Once infection has progressed to AIDS, the survival period is usually less than 2 years in untreated patients. Because reverse transcriptase lacks the “proofreading” function that most DNA-synthesizing enzymes have, many mutations arise as the virus replicates, further hindering the ability of the immune system to combat the virus. Those mutations allow the virus to evolve very rapidly, approximately one million times faster than the human genome evolves. That rapid evolution allows the virus to escape from antiviral immune responses and antiretroviral drugs. The next step in the virus life cycle is the integration of the viral genome into the host cell DNA. Integration at essentially any accessible site in the host genome and results in the permanent acquisition of viral genes by the host cell. Under appropriate conditions those genes are transcribed into viral RNA molecules. Some viral RNA molecules are incorporated into new virus particles, whereas others are used as messenger RNA for the production of new viral proteins. Viral proteins assemble at the plasma membrane together with the genomic viral RNA to form a virus particle that buds from the surface of the infected cell, taking with it some of the host cell membrane that serves as the viral envelope. Embedded in that envelope are the gp120/gp41 complexes that allow attachment of the helper T cells in the next round of infection. Most infected cells die quickly (in about one day). The number of helper T cells that are lost through direct infection or other mechanisms exceeds the number of new cells produced by the immune system, eventually resulting in a decline in the number of helper T cells. Physicians follow the course of the disease by determining the number of helper T cells (CD4+ cells) in the blood. That measurement, called the CD4 count, provides a good indication of the status of the immune system. Physicians also measure the amount of virus in the bloodstream—i.e., the viral load—which provides an indication of how fast the virus is replicating and destroying helper T cells.

Many viruses cause an acute but limited infection inducing lasting protective immunity. Others, such as herpes viruses, set up a latent infection that is not eliminated but is controlled adequately by an adaptive immune response. However, infection with HIV seems rarely, if ever, to lead to an immune response that can prevent ongoing replication of the virus. We’ve come a long way from the days when diagnosis with HIV equaled a death sentence. Today, there are a variety of treatments that, when used in combination can significantly slow down and in some cases stop altogether, the progression of HIV infection.

“He was immediately put on treatment, strong antiviral drugs, which has suppressed the virus, to the point that he is absolutely healthy from that vantage,” Huizenga said. “Individuals who are optimally treated with undetectable viral loads, (the risk is) incredibly low to transmit the virus. We can’t say it’s zero, but it’s an incredibly low number.”

Most healthy people have a CD4 count of 500 to 1,000 cells per microliter of blood. Typically, the number of CD4+ lymphocytes is reduced during the first few months of infection. After about 3 to 6 months, the CD4 count stabilizes, but without treatment, it usually continues to decline at rates that vary from slow to rapid.

Following decades of inadequate funding, our nation’s public health infrastructure lacks the resources it needs to respond aggressively to the HIV and AIDS epidemic. This arrangement has been devastating for members of the LGBTQ community, since the little funding that does exist for HIV prevention, treatment, and care has not been focused on or funded in the communities most impacted by HIV. The Ryan White Care Program, for instance, has been flat funded (i.e, remained the same) since its reauthorization in 2009 despite an increasing number of people living with HIV in the U.S. coming to rely on it for medical and social suport.

There are some people who do not want people to know about condoms or clean needles. They believe that if people know about condoms and have condoms they will have more sex. They believe that if people have clean needles they will use illegal drugs more. Many of these people think this because of their religion. For example, the Catholic church does not want people to have or use condoms.[5] They do not want people to have condoms because they do not think people should have sex unless they are married. They also think that married people should not use condoms, because they believe that if people have sex, they should be prepared to accept a possible pregnancy.

A family history of primary immunodeficiency is the strongest predictor of a disorder. At birth and for only a few months, babies are partially protected from infections by antibodies transmitted to them by their mothers. Typically, the earlier the age at onset of signs of an immunodeficiency in children, the more severe the disorder. Testing can be done within the first few months, but it is also important to recognize the early signs: recurrent infections and failure to thrive. Initial laboratory screening should include a complete blood count with differential and measurement of serum immunoglobulin and complement levels.

The first available drug in this class was RAL, which is very potent at suppressing HIV in all patients who have never been on this drug or others in the class. It was initially approved for treatment-experienced patients with drug-resistant virus. It is also now approved for those starting therapy for the first time. The approved dose of RAL is 400 mg twice daily with a recently approved new formulation that can be given to those starting therapy for the first time or stably suppressed on RAL twice daily that can be given as two 600 mg tablets once daily. As noted above, a second drug in this class, EVG, is approved for use as first-line therapy as part of the fixed-dose combination pill of TDF/FTC/COBI/EVG and more recently TAF/FTC/COBI/EVG as a stand-alone drug for use in treatment-experienced patients combining it with a ritonavir-boosted PI. This drug is well tolerated and given as one pill per day, but unlike RAL it does need to be taken with food and it has interactions with other drugs since it must be used with RTV or COBI, so it must be used with caution in those on multiple medications. Another InSTI, DTG is currently recommended for those starting therapy for the first time with either TDF/FTC or ABC/3TC and is available as a fixed-dose combination of ABC/3TC/DTG that can be given as a single pill per day. This drug has a limited number of drug-drug interactions and is generally well tolerated with resistance rarely emerging in those experience virologic failure. Another InSTI in advanced stages of development is called bictegravir (BIC) that has few drug-drug interactions, is potent, well-tolerated, and can be given with or without food. It is expected to be approved as a single-tablet regimen as BIC/FTC/TAF.

RNA testing (viral load test) detects HIV RNA in the blood. It is not commonly used for screening but can be helpful in detecting early HIV infection when a person is in the window period or if the screening tests are unclear.

HIV/AIDS is a global pandemic.[207] As of 2016, approximately 36.7 million people have HIV worldwide with the number of new infections that year being about 1.8 million.[16] This is down from 3.1 million new infections in 2001.[208] Slightly over half the infected population are women and 2.1 million are children.[16] It resulted in about 1 million deaths in 2016, down from a peak of in 2005.[16]

HIV can be transmitted from mother to child during pregnancy, during delivery, or through breast milk, resulting in the baby also contracting HIV.[76][77] This is the third most common way in which HIV is transmitted globally.[12] In the absence of treatment, the risk of transmission before or during birth is around 20% and in those who also breastfeed 35%.[77] As of 2008, vertical transmission accounted for about 90% of cases of HIV in children.[77] With appropriate treatment the risk of mother-to-child infection can be reduced to about 1%.[77] Preventive treatment involves the mother taking antiretrovirals during pregnancy and delivery, an elective caesarean section, avoiding breastfeeding, and administering antiretroviral drugs to the newborn.[78] Antiretrovirals when taken by either the mother or the infant decrease the risk of transmission in those who do breastfeed.[79] However, many of these measures are not available in the developing world.[78] If blood contaminates food during pre-chewing it may pose a risk of transmission.[74]

How quickly HIV progresses through the chronic stage varies significantly from person to person. Without treatment, it can last up to a decade before advancing to AIDS. With treatment, it can last indefinitely.

As patients near the end of life, clinicians may need to prescribe drugs to relieve pain, anorexia, agitation, and other distressing symptoms. The profound weight loss in many people during the last stages of AIDS makes good skin care difficult. The comprehensive support provided by hospice programs helps many patients because hospice providers are unusually skilled at symptom management, and they support caregivers and patient autonomy.

Although most HIV-1 infected individuals have a detectable viral load and in the absence of treatment will eventually progress to AIDS, a small proportion (about 5%) retain high levels of CD4+ T cells (T helper cells) without antiretroviral therapy for more than 5 years.[28][33] These individuals are classified as HIV controllers or long-term nonprogressors (LTNP).[33] Another group consists of those who maintain a low or undetectable viral load without anti-retroviral treatment, known as “elite controllers” or “elite suppressors”. They represent approximately 1 in 300 infected persons.[34]

Keep in mind that the body hasn’t produced antibodies to HIV yet so an antibody test may not pick it up. (It can take a few weeks to a few monthsfor HIV antibodies to show in a blood test). Investigate other test options such as one that detects viral RNA, typically within nine days of infection.

CDC recommends routine testing for HIV infection for persons aged 13–64 years in health care settings and testing at least annually for persons at high risk for HIV infection (7). Yet, according to National HIV Behavioral Surveillance (NHBS), one third of gay, bisexual, and other men who have sex with men (MSM) have not been tested in the past year, with even lower percentages of recent testing reported among other population segments at high risk for HIV infection.

Dendritic cells (DCs). DCs are large cells with dendritic cytoplasmic extensions. These cells present processed antigens to T lymphocytes in lymph nodes. Epidermal DCs, expressing CD1a and Birbeck granules, are probably among the first immune cells to combat HIV at the mucosal surfaces. These cells transport HIV from the site of infection to lymphoid tissue. The follicular DCs, found in lymphoid tissue, are also key antigen-presenting cells that trap and present antigens on their cell surfaces. Hematologic disorders (eg, cytopenias, lymphomas, cancers) are common and may be usefully evaluated with bone marrow aspiration and biopsy. This procedure can also help diagnose disseminated infections with MAC, M. tuberculosis, Cryptococcus, Histoplasma, human parvovirus B19, P. jirovecii, and Leishmania. Most patients have normocellular or hypercellular marrow despite peripheral cytopenia, reflecting peripheral destruction. Iron stores are usually normal or increased, reflecting anemia of chronic disease (an iron-reutilization defect). Mild to moderate plasmacytosis, lymphoid aggregates, increased numbers of histiocytes, and dysplastic changes in hematopoietic cells are common.

You don’t actually “get” AIDS. You might get infected with HIV, and later you might develop AIDS. You can get infected with HIV from anyone who’s infected, even if they don’t look sick and even if they haven’t tested HIV-positive yet. The blood, vaginal fluid, semen, and breast milk of people infected with HIV has enough of the virus in it to infect other people. Most people get the HIV virus by:

Many people with HIV do not know they are infected. In the United States, it is likely that 14% of HIV-positive individuals are unaware of their infection. HIV infection progresses in three very general stages.

Political denial and inaction have also likely caused considerable damage. Several governments in countries with high HIV infection rates were slow to admit that they had an HIV epidemic, and at least one (South Africa) initially rejected that AIDS was even a problem, then that the disease was caused by HIV infection, and, most recently, that antiretroviral therapy was effective in treating HIV infection and preventing MTCT. Changes have now occurred but have been slow and have cost hundreds of thousands of lives.

Data reported to CDC’s National HIV Surveillance System from 50 states and the District of Columbia through June 2017 were used to estimate the total number of persons living with HIV infection (diagnosed and undiagnosed infection, or prevalence) at year-end 2015 and the median number of years and interquartile range between infection and diagnosis (diagnosis delay) of persons with HIV diagnosed in 2015 (8,9). The first CD4 test after HIV diagnosis and a CD4 depletion model indicating disease progression were used to estimate year of infection and the distribution of time from HIV infection to diagnosis among persons with diagnosed infection (9). The distribution of diagnosis delay was used to estimate the annual number of HIV infections, which includes persons with diagnosed infection and persons with undiagnosed infection. HIV prevalence (persons with diagnosed or undiagnosed HIV infection) was estimated by subtracting reported cumulative deaths among persons with HIV infection from cumulative HIV infections.

Taking HAART therapy is very manageable yet isn’t necessarily easy. These drugs must be taken at the right time, every single day. Also, a range of side effects may occur, including: diarrhea, nausea, rash, vivid dreams, or abnormal distribution of body fat. And, especially if medications are taken incorrectly or inconsistently, the virus can mutate, or change, into a strain resistant to treatment. The good news is that there are now several HIV medications that are only taken once a day. If there is resistant virus, however, these may not work and other medication options must be used.

A. there are no effective natural remedy for HIV. the medications are very hard ones that try to control the virus from spreading (cannot eliminate it though). no herbal remedy or nutrition change will do that.

Current HAART options are combinations (or “cocktails”) consisting of at least three medications belonging to at least two types, or “classes,” of antiretroviral agents.[149] Initially treatment is typically a non-nucleoside reverse transcriptase inhibitor (NNRTI) plus two nucleoside analog reverse transcriptase inhibitors (NRTIs).[150] Typical NRTIs include: zidovudine (AZT) or tenofovir (TDF) and lamivudine (3TC) or emtricitabine (FTC).[150] Combinations of agents which include protease inhibitors (PI) are used if the above regimen loses effectiveness.[149]

As the disease progresses, both women and men may experience yeast infections on the tongue (thrush), and women may develop severe vaginal yeast infections or pelvic inflammatory disease. Shingles is often seen early on, often before someone is diagnosed with HIV.

human T-cell lymphotropic virus type III; a cytopathic retrovirus (genus Lentvirus, family Retroviridae) that is 100-120 nm in diameter, has a lipid envelope, and has a characteristic dense cylindric nucleoid containing core proteins and genomic RNA. There are currently two types: HIV-1 infects only humans and chimpanzees and is more virulent than HIV-2, which is more closely related to Simian or monkey viruses. HIV-2 is found primarily in West Africa and is not as widespread as HIV-1. In addition to the usual gene associated with retroviruses, this virus has at least six genes that regulate its replication. It is the etiologic agent of acquired immunodeficiency syndrome (AIDS). Formerly or also known as the lymphadenopathy virus (LAV) or the human T-cell lymphotropic virus type III (HTLV-III). HIV is different in structure from other retroviruses. It is roughly spherical[19] with a diameter of about 120 nm, around 60 times smaller than a red blood cell.[20] It is composed of two copies of positive-sense single-stranded RNA that codes for the virus’s nine genes enclosed by a conical capsid composed of 2,000 copies of the viral protein p24.[21] The single-stranded RNA is tightly bound to nucleocapsid proteins, p7, and enzymes needed for the development of the virion such as reverse transcriptase, proteases, ribonuclease and integrase. A matrix composed of the viral protein p17 surrounds the capsid ensuring the integrity of the virion particle.[21]

The Centers for Disease Control reported cases of Pneumocystis carinii pneumonia and Kaposi’s sarcoma in otherwise healthy young male homosexuals in 1981. Until then, pneumocystis carinii was mainly known to occur in immunodepressed patients after organ transplants or suffering from congenital immunodeficiencies. Soon thereafter, the same condition was seen in IV drug abusers, haemophilliacs and babies of IV drug abusing mothers. These patients had profound immunosuppression due to the depletion of T4 helper lymphocytes and the name ‘acquired immunodeficiency’ was coined for this syndrome. Epidemiological studies have now established that the disease is infectious and can be transmitted by sexual intercourse, blood or blood products. The lymphocytes of patients died early, creating a difficulty in isolating the virus. Montagnier and Gallo eventually isolated the virus in 1984 and HIV-2 was isolated in 1986 from West Africa. There is less evidence that treatment of HIV-2 infection slows progression, and certain antiretroviral medications (specifically the non-nucleoside–analogue reverse-transcriptase inhibitors) are not effective against HIV-2. The HIV-1 viral-load assays are much less reliable at quantifying HIV-2, if they work at all. HIV-2 viral load assays have been developed, but none has been approved by the US Food and Drug Administration except as blood donor–screening tools.

Bangui definition A points-based system used to define AIDS in countries where HIV testing is not available. It was developed by workers from the CDC and WHO a conference held in Bangui, Central African Republic, in 1985, and gives the most points for severe weight loss, protracted asthenia, recalcitrant fever and diarrhoea. AIDS is diagnosed with scores of 12 or more.

When initially infected, many people have no noticeable symptoms, but within 1 to 4 weeks, fever, rashes, sore throat, swollen lymph nodes, fatigue, and a variety of less common symptoms develop in some people. Symptoms of initial (primary) HIV infection last from 3 to 14 days.

First of all, there is no evidence that people infected with HIV can be cured by the currently available therapies, although research related to curing people of infection will be discussed later. In general, those who are treated for years and are repeatedly found to have no virus in their blood by standard viral load assays will experience a prompt rebound in the number of viral particles when therapy is discontinued. Consequently, the decision to start therapy must balance the risk versus the benefits of treatment. The risks of therapy include the short- and long-term side effects of the drugs, described in subsequent sections, as well as the possibility that the virus will become resistant to the therapy, which can limit options for future treatment. The risks of both of these problems are quite small with the treatment options currently available.

Major advancements in HIV prevention, treatment, and care have put an AIDS-free generation squarely within reach. HIV tests are faster and more reliable than ever before. HIV medications are safer and more effective, and there are now several ways to prevent the spread of HIV, including condoms and Pre-Exposure Prophylaxis (PrEP). PrEP is an HIV prevention strategy that currently involves taking a once daily-pill called Truvada ®. When taken as prescribed, PrEP is safe and highly effective at preventing people from becoming HIV-positive.

HIV is a retrovirus. That is, it stores its genetic information as ribonucleic acid (RNA). Once inside a CD4+ lymphocyte, the virus uses an enzyme called reverse transcriptase to make a copy of its RNA, but the copy is made as deoxyribonucleic acid (DNA). HIV mutates easily at this point because reverse transcriptase is prone to making errors during the conversion of HIV RNA to DNA. These mutations make HIV more difficult to control because the many mutations increase the chance of producing HIV that can resist attacks by the person’s immune system and/or antiretroviral drugs.

The HIV virion enters macrophages and CD4+ T cells by the adsorption of glycoproteins on its surface to receptors on the target cell followed by fusion of the viral envelope with the target cell membrane and the release of the HIV capsid into the cell.[55][56]

The topic of religion and AIDS has become highly controversial in the past twenty years, primarily because some religious authorities have publicly declared their opposition to the use of condoms.[261][262] The religious approach to prevent the spread of AIDS according to a report by American health expert Matthew Hanley titled The Catholic Church and the Global AIDS Crisis argues that cultural changes are needed including a re-emphasis on fidelity within marriage and sexual abstinence outside of it.[262]

HIV-1 originated in Central Africa in the first half of the 20th century, when a closely related chimpanzee virus first infected humans. Epidemic global spread began in the late 1970s, and AIDS was recognized in 1981.

Because live-virus vaccines are potentially dangerous for patients with severe immunosuppression, expert opinion should be sought when dealing with patients at risk of primary varicella; recommendations vary (see Human Immunodeficiency Virus (HIV) Infection in Infants and Children : Vaccination and Considerations for Use of Live Vaccines in Children With HIV Infection).

Diagnosis is made through a blood test that screens specifically for the virus. If HIV has been found, the test result is “positive.” The blood is re-tested several times before a positive result is given.

Post-exposure prophylaxis (PEP) is the use of ARV drugs within 72 hours of exposure to HIV in order to prevent infection. PEP includes counselling, first aid care, HIV testing, and administration of a 28-day course of ARV drugs with follow-up care. WHO recommends PEP use for both occupational and non-occupational exposures and for adults and children.

Tuberculosis is diagnosed with the Mantoux test, in which a small sample of tuberculin is placed under the skin in an arm, and if a bump persists in the area, the individual may have TB. If the person is suspected positive for TB, the doctor may advise a chest x-ray and a mucous analysis as a follow-up. The treatment and prognosis vary for individuals who are TB infected and individuals who are experiencing symptoms of the TB disease. Various drugs therapies are used to treat both individuals. Tuberculosis can be cured if treated well. The best way to prevent TB is to treat and cure people who have it.

After many years of research, an untested HIV vaccine has been created.[113] Bi-specific antibodies, that target both the surface of T-cells and viral epitopes, can prevent entry of the virus into human cells.[114] Another group has utilised the same technology to develop a bi-specific antibody that neutralises viral particles by cross-linking of envelope glycoproteins.[115]

In place of effective sex education, these programs often discouraged condom use while teaching abstinence as the only way to prevent the spread of AIDS — even as well-regarded research established that this kind of sex education does not lower the risk of contracting H.I.V. and other sexually transmitted diseases.

Sturdevant, born and raised in Metcalfe, a tiny Mississippi Delta town of about 1,000, understands all too well the fear, stigma and isolation that can come with being a black gay man in the South. “Growing up, I was taught that God was not fixing to forgive a person who was homosexual,” Sturdevant said. “The Bible supposedly said you’re going straight to hell, automatically, there’s no forgiveness. There were several times I thought about suicide. There were several times I wanted to get sick and die. Finally, my thought was, I just want to get out of here.” He moved to Dallas, and then to Memphis.

The goal is to start PEP as soon after exposure as possible if prophylaxis is warranted. CDC recommends providing PEP within 24 to 36 h after exposure; a longer interval after exposure requires the advice of an expert.

If treatment fails, drug susceptibility (resistance) assays can determine the susceptibility of the dominant HIV strain to all available drugs. Genotypic and phenotypic assays are available and can help clinicians select a new regimen that should contain at least 2 and preferably 3 drugs to which the HIV strain is more susceptible. The dominant HIV strain in the blood of patients who are taken off antiretroviral therapy may revert over months to years to the wild-type (ie, susceptible) strain because the resistant mutants replicate more slowly and are replaced by the wild type. Thus, if patients have not been treated recently, the full extent of resistance may not be apparent through resistance testing, but when treatment resumes, strains with resistance mutations often reemerge from latency and again replace the wild-type HIV strain.

An alternative view holds that unsafe medical practices in Africa after World War II, such as unsterile reuse of single use syringes during mass vaccination, antibiotic and anti-malaria treatment campaigns, were the initial vector that allowed the virus to adapt to humans and spread.[238][241][242]

The stage of symptomatic infection, in which the body’s immune (or defense) system has been suppressed and complications have developed, is called the acquired immunodeficiency syndrome (AIDS). The symptoms are caused by the complications of AIDS, which include one or more unusual infections or cancers, severe loss of weight, and intellectual deterioration (called dementia).

The rapid replication of HIV, with the generation of 109 to 1010 virions every day, coupled with a mutation rate of approximately 3 × 10-5 per nucleotide base per cycle of replication, leads to the generation of many variants of HIV in a single infected patient in the course of one day. Replication of a retroviral genome depends on two error-prone steps. Reverse transcriptase lacks the proofreading mechanisms associated with cellular DNA polymerases, and the RNA genomes of retroviruses are therefore copied into DNA with relatively low fidelity; the transcription of the proviral DNA into RNA copies by the cellular RNA polymerase is similarly a low-fidelity process. A rapidly replicating persistent virus that is going through these two steps repeatedly in the course of an infection can thereby accumulate many mutations, and numerous of HIV, sometimes called quasi-species, are found within a single infected individual. This very high variability was first recognized in HIV and has since proved to be common to the other lentiviruses.

However, clear clinical implications arose before society became aware of the disease; for example, prior to the recognition of HIV, only one case of Pneumocystis pneumonia not clearly associated with immune suppression was diagnosed in the United States between January 1976 and June 1980. In 1981 alone, 42 similar diagnoses were made, and by December 1994, 127,626 cases of Pneumocystis pneumonia with HIV infection as the only identified cause of immune suppression had been reported to the Centers for Disease Control and Prevention (CDC). Also, Kaposi sarcoma is up to 30,000 times more likely to develop in persons with HIV infection than in immunocompetent persons.

When CD4 T-cell numbers decline below a critical level, cell-mediated immunity is lost, and infections with a variety of opportunistic microbes appear (Fig. 11.29). Typically, resistance is lost early to oral Candida species and to Mycobacterium tuberculosis, which shows as an increased prevalence of thrush (oral candidiasis) and tuberculosis. Later, patients suffer from shingles, caused by the activation of latent herpes zoster, from EBV-induced B-cell lymphomas, and from Kaposi’s sarcoma, a tumor of endothelial cells that probably represents a response both to cytokines produced in the infection and to a novel herpes virus called HHV-8 that was identified in these lesions. Pneumonia caused by the fungus Pneumocystis carinii is common and often fatal. In the final stages of AIDS, infection with cytomegalovirus or Mycobacterium avium complex is more prominent. It is important to note that not all patients with AIDS get all these infections or tumors, and there are other tumors and infections that are less prominent but still significant. Rather, this is a list of the commonest opportunistic infections and tumors, most of which are normally controlled by robust CD4 T cell-mediated immunity that wanes as the CD4 T-cell counts drop toward zero (see Fig. 11.21).

The course of HIV infection involves three stages: primary HIV infection, the asymptomatic phase, and AIDS. During the first stage the transmitted HIV replicates rapidly, and some persons may experience an acute flulike illness that usually persists for one to two weeks. During that time a variety of symptoms may occur, such as fever, enlarged lymph nodes, sore throat, muscle and joint pain, rash, and malaise. Standard HIV tests, which measure antibodies to the virus, are initially negative, because HIV antibodies generally do not reach detectable levels in the blood until a few weeks after the onset of the acute illness. As the immune response to the virus develops, the level of HIV in the blood decreases.

During the 2004 election, the PBS journalist Gwen Ifill brought the issue to the mainstream stage as the moderator for the vice-presidential debate. She asked the candidates Dick Cheney and John Edwards what they planned to do to end the spread of H.I.V./AIDS — “not about AIDS in China or Africa, but AIDS right here in this country” — among black women. Cheney replied that he was not aware of the numbers, while Edwards spent more than a minute discussing AIDS in Africa. In 2006, I attended the International AIDS Conference in Toronto with a delegation of black journalists, civil rights leaders, government officials, politicians and celebrities, including the singer Sheryl Lee Ralph, Representatives Maxine Waters and Barbara Lee, the Rev. Jesse Jackson and Julian Bond, chairman of the N.A.A.C.P., who famously announced, “Now is the time for us to face the fact that AIDS has become a black disease.”

Epidemiologic studies have shown that the risk of HIV transmission from patient to health care professional is exceedingly low and is related to needle stick or intraoperative injury or to potentially infectious fluid that comes in contact with a mucous membrane (17). Most contacts between health care professionals and women who are infected with HIV occur, however, during routine obstetric and gynecologic care. Health care practitioners should observe standard precautions with all patients to minimize skin, mucous membrane, and percutaneous exposure to blood and body fluids to protect against a variety of pathogens, including HIV.

AIDS was first clinically observed in 1981 in the United States.[37] The initial cases were a cluster of injecting drug users and homosexual men with no known cause of impaired immunity who showed symptoms of Pneumocystis carinii pneumonia (PCP), a rare opportunistic infection that was known to occur in people with very compromised immune systems.[218] Soon thereafter, an unexpected number of homosexual men developed a previously rare skin cancer called Kaposi’s sarcoma (KS).[219][220] Many more cases of PCP and KS emerged, alerting U.S. Centers for Disease Control and Prevention (CDC) and a CDC task force was formed to monitor the outbreak.[221]

There is no fixed site of integration, but the virus tends to integrate in areas of active transcription, probably because these areas have more open chromatin and more easily accessible DNA. Within weeks of infection, many people will develop the varied symptoms of primary or acute infection, which typically has been described as a mononucleosis- or influenza-like illness but can range from minimal fever, aches, and pains to very severe symptoms. The second most frequent mode of HIV transmission is via blood and blood products.[12] Blood-borne transmission can be through needle-sharing during intravenous drug use, needle stick injury, transfusion of contaminated blood or blood product, or medical injections with unsterilized equipment. The risk from sharing a needle during drug injection is between 0.63 and 2.4% per act, with an average of 0.8%.[66] The risk of acquiring HIV from a needle stick from an HIV-infected person is estimated as 0.3% (about 1 in 333) per act and the risk following mucous membrane exposure to infected blood as 0.09% (about 1 in 1000) per act.[49] In the United States intravenous drug users made up 12% of all new cases of HIV in 2009,[67] and in some areas more than 80% of people who inject drugs are HIV positive.[12]

Some religious organizations have claimed that prayer can cure HIV/AIDS. In 2011, the BBC reported that some churches in London were claiming that prayer would cure AIDS, and the Hackney-based Centre for the Study of Sexual Health and HIV reported that several people stopped taking their medication, sometimes on the direct advice of their pastor, leading to a number of deaths.[263] The Synagogue Church Of All Nations advertised an “anointing water” to promote God’s healing, although the group denies advising people to stop taking medication.[263]

According to the Centers for Disease Control and Prevention (CDC), from 2010-2015, the estimated rate of HIV infection diagnoses in all 50 US states decreased from 14.2 per 100,000 population in 2010 to 12.3 per 100,000 population in 2015. [72] In 2015, 39,513 individuals were diagnosed with HIV infection. From 2010 to 2014, the annual number of new HIV infection diagnoses decreased 9%.

AIDS is currently defined as an illness characterised by the development of one or more AIDS-indicating conditions. It is diagnosed in people infected with HIV when they develop certain opportunistic infections or malignancies for the first time. The following list relates to diagnosis in adults. AIDS is the more advanced stage of HIV infection. When the immune system CD4 cells drop to a very low level, a person’s ability to fight infection is lost. In addition, there are several conditions that occur in people with HIV infection with this degree of immune system failure — these are called AIDS-defining illnesses.

There are different variants of HIV, and the cell types that they infect are determined to a large degree by which chemokine receptor they bind as co-receptor. The variants of HIV that are associated with primary infections use CCR5, which binds the CC chemokines RANTES, MIP-1α, and MIP-1β (see Chapter 2), as a co-receptor, and require only a low level of CD4 on the cells they infect. These variants of HIV infect dendritic cells, macrophages, and T cells in vivo. However, they are often described simply as ‘macrophage-tropic’ because they infect macrophage but not T-cell lines in vitro and the cell tropism of different HIV variants was originally defined by their ability to grow in different cell lines.

Many patients develop low-grade fevers, chronic fatigue, and general weakness. HIV also may cause a combination of food malabsorption, loss of appetite, and increased metabolism that contribute to AIDS wasting syndrome.

HIV is one of a group of viruses known as retroviruses. After getting into the body, the virus enters many different cells, incorporates its genes into the human DNA, and hijacks the cell to produce HIV virus. Most importantly, HIV attacks cells of the body’s immune system called CD4 or T-helper cells (T cells). These cells are destroyed by the infection. The body tries keep up by making new T cells or trying to contain the virus, but eventually the HIV wins out and progressively destroys the body’s ability to fight infections and certain cancers. The virus structure has been studied extensively, and this ongoing research has helped scientists develop new treatments for HIV/AIDS. Although all HIV viruses are similar, small variations or mutations in the genetic material of the virus create drug-resistant viruses. Larger variations in the viral genes are found in different viral subtypes. Currently, HIV-1 is the predominant subtype that causes HIV/AIDS. HIV-2, another form of HIV, occurs almost exclusively in West Africa.

In 2016, about 36.7 million people, including about 2.1 million children (< 15 yr), were living with HIV worldwide, according to the World Health Organization (WHO [1]). Almost half do not know they are infected. In 2016, about 1 million died, and 1.8 million were newly infected. Most new infections (95%) occur in the developing world; > 1/2 are in women. Unsafe medical injections play a significant role in HIV spread in sub-Saharan Africa. In 2007, between 12 and 17% of infections in this region were attributed to medical syringe use.[73] The World Health Organization estimates the risk of transmission as a result of a medical injection in Africa at 1.2%.[73] Significant risks are also associated with invasive procedures, assisted delivery, and dental care in this area of the world.[73]

You might not know if you are infected by HIV. Within a few weeks of being infected, some people get fever, headache, sore muscles and joints, stomach ache, swollen lymph glands, or a skin rash for one or two weeks. Most people think it’s the flu. Some people have no symptoms. Fact Sheet 103 has more information on the early stage of HIV infection.

In contrast, when these strains infect species that have not adapted to SIV (“heterologous” or similar hosts such as rhesus or cynomologus macaques), the animals develop AIDS and the virus generates genetic diversity similar to what is seen in human HIV infection.[94] Chimpanzee SIV (SIVcpz), the closest genetic relative of HIV-1, is associated with increased mortality and AIDS-like symptoms in its natural host.[95] SIVcpz appears to have been transmitted relatively recently to chimpanzee and human populations, so their hosts have not yet adapted to the virus.[90] This virus has also lost a function of the Nef gene that is present in most SIVs. For non-pathogenic SIV variants, Nef suppresses T cell activation through the CD3 marker. Nef’s function in non-pathogenic forms of SIV is to downregulate expression of inflammatory cytokines, MHC-1, and signals that affect T cell trafficking. In HIV-1 and SIVcpz, Nef does not inhibit T-cell activation and it has lost this function. Without this function, T cell depletion is more likely, leading to immunodeficiency.[95][96]

Specific proposed high-risk transmission channels, allowing the virus to adapt to humans and spread throughout the society, depend on the proposed timing of the animal-to-human crossing. Genetic studies of the virus suggest that the most recent common ancestor of the HIV-1 M group dates back to circa 1910.[148] Proponents of this dating link the HIV epidemic with the emergence of colonialism and growth of large colonial African cities, leading to social changes, including different patterns of sexual contact (especially multiple, concurrent partnerships), the spread of prostitution, and the concomitant high frequency of genital ulcer diseases (such as syphilis) in nascent colonial cities.[149] While transmission rates of HIV during vaginal intercourse are typically low, they are increased many fold if one of the partners suffers from a sexually transmitted infection resulting in genital ulcers. Early 1900s colonial cities were notable due to their high prevalence of prostitution and genital ulcers to the degree that as of 1928 as many as 45% of female residents of eastern Leopoldville were thought to have been prostitutes and as of 1933 around 15% of all residents of the same city were infected by one of the forms of syphilis.[149]

In the United States, 1.2 million people aged 13 years or older were estimated to have HIV infection in 2012. About 12.8% of them do not know they have HIV infection. About 50,000 new cases are estimated to occur each year in the United States. Most new infections occur in gay and bisexual men, and black men and black women are disproportionately affected (see also HIV in the United States: At A Glance).

Although the symptoms immune deficiency characteristic of AIDS do not appear for years after a person is infected, the bulk of CD4+ T cell loss occurs during the first weeks of infection, especially in the intestinal mucosa, which harbors the majority of the lymphocytes found in the body.[99] The reason for the preferential loss of mucosal CD4+ T cells is that the majority of mucosal CD4+ T cells express the CCR5 protein which HIV uses as a co-receptor to gain access to the cells, whereas only a small fraction of CD4+ T cells in the bloodstream do so.[100] A specific genetic change that alters the CCR5 protein when present in both chromosomes very effectively prevents HIV-1 infection.[101]

The vast majority of infections remain in sub-Saharan Africa, where 5.2% of the population is thought to be infected. Between 2004 and 2006, the prevalence of HIV infection in central and eastern Asia and Eastern Europe increased by 21%. During this period, the number of new HIV infections in persons aged 15 to 64 years rose by 70% in Eastern Europe and central Asia.

The resistance of these rare individuals to HIV infection has now been explained by the discovery that they are homozygous for an allelic, nonfunctional variant of CCR5 caused by a 32-base-pair deletion from the coding region that leads to a frameshift and truncation of the translated protein. The gene frequency of this mutant allele in Caucasoid populations is quite high at 0.09 (meaning that about 10% of the Caucasoid population are heterozygous carriers of the allele and about 1% are homozygous). The mutant allele has not been found in Japanese or black Africans from Western or Central Africa. Heterozygous deficiency of CCR5 might provide some protection against sexual transmission of HIV infection and a modest reduction in the rate of progression of the disease. In addition to the structural polymorphism of the gene, variation of the promoter region of the CCR5 gene has been found in both Caucasian and African Americans. Different promoter variants were associated with different rates of progression of disease.

This expensive test isn’t used for general screening. It’s for people who have early symptoms of HIV or recently had a high-risk exposure. This test doesn’t look for antibodies, but for the virus itself. It takes from seven to 28 days for HIV to be detectable in the blood. This test is usually accompanied by an antibody test.

Sub-Saharan Africa is the region most affected. In 2010, an estimated 68% (22.9 million) of all HIV cases and 66% of all deaths (1.2 million) occurred in this region.[209] This means that about 5% of the adult population is infected[210] and it is believed to be the cause of 10% of all deaths in children.[211] Here in contrast to other regions women compose nearly 60% of cases.[209] South Africa has the largest population of people with HIV of any country in the world at 5.9 million.[209] Life expectancy has fallen in the worst-affected countries due to HIV/AIDS; for example, in 2006 it was estimated that it had dropped from 65 to 35 years in Botswana.[19] Mother-to-child transmission, as of 2013, in Botswana and South Africa has decreased to less than 5% with improvement in many other African nations due to improved access to antiretroviral therapy.[212]

Some people think that HIV is not the cause of AIDS. They dispute the connection between HIV and AIDS,[6] the existence of HIV itself, or the validity of HIV testing and treatment methods.[7][8] These claims, known as “AIDS denialism”, are rejected by the scientific community.[9] However, they have had a significant impact, particularly in South Africa. There the government’s official embrace of AIDS denialism (1999–2005) was responsible for its weak response to that country’s AIDS epidemic. It has been blamed for hundreds of thousands of avoidable deaths and HIV infections.[10][11][12]

There is no cure for HIV infection. Before there were treatments for the virus, people with AIDS lived only for a couple of years. Fortunately, medications have substantially improved the outlook and survival rates. Prevention efforts have reduced HIV infection in young children and have the potential to limit new infections in other populations.

As the center of the epidemic has moved from New York and San Francisco to the smaller cities in the South, and from gay white men of means to poorer people of color, L.G.B.T. advocacy and fund-raising has shifted to marriage equality. In 2013, H.I.V. activists persuaded 35 L.G.B.T. leaders to sign a statement and create a video imploring the greater gay community to recommit to the AIDS struggle. The message: “We need you to come back.” But of $168 million in H.I.V./AIDS philanthropic dollars spent in the United States in 2015, $31 million was disbursed to the South, just 19 percent of total H.I.V. philanthropy in the United States; only $26 million directly targeted African-Americans, and just $16 million went directly to gay and bisexual men, according to the organization Funders Concerned About AIDS.

The interval from HIV infection to diagnosis has decreased in recent years, but diagnosis delays continue to be substantial for some population segments. Whereas testing in the past 12 months has increased in recent years among groups at high risk, a high proportion of persons in all risk groups remain untested, with many missed opportunities for testing. Diagnosis delays lead to missed opportunities for HIV care and treatment and prolong the time a person is unaware of their infection, increasing the potential for HIV transmission. For care and treatment to reduce HIV incidence effectively, a high proportion of cases need to be diagnosed and treated soon after infection occurs. Continued efforts to determine why cases are not being diagnosed soon after infection and to assure implementation of routine and targeted testing can help reduce both the number of persons unaware of their infection and diagnosis delays. HIV is now known to spread between CD4+ T cells by two parallel routes: cell-free spread and cell-to-cell spread, i.e. it employs hybrid spreading mechanisms.[89] In the cell-free spread, virus particles bud from an infected T cell, enter the blood/extracellular fluid and then infect another T cell following a chance encounter.[89] HIV can also disseminate by direct transmission from one cell to another by a process of cell-to-cell spread.[90][91] The hybrid spreading mechanisms of HIV contribute to the virus’s ongoing replication against antiretroviral therapies.[89][92]

One interesting issue is that the co-receptor usage of the virus strains tends to change over time. The initial infection nearly always involves a strain that uses the chemokine receptor 5 (CCR5), which is found on macrophages and dendritic cells, as a co-receptor with CD4. People who are homozygous for deletions in the CCR5 gene (ie, CCR5-delta32) tend to be resistant to infection, [46, 47] and those with heterozygosity for the polymorphism tend to show slower progression of disease. Scientists identified a type of chimpanzee in Central Africa as the source of HIV infection in humans. They believe that the chimpanzee version of the immunodeficiency virus (called simian immunodeficiency virus, or SIV) most likely was transmitted to humans and mutated into HIV when humans hunted these chimpanzees for meat and came into contact with their infected blood. Studies show that HIV may have jumped from apes to humans as far back as the late 1800s. Over decades, the virus slowly spread across Africa and later into other parts of the world. We know that the virus has existed in the United States since at least the mid to late 1970s. Antiviral treatment options have primarily included combinations of two NRTIs, often referred to as “nucs,” and a third drug, typically being a boosted PI, a NNRTI, often called “non-nucs, and InSTIs such as RAL, EVG or dolutegravir (Tivicay, DTG).
