Scientists suspect the simian immunodeficiency virus (SIV) jumped from chimps to humans when people consumed infected chimpanzee meat. Once inside the human population, the virus mutated into what we now know as HIV.

Several classes of antiretroviral drugs are used together to treat HIV infection. These drugs block HIV from entering human cells or block the activity of one of the enzymes HIV needs to replicate inside human cells and/or integrate its genetic material into human DNA.

Testing and diagnosis of HIV-exposed infants has been a challenge. For infants and children less than 18 months of age, serological testing is not sufficient to identify HIV infection – virological testing must be provided (at 6 weeks of age, or as early as birth) to detect the presence of the virus in infants born to mothers living with HIV. However, new technologies are now becoming available to perform the test at the point of care and enable return of the result on the same day to accelerate appropriate linkage and treatment initiation.

Modern HIV testing is extremely accurate. A single screening test is correct more than 99% of the time.[108][needs update] The chance of a false-positive result in standard two-step testing protocol is estimated to be about 1 in 250,000 in a low risk population.[108] Testing post-exposure is recommended immediately and then at six weeks, three months, and six months.[109]

The disease usually spreads through the inhalation of infectious drops from coughs and can be transmitted easily to immune- compromised patients, including patients with acquired immune deficiency syndrome (AIDS) and human immuno-deficiency virus (HIV) infection.

Consider the drug Truvada. The drug emtricitabine-tenofovir (Truvada) can reduce the risk of sexually transmitted HIV infection in people at very high risk. need to take it every day. It doesn’t prevent other STIs, so you’ll still need to practice safe sex. If you have hepatitis B you should be evaluated by an infectious disease or liver specialist before beginning therapy. You will need a blood test to check your kidney function before taking this drug.

New technologies to help people test themselves are being introduced, with many countries implementing self-testing as an additional option to encourage HIV diagnosis. HIV self-testing is a process whereby a person who wants to know his or her HIV status collects a specimen, performs a test and interprets the test results in private or with someone they trust. HIV self-testing does not provide a definitive HIV-positive diagnosis – instead, it is an initial test which requires further testing by a health worker.

In 2011, HPTN 052, a study of 1,763 couples in 13 cities on four continents funded by the National Institute of Allergy and Infectious Diseases, found that people infected with H.I.V. are far less likely to infect their sexual partners when put on treatment immediately instead of waiting until their immune systems begin to fall apart. This “test and treat” strategy also significantly reduces the risk of illness and death. The data was so persuasive that the federal government began pushing new H.I.V./AIDS treatment guidelines to health care providers the following year. And in 2012, the Food and Drug Administration approved the preventive use of Truvada, in the form of a daily pill to be taken as pre-exposure prophylaxis (commonly called PrEP). It has been found to be up to 99 percent effective in preventing people who have not been infected with H.I.V. from contracting the virus, based on the results of two large clinical trials; an estimated 80,000 patients have filled prescriptions over the past four years.

Use a new condom every time you have sex. Use a new condom every time you have anal or vaginal sex. Women can use a female condom. If using lubricant, make sure it’s water-based. Oil-based lubricants can weaken condoms and cause them to break. During oral sex use a nonlubricated, cut-open condom or a dental dam — a piece of medical-grade latex.

However, viruses are highly antigenic. HIV needs the integrase enzyme to infect T cells. This drug prevents that step. Integrase inhibitors are often used in the first line of treatment because they are effective for many people, and cause minimal side effects. Integrase inhibitors include elvitegravir (Vitekta), dolutegravir (Tivicay), and raltegravir (Isentress)

Early advances in preventing HIV transmission resulted from educational programs describing how transmission occurs and providing barrier protection for those exposed to genital secretions and new needles or bleach to those exposed to blood by sharing needles. Despite these efforts, new infection in both the developed and developing worlds has continued at high rates.

Macrophages. Tissue macrophages are one of the target cells for HIV. These macrophages harbour the virus and are known to be the source of viral proteins. However, the infected macrophages are shown to lose their ability to ingest and kill foreign microbes and present antigen to T cells. This could have a major contribution in overall immune dysfunction caused by HIV infection.

Protease inhibitors (PIs) interrupt virus replication at a later step in the HIV life cycle, preventing cells from producing new viruses. Currently, these include ritonavir (Norvir), darunavir (Prezista), and atazanavir (Reyataz). Using PIs with NRTIs reduces the chances that the virus will become resistant to medications. Atazanavir and darunavir are available in combination with cobicistat as atazanavir/cobicistat (Evotaz) and darunavir/cobicistat (Prezcobix). Cobicistat and ritonavir inhibit the breakdown of other drugs, so they are used as boosters to reduce the number of pills needed.

HIV progressively destroys some types of white blood cells called CD4+ lymphocytes. Lymphocytes help defend the body against foreign cells, infectious organisms, and cancer. Thus, when HIV destroys CD4+ lymphocytes, people become susceptible to attack by many other infectious organisms. Many of the complications of HIV infection, including death, usually result from these other infections and not from HIV infection directly.

The HIV enzyme reverse transcriptase converts the viral RNA into DNA, which is compatible to human genetic material, when the virus is inside the cell. This DNA is transported to the cell’s nucleus, where it is spliced into human DNA by the HIV enzyme integrase. The HIV DNA is known as provirus after it is integrated.

You don’t actually “get” AIDS. You might get infected with HIV, and later you might develop AIDS. You can get infected with HIV from anyone who’s infected, even if they don’t look sick and even if they haven’t tested HIV-positive yet. Patients with AIDS have had their immune system depleted by HIV and are very susceptible to such opportunistic infections. Common symptoms are fevers, sweats (particularly at night), swollen glands, chills, weakness, and weight loss.

The O (“Outlier”) group is not usually seen outside of West-central Africa. It is reportedly most common in Cameroon, where a 1997 survey found that about 2% of HIV-positive samples were from Group O.[13] The group caused some concern because it could not be detected by early versions of the HIV-1 test kits. More advanced HIV tests have now been developed to detect both Group O and Group N.[14]

Once HIV has entered the cell, it can replicate intracellularly and kill the cell in ways that are still not completely understood. In addition to killing some lymphocytes directly, the AIDS virus disrupts the functioning of the remaining immune system cells. Because the immune system cells are destroyed, a wide variety of infections and cancers can take advantage of a person’s weakened immune system (opportunistic infections/diseases).

However, with effective treatment, the HIV RNA level decreases to undetectable levels, CD4 counts increase dramatically, and people can continue to lead productive, active lives. The risk of illness and death decreases but remains higher than that of people who are of similar age and who are not infected with HIV. However, if people cannot tolerate or take drugs consistently, HIV infection and immune deficiency progresses, causing serious symptoms and complications.

The ethical underpinning of this opposition is that it is not felt to be in the best interest of the child to be born to a parent who may not be available for continued child-rearing. In addition, the risk of mother-to-infant transmission places the infant at risk of acquiring a highly debilitating illness. Yet as stated previously, HIV infection currently is a manageable chronic illness with a life-expectancy equivalent to that with many other chronic diseases for which assisted reproductive technology is not routinely precluded. Further, interventions, such as antiretroviral therapy or cesarean delivery or both, reduce the absolute risk of transmission to a level comparable, again, to risks significantly lower than those tolerated among couples choosing assisted reproductive technology (eg, parents who are carriers of autosomal recessive conditions) or risks often assumed as part of assisted reproductive technology (eg, risks of prematurity from multiple pregnancies).

Some conspiracy theories have been put about. Operation INFEKTION was a worldwide Soviet active measures operation to spread the claim that the United States had created HIV/AIDS. Surveys show that a significant number of people believed – and continue to believe – in such claims.[13]

Estimation of current incidence of HIV is difficult. A back-calculation analysis (a statistical method using incubation period to project future distribution of infection) suggests there has been little change in HIV incidence in MSM over recent years. If there has been a decrease in transmissibility associated with antiretroviral treatment in those diagnosed it may have been offset by an increase in risky behaviours. In 2012, there were 2,300-2,500 new infections annually and 7,200 MSM undiagnosed.[5]London has been the main focus of the HIV epidemic in the UK. Of those MSM receiving HIV care in 2012, 50% lived in London.[7]

In other respects, health care is a distinct area of concern for AIDS patients and health professionals alike. Discrimination has often taken place. State and federal statutes, including the Rehabilitation Act, guarantee access to health care for AIDS patients, and courts have upheld that right. In the 1988 case of Doe v. Centinela Hospital, 57 U.S.L.W. 2034 (C.D. Cal.), for example, an HIV-infected person with no symptoms was excluded from a federally funded hospital residential program for drug and alcohol treatment because health care providers feared exposure to the virus. The case itself exposed the irrationality of such discrimination. Although its employees had feared HIV, the hospital argued in court that the lack of symptoms meant that the patient was not disabled and thus not protected by the Rehabilitation Act. A federal trial court in California rejected this argument, ruling that a refusal to grant services based solely on fear of contagion is discrimination under the Rehabilitation Act.

Everybody knows everybody else in Jackson’s small, tight-knit black gay community, and most men will find their sexual partners in this network. Most scientists now believe that risk of contracting H.I.V. boils down to a numbers game rather than a blame game: If the virus is not present in your sexual network, you can have unprotected sex and not get infected. But if you are in a community, like Jackson, where a high percentage of gay and bisexual men are infected with H.I.V. — and many don’t know it and go untreated — any unprotected sexual encounter becomes a potential time bomb. This explanation of “viral load” helps dispel the stubbornly held notion that gay and bisexual black men have more sex than other men, a false perception embedded in the American sexual imagination and fueled by stereotypes of black men as hypersexual Mandingos dating back to slavery.

At the same time, it is important to recognise that reaching an undetectable viral load is determined by many factors, including treatment adherence, HIV resistance to certain anti-retroviral drugs, stigma, and inadequate health systems.[119]

Because of the great efficacy of the protease inhibitors, it is possible to learn much about the kinetics of HIV replication in vivo by measuring the decline in viremia after the initiation of protease inhibitor therapy. For the first 2 weeks after starting treatment there is an exponential fall in plasma virus levels with a half-life of viral decay of about 2 days (Fig. 11.26). This phase reflects the decay in virus production from cells that were actively infected at the start of drug treatment, and indicates that the half-life of productively infected cells is similarly about 2 days. The results also show that free virus is cleared from the circulation very rapidly, with a half-life of about 6 hours. After 2 weeks, levels of virus in plasma have dropped by more than 95%, representing an almost total loss of productively infected CD4 lymphocytes. After this time, the rate of decline of plasma virus levels is much slower, reflecting the very slow decay of virus production from cells that provide a longer-lived reservoir of infection, such as dendritic cells and tissue macrophages, and from latently infected memory CD4 T cells that have been activated. Very long-term sources of infection might be CD4 memory T cells that continue to carry integrated provirus, and virus stored as immune complexes on follicular dendritic cells. These very long-lasting reservoirs of infection might prove to be resistant to drug therapy for HIV.

Programs to prevent the vertical transmission of HIV (from mothers to children) can reduce rates of transmission by 92–99%.[77][134] This primarily involves the use of a combination of antiviral medications during pregnancy and after birth in the infant and potentially includes bottle feeding rather than breastfeeding.[77][140] If replacement feeding is acceptable, feasible, affordable, sustainable, and safe, mothers should avoid breastfeeding their infants; however exclusive breastfeeding is recommended during the first months of life if this is not the case.[141] If exclusive breastfeeding is carried out, the provision of extended antiretroviral prophylaxis to the infant decreases the risk of transmission.[142] In 2015, Cuba became the first country in the world to eradicate mother-to-child transmission of HIV.[143]

Sexual contact. People at greatest risk are those who do not practice safer sex by always using a condom, those who have multiple sexual partners, those who participate in anal intercourse, and those who have sex with a partner who has HIV infection and/or other sexually transmitted diseases (STDs). In the United States and Europe, most cases of sexually transmitted HIV infection result from homosexual contact, whereas in Africa, the disease is spread primarily through sexual intercourse among heterosexuals. Most people with AIDS in the United States are between 25 and 44 years of age.

Risk of transmission from infected health care practitioners who take appropriate precautions is unclear but appears minimal. In the 1980s, one dentist transmitted HIV to ≥ 6 of his patients by unknown means. However, extensive investigations of patients cared for by other HIV-infected physicians, including surgeons, have uncovered few other cases.

Numerous young homosexual men presented with opportunistic infections that, at the time, were typically associated with severe immune deficiency: Pneumocystis pneumonia (PCP) and aggressive Kaposi sarcoma. Initially, some researchers referred to the syndrome as gay-related immune deficiency (GRID), since it appeared to be limited to homosexuals. In the media the disease commonly was referred to as the “gay plague.” But the disease had also been detected in intravenous drug users, who became infected mainly by sharing contaminated hypodermic needles. It also had been observed in women with male sexual partners. As a result, the term acquired immunodeficiency syndrome, or AIDS, was introduced to describe the disease; the CDC published its first report using the term in 1982.

The fourth problem is the ability of the virus to persist in latent form as a transcriptionally silent provirus, which is invisible to the immune system. This might prevent the immune system from clearing the infection once it has been established. In summary, the ability of the immune system to clear infectious virus remains uncertain.

A few exceptional patients can control their HIV strain without treatment; they maintain normal CD4 counts and very low blood levels of HIV (long-term nonprogressors) or normal CD4 counts and undetectable blood levels of HIV (elite controllers). These patients may not require ART, but studies to determine whether treating them is helpful have not been done and would be difficult because there are few of these patients and they would likely do well not taking ART for long periods.

There are theoretical reasons why patients identified with HIV around the time they are first infected (primary, acute infection) may benefit from the immediate initiation of potent antiviral therapy. Preliminary evidence suggests that unique aspects of the body’s immune response to the virus may be preserved by this strategy. It is thought that treatment during the primary infection may be an opportunity to help the body’s natural defense system to work against HIV. Thus, patients may gain improved control of their infection while on therapy and perhaps even after therapy is stopped. At one time, the hope was that if therapy was started very early in the course of the infection, HIV could be eradicated. Most evidence today, however, suggests that this is not the case, although research will certainly continue in the coming years in this area. In addition, recent data demonstrated that a subset those starting ART within the first weeks of infection were able to stop therapy after many years and maintain good viral control off treatment. While this response does not occur in the majority of similarly treated patients, the observations are intriguing and an area of ongoing research. Regardless, at least for now it is premature to think that early treatment may result in a cure, although other benefits may still exist, including avoiding the substantial damage to the immune system that occurs during the first weeks of infection. In addition, these individuals have very high levels of virus in their blood and genital secretions, and early treatment might reduce their risk of transmitting HIV to others. There also is evidence that those who develop such symptoms during the early days of infection may be at greater risk of disease progression than those who become infected with minimal or no symptoms. Due to the absence of definitive data, guidelines vary, but since it is now recommended that all patients initiate therapy at the time of diagnosis it is generally recommended that patients with primary infection be offered early therapy.

As a consequence of its high variability, HIV rapidly develops resistance to antiviral drugs. When antiviral drugs are administered, variants of the virus that carry mutations conferring resistance to their effects emerge and expand until former levels of plasma virus are regained. Resistance to some of the protease inhibitors appears after only a few days (Fig. 11.27). Resistance to some of the nucleoside analogues that are potent inhibitors of reverse transcriptase develops in a similarly short time. By contrast, resistance to the nucleoside zidovudine (AZT), the first drug to be widely used for treating AIDS, takes months to develop. This is not because AZT is a more powerful inhibitor, but because resistance to zidovudine requires three or four mutations in the viral reverse transcriptase, whereas a single mutation can confer resistance to the protease inhibitors and other reverse-transcriptase inhibitors. As a result of the relatively rapid appearance of resistance to all known anti-HIV drugs, successful drug treatment might depend on the development of a range of antiviral drugs that can be used in combination. It might also be important to treat early in the course of an infection, thereby reducing the chances that a variant virus has accumulated all the necessary mutations to resist the entire cocktail. Current treatments follow this strategy and use combinations of viral protease inhibitors together with nucleoside analogues (see Fig. 11.26).

Effective chemoprophylaxis is available for many opportunistic infections and reduces rates of disease due to P. jirovecii, Candida, Cryptococcus, and MAC. If therapy restores CD4 counts to above threshold values for > 3 mo, chemoprophylaxis can be stopped.

Mother-to-child transmission is the most common way that children become infected with HIV. HIV medicines, given to women with HIV during pregnancy and childbirth and to their babies after birth, reduce the risk of mother-to-child transmission of HIV. 

Clinical findings Weight loss exceeding 10% of body weight, protracted asthenia, continuous fever for >1 month, diarrhoea >1 month, persistent cough, oropharyngeal candidiasis, relapsing cutaneous herpes, generalised pruritic dermatosis, generalised lymphadenopathy, Kaposi’s sarcoma.

The longer diagnosis delay among non-white racial/ethnic groups might partly reflect the higher proportion of infections attributable to heterosexual contact among these groups compared with whites (14), given that heterosexual persons had longer diagnosis delays. Among all transmission categories, males with infection attributed to heterosexual contact had the longest median diagnosis delay (4.9 years). This observation was consistent with the finding that heterosexual males at increased risk for infection were less likely to report testing in the past 12 months than were heterosexual females at increased risk. Heterosexual men are less likely to visit a health care provider than are both women and MSM, leading to fewer opportunities for testing (15). Moreover, compared with other risk groups, heterosexual persons at increased risk were less likely to have been offered an HIV test even when visiting a health care provider in the past 12 months, possibly because of low perceived risk for infection (15,16). This finding highlights the importance of implementing routine screening in health care settings.

These patients of Sturdevant’s are the faces of one of America’s most troubling public-health crises. Thanks to the success of lifesaving antiretroviral medication pioneered 20 years ago and years of research and education, most H.I.V.-positive people today can lead long, healthy lives. In cities like New York and San Francisco, once ground zero for the AIDS epidemic, the virus is no longer a death sentence, and rates of infection have plummeted. In fact, over the past several years, public-health officials have championed the idea that an AIDS-free generation could be within reach — even without a vaccine. But in certain pockets of the country, unknown to most Americans, H.I.V. is still ravaging communities at staggering rates.

Rapid screening tests: These tests are being increasingly used to detect antibodies because they are quicker and simpler than ELISA, can be done in almost any setting, and provide immediate results. These tests can be done using a sample of blood or saliva in a doctor’s office.

HIV progressively destroys some types of white blood cells called CD4+ lymphocytes. Lymphocytes help defend the body against foreign cells, infectious organisms, and cancer. Thus, when HIV destroys CD4+ lymphocytes, people become susceptible to attack by many other infectious organisms. Many of the complications of HIV infection, including death, usually result from these other infections and not from HIV infection directly.

HIV most often spreads through unprotected sex with an infected person. It may also spread by sharing drug needles or through contact with the blood of an infected person. Women can give it to their babies during pregnancy or childbirth.

The second phase of HIV infection, the asymptomatic period, lasts an average of 10 years. During that period the virus continues to replicate, and there is a slow decrease in the CD4 count (the number of helper T cells). When the CD4 count falls to about 200 cells per microlitre of blood (in an uninfected adult it is typically about 1,000 cells per microlitre), patients begin to experience opportunistic infections—i.e., infections that arise only in individuals with a defective immune system. That is AIDS, the final stage of HIV infection. The most-common opportunistic infections are Pneumocystis carinii pneumonia, tuberculosis, Mycobacterium avium infection, herpes simplex infection, bacterial pneumonia, toxoplasmosis, and cytomegalovirus infection. In addition, patients can develop dementia and certain cancers, including Kaposi sarcoma and lymphomas. Death ultimately results from the relentless attack of opportunistic pathogens or from the body’s inability to fight off malignancies.

A fusion inhibitor blocks an early step in the viral life cycle. Enfuvirtide (Fuzeon, T-20) attaches to the envelope surrounding the virus and prevents it from entering the CD4 cells. This prevents the infection of CD4 cells by HIV. T-20 is the first approved drug in this class. It is given as a twice-daily subcutaneous injection (90 mg). It is used primarily in individuals who have developed resistance to other classes of drugs in order to create a new potent combination. Like all other antivirals, it is most useful in those taking other active drugs at the same time in order to optimize the chance of getting viral loads to undetectable levels and to prevent the development of drug resistance.

Sheen rose to the top again with “Two and a Half Man,” playing free-spirited jingle writer Charlie Harper. The show was one of the highest-rated on television, and Sheen soon became the highest-paid actor on TV, eventually making close to $2 million an episode. But a rehab stint shut down production in 2010, and he and show creator Chuck Lorre were soon at loggerheads. Sheen was fired after the eighth season.

Other major factors in the early days of AIDS were injection drug use (IDU) through needle sharing and transfusions of blood and blood components. Numerous hemophiliacs and surgical patients were infected through tranfusions before the ability to test for the virus in donated blood became available.

For example, the Catholic church does not want people to have or use condoms.[5] They do not want people to have condoms because they do not think people should have sex unless they are married. They also think that married people should not use condoms, because they believe that if people have sex, they should be prepared to accept a possible pregnancy.

Developing AIDS requires that the person acquire HIV infection. Risks for acquiring HIV infection include behaviors that result in contact with infected blood or sexual secretions, which pose the main risk of HIV transmission. These behaviors include sexual intercourse and injection drug use. The presence of sores in the genital area, like those caused by herpes, makes it easier for the virus to pass from person to person during intercourse. HIV also has been spread to health care workers through accidental sticks with needles contaminated with blood from HIV-infected people, or when broken skin has come into contact with infected blood or secretions. Blood products used for transfusions or injections also may spread infection, although this has become extremely rare (less than one in 2 million transfusions in the U.S.) due to testing of blood donors and blood supplies for HIV. Finally, infants may acquire HIV from an infected mother either while they are in the womb, during birth, or by breastfeeding after birth.

Without treatment, risk of progression to AIDS is about 1 to 2%/yr in the first 2 to 3 yr of infection and about 5 to 6%/yr thereafter. Eventually, AIDS almost invariably develops in untreated patients.

From a legal, ethical, and moral standpoint, they should warn any prospective sexual partner of their HIV positive status. They should not exchange body fluids during sexual activity and must use whatever preventative measures (such as a latex condom) will afford the partner the most protection.

Practising safer sex – use condoms and water based lubricants for penetrative sex. These reduce the risk of getting HIV, as well as other sexually transmitted infections (STIs). Having any STI increases the risk of getting HIV infection.

ABSTRACT: Early diagnosis and treatment of human immunodeficiency virus (HIV) can improve survival and reduce morbidity. The Centers for Disease Control and Prevention and the American College of Obstetricians and Gynecologists recommend that females aged 13–64 years be tested at least once in their lifetime and annually thereafter based on factors related to risk. In addition, obstetrician–gynecologists should annually review patients’ risk factors for HIV and assess the need for retesting. The opportunity for repeat testing should be made available to all women even in the absence of identified risk factors. Women who are infected with HIV should receive or be for appropriate clinical and supportive care. Obstetrician–gynecologists who use rapid tests must be prepared to provide counseling to women who receive positive test results the same day that the specimen is collected. Obstetrician–gynecologists should be aware of and comply with legal requirements regarding HIV testing in their jurisdictions and institutions.

In the United States, guidelines for using antiviral therapy have been developed and are updated on a regular basis by an expert panel assembled by the DHHS, the IAS-USA panel, and others. Cultural factors (e.g., stigma, fear, discrimination, and homophobia) might contribute to longer diagnosis delays in some populations (12). Asians accounted for the highest percentage of persons living with undiagnosed HIV infection compared with all other race/ethnicity groups (13). Although blacks were more likely than whites to report testing in the past 12 months across all groups at risk, the median diagnosis delay was 1 year longer for blacks (median = 3.3 years) than for whites (median = 2.2 years). The testing results might reflect national efforts to improve access to testing among blacks, and black MSM in particular, through prevention programs and media campaigns. In 2007, CDC launched the Expanded Testing Initiative (https://www.cdc.gov/hiv/policies/eti.html) to facilitate HIV diagnosis and linkage to care among blacks and continues to support high levels of testing. CDC’s MSM Testing Initiative (https://www.researchgate.net/publication/287201580) scaled up HIV testing and linkage-to-care activities among black and Hispanic or Latino MSM in 11 cities. In addition, CDC implemented Testing Makes Us Stronger (https://www.cdc.gov/actagainstaids/campaigns/tmus), a public education campaign to increase testing among black MSM, from 2011 to 2015.

Teaching young people about AIDS is an enormously popular idea. Since the late 1980s, Gallup Polls have revealed that over 90 percent of respondents think public schools should do so. Agreement ends there, however. In the 1990s, more angry debate focused on AIDS education than on any issue facing schools since court-ordered busing in the 1970s. The core question of the debate is simple: What is the best way to equip students to protect themselves from this fatal disease? The answers may be miles apart. For one side, “equipping” means advocating the only sure means of protection, sexual and drug abstinence. For the other, it means supporting abstinence along with knowledge of sexual practices, the use of clean drug needles, and the use of prophylactics (condoms), which are distributed in some schools. Between these positions lie a great many issues of disagreement that have bitterly divided school districts, provoked lawsuits, and cost high-ranking Washington, D.C., officials their jobs.

Each year about 5 million people contract AIDS worldwide, and 3 million die of it. Some 40-50 million are estimated to be living with the disease. The gender incidence is approximately equal. The highest prevalence is in some African countries, where as many as 25% of the adult population may test HIV positive; about 70% of the world’s infected population lives in sub-Saharan Africa. The first cases of AIDS were reported in the U.S. in June 1981. During the succeeding 2 decades an estimated 1.4 million people in this country were infected with HIV and 816,149 cases of AIDS and 467,910 deaths were reported to the U.S. Centers for Disease Control and Prevention (CDC). The numbers of new AIDS cases and deaths declined substantially after introduction of combination antiretroviral therapy in the late 1990s. The annual number of new cases of AIDS in the U.S. has remained stable at about 40,000, with 16,000 deaths since 1998. The number of people infected with HIV continues to increase, and of an estimated 1 million, one fourth are unaware that they are infected. In the U.S., AIDS is the leading cause of death among men 25-44 years old, and the fourth leading cause of death among women in the same age group. The development of effective antiretroviral agents (for example, reverse transcriptase inhibitors and protease inhibitors) and of quantitative plasma HIV RNA assays that can monitor progression of disease and response to treatment has shifted the goal of management in AIDS from prophylaxis and treatment of opportunistic infections to achievement of remission through suppressive therapy. Immune compromise is monitored by serial CD4 counts, viral replication by plasma HIV RNA assay (that is, plasma viral load, PVL). Indications for starting antiretroviral therapy are the appearance of symptoms of opportunistic infection, decline of the CD4 count below 350/mm3, or viral load exceeding 30,000 copies/mL. The CD4 count is considered a more sensitive predictor of disease progression than viral load. Empiric treatment may be begun early (within 6 months after conversion to HIV-positive status) in an effort to preserve immune function and mobilize the patient’s own defenses against the virus. But current guidelines advise deferring treatment as long as possible so as to limit induction of drug resistance. Protease inhibitors have been shown to be highly effective antiretroviral agents and standard treatment regimens combining 2 reverse transcriptase inhibitors with 1 protease inhibitor (“triple therapy”) have clearly demonstrated superiority over monotherapy. These drugs are expensive. Regimens are often complex, with varying requirements for fasting and timing of doses, and adverse effects and drug interactions are common. Protease inhibitors have been associated with elevation of cholesterol and triglycerides, insulin resistance, and disfiguring lipodystrophy. In one large study, more than one half of HIV-infected adults under treatment were found to be infected with strains of virus resistant to one or more antiretroviral drugs, and strains of HIV that are resistant to all available protease inhibitors have appeared. The rationale for current AIDS regimens is an effort to eradicate HIV infection by inhibiting spread of virus to new cells until all infected cells have died. However, actual cure seldom if ever occurs. A small number of resting CD4 memory cells in treated patients with undetectable plasma HIV RNA levels harbor HIV proviral DNA capable of replication, and these cells may survive for months or years. Macrophages and CNS neurons may serve as an anatomic sanctuary for HIV into which antretroviral drugs cannot penetrate in adequate concentration. When antiretroviral therapy is initiated early, CD4 helper cell counts rise, CD4 cell activity is preserved, and HIV RNA levels may remain undetectable for long periods. But in about 50% of patients with advanced disease, even multidrug regimens fail to suppress plasma viral RNA to undetectable levels. Many treatment failures result from poor compliance with multidrug regimens. Failure of one therapeutic regimen often precludes success with others because of the high degree of cross-resistance among antiretroviral drugs. After failure of an initial regimen, genotypic testing can be used to identify mutations in the HIV genome that confer resistance to one or more classes of HIV drugs. Many patients remain vulnerable to opportunistic infections despite restoration of CD4 counts to normal, probably because some subpopulations of T cells have been annihilated and cannot be recovered even after HIV has been suppressed. Moreover, even HIV-infected patients with undetectable viral loads must still be considered infectious. In a small set of those infected with HIV, impairment of immunity progresses to AIDS slowly or not at all. CD8 T-cells from such nonprogressors have been found to produce proteins called α-defensins. Evolving standards of treatment in HIV disease include aggressive prophylaxis in pregnancy and after accidental needle stick and sexual assault. Administration of antiretroviral agents to HIV-positive mothers before birth and during labor and delivery, and to newborns for the first 6 weeks of life, markedly decrease the risk of vertical transmission of HIV infection. The risk of HIV infection after occupational parenteral exposure to blood from an HIV-infected patient is approximately 0.3%. Postexposure prophylaxis with antiretroviral agents continued for 28 days have been shown to reduce the risk by 80%. The selection of agents depends on the source patient’s therapeutic history. Efforts to develop a vaccine against HIV have been hampered by the unique properties of the virus and the long incubation period of AIDS. Early in the 21st century, public health authorities sought to make HIV testing a routine part of medical care, to facilitate diagnosis outside formal clinical settings, to prevent new infections by educating people and their sexual partners, and to decrease perinatal HIV transmission through routine HIV testing of pregnant women and of infants whose mothers were not screened.

Compliance with medications is important to provide the best outcome for mother and child. Even though a physician might highly recommend a medication regimen, the pregnant woman has a choice of whether or not to take the medicines. Studies have shown that compliance is improved when there is good communication between the woman and her doctor, with open discussions about the benefits and side effects of treatment. Compliance also is improved with better social support, including friends and relatives.

As the disease progresses, both women and men may experience yeast infections on the tongue (thrush), and women may develop severe vaginal yeast infections or pelvic inflammatory disease. Shingles is often seen early on, often before someone is diagnosed with HIV.

Dealing with the potential consequences of bias and discrimination – job loss, homelessness, lack of healthcare insurance – often results in LGBTQ people engaging in behaviors that facilitate the spread of HIV. For example, in the face of persistent employment discrimination, many transgender women are left with few other options but to engage in survival sex work in order to meet their most basic needs. According to a 2015 survey of more than 27,000 transgender people, “The rate of HIV [diagnosis] was…five times higher among those who have participated in sex work at any point in their lifetime” than among those who have not.

Abnormal elevation of immune activation may be caused in part by absorption of components of bowel bacteria. Immune activation contributes to CD4+ depletion and immunosuppression by mechanisms that remain unclear.

Sexual contact. People at greatest risk are those who do not practice safer sex by always using a condom, those who have multiple sexual partners, those who participate in anal intercourse, and those who have sex with a partner who has HIV infection and/or other sexually transmitted diseases (STDs). In the United States and Europe, most cases of sexually transmitted HIV infection result from homosexual contact, whereas in Africa, the disease is spread primarily through sexual intercourse among heterosexuals. Most people with AIDS in the United States are between 25 and 44 years of age.

Blood transmission — the risk of transmitting HIV through blood transfusion is extremely low in developed countries, thanks to meticulous screening and precautions. However, among people who inject drugs, sharing and reusing syringes contaminated with HIV-infected blood is extremely hazardous.

HIV can be transmitted from mother to child during pregnancy, during delivery, or through breast milk, resulting in the baby also contracting HIV.[76][77] This is the third most common way in which HIV is transmitted globally.[12] In the absence of treatment, the risk of transmission before or during birth is around 20% and in those who also breastfeed 35%.[77] As of 2008, vertical transmission accounted for about 90% of cases of HIV in children.[77] With appropriate treatment the risk of mother-to-child infection can be reduced to about 1%.[77] Preventive treatment involves the mother taking antiretrovirals during pregnancy and delivery, an elective caesarean section, avoiding breastfeeding, and administering antiretroviral drugs to the newborn.[78] Antiretrovirals when taken by either the mother or the infant decrease the risk of transmission in those who do breastfeed.[79] However, many of these measures are not available in the developing world.[78] If blood contaminates food during pre-chewing it may pose a risk of transmission.[74]

Many governments and research institutions participate in HIV/AIDS research. This research includes behavioral health interventions such as sex education, and drug development, such as research into microbicides for sexually transmitted diseases, HIV vaccines, and antiretroviral drugs. Other medical research areas include the topics of pre-exposure prophylaxis, post-exposure prophylaxis, and circumcision and HIV.

If people at low risk have a negative test result, the screening test is not repeated unless their risk status changes. If people at the highest risk have a negative test result (especially if they are sexually active, have several partners, or do not practice safe sex), testing should be repeated every 6 to 12 months.

In June 2001, the United Nations (UN) General Assembly called for the creation of a “global fund” to support efforts by countries and organisations to combat the spread of HIV through prevention, treatment and care including buying medication.73

In people with unmasked IRIS, doctors treat the newly identified opportunistic infection with antimicrobial drugs. Occasionally, when the symptoms are severe, corticosteroids are also used. Usually, when unmasked IRIS occurs, cART is continued. An exception is when a cryptococcal infection affects the brain. Then cART is temporarily interrupted until the infection is controlled.

HIV/AIDS research includes all medical research that attempts to prevent, treat, or cure HIV/AIDS, as well as fundamental research about the nature of HIV as an infectious agent and AIDS as the disease caused by HIV.

HIV attacks the body’s immune system, specifically the CD4 cells (T cells), which help the immune system fight off infections. Untreated, HIV reduces the number of CD4 cells (T cells) in the body, making the person more likely to get other infections or infection-related cancers. Over time, HIV can destroy so many of these cells that the body can’t fight off infections and disease. These opportunistic infections or cancers take advantage of a very weak immune system and signal that the person has AIDS, the last stage of HIV infection.

HIV/Aids is caused by the Human Immunodeficiency Virus (HIV). HIV is mainly transmitted through sexual intercourse, but can also be passed down from mother to child, acquired via blood transfusion with infected blood, or other methods. Once a person is infected, the virus remains in the body for life. There is no cure for HIV/Aids, but there are drugs that help control the virus, enabling people with symptoms of HIV to live full and healthy lives. The PrEP Heroes aims to increase awareness of drugs that prevent HIV from establishing itself if a person is exposed. “Being a part of the PrEP Hero campaign was important because it was an opportunity to show diversity in communities where HIV and LGBT intersect,” Franco De Marco said.

During this time, many scientists, researchers and government administrators were afraid to speak openly about condoms, needle exchange and L.G.B.T. issues for fear of reprisal and loss of funding. Community organizations became targets of anti-gay crusades, subjected to intense scrutiny, including exhaustive audits, by federal agencies. “It is no coincidence that new rates of H.I.V. infection among gay men, especially gay black men, began to spike sharply from 2000 on, because of an anti-science campaign that allowed for little or nothing to be done for a maligned community simply due to ideology and bigotry,” Millett said. “The hostile environment made funding effective H.I.V.-prevention programs, messages or research impossible for U.S. communities most impacted by H.I.V.”

In 1981, cases of a rare lung infection called Pneumocystis carinii pneumonia (PCP) were found in five young, previously healthy gay men in Los Angeles.2 At the same time, there were reports of a group of men in New York and California with an unusually aggressive cancer named Kaposi’s Sarcoma.3

HIV infection is not spread by casual contact (such as hugging and touching), by touching dishes, doorknobs, or toilet seats previously touched by a person infected with the virus, during participation in sports, or by mosquitoes.

Exposure to HIV does not always lead to infection, and some people who have had repeated exposures over many years remain uninfected. Moreover, many HIV-infected people remain well for more than a decade. A very few HIV-infected, untreated people have remained well for over 20 years. Why some people become ill so much sooner than others is not fully understood, but a number of genetic factors appear to influence both susceptibility to infection and progression to AIDS after infection.

HIV infection does not immediately cause AIDS, and the issues of how it does, and whether all HIV-infected patients will progress to overt disease, remain controversial. Nevertheless, accumulating evidence clearly implicates the growth of the virus in CD4 T cells, and the immune response to it, as the central keys to the puzzle of AIDS. HIV is a worldwide pandemic and, although great strides are being made in understanding the pathogenesis and epidemiology of the disease, the number of infected people around the world continues to grow at an alarming rate, presaging the death of many people from AIDS for many years to come. Estimates from the World Health Organization are that 16.3 million people have died from AIDS since the beginning of the epidemic and that there are currently around 34.3 million people alive with HIV infection (Fig. 11.19), of whom the majority are living in sub-Saharan Africa, where approximately 7% of young adults are infected. In some countries within this region, such as Zimbabwe and Botswana, over 25% of adults are infected. (AIDS in Mother and Child, in Case Studies in Immunology, see Preface for details)

The US Centers for Disease Control and Prevention (CDC) estimates that about 1.3 million  people are living with HIV infection or AIDS; about 15% of them do not know they have it. About 73 percent of the 56,000 new infections each year are in men and about 27 percent are in women. About half of the new infections are in Blacks, even though they make up only 12 percent of the US population. In the mid-1990s, AIDS was a leading cause of death. Screening of blood and organs: Transmission by blood transfusion is still remotely possible in the US because antibody results may be false-negative during early infection. Currently, screening blood for antibody and p24 antigen is mandated in the US and probably further reduces risk of transmission. Risk is reduced further by asking people with risk factors for HIV infection, even those with recent negative HIV antibody test results, not to donate blood or organs for transplantation. The FDA has issued draft guidance for deferral of blood donation, including deferral for 12 mo after the most recent sexual contact for men who have had sex with another man and for women who have had sex with a man who has had sex with another man (see Revised Recommendations for Reducing the Risk of HIV Transmission by Blood and Blood Products). However, use of sensitive HIV screening tests and deferral of donors of organs, blood, and blood products have not been implemented consistently in developing countries.

“At this point the virus is moving into the blood stream and starting to replicate in large numbers,” says Carlos Malvestutto, MD, instructor of infectious diseases and immunology in the department of medicine at NYU School of Medicine in New York City. “As that happens, there is an inflammatory reaction by the immune system.”

Counseling for pregnant women:Mother-to-child transmission has been virtually eliminated by HIV testing, treatment with ART, and, in developed countries, use of breast milk substitutes. If pregnant women test positive for HIV, risk of mother-to-child transmission should be explained. Pregnant women who do not accept immediate treatment for their HIV infection should be encouraged to accept therapy to protect the unborn baby, typically beginning at about 14 wk gestation. Combination therapy is typically used because it is more effective than monotherapy and less likely to result in drug resistance. Some drugs can be toxic to the fetus or woman and should be avoided. If women meet criteria for ART, they should begin a regimen tailored to their history and stage of pregnancy and continue it throughout pregnancy. Cesarean delivery can also reduce risk of transmission. Regardless of the antepartum regimen used or mode of delivery, all HIV-infected women should be given IV zidovudine during labor, and after birth, neonates should be given oral zidovudine, which is continued for 6 wk after delivery (see also Prevention of Perinatal Transmission). Some women choose to terminate their pregnancy because HIV can be transmitted in utero to the fetus or for other reasons.

Activated cells that become infected with HIV produce virus immediately and die within one or two days. The vast majority of viruses present in the plasma can be attributed to the short-lived, activated cells. It takes approximately 1.5 days to complete a single HIV life-cycle. Resting cells that become infected produce virus only after immune stimulation and these cells have a half-life of at least 5-6 months. Because viral reproduction is almost completely carried out by host cell mechanisms, there are few points in the process where stopping viral reproduction will not also kill host cells. For this reason there are no chemotherapeutic agents for most viral diseases. acyclovir is an antiviral that requires viral proteins to become active. Some viral infections can be prevented by vaccination (active immunization), and others can be treated by passive immunization with immune globulin, although this has been shown to be effective against only a few dozen viruses.

HIV is one of a group of viruses known as retroviruses. After getting into the body, the virus enters many different cells, incorporates its genes into the human DNA, and hijacks the cell to produce HIV virus. Most importantly, HIV attacks cells of the body’s immune system called CD4 or T-helper cells (T cells). These cells are destroyed by the infection. The body tries to keep up by making new T cells or trying to contain the virus, but eventually the HIV wins out and progressively destroys the body’s ability to fight infections and certain cancers. The virus structure has been studied extensively, and this ongoing research has helped scientists develop new treatments for HIV/AIDS. Although all HIV viruses are similar, small variations or mutations in the genetic material of the virus create drug-resistant viruses. Larger variations in the viral genes are found in different viral subtypes. Currently, HIV-1 is the predominant subtype that causes HIV/AIDS. HIV-2, another form of HIV, occurs almost exclusively in West Africa.

These sub-epidemics each follow their own pattern, although there is some degree of interdependence. Early on, nearly all cases of HIV infection detected in the Western Hemisphere were in homosexual men, but the spread of the disease to female partners of bisexual men with HIV infection gave rise to an increased rate among heterosexual persons.

Not everyone who has HIV have AIDS. When people first get HIV, they can be healthy for years. A person is diagnosed as having AIDS when he or she gets specific types of illnesses or gets sick in certain ways due to their HIV. Once a person’s HIV progresses to (or turns into) AIDS, the person will continue to have AIDS for the rest of their life. While there are many treatments for HIV/AIDS, at this point there is no cure. Nathan King wants to help fight the stigma associated with PrEP. “Unlike many medical breakthroughs and preventive strategies, PrEP, and its users, faced criticism from the beginning,” he said. “People who used the medication are stigmatized and stereotyped, rather than supported for taking steps to protect the health of themselves and their communities.”

Despite the persistent anti-L.G.B.T. stigma and entrenched social and economic issues that cling to the South, Sturdevant feels a complicated, bone-deep tie to the people and the place. When he encourages his “sons” and “daughters” to take care of themselves and others, he is echoing the love and acceptance he received from his own large family. After years of hiding, when he came out to his mother in his 20s, she told him, “I love you regardless.” When his family eventually found out that he was sick, his mother and sister drove up to where he was living in Memphis, along with six carloads of aunts, uncles, nieces, nephews and cousins. They tried to serve him plates laden with down-home food that he was too ill to eat and did their best to love him back to health. In the hospital, he finally admitted to his mother he had AIDS. “She told me, ‘Boy, you gonna be all right; God got you,’ ” he recalls, tearing up. In the end, they took him home. He moved back to his mother’s house in Metcalfe, with somebody from the sprawling network of nearly 100 family members always close by, until he recovered. “They saved my life, and I’ll never forget that,” he said.

These drugs, also referred to as “nukes,” interfere with HIV as it tries to replicate and make more copies of itself. NRTIs include abacavir (Ziagen), lamivudine/zidovudine (Combivir), and emtricitabine (Emtriva)

TB, or tuberculosis, is a disease caused by bacteria called Mycobacterium tuberculosis that can affect anyone at any age. The bacteria usually attacks the lungs. Particular groups of individuals, however, are shown to be at a higher risk of acquiring the disease than others. These include HIV/AIDS patients, individuals in close contact with TB patients, diabetics, individuals with suppressed immune systems, foreign-born individuals in countries with high TB incidences, healthcare workers, alcoholics, and others. Symptoms of the disease include a persistent cough, fatigue, weight loss, fever, coughing blood, and sweating at night. When an infected individual coughs or sneezes, others nearby are at risk for breathing in the bacteria.

In 1991, the Visual AIDS Artists Caucus launched the Red Ribbon Project to create a symbol of compassion for people living with HIV and their carers. The red ribbon became an international symbol of AIDS awareness.51

Sheen told Lauer that he had unprotected sex “under the care of my doctor” with two women since his diagnosis, but that it was “impossible” that he had transferred the virus to them. While Huizenga did not agree that it’s “impossible,” he did say it was highly unlikely.

Vaccines against HIV have been difficult to develop because HIV surface proteins mutate easily, resulting in an enormous diversity of antigenic types. Nonetheless, various vaccine candidates are under study, and a few have shown promise in clinical trials. At the present time, there is no effective AIDS vaccine.

Human T-cell lymphotropic virus type III; a cytopathic retrovirus that is 100-120 nm in diameter, has a lipid envelope, and has a characteristic dense cylindric nucleoid containing core proteins and genomic RNA; two types exist: HIV-1 infects only humans and chimpanzees and is more virulent than HIV-2, which is more closely related to Simian or monkey viruses. HIV-2 is found primarily in West Africa. It is the etiologic agent of acquired immunodeficiency syndrome (AIDS).

Federal and state programs are also hampered by policy decisions grounded in ideology rather than science such as the allocation of more than $1 billion to failed abstinence-only sex education programs or the enactment of outdated HIV criminalization statutes. In more than 30 states, people living with HIV can be tried and imprisoned simply because a partner accuses them of withholding their HIV status. There’s no proof these laws work, and they run counter to public health by perpetuating stigma and subsequently deterring people from getting tested or treated for HIV.

In general, most antiviral regimens for HIV disease contain a backbone of at least two NRTIs. The NRTIs include zidovudine (Retrovir, ZDV), stavudine (Zerit, d4T), didanosine (Videx, ddI), zalcitabine (HIVID, ddC), lamivudine (Epivir, 3TC), emtricitabine (Emtriva, FTC), abacavir (Ziagen, ABC), tenofovir disoproxil fumarate (Viread, TDF), and tenofovir alafenamide (Descovy, TAF). The latter drug is a new formulation of tenofovir that has become available as tenofovir alafenamide (TAF) as part of multiple fixed-dose combinations. This form of tenofovir has been shown to be equally effective as TDF but with less renal and bone toxicity. The NRTIs FTC and 3TC are highly related compounds and, although data is somewhat limited, most experts agree that they probably can be used interchangeably. That said, many combinations of NRTIs can be used together, with current guidelines generally recommending the fixed-dose combination of TDF with FTC (Truvada), or TAF with FTC (Descovy), both of which are also available as part of single tablet regimens. An alternative regimen uses the fixed-dose combination of ABC/3TC (Epzicom) alone or combined as a single tablet regimen with dolutegravir (Triumeq). ABC has been associated with severe allergic reactions in approximately 5% of patients. Recent studies have shown that a blood test (HLA-B*5701) can be performed to determine who is at risk for this reaction so that the drug can be avoided in these individuals and be used in others with greater confidence that there will not be such a reaction. In fact, when available, it is now the standard of care to perform this test prior to initiation of ABC. The main side effects associated with TDF are reduced kidney function and bone density.

At least once a week, I am asked by one of my HIV-infected patients whether they need to continue to practice safe sex if they are in a monogamous (one mate only) relationship with an HIV-infected partner. Put another way, since both partners already have HIV, what’s the harm of unprotected sex? Actually, this is not an easy question to answer fully.

There has been a great deal of attention given to the more recently identified problem of “lipodystrophy.” Individuals suffering from this syndrome can be categorized as having lipohypertrophy (fat accumulation) syndromes, such as the “buffalo hump” on the back of the neck, breast enlargement, or increased abdominal girth. Others primarily suffer from lipoatrophy with fat loss under the skin with complaints of prominent veins on the arms and legs, sunken cheeks, and decreased gluteal (buttock) size. These syndromes appear to be related to multiple factors, including, but not limited to, drug therapy. The NRTIs appear to be most closely linked to lipoatrophy, in particular D4T and to a lesser extent ZDV. In fact, some studies have suggested slow accumulation of fat in those who modify the NRTI component of their regimen. Some NRTIs also have been linked to elevation in lipid (fat) levels in the blood. While switching therapy is always a consideration in those experiencing potential drug-related toxicity, this should only be done under the careful supervision of an experienced HIV provider.

Cost is another concern associated with protease inhibitors. To be effective, protease inhibitors must be used in combination with at least two other anti-HIV drugs. Annual costs for this treatment ranges between $12,000-$15,000 per person. Those persons without private health insurance must rely on public programs such as the AIDS Drug Assistance Program (ADAP), a federally funded initiative to provide AIDS-related drugs to people with HIV. Most ADAP programs, which are administered by states, have lacked the funding to enroll everyone in need.

Epidemics have no single answer beyond a cure. Since no cure for AIDS existed as of the early 2000s, the law continued to grapple with a vast number of problems. The federal government has addressed AIDS in two broad ways: by spending money on research and treatment of the disease and by prohibiting unfairness to people with HIV or AIDS. It has funded medical treatment, research, and public education, and it has passed laws prohibiting discrimination against people who are HIV-positive or who have developed AIDS. States and local municipalities have joined in these efforts, sometimes with federal help. In addition, states have criminalized the act of knowingly transmitting the virus through sexual behavior or blood donation. The courts, of course, are the decision makers in AIDS law. They have heard a number of cases in areas that range from employment to education and from crimes to torts. Although a body of case law has developed, it remains relatively new with respect to most issues and controversial in all.

Cytomegalovirus. This common herpes virus is transmitted in body fluids such as saliva, blood, urine, semen and breast milk. A healthy immune system inactivates the virus, and it remains dormant in your body. If your immune system weakens, the virus resurfaces — causing damage to your eyes, digestive tract, lungs or other organs.

The course of HIV infection involves three stages: primary HIV infection, the asymptomatic phase, and AIDS. During the first stage the transmitted HIV replicates rapidly, and some persons may experience an acute flulike illness that usually persists for one to two weeks. During that time a variety of symptoms may occur, such as fever, enlarged lymph nodes, sore throat, muscle and joint pain, rash, and malaise. Standard HIV tests, which measure antibodies to the virus, are initially negative, because HIV antibodies generally do not reach detectable levels in the blood until a few weeks after the onset of the acute illness. As the immune response to the virus develops, the level of HIV in the blood decreases.

The prognosis in patients with untreated HIV infection is poor, with an overall mortality rate of more than 90%. The average time from infection to death is 8-10 years, although individual variability ranges from less than 1 year to long-term nonprogression. Many variables have been implicated in HIV’s rate of progression, including CCR5-delta32 heterozygosity, mental health, [78] concomitant drug or alcohol abuse, superinfection with another HIV strain, nutrition, and age.

In 2009, a newly analyzed HIV sequence was reported to have greater similarity to a simian immunodeficiency virus recently discovered in wild gorillas (SIVgor) than to SIVs from chimpanzees (SIVcpz). HIV/AIDS has had a great impact on society, both as an illness and as a source of discrimination.[22] The disease also has large economic impacts.[22] There are many misconceptions about HIV/AIDS such as the belief that it can be transmitted by casual non-sexual contact.[23] The disease has become subject to many controversies involving religion including the Catholic Church’s position not to support condom use as prevention.[24] It has attracted international medical and political attention as well as large-scale funding since it was identified in the 1980s.[25]

Acquired immunodeficiency syndrome (AIDS) is the most advanced stage of HIV infection. Guidelines for starting antiviral therapy have been proposed by panels of experts from several groups, including the DHHS (https://aidsinfo.nih.gov/) and IAS-USA. There are similar guidelines for treatment throughout Europe and by the World Health Organization for treatment in resource-limited countries. Until recently a recommendation supporting the start of therapy in those with CD4 cells greater than 500 cells was based upon evidence that ongoing viral replication, even in the setting of high CD4 cell counts, may be associated with damage to the brain, kidneys, heart, and possibly even liver. Along with this rationale, it was clear that newer regimens were easy to take, including a growing number of one-pill-per-day options, with minimal side effects. Another compelling argument that can be made for early therapy is the ability to reduce the risk of transmission to uninfected partners. A study called HPTN 052 demonstrated that amongst couples where one person is HIV-infected and the other is not, those who were on antiretroviral therapy were 96% less likely to transmit HIV to their uninfected partner than those not on treatment. Finally, a large study was recently reported that demonstrated unequivocally that starting therapy even with a CD4 cell count of greater than 500 cells/mm3 was associated with less risk of disease progression than waiting until CD4 cells were less than 350 cells/mm3. This study was called the START study and demonstrated a major reduction in disease progression with early therapy with virtually no increased risk for side effects. Based upon START, HPTN 052 and other accumulated data, currently all major guidelines around the world, including those of the World Health Organization recommend that antiretroviral therapy be initiated in all HIV-infected patients at the time of diagnosis. It is worth noting that these recommendations for universal treatment of HIV-infected patients will be limited by resources available for antiviral treatment in resource-limited countries. The next year, two research teams—one led by Luc Montagnier and Françoise Barré-Sinoussi, of the Pasteur Institute, in Paris, the other by Robert Gallo, at the National Cancer Institute, in Maryland—published papers in Science that described a new retrovirus in the lymph nodes and blood cells of AIDS patients. A retrovirus has a pernicious way of reproducing: it permanently inserts a DNA copy of its genome into the nucleus of a host cell, hijacking the cell’s machinery for its own purposes. When the retrovirus mutates, which it often does, its spawn becomes difficult for the body or a vaccine to target and chase out. Retroviral diseases were widely believed to be incurable. In May of 1986, after much dispute about credit for the discovery (the French finally won the Nobel, in 2008), an international committee of scientists agreed on the name H.I.V., or human immunodeficiency virus. By the end of that year, about twenty-five thousand of the nearly twenty-nine thousand Americans with reported AIDS diagnoses had died.

The viral load of an infected person is an important risk factor in both sexual and mother-to-child transmission.[60] During the first 2.5 months of an HIV infection a person’s infectiousness is twelve times higher due to this high viral load.[58] If the person is in the late stages of infection, rates of transmission are approximately eightfold greater.[53] An HIV-positive person who has an undetectable viral load as a result of long-term treatment has effectively no risk of transmitting HIV sexually.[61]

Consistent condom use reduces the risk of HIV transmission by approximately 80% over the long term.[110] When condoms are used consistently by a couple in which one person is infected, the rate of HIV infection is less than 1% per year.[111] There is some evidence to suggest that female condoms may provide an equivalent level of protection.[112] Application of a vaginal gel containing tenofovir (a reverse transcriptase inhibitor) immediately before sex seems to reduce infection rates by approximately 40% among African women.[113] By contrast, use of the spermicide nonoxynol-9 may increase the risk of transmission due to its tendency to cause vaginal and rectal irritation.[114]

In patients with HIV infection, certain syndromes are common and may require different considerations (see Table: Common Manifestations of HIV Infection by Organ System). Some patients present with cancers (eg, Kaposi sarcoma, B-cell lymphomas) that occur more frequently, are unusually severe, or have unique features in patients with HIV infection (see Cancers Common in HIV-Infected Patients). In other patients, neurologic dysfunction may occur.

Medications that fight HIV are called antiretroviral medications. Different antiretroviral medications target the virus in different ways. When used in combination with each other, they are very effective at suppressing the virus. It is important to note that there is no cure for HIV. ART only suppresses reproduction of the virus and stops or delays the disease from progressing to AIDS. Most guidelines currently recommend that all HIV-infected people who are willing to take medications should have them initiated shortly after being diagnosed with the infection. This delays or prevents disease progression, improves overall health of an infected person, and makes it less likely that they will transmit the virus to their partners.

Mutations that occur as HIV replicates can allow variants of the virus to escape recognition by antibody or cytotoxic T cells and can contribute to the failure of the immune system to contain the infection in the long term. Direct escape of virus-infected cells from killing by cytotoxic T lymphocytes has been shown by the occurrence of mutations of immunodominant viral peptides presented by MHC class I molecules. In other cases, variant peptides produced by the virus have been found to act as antagonists (see Section 6-12) for T cells responsive to the wild-type epitope, thus allowing both mutant and wild-type viruses to survive. Mutant peptides acting as antagonists have also been reported in hepatitis B virus infections, and similar mutant peptides might contribute to the persistence of some viral infections, especially when, as often happens, the immune response of an individual is dominated by T cells specific for a particular epitope.

Programs to prevent the vertical transmission of HIV (from mothers to children) can reduce rates of transmission by 92–99%.[77][134] This primarily involves the use of a combination of antiviral medications during pregnancy and after birth in the infant and potentially includes bottle feeding rather than breastfeeding.[77][140] If replacement feeding is acceptable, feasible, affordable, sustainable, and safe, mothers should avoid breastfeeding their infants; however exclusive breastfeeding is recommended during the first months of life if this is not the case.[141] exclusive breastfeeding is carried out, the provision of extended antiretroviral prophylaxis to the infant decreases the risk of transmission.[142] In 2015, Cuba became the first country in the world to eradicate mother-to-child transmission of HIV.[143]

Sub-Saharan Africa is the region most affected. In 2010, an estimated 68% (22.9 million) of all HIV cases and 66% of all deaths (1.2 million) occurred in this region.[209] This means that about 5% of the adult population is infected[210] and it is believed to be the cause of 10% of all deaths in children.[211] Here in contrast to other regions women compose nearly 60% of cases.[209] South Africa has the largest population of people with HIV of any country in the world at 5.9 million.[209] Life expectancy has fallen in the worst-affected countries due to HIV/AIDS; for example, in 2006 it was estimated that it had dropped from 65 to 35 years in Botswana.[19] Mother-to-child transmission, as of 2013, in Botswana and South Africa has decreased to less than 5% with improvement in many other African nations due to improved access to antiretroviral therapy.[212]

When HIV infection is advanced, either through treatment failure or in untreated infection, and has caused immune system destruction, secondary infections (opportunistic infections) can occur. Earlier-generation enzyme-linked immunosorbent assay (ELISA) antibody assays are highly sensitive, but because they do not test for antigen, they are not positive as early as the 4th-generation combination test. Asymptomatic, mild-to-moderate cytopenias (eg, leukopenia, anemia, thrombocytopenia) are also common. Some patients experience progressive wasting (which may be related to anorexia and increased catabolism due to infections) and low-grade fevers or diarrhea.

The term viral tropism refers to the cell types a virus infects. HIV can infect a variety of immune cells such as CD4+ T cells, macrophages, and microglial cells. Practising safer sex – use condoms and water based lubricants for penetrative sex. These reduce the risk of getting HIV, as well as other sexually transmitted infections (STIs). Having any STI increases the risk of getting HIV infection.

While switching therapy is always a consideration in those experiencing potential drug-related toxicity, this should only be done under the careful supervision of an experienced HIV provider.

PrEP is short for pre-exposure prophylaxis. People who do not have HIV can take a daily pill to reduce their risk of becoming infected. PrEP is not right for everyone and must still be used in combination with safer sex and injection practices. It requires commitment to treatment and does not replace other prevention measures like condom use. It also requires very regular medical visits and frequent blood tests for STDs and HIV, because unknowingly continuing PrEP medication while HIV-infected can lead to resistance and limit HIV treatment options. Resistance has already been reported in a person who became infected while taking PrEP.

Opportunistic infections may be caused by bacteria, viruses, fungi, and parasites that are normally controlled by the immune system.[35] Which infections occur depends partly on what organisms are common in the person’s environment.[28] These infections may affect nearly every organ system.[36]

Most people infected by HIV develop a flu-like illness within a month or two after the virus enters the body. This illness, known as primary or acute HIV infection, may last for a few weeks. Possible signs and symptoms include:

A transmissible retrovirus that causes AIDS in humans. Two forms of HIV are now recognized: HIV-1, which causes most cases of AIDS in Europe, North and South America, and most parts of Africa; and HIV-2, which is chiefly found in West African patients. HIV-2, discovered in 1986, appears to be less virulent than HIV-1 and may also have a longer latency period.

The RNA genome consists of at least seven structural landmarks (LTR, TAR, RRE, PE, SLIP, CRS, and INS), and nine genes (gag, pol, and env, tat, rev, nef, vif, vpr, vpu, and sometimes a tenth tev, which is a fusion of tat, env and rev), encoding 19 proteins. Three of these genes, gag, pol, and env, contain information needed to make the structural proteins for new virus particles.[21] For example, env codes for a protein called gp160 that is cut in two by a cellular protease to form gp120 and gp41. The six remaining genes, tat, rev, nef, vif, vpr, and vpu (or vpx in the case of HIV-2), are regulatory genes for proteins that control the ability of HIV to infect cells, produce new copies of virus (replicate), or cause disease.[21]

In order for a person to be infected, HIV must be present in the transmitted body fluids, and its concentration (very high in blood) determines whether infection takes place. HIV must get into the blood stream and can only enter via an open cut or sore or by contact through the mucous membranes of the anus, rectum, genitalia, mouth or eyes. Outside the body HIV can live up to 15 days in a stable temperature and humidity, if it is in high concentration, but usually only for a short time (a few hours). It is not transmitted by insect bites, through saliva, tears, sweat, faeces or urine. There are documented cases of oral infection and male to female transmission is much more frequent than female to male. There are records of Simian immunodeficiency virus being transmitted to humans, but these have so far not given rise to the disease. The virus in chimpanzees can be transmitted but not similiar viruses from other animals.

Because many patients with AIDS have abnormally low levels of both red and white blood cells, they may be given medications to stimulate blood cell production. Epoetin alfa (erythropoietin) may be given to anemic patients. Patients with low white blood cell counts may be given filgrastim or sargramostim.
