are not a source of infectious virus.

Gut-associated lymphoid tissue (GALT) plays a role in HIV replication. [28] Although the portal of entry for HIV infection is typically through direct blood inoculation or exposure of the virus to genital mucosal surfaces, the GI tract contains a large amount of lymphoid tissue, making this an ideal site for HIV replication.

Approximately 20% of new diagnoses are in women. In the United States, heterosexual transmission accounts for approximately one-quarter of new diagnoses, with intravenous drug use contributing to the remaining cases in the U.S.

Talk to your sexual partner(s). Get tested for other sexually transmitted diseases (STDs), and use protection time you have sex. Talk to your doctor about pre-exposure prophylaxis (PrEP). When used consistently, this daily medication can lower the chances of transmission.

Early advances in preventing HIV transmission resulted from educational programs describing how transmission occurs and providing barrier protection for those exposed to genital secretions and new needles or bleach to those exposed to blood by sharing needles. Despite these efforts, new infection in both the developed and developing worlds has continued at high rates.

Cellular immune response to HIV. The cellular immune response is induced upon the entry of HIV into the target cells (e.g., T cells) and synthesis of viral proteins (Figure 1). MHC class I on the cell surface displays the intracellularly degraded HIV peptide fragments for recognition by T-cell receptors (TCR) on CD8+ T cells (Figure 3). CD8+ T cells lyse HIV infected cells and secrete cytokines, i.e. interferon-γ (IFN-γ), tumor necrosis factor α (TNF-α), and chemokines, i.e. MIP-1 α, MIP β and RANTES, that inhibit virus replication and block  viral entry into CD4+ T cells. Development of CD8+ T cells is crucial for control of HIV replication. This results in declining viraemia after primary infection. In the early stages of infection, CD4+ T cells lose their proliferative capacity and therefore their contribution to viral control is minor. However, during chronic infection CD4+T cells are present and secrete interleukin-2 (IL-2) or cytokines, such as IFN-γ, to control viraemia.

During this phase, the infection is established and a proviral reservoir is created. [60, 61] This reservoir consists of persistently infected cells, typically macrophages, and appears to steadily release virus. Some of the viral release replenishes the reservoir, and some goes on to produce more active infection.

Within weeks of infection, many people will develop the varied symptoms of primary or acute infection, which typically has been described as a mononucleosis- or influenza-like illness but can range from minimal fever, aches, and pains to very severe symptoms. The most common symptoms of primary HIV infection are

Universal precautions within the health care environment are believed to be effective in decreasing the risk of HIV.[133] Intravenous drug use is an important risk factor and harm reduction strategies such as needle-exchange programs and opioid substitution therapy appear effective in decreasing this risk.[134][135]

Counseling for parenteral drug users: Counseling about the risk of sharing needles is important but is probably more effective if combined with provision of sterile needles, treatment of drug dependence, and rehabilitation.

In other respects, health care is a distinct area of concern for AIDS patients and health professionals alike. Discrimination has often taken place. State and federal statutes, including the Rehabilitation Act, guarantee access to health care for AIDS patients, and courts have upheld that right. In the 1988 case of Doe v. Centinela Hospital, 57 U.S.L.W. 2034 (C.D. Cal.), for example, an HIV-infected person with no symptoms was excluded from a federally funded hospital residential program for drug and alcohol treatment because health care providers feared exposure to the virus. The case itself exposed the irrationality of such discrimination. Although its employees had feared HIV, the hospital argued in court that the lack of symptoms meant that the patient was not disabled and thus not protected by the Rehabilitation Act. A federal trial court in California rejected this argument, ruling that a refusal to grant services based solely on fear of contagion is discrimination under the Rehabilitation Act.

During all stages of infection, literally billions of HIV particles (copies) are produced every day and circulate in the blood. This production of virus is associated with a decline (at an inconsistent rate) in the number of CD4 cells in the blood over the ensuing years. Although the precise mechanism by which HIV infection results in CD4 cell decline is not known, it probably results from a direct effect of the virus on the cell as well as the body’s attempt to clear these infected cells from the system. In addition to virus in the blood, there is also virus throughout the body, especially in the lymph nodes, brain, and genital secretions.

Macrophage-tropic (M-tropic) strains of HIV-1, or non-syncytia-inducing strains (NSI; now called R5 viruses[41]) use the β-chemokine receptor CCR5 for entry and are, thus, able to replicate in both macrophages and CD4+ T cells.[42] This CCR5 co-receptor is used by almost all primary HIV-1 isolates regardless of viral genetic subtype. Indeed, macrophages play a key role in several critical aspects of HIV infection. They appear to be the first cells infected by HIV and perhaps the source of HIV production when CD4+ cells become depleted in the patient. Infected mothers should not breastfeed if they live in countries where formula feeding is safe and affordable. In some individuals treatment may not be commenced as recommended and disease progression may occur. The length of time that people with untreated HIV infection may live without symptoms varies widely. Some people experience rapid development of symptoms or disease due to their HIV infection, whereas others may remain free of any symptoms for years.

AIDS (acquired immunodeficiency syndrome) is a syndrome caused by a virus called HIV (human immunodeficiency virus). The disease alters the immune system, making people much vulnerable to infections and diseases. This susceptibility worsens if the syndrome progresses.

Visible effects of HIV infection come in the form of disrupted lymph-node architecture. This disruption is temporal, and, at one point, lymph-node biopsy was considered as a form of staging the disease. [54, 55] The disruption of the follicular dendritic network in the lymph nodes and subsequent failure of normal antigen presentation are likely contributors to the disease process.

Every 9.5 minutes, someone in the United States becomes infected. That’s more than 56,000 new cases a year. It is estimated that 1.1 million Americans are currently living with HIV. And 1 in 5 are unaware they are infected.

The transmission of HIV requires contact with a body fluid that contains the virus or cells infected with the virus. HIV can appear in nearly any body fluid, but transmission occurs mainly through blood, semen, vaginal fluids, and breast milk. Although tears, urine, and saliva may contain low concentrations of HIV, transmission through these fluids is extremely rare, if it occurs at all. HIV is not transmitted by casual contact (such as touching, holding, or dry kissing) or by close, nonsexual contact at work, school, or home. No case of HIV transmission has been traced to the coughing or sneezing of an infected person or to a mosquito bite. Transmission from an infected doctor or dentist to a patient is extremely rare.

In the United States, guidelines for using antiviral therapy have been developed and are updated on a regular basis by an expert panel assembled by the DHHS, the IAS-USA panel, and others. The DHHS guidelines are available at https://aidsinfo.nih.gov/. The most recent IAS-USA guidelines were published in the Journal of the American Medical Association (JAMA) in the summer of 2016.

By affecting mainly young adults, AIDS reduces the taxable population, in turn reducing the resources available for public expenditures such as education and health services not related to AIDS resulting in increasing pressure for the state’s finances and slower growth of the economy. This causes a slower growth of the tax base, an effect that is reinforced if there are growing expenditures on treating the sick, training (to replace sick workers), sick pay and caring for AIDS orphans. This is especially true if the sharp increase in adult mortality shifts the responsibility and blame from the family to the government in caring for these orphans.[258]

Viral replication requires that reverse transcriptase (an RNA-dependent DNA polymerase) copy HIV RNA, producing proviral DNA; this copying mechanism is prone to errors, resulting in frequent mutations and thus new HIV genotypes. These mutations facilitate the generation of HIV that can resist control by the host’s immune system and by antiretroviral drugs.

Baseline HIV genotype can be determined using a sample of blood; availability of this testing varies by location. HIV genotyping is used to identify mutations known to cause resistance to certain antiretroviral drugs and to help select a drug regimen likely to be effective for a specific patient with HIV infection.

Due to their nonspecific character, these symptoms are not often recognized as signs of HIV infection. Even cases that do get seen by a family doctor or a hospital are often misdiagnosed as one of the many common infectious diseases with overlapping symptoms. Thus, it is recommended that HIV be considered in people presenting an unexplained fever who may have risk factors for the infection.[28]

Finally, there are difficult ethical issues in the development of a vaccine. It would be unethical to conduct a vaccine trial without trying at the same time to minimize the exposure of a vaccinated population to the virus itself. However, the effectiveness of a vaccine can only be assessed in a population in which the exposure rate to the virus is high enough to assess whether vaccination is protective against infection. This means that initial vaccine trials might have to be conducted in countries where the incidence of infection is very high and public health measures have not yet succeeded in reducing the spread of HIV. During the first few months of infection, an HIV test may provide a false-negative result. This is because it takes time for the immune system to build up enough antibodies to be detected in a blood test. But the virus is active and highly contagious during this time.

Paradoxical IRIS typically occurs during the first few months of treatment and usually resolves on its own. If it does not, corticosteroids, given for a short time, are often effective. Paradoxical IRIS is more likely to cause symptoms and symptoms are more likely to be severe when cART is started soon after treatment of an opportunistic infection is started. Thus, for some (but not all) opportunistic infections, cART is delayed until treatment of the opportunistic infection has reduced or eliminated the infection.

Asymptomatic, mild-to-moderate cytopenias (eg, leukopenia, anemia, thrombocytopenia) are also common. Some patients experience progressive wasting (which may be related to anorexia and increased catabolism due to infections) and low-grade fevers or diarrhea.

No firm evidence has shown that the initiation of therapy early in the asymptomatic period is effective. However, very late initiation is known to result in a less effective response to therapy and a lower level of immune reconstitution.

The CDC and the College recommend that females aged 13–64 years be tested at least once in their lifetime and annually thereafter based on factors related to risk. Obstetrician–gynecologists should annually review patients’ risk factors for HIV and assess the need for retesting. Repeat HIV testing should be offered at least annually to women who

The α-chemokine SDF-1, a ligand for CXCR4, suppresses replication of T-tropic HIV-1 isolates. It does this by down-regulating the expression of CXCR4 on the surface of HIV target cells. M-tropic HIV-1 isolates that use only the CCR5 receptor are termed R5; those that use only CXCR4 are termed X4, and those that use both, X4R5. However, the use of co-receptor alone does not explain viral tropism, as not all R5 viruses are able to use CCR5 on macrophages for a productive infection[42] and HIV can also infect a subtype of myeloid dendritic cells,[45] which probably constitute a reservoir that maintains infection when CD4+ T cell numbers have declined to extremely low levels.

Medications are also used to prevent opportunistic infections (such as Pneumocystis carinii pneumonia) and can keep AIDS patients healthier for longer periods of time. Opportunistic infections are treated as they occur.

HIV is one of a group of viruses known as retroviruses. After getting into the body, the virus enters many different cells, incorporates its genes into the human DNA, and hijacks the cell to produce HIV virus. Most importantly, HIV attacks cells of the body’s immune system called CD4 or T-helper cells (T cells). These cells are destroyed by the infection. The body tries to keep up by making new T cells or trying to contain the virus, but eventually the HIV wins out and progressively destroys the body’s ability to fight infections and certain cancers. The virus structure has been studied extensively, and this ongoing research has helped scientists develop new treatments for HIV/AIDS. Although all HIV viruses are similar, small variations or mutations in the genetic material of the virus create drug-resistant viruses. Larger variations in the viral genes are found in different viral subtypes. Currently, HIV-1 is the predominant subtype that causes HIV/AIDS. HIV-2, another form of HIV, occurs almost exclusively in West Africa.

The number of new cases of AIDS acquired from heterosexual intercourse used to be greater than from men who have sex with men, but this situation was reversed in 2011. Approximately half (52%, 1,560/2,990 in 2011) of all infections among heterosexuals were probably acquired in the UK and this proportion has increased over recent years. The figure in 2002 was 27%.[5]

All HIV-infected pregnant women should be managed by an obstetrician with experience in dealing with HIV-infected women. Maximal obstetric precautions to minimize transmission of the HIV virus, such as avoiding scalp monitors and minimizing labor after rupture of the uterine membranes, should be observed. In addition, the potential use of an elective Caesarean section (C-section) should be discussed, particularly in those women without good viral control of their HIV infection where the risk of transmission may be increased. Breastfeeding should be avoided if alternative nutrition for the infant is available since HIV transmission can occur by this route. When breastfeeding is done, it should be in conjunction with antiretroviral therapy for the mother if at all possible. Antiviral therapy suppresses the replication of the HIV virus in the body. A combination of several antiretroviral agents, termed Highly Active Anti-Retroviral Therapy (HAART), has been highly effective in reducing the number of HIV particles in the blood stream (as measured by a blood test called the viral load). This can help the immune system bounce back for a and improve T-cell counts.

Human immunodeficiency virus (HIV)-associated cholangiopathy has been described in children.25 As in adults, the biliary abnormalities include irregularities of contour and caliber of the intrahepatic and extrahepatic ducts and papillary stenosis. The changes may result from concomitant infection with opportunistic organisms such as cytomegalovirus and Cryptosporidium parvum. Ascariasis infestation may be the most prevalent biliary infection worldwide, although concentrated within tropical climates. Among 214 children admitted to hospital in northern India for management of hepatobiliary and pancreatic ascariasis, 20 (9%) underwent endoscopic and 7 (4%) surgical intervention.26

The most powerful known cause of innate human immunodeficiency virus resistance is CCR5Δ32, a mutant allele, coding for a truncated inactive form of CCR5 (Dean et al., 1996; Dragic et al., 1996; Huang et al., 1996; Liu et al., 1996; Michael et al., 1997; Samson et al., 1996; Zimmerman et al., 1997). CX3CR1 that recognizes ABCD-3 is a recently identified human immunodeficiency virus coreceptor too (Combadiere et al., 1998; Reeves et al., 1997; Rucker et al., 1997). CX3CR1 interacts only with a limited number of human immunodeficiency virus envelopes, and ABCD-3 can efficiently block human immunodeficiency virus coreceptor activity of CX3CR1 (Combadiere et al., 1998). That CX3CR1 functions as a human immunodeficiency virus coreceptor suggests that nucleotide polymorphic variations of it may slow or accelerate disease progression. Indeed, rapid progression to acquired immunodeficiency syndrome was observed in human immunodeficiency virus individuals with a structural variant of CX3CR1 (Faure et al., 2000).

Cesarean delivery may be recommended for HIV-positive women. This also helps reduce the risk of transmission of the virus to the baby, especially when the mother receives medications. HIV may also be transmitted through breast milk. Because breast milk contains the virus, HIV-positive mothers should not breastfeed their babies.

HIV infects T cells via high-affinity interaction between the virion envelope glycoprotein (gp120) and the CD4 molecule. The infection of T cells is assisted by the T-cell co-receptor called CXCR4 while HIV infects monocytes by interacting with CCR5 co-receptor (Figure 1). As illustrated in Figure 2, after gp120 binds to CD4 on the T cell (1). Nucleocapsids containing viral genome and enzymes enters the target cell (2). Following the release of viral genome and enzymes from the core protein, viral reverse transcriptase catalyses reverse transcription of ssRNA to form RNA-DNA hybrids (3). To yield HIV dsDNA the viral RNA template is partially degraded by ribonuclease H and the second DNA strand is synthesized (4). The viral dsDNA is translocated into the nucleus and integrated into the host genome by the viral integrase enzyme (5). Transcription factors transcribe the proviral DNA into genomic ssRNA (6), which is exported to cytoplasm (7). In the cytoplasm, host-cell ribosomes catalyse synthesis of viral precursor proteins (8). The viral precursor proteins are cleaved into viral proteins by viral proteases (9). HIV ssRNA and proteins assemble beneath the host-cell plasma membrane (10) forming virion buds from it (11). Maturation occurs either in the forming buds or after budding from the host cell (12). During maturation, HIV proteases cleave the poly-proteins into individual functional HIV proteins. The mature virions are able to infect another host cell.

Programs encouraging sexual abstinence do not appear to affect subsequent HIV risk.[124] Evidence of any benefit from peer education is equally poor.[125] Comprehensive sexual education provided at school may decrease high risk behavior.[126][127] A substantial minority of young people continues to engage in high-risk practices despite knowing about HIV/AIDS, underestimating their own risk of becoming infected with HIV.[128] Voluntary counseling and testing people for HIV does not affect risky behavior in those who test negative but does increase condom use in those who test positive.[129] It is not known whether treating other sexually transmitted infections is effective in preventing HIV.[59]

People who have been exposed to HIV from a blood splash, needlestick, or sexual contact may reduce the chance of infection by taking antiretroviral drugs for 4 weeks. These drugs are more effective when they are started as soon as possible after the exposure. Taking three or more drugs is currently recommended.

Cellular: Cell-mediated immunity is a more important means of controlling the high levels of viremia (usually over 106 copies/mL) at first. But rapid mutation of viral antigens that are targeted by lymphocyte-mediated cytotoxicity subvert control of HIV in all but a small percentage of patients.

The training and qualifications of providers treating patients with HIV/AIDS is very important. If we can safely and affordably eliminate the viral reservoir, we will finally have defeated H.I.V.

The infections that occur with AIDS are called opportunistic infections because they take advantage of the opportunity to infect a weakened host. A person diagnosed with AIDS may need to be on antibiotic prophylaxis to prevent certain opportunistic infections from occurring. The infections include (but are not limited to) the following:

Certain students with AIDS may assert their right to public education under the Education for All Handicapped Children Act of 1975 (EAHCA), but the law is only relevant in cases involving special education programs. More commonly, students’ rights are protected by the Rehabilitation Act. Perhaps the most important case in this area is Thomas v. Atascadero Unified School District, 662 F. Supp. 376 (C.D. Cal.1986), which illustrates how far such protections go. Thomas involved an elementary school student with AIDS who had bitten another youngster in a fight. Based on careful review of medical evidence, the U.S. District Court for the Central District of California concluded that biting was not proved to transmit AIDS, and it ordered the school district to readmit the girl. Similarly, schools that excluded teachers with AIDS have been successfully sued on the ground that those teachers pose no threat to their students or others and that their right to work is protected by the Rehabilitation Act, as in Chalk.

One of the problems with finding a cure is that the virus can persist in cells throughout the body and potentially hide in areas that are difficult for drugs to reach, like the brain. New research is helping us understand how to effectively treat viruses in these secluded areas of the body. In addition, those infected cells that persist in the body are being studied to determine how they can be stimulated to produce virus and/or be targeted for clearance from the body by novel therapies.

HIV is spread primarily by unprotected sex (including anal and oral sex), contaminated blood transfusions, hypodermic needles, and from mother to child during pregnancy, delivery, or breastfeeding.[12] Some bodily fluids, such as saliva and tears, do not transmit HIV.[13] Methods of prevention include safe sex, needle exchange programs, treating those who are infected, and male circumcision.[5] Disease in a baby can often be prevented by giving the mother and child antiretroviral medication.[5] There is no cure or vaccine; however, antiretroviral treatment can slow the course of the disease and may lead to a near-normal life expectancy.[6][7] Treatment is recommended as soon as the diagnosis is made.[14] Without treatment, the average survival time after infection is 11 years.[15]

He introduced me to one of his patients, whom I’ll call Gordon. A tall, genial man with rimless glasses stood up to shake my hand, and I saw that he had the signature protruding belly. He has been H.I.V.-positive for almost forty years, and he said he felt lucky to be alive: “A ten-year partner of mine who had the same strain of H.I.V., who ate the same food, had the same doctors, took the same early H.I.V. meds, died in June, 1990, almost twenty-five years ago.”

Many say rejection feels most acute and painful from the institution that should offer sanctuary and support: the black church. Individual congregations, religious organizations and clerics have made strides in openness and acceptance, but in general the black church remains largely absent from and often hostile toward the L.G.B.T. community. An African-American pastor in Jackson, the Rev. Edward James of Bertha Chapel Missionary Baptist Church, became a cringe-worthy symbol of homophobia in December 2014 for his protest against same-sex marriage equality. News outlets and social-media accounts shared a photo of him in his clerical robe, holding a sign that read: “Marriage is one man and one woman. Anything else is a perversion,” next to a horse clad in a white wedding dress. “The church is someplace to go for release and spiritual comfort, but the church is actually fearful for me,” said Buckley, who, growing up, attended Baptist services with his grandmother in the delta. “Now I stay at home on Sunday. It’s too hard.”

AID Atlanta, the largest non-profit HIV healthcare organization in the Southeast transforms lives with a continuum of care that provides access, linkage, and retention to HIV care. The Agency serves over 5,000 patients yearly. AID Atlanta’s major fundraiser – AIDS Walk Atlanta 5K & Run – now in its 25th year draws nearly 10 thousand and raises about $1 million annually. With an annual budget of $7.6 million and two locations in Midtown Atlanta and Newnan, GA, AID Atlanta provides services to over 50,000 individuals per year. Both locations provide services to newly diagnosed individuals who are then linked to primary health care and a comprehensive suite of programs that improve their health outcomes, provide basic needs and address mental health issues.  AID Atlanta programs have been proven effective at improving health outcomes as measured by reduced viral loads and higher CD4 counts, the two key indicators of health for those who are HIV-positive.

Consider the drug Truvada. The drug emtricitabine-tenofovir (Truvada) can reduce the risk of sexually transmitted HIV infection in people at very high risk. You need to take it every day. It doesn’t prevent other STIs, so you’ll still need to practice safe sex. If you have hepatitis B you should be evaluated by an infectious disease or liver specialist before beginning therapy. You will need a blood test to check your kidney function before taking this drug.

The last stage of HIV infection is AIDS (acquired immunodeficiency syndrome). People with AIDS have a low number of CD4+ cells and get infections or cancers that rarely occur in healthy people. These can be deadly.

A type of protein molecule in human blood, sometimes called the T4 antigen, that is present on the surface of 65% of immune cells. The HIV virus infects cells with CD4 surface proteins, and as a result, depletes the number of immune system cells (T cells, B cells, natural killer cells, monocytes) in the individual’s blood. Most of the damage to an AIDS patient’s immune system is done by the virus’ destruction of CD4+ lymphocytes.

Taking an antiretroviral drug beforebeing exposed to HIV can reduce the risk of HIV infection. Such preventive treatment is called preexposure prophylaxis (PrEP). However, PrEP is expensive and is effective only if people take the drug every day. Thus, PrEP is recommended only for people who have a very high risk of becoming infected, such as people who have a partner who is infected with HIV.

Because HIV infection produces a wide range of symptoms, the CDC has compiled a list of conditions regarded as defining AIDS. The physician will use the CDC list to decide whether the patient falls into one of these three groups:

It is possible for HIV to become resistant to some antiretroviral medications. The best way to prevent resistance is for the patient to take their ART as directed. If the patient wants to stop a drug because of side effects, he or she should call the physician immediately.

During pregnancy or delivery or through breast-feeding. Infected mothers can pass the virus on to their babies. HIV-positive mothers who get treatment for the infection during pregnancy can significantly lower the risk to their babies.

This expensive test isn’t used for general screening. It’s for people who have early symptoms of HIV or recently had a high-risk exposure. This test doesn’t look for antibodies, but for the virus itself. It takes from seven to 28 days for HIV to be detectable in the blood. This test is usually accompanied by an antibody test. One interesting issue is that the co-receptor usage of the virus strains tends to change over time. The initial infection nearly always involves a strain that uses the chemokine receptor 5 (CCR5), which is found on macrophages and dendritic cells, as a co-receptor with CD4. People who are homozygous for deletions in the CCR5 gene (ie, CCR5-delta32) tend to be resistant to infection, [46, 47] and those with heterozygosity for the polymorphism tend to show slower progression of disease. By 1984 researchers working in Africa had provided clear evidence for heterosexual transmission of the causative agent, HIV. The virus had been isolated the year before by a team of French researchers led by virologist Luc Montagnier. Montagnier and his colleagues identified the virus as a new type of human retrovirus, and they suspected that it was the cause of AIDS. But more-detailed characterization was needed to confirm the connection, so Montagnier sent samples to American virologist Robert C. Gallo, who had contributed to the discovery of the first known human retrovirus (human T-lymphotropic virus) several years earlier. Gallo helped establish that HIV caused AIDS, and he contributed to the subsequent development of a blood test for its detection. Montagnier initially called the new infectious agent lymphadenopathy-associated virus (LAV), but in 1986 the International Committee on Taxonomy of Viruses renamed it HIV. Montagnier and French virologist Françoise Barré-Sinoussi were awarded the 2008 Nobel Prize for Physiology or Medicine for their discovery of HIV; despite Gallo’s role in confirming HIV as the cause of AIDS, Montagnier and colleagues were the first to isolate the virus.

The second problem is our uncertainty over what form protective immunity to HIV might take. It is not known whether antibodies, cytotoxic T lymphocyte responses, or both are necessary to achieve protective immunity, and which epitopes might provide the targets of protective immunity. Third, if strong cytotoxic responses are necessary to provide protection against HIV, these might be difficult to develop and sustain through vaccination. Other effective viral vaccines rely on the use of live, attenuated viruses and there are concerns over the safety of pursuing this approach for HIV. Another possible approach is the use of DNA vaccination, a technique that we discuss in Section 14-25. Both of these approaches are being tested in animal models.

In 2006, male circumcision was found to reduce the risk of female-to-male HIV transmission by 60%.81 Since then, the WHO and UNAIDS have emphasised that male circumcision should be considered in areas with high HIV and low male circumcision prevalence.82

You can help prolong your life by taking good care of yourself and developing a good relationship with an experienced doctor specializing in HIV and AIDS. Also, be consistent about taking your HIV medications as prescribed and getting regular lab work to catch any problems early.

Needle sticks or body fluid splashes among health care professionals. Transmission through theses sources accounts for fewer than 0.3% of all HIV infections in the United States. This rate reflects the emphasis on universal safety precautions (e.g., use of gloves, face shields, proper disposal of needles) among health care professionals and first responders.

One of the proteins that enters the cell with the viral genome is the viral reverse transcriptase, which transcribes the viral RNA into a complementary DNA (cDNA) copy. The viral cDNA is then integrated into the host cell genome by the viral integrase, which also enters the cell with the viral RNA. The integrated cDNA copy is known as the provirus. The infectious cycle up to the integration of the provirus is shown in Fig. 11.23. In activated CD4 T cells, virus replication is initiated by transcription of the provirus, as we will see in the next section. However, HIV can, like other retroviruses, establish a latent infection in which the provirus remains quiescent. This seems to occur in memory CD4 T cells and in dormant macrophages, and these cells are thought to be an important reservoir of infection.

Preexposure prophylaxis with antiretrovirals (PrEP): In PrEP, people who are not infected with HIV but are at high risk (eg, by having an HIV-infected sexual partner) take an antiretroviral drug daily to reduce their risk of infection. The combination of tenofovir disoproxil fumarate plus emtricitabine (TDF/FTC) can be used. Use of PrEP does not eliminate the need to use other methods of reducing risk of HIV infection, including using condoms and avoiding high-risk behaviors (eg, needle sharing). Data concerning infants of HIV-negative mothers taking TDF/FTC PrEP during pregnancy are incomplete, but currently, no adverse effects have been reported in children born to HIV-infected women treated with TDF/FTC. Use of PrEP to reduce the risk of HIV infection in injection drug users is being studied. For the current CDC recommendations, see Pre-Exposure Prophylaxis (PrEP).

One of the greatest advances in the management of HIV infection has been in pregnant women. Prior to antiviral therapy, the risk of HIV transmission from an infected mother to her newborn was approximately 25%-35%. The first major advance in this area came with studies giving ZDV after the first trimester of pregnancy, then intravenously during the delivery process, and then after delivery to the newborn for six weeks. This treatment showed a reduction in the risk of transmission to less than 10%. There is strong data that women who have viral suppression during pregnancy have very low risk of transmitting HIV to their baby. Current recommendations are to advise HIV-infected pregnant women regarding both the unknown side effects of antiviral therapy on the fetus and the promising clinical experience with potent therapy in preventing transmission. In the final analysis, however, pregnant women with HIV should be treated essentially the same as nonpregnant women with HIV. Exceptions would be during the first trimester, where therapy remains controversial, and avoiding certain drugs that may cause greater concern for fetal toxicity, such as EFV.

Complementary and alternative medicine, including Chinese medicine (CM), has been used to treat acquired immune deficiency syndrome (AIDS) for almost 30 years. We aimed to compare the main differences between AIDS treatment and evaluation strategies between CM and Western Medicine (WM), and analyze advantages and disadvantages. The characteristics of integrative medicine (IM), based on CM and WM, include a patient-centered mode of medicine based on evidence. IM focuses on complex intervention and management with systemic and individual treatment. The evaluation indexes of IM might consist of objective indicators and subjective indexes. IM might be a more valuable method for treating AIDS in the future instead of WM or CM alone.

The following is a list of AIDS-related infections and cancers that people with AIDS acquire as their CD4 count decreases. The most powerful known cause of innate human immunodeficiency virus resistance is CCR5Δ32, a mutant allele, coding for a truncated inactive form of CCR5 (Dean et al., 1996; Dragic et al., 1996; Huang et al., 1996; Liu et al., 1996; Michael et al., 1997; Samson et al., 1996; Zimmerman et al., 1997). CX3CR1 that recognizes ABCD-3 is a recently identified human immunodeficiency virus coreceptor too (Combadiere et al., 1998; Reeves et al., 1997; Rucker et al., 1997). CX3CR1 interacts only with a limited number of human immunodeficiency virus envelopes, and ABCD-3 can efficiently block human immunodeficiency virus coreceptor activity of CX3CR1 (Combadiere et al., 1998). That CX3CR1 functions as a human immunodeficiency virus coreceptor suggests that nucleotide polymorphic variations of it may slow or accelerate disease progression. Indeed, rapid progression to acquired immunodeficiency syndrome was observed in human immunodeficiency virus individuals with a structural variant of CX3CR1 (Faure et al., 2000).
