During late-stage HIV infection, the risk developing a life-threatening illness is much greater. Serious conditions may be controlled, avoided, and/or treated with other medications, alongside HIV treatment.
In 2010, the iPrEx study reported the results of the first large study testing the effectiveness of PrEP using orally administered therapy, as opposed to topical agents as in the vaginal PrEP studies. In this study, HIV-uninfected men who had sex with men who took TDF/FTC once daily along with a comprehensive program to promote safe-sex practices and early treatment of sexually transmitted diseases experienced a markedly reduced risk of acquiring HIV compared with those receiving similar prevention practice without TDF/FTC. There are several other studies that have shown that once daily TDF or TDF/FTC have been effective for PrEP in heterosexual men, women, and intravenous drug users. Nevertheless, there are other studies of high-risk HIV-uninfected women that have shown no benefit, with convincing data in both studies demonstrating extremely low levels of treatment adherence with study medications. Based upon the data available, the United States FDA has approved TDF/FTC for use in high-risk HIV-uninfected individuals. When this therapy is utilized, it is clear that people need to be extensively counseled regarding the importance of continued use of condoms as well as diligent screening for HIV infection, acquisition of sexually transmitted diseases, as well as treatment adherence. Treated individuals also need to be made aware of potential side effects of treatment, including gastrointestinal symptoms, kidney damage, and decreases in bone mineral density.
Treatment cannot (with rare exceptions) eliminate the virus from the body, although the HIV level often decreases so much that it cannot be detected in blood or other fluids or tissues. An undetectable level is the goal of treatment. If treatment is stopped, the HIV level increases, and the CD4 count begins to fall.
Human immunodeficiency virus infection and acquired immune deficiency syndrome (HIV/AIDS) is a spectrum of conditions caused by infection with the human immunodeficiency virus (HIV). Following initial infection, a person may not notice any symptoms or may experience a brief period of influenza-like illness. Typically, this is followed by a prolonged period with no symptoms. As the infection progresses, it interferes more with the immune system, increasing the risk of common infections like tuberculosis, as well as other opportunistic infections, and tumors that rarely affect people who have working immune systems. These late symptoms of infection are referred to as acquired immunodeficiency syndrome (AIDS). This stage is often also associated with weight loss.
Other major factors in the early days of AIDS were injection drug use (IDU) through needle sharing and transfusions of blood and blood components. Numerous hemophiliacs and surgical patients were infected through tranfusions before the ability to test for the virus in donated blood became available.

The CDC and the College recommend that females aged 13–64 years be tested at least once in their lifetime and annually thereafter based on factors related to risk. Obstetrician–gynecologists should annually review patients’ risk factors for HIV and assess the need for retesting. Repeat HIV testing should be offered at least annually to women who

However, against this pessimistic background, there are grounds for hope that successful vaccines can be developed. Of particular interest are rare groups of people who have been exposed often enough to HIV to make it virtually certain that they should have become infected but who have not developed the disease. In some cases this is due to an inherited deficiency in the chemokine receptor used as co-receptor for HIV entry, as we explained in Section 11-19. However, this mutant chemokine receptor does not occur in Africa, where one such group has been identified. A small group of Gambian and Kenyan prostitutes who are estimated to have been exposed to many HIV-infected male partners each month for up to 5 years were found to lack antibody responses but to have cytotoxic T lymphocyte responses to a variety of peptide epitopes from HIV. These women seem to have been naturally immunized against HIV.
Fifty percent of persons with HIV infection diagnosed in 2015 had been infected for at least 3 years, and a quarter had been infected for ≥7 years. Diagnosis delays varied substantially by population. Although the percentage of persons testing increased over time among groups at high risk, overall, 15% of persons were unaware of their infection. The prevalence of persons unaware of their infection varied among states, and half (50.5%) of persons with undiagnosed HIV infection in 2015 were living in the South. Gaps in testing remain, and missed opportunities for testing at health care visits are prevalent. Improved testing coverage and frequency are needed to meet the goal of at least 90% of persons living with HIV knowing their infection status and to reduce diagnosis delays and ultimately reduce HIV incidence in the United States (11).
A safe and effective vaccine for the prevention of HIV infection and AIDS is an attractive goal, but its achievement is fraught with difficulties that have not been faced in developing vaccines against other diseases. The first problem is the nature of the infection itself, featuring a virus that proliferates extremely rapidly and causes sustained infection in the face of strong cytotoxic T-cell and antibody responses. As we discussed in Section 11-25, HIV evolves in individual patients by the selective proliferative advantage of mutant virions encoding peptide sequence changes that escape recognition by antibodies and by cytotoxic T lymphocytes. This evolution means that the development of therapeutic vaccination strategies to block the development of AIDS in HIV-infected patients will be extremely difficult. Even after the viremia has been largely cleared by drug therapy, immune responses to HIV fail to prevent drug-resistant virus from rebounding and replicating at pretreatment levels.

Use a condom in other situations. Condoms offer some protection if used properly and consistently. Occasionally, they may break or leak. Only condoms made of latex should be used. Only water-based lubricants should be used with latex condoms; petroleum jelly dissolves latex.
Left untreated, HIV is almost always a fatal illness with half of people dying within nine months of diagnosis of an AIDS-defining condition. The use of ART has dramatically changed this grim picture. People who are on an effective ART regimen have life expectancies that are similar to or only moderately less than the uninfected population. Unfortunately, many people with HIV deal with socioeconomic issues, substance-abuse issues, or other problems that interfere with their ability or desire to take medications.
If screening test results are positive, they are confirmed by a more accurate, specific tests such as the Western blot. The Western blot test is more difficult to do than screening tests but is more accurate.
For HIV treatment to be effective in reducing HIV incidence, infections need to be diagnosed as quickly as possible. This requires increasing HIV testing coverage and frequency. CDC recommends testing all persons aged 13–64 years at least once as a routine part of medical care and more frequent testing (at least annually) for persons at high risk for HIV infection (7). A large proportion (84%) of HIV sexually transmitted from MSM and heterosexual persons is transmitted by MSM (1). Some sexually active MSM might benefit from more frequent testing (e.g., every 3 to 6 months) (18). Testing according to CDC guidelines is critical to diagnosing HIV infection, so that anyone who receives a diagnosis of HIV infection can start antiretroviral treatment. Overall, prior year testing increased among groups at high risk over time. However, 29% of MSM (in 2014), 42% of persons who inject drugs (in 2015), and 59% of heterosexual persons at increased risk (in 2016) did not report testing in the past 12 months. In addition, it is important to note that these data are from persons residing in large metropolitan statistical areas in the United States. Studies have found that persons residing in rural areas are less likely to report prior HIV testing, including in the past 12 months, compared with their urban counterparts, and that persons living in rural areas are more likely to have HIV infection diagnosed at a late stage (19,20). Barriers to implementing routine testing include lack of time, competing priorities, and concerns about reimbursement on the health care provider’s part and stigma and lack of perceived risk on the client’s part (21). Lack of perceived risk was also one of the main reasons cited by MSM in NHBS for not testing in the past 12 months.

Diagnostic blood tests for AIDS are given to individuals in high-risk populations, pregnant women, health care and public service workers who have been exposed to HIV, those who have symptoms associated with AIDS, or others who fear they may have been exposed to the virus. The first blood test for AIDS was developed in 1985. Patients who are being tested for HIV infection are usually given an enzyme-linked immunosorbent assay (ELISA) test for the presence of HIV antibody in their blood. Positive ELISA results are then tested with a Western blot or immunofluorescence (IFA) assay for confirmation. The combination of the ELISA and Western blot tests is more than 99.9% accurate in detecting HIV infection within four to eight weeks following exposure. The polymerase chain reaction (PCR) test can be used to detect the presence of viral nucleic acids in the very small number of HIV patients who have false-negative results on the ELISA and Western blot tests. These tests are also used to detect viruses and bacteria other than HIV and AIDS.
Background: Persons unaware of their human immunodeficiency virus (HIV) infection account for approximately 40% of ongoing transmissions in the United States. Persons are unaware of their infection because of delayed HIV diagnoses that represent substantial missed opportunities to improve health outcomes and prevent HIV transmission.
Plasma HIV RNA level (viral load) reflects HIV replication rates. The higher the set point (the relatively stable virus levels that occur after primary infection), the more quickly the CD4 count decreases and the greater the risk of opportunistic infection, even in patients without symptoms.
Drug therapy is often recommended for patients who are committed to taking all their medications and have a CD4 count less than 500 (indicating immune system suppression) or a high viral load (amount of HIV virus in the bloodstream).
When a patient is infected with HIV, the virus slowly begins to destroy that patient’s immune system. How fast this occurs is different in each individual. Treatment with HAART can help slow and even halt the destruction of the immune system.

Stevens-Johnson syndrome (SJS) is a rare allergic reaction to HIV medication. Symptoms include fever and swelling of the face and tongue. Rash, which can involve the skin and mucous membranes, appears and spreads quickly.
These drugs prevent HIV from replicating in cells and dramatically reduce the amount of HIV in the blood over a few days to weeks. If replication is sufficiently slowed, the destruction of CD4+ lymphocytes by HIV is decreased and the CD4 count begins to increase. As a result, much of the damage to the immune system caused by HIV can be reversed. Doctors can detect this reversal by measuring the CD4 count, which begins to return toward normal levels over weeks to months. The CD4 count continues to increase for several years but at a slower rate.
The Centers for Disease Control and Prevention (CDC) estimates that approximately 40,000–50,000 new human immunodeficiency virus (HIV) infections occurred annually in the United States from 2006 to 2009 (1). Almost 1 in 5 (18.1%) of all individuals infected with HIV are unaware of their HIV status (2). In order to identify individuals with undiagnosed HIV infection, the CDC recommends HIV screening for all patients aged 13–64 years in health care settings (3). Because obstetrician–gynecologists provide primary and preventive care for adolescents and women, they are ideally suited to play an important role in promoting HIV screening for their patients. The American College of Obstetricians and Gynecologists (the College) recommends routine HIV screening for females aged 13–64 years and older women with risk factors. Screening after age 64 years is indicated if there is ongoing risk of HIV infection, as indicated by risk assessment (eg, new sexual partners).
Antibody tests in children younger than 18 months are typically inaccurate due to the continued presence of maternal antibodies. Thus HIV infection can only be diagnosed by PCR testing for HIV RNA or DNA, or via testing for the p24 antigen. Much of the world lacks access to reliable PCR testing and many places simply wait until either symptoms develop or the child is old enough for accurate antibody testing. In sub-Saharan Africa as of 2007–2009 between 30 and 70% of the population were aware of their HIV status. In 2009, between 3.6 and 42% of men and women in Sub-Saharan countries were tested which represented a significant increase compared to previous years.
Most people infected with HIV develop specific antibodies (i.e. seroconvert) within three to twelve weeks of the initial infection. Diagnosis of primary HIV before seroconversion is done by measuring HIV-RNA or p24 antigen. Positive results obtained by antibody or PCR testing are confirmed either by a different antibody or by PCR.
The humoral immune system is also affected. Hyperplasia of B cells in lymph nodes causes lymphadenopathy, and secretion of antibodies to previously encountered antigens increases, often leading to hyperglobulinemia. Total antibody levels (especially IgG and IgA) and titers against previously encountered antigens may be unusually high. However, antibody response to new antigens (eg, in vaccines) decreases as the CD4 count decreases.
