HIV attacks and destroys the infection-fighting CD4 cells of the immune system. The loss of CD4 cells makes it difficult for the body to fight infections and certain cancers. Without treatment, HIV can gradually destroy the immune system and advance to AIDS.

HIV-positive women who might become pregnant should talk to their provider about the risk to their unborn child. They should also discuss methods to prevent their baby from becoming infected, such as taking antiretroviral medicines during pregnancy.

These are federally approved medical practice guidelines for HIV/AIDS. Each set of guidelines is developed by a panel of experts in HIV care and research that includes health professionals, researchers, and community members. The Panels meet regularly to review the latest clinical research and update the prevention and treatment recommendations.

HIV is the causative agent of Acquired Immunodeficiency Syndrome (AIDS). AIDS is a severe, life-threatening disease that represents the late clinical stage of infection with the HIV. 2.5 million people died of AIDS in 2005 alone, and estimates place the number of people living with HIV/AIDS at 38.6 million. HIV/AIDS has more than 25 million lives since 1981.

From a legal, ethical, and moral standpoint, they should warn any prospective sexual partner of their HIV positive status. They should not exchange body fluids during sexual activity and must use whatever preventative measures (such as a latex condom) will afford the partner the most protection.

If people at low risk have a negative test result, the screening test is not repeated unless their risk status changes. If people at the highest risk have a negative test result (especially if they are sexually active, have several partners, or do not practice safe sex), testing should be repeated every 6 to 12 months.

Patients with HIV infection should be counseled about the risks of infecting their sexual partners with HIV. Safer sex practices and treatment of concurrent sexually transmitted diseases, both in the patient and in sexual partners, considerably reduces the risk of transmission. Patients with HIV infection should be encouraged to inform their sexual partners of their status; failure to do so has resulted in successful prosecutions in several countries. Sexual contacts should be tested.

Stage III: Advanced symptoms which may include unexplained chronic diarrhea for longer than a month, severe bacterial infections including tuberculosis of the lung, and a CD4 count of less than 350/µl.[26]

HIV attaches to and penetrates host T cells, then releases HIV RNA and enzymes into the host cell. HIV reverse transcriptase copies viral RNA as proviral DNA. Proviral DNA enters the host cell’s nucleus, and HIV integrase facilitates the proviral DNA’s integration into the host’s DNA. The host cell then produces HIV RNA and HIV proteins. HIV proteins are assembled into HIV virions and budded from the cell surface. HIV protease cleaves viral proteins, converting the immature virion to a mature, infectious virion.

These symptoms can come and go or get progressively worse. If you’ve been exposed to HIV, you may also have been exposed to other sexually transmitted diseases (STDs). Men are more likely than women to notice symptoms like sores on their genitals. But men typically don’t seek medical care as often as women.

Blood and genital secretions from people with HIV are considered infectious and the utmost care should be taken in handling them. Fluids that are contaminated with blood also are potentially infectious. Feces, nasal secretions, saliva, sputum, sweat, tears, urine, and vomit are not considered infectious unless visibly bloody.

Methods: Data from CDC’s National HIV Surveillance System were used to estimate, among persons with HIV infection diagnosed in 2015, the median interval (and range) from infection to diagnosis (diagnosis delay), based on the first CD4 test after HIV diagnosis and a CD4 depletion model indicating disease progression and, among persons living with HIV in 2015, the percentage with undiagnosed infection. Data from CDC’s National HIV Behavioral Surveillance were analyzed to determine the percentage of persons at increased risk for HIV infection who had tested in the past 12 months and who had missed opportunities for testing.

Although there is no HIV vaccine, HIV infections are entirely preventable through safe behaviour. Everyone has a responsibility to help prevent transmission of HIV and to take care of themselves and others. This means:

Most scientists now believe that risk of contracting H.I.V. boils down to a numbers game rather than a blame game: If the virus is not present in your sexual network, you can have unprotected sex and not get infected. But if you are in a community, like Jackson, where a high percentage of gay and bisexual men are infected with H.I.V. — and many don’t know it and go untreated — any unprotected sexual encounter becomes a potential time bomb. This explanation of “viral load” helps dispel the stubbornly held notion that gay and bisexual black men have more sex than other men, a false perception embedded in the American sexual imagination and fueled by stereotypes of black men as hypersexual Mandingos dating back to slavery.

Antibody tests in children younger than 18 months are typically inaccurate due to the continued presence of maternal antibodies.[106] Thus HIV infection can only be diagnosed by PCR testing for HIV RNA or DNA, or via testing for the p24 antigen.[26] Much of the world lacks access to reliable PCR testing and many places simply wait until either symptoms develop or the child is old enough for accurate antibody testing.[106] In sub-Saharan Africa as of 2007–2009 between 30 and 70% of the population were aware of their HIV status.[107] In 2009, between 3.6 and 42% of men and women in Sub-Saharan countries were tested[107] which represented a significant increase compared to previous years.[107]

Advances in Treatment Though the search for an AIDS vaccine has consumed many researchers, by 2003 no breakthroughs had appeared. However, other researchers have concentrated on ways of controlling AIDS through drug treatment regimens that require individuals to consume many different types of medications at the same time. These anti-AIDS “cocktails” undergo constant study and modification as researchers learn more about the working of HIV. The medications are from a family of drugs called protease inhibitors.

Alternative treatments for AIDS can be grouped into two categories: those intended to help the immune system and those aimed at pain control. Treatments that may enhance the function of the immune system include Chinese herbal medicine and western herbal medicine, macrobiotic and other special diets, guided imagery and creative visualization, homeopathy, and vitamin therapy. Pain control therapies include hydrotherapy, reiki, acupuncture, meditation, chiropractic treatments, and therapeutic massage. Alternative therapies also can be used to help with side effects of the medications used in the treatment of AIDS.

If you have these symptoms, that doesn’t mean you have HIV. Each of these symptoms can be caused by other illnesses. People with AIDS have had their immune system damaged by HIV. They are at very high risk of getting infections that are uncommon in people with a healthy immune system. These infections are called opportunistic infections. These can be caused by bacteria, viruses, fungi, or protozoa, and can affect any part of the body. People with AIDS are also at higher risk for certain cancers, especially lymphomas and a skin cancer called Kaposi sarcoma.

Seroconversion may take a few weeks, up to several months. Symptoms during this time may include fever, flulike illness, lymphadenopathy, and rash. These manifestations develop in approximately half of all people infected with HIV.

Patients with AIDS have had their immune system depleted by HIV and are very susceptible to such opportunistic infections. Common symptoms are fevers, sweats (particularly at night), swollen glands, chills, weakness, and weight loss.

Historically, the greatest success in preventing viral transmission has resulted from the development of preventative vaccines. Unfortunately, decades of research to develop an HIV vaccine has led to little hope for success. In 2007, a major setback in this area occurred when the STEP study investigating a promising vaccine candidate was prematurely stopped due to the lack of evidence that it produced any protection from HIV infection. In contrast, a glimmer of hope did emerge with the report in 2009 of the results of the RV 144 Thai HIV vaccine trial, which demonstrated borderline effectiveness in the more than 16,000 recipients. While this vaccine demonstrated only limited evidence of protection, research is under way to further explore what can be learned for future vaccine development from this modest success.

Protease inhibitors. Protease inhibitors work by disabling protease, an enzyme necessary for HIV reproduction. Protease inhibitors include saquinavir (Invirase), ritonavir (Norvire), indinavir (Crixivan), nelfinavir (Viracept), amprenavir (Agenerase), kaletra, and many others.

At the same time, it is important to recognise that reaching an undetectable viral load is determined by many factors, including treatment adherence, HIV resistance to certain anti-retroviral drugs, stigma, and inadequate health systems.[119]

The incidence of AIDS by date of diagnosis (assuming an almost constant population at risk) has roughly doubled every half-year since the second half of 1979 (Table 1). An average of one to two cases are now diagnosed every day. Although the overall case-mortality rate for the current total of 593 is 41%, the rate exceeds 60% for cases diagnosed over a year ago.

Studies with powerful drugs that completely block the cycle of HIV replication indicate that the virus is replicating rapidly at all phases of infection, including the asymptomatic phase. Two viral proteins in particular have been the target of drugs aimed at arresting viral replication. These are the viral reverse transcriptase, which is required for synthesis of the provirus, and the viral protease, which cleaves the viral polyproteins to produce the virion proteins and viral enzymes. Inhibitors of these enzymes prevent the establishment of further infection in uninfected cells. Cells that are already infected can continue to produce virions because, once the provirus is established, reverse transcriptase is not needed to make new virus particles, while the viral protease acts at a very late maturation step of the virus, and inhibition of the protease does not prevent virus from being released. However, in both cases, the released virions are not infectious and further cycles of infection and replication are prevented.

Some people may develop a flu-like illness within a month or two after exposure to the HIV virus, although many people do not develop any symptoms at all when they first become infected. Many people mistake this flu-like illness as being caused by something else. Persistent or severe symptoms may not surface for 10 years or more, after HIV first enters the body in adults, or within two years in children born with an HIV infection.

There are medicines that help people with AIDS. These are called antiretroviral medicines (or antiretrovirals.) Anti- means against. HIV is a retrovirus. So antiretroviral means it fights retroviruses.

HIV is one of a group of viruses known as retroviruses. After getting into the body, the virus enters many different cells, incorporates its genes into the human DNA, and hijacks the cell to produce HIV virus. Most importantly, HIV attacks cells of the body’s immune system called CD4 or T-helper cells (T cells). These cells are destroyed by the infection. The body tries to keep up by making new T cells or trying to contain the virus, but eventually the HIV wins out and progressively destroys the body’s ability to fight infections and certain cancers. The virus structure has been studied extensively, and this ongoing research has helped scientists develop new treatments for HIV/AIDS. Although all HIV viruses are similar, small variations or mutations in the genetic material of the virus create drug-resistant viruses. Larger variations in the viral genes are found in different viral subtypes. Currently, HIV-1 is the predominant subtype that causes HIV/AIDS. HIV-2, another form of HIV, occurs almost exclusively in West Africa.

Near the end of life, many people have pain and other distressing symptoms (such as agitation) and usually lose their appetite. Hospice programs are particularly equipped to deal with such problems. They can provide comprehensive support and care, which focuses on managing symptoms, helping dying people maintain their independence, and supporting their caregivers.

Fusion and entry inhibitors are agents that keep HIV from entering human cells. Enfuvirtide (Fuzeon/T20) was the first drug in this group and was given in injectable form like insulin. Maraviroc (Selzentry) can be given by mouth and is used in combination with other ARTs.

Routine social or community contact with an HIV infected person carries no risk of infection. There is no evidence of spread of HIV through social contact in schools, at home or in the work place. HIV has not been transmitted through:

Ideally, prior to initiating treatment, the viral load and the CD4 cell count should be checked and the viral load test then repeated after approximately four weeks of treatment. If the patient is beginning a regimen that includes two to three drugs for which the patient’s virus does not appear to be resistant, it is expected that the amount of virus should decrease by at least a hundredfold during this interval. The ultimate goal is for the viral load to decrease to undetectable levels which should occur by approximately 12-24 weeks. There are some individuals that despite taking all of their medications correctly will suppress their viral load to less than 200 copies/mL but not consistently undetectable levels. It is not completely known how to optimally manage this situation but many experts would continue to monitor on current therapy as long as viral load remains below 200 copies/mL. Those who are not having an appropriate response to therapy need to be questioned to make sure that they are taking their medications correctly, and if not, why. If the viral load is not going to undetectable levels and the patient is taking the medications correctly, then it is likely that there is a resistant virus to some of the medications. Drug-resistance testing then should be performed and the patient managed as described in the next section. Once the patient’s viral load is suppressed, they can often have viral load and CD4 cell counts performed less frequently (for example, every three to four months and in select cases every six months or possibly even less).

The classical process of infection of a cell by a virion can be called “cell-free spread” to distinguish it from a more recently-recognized process called “cell-to-cell spread”.[81] In cell-free spread (see figure), virus particles bud from an infected T cell, enter the blood or extracellular fluid and then infect another T cell following a chance encounter.[81] HIV can also disseminate by direct transmission from one cell to another by a process of cell-to-cell spread, for which two pathways have been described. Firstly, an infected T cell can transmit virus directly to a target T cell via a virological synapse.[57][82] Secondly, an antigen-presenting cell (APC), such as a macrophage or dendritic cell, can transmit HIV to T cells by a process that either involves productive infection (in the case of macrophages) or capture and transfer of virions in trans (in the case of dendritic cells).[83] Whichever pathway is used, infection by cell-to-cell transfer is reported to be much more efficient than cell-free virus spread.[84] A number of factors contribute to this increased efficiency, including polarised virus budding towards the site of cell-to-cell contact, close apposition of cells, which minimizes fluid-phase diffusion of virions, and clustering of HIV entry receptors on the target cell to the contact zone.[82] Cell-to-cell spread is thought to be particularly important in lymphoid tissues where CD4+ T cells are densely packed and likely to interact frequently.[81] Intravital imaging studies have supported the concept of the HIV virological synapse in vivo.[85] The hybrid spreading mechanisms of HIV contribute to the virus’ ongoing replication in spite of anti-retroviral therapies.[81][86]

Compliance with medications is important to provide the best outcome for mother and child. Even though a physician might highly recommend a medication regimen, the pregnant woman has a choice of whether or not to take the medicines. Studies have shown that compliance is improved when there is good communication between the woman and her doctor, with open discussions about the benefits and side effects of treatment. Compliance also is improved with better social support, including friends and relatives.

Patients with late-stage AIDS may develop Kaposi’s sarcoma (KS), a skin tumor that primarily affects homosexual men. KS is the most common AIDS-related malignancy. It is characterized by reddish-purple blotches or patches (brownish in people with dark skin) on the skin or in the mouth. About 40% of patients with KS develop symptoms in the digestive tract or lungs. KS may be caused by a herpes virus-like sexually transmitted disease agent rather than HIV.

In the United States, 1.2 million people aged 13 years or older were estimated to have HIV infection in 2012. About 12.8% of them do not know they have HIV infection. About 50,000 new cases are estimated to occur each year in the United States. Full blood count: This is a test to check on the levels of white blood cells, red blood cells, platelets and haemoglobins in your blood. This test needs to be done before and regularly after treatment to check for anaemia (reduced blood haemoglobin) and reduction of other blood cells.

The source is qualified by whether it is known or unknown. If the source is unknown (eg, a needle on the street or in a sharps disposal container), risk should be assessed based on the circumstances of the exposure (eg, whether the exposure occurred in an area where injection drug use is prevalent, whether a needle discarded in a drug-treatment facility was used). Dealing with the potential consequences of bias and discrimination – job loss, homelessness, lack of healthcare insurance – often results in LGBTQ people engaging in behaviors that facilitate the spread of HIV. For example, in the face of persistent employment discrimination, many transgender women are left with few other options but to engage in survival sex work in order to meet their most basic needs. In 2003, President george w. bush proposed spending $15 billion over five years to support international AIDS prevention and the purchase of anti-viral drugs. The largest share of the money would be contributed directly by the United States to other countries, such as through programs sponsored by the U.S. Agency for International Development. The proposal would account for almost half the money in a global fund committed to fight HIV and AIDS.

Protease is an enzyme that HIV needs to replicate. As the name suggests, protease inhibitors bind to the enzyme and inhibit its action, preventing HIV from making copies of itself. These include atazanavir/cobicistat (Evotaz), lopinavir/ritonavir (Kaletra), and darunavir/cobicistat (Prezcobix).

There are many misconceptions about HIV and AIDS. Three of the most common are that AIDS can spread through casual contact, that sexual intercourse with a virgin will cure AIDS,[275][276][277] and that HIV can infect only gay men and drug users. In 2014, some among the British public wrongly thought one could get HIV from kissing (16%), sharing a glass (5%), spitting (16%), a public toilet seat (4%), and coughing or sneezing (5%).[278] Other misconceptions are that any act of anal intercourse between two uninfected gay men can lead to HIV infection, and that open discussion of HIV and homosexuality in schools will lead to increased rates of AIDS.[279][280]

The profound immunosupression seen in AIDS is due to the depletion of T4 helper lymphocytes. HIV is present at a high level in the blood immediately after exposure. It then settles down to a certain low level set-point during the incubation period that lasts from 3-8 weeks. During the incubation perid, there is a massive turnover of CD4 cells as the CD4 cells killed by HIV are replaced rapidly and efficiently. The immune system eventually succumbs and AIDS is developed when killed CD4 cells can no longer be replaced, as witnessed by high HIV-RNA, HIV-Antigen and low CD4 counts.

If a woman is untreated, two years of breastfeeding results in an HIV/AIDS risk in her baby of about 17%. Treatment decreases this risk to 1 to 2% per year. Due to the increased risk of death without breastfeeding in many areas in the developing world, the World Health Organization recommends either: (1) the mother and baby being treated with antiretroviral medication while breastfeeding being continued (2) the provision of safe formula.[80] Infection with HIV during pregnancy is also associated with miscarriage.[81]

In September 2014, new UNAIDS “Fast Track” targets called for the dramatic scaling-up of HIV prevention and treatment programmes to avert 28 million new infections and end the epidemic as a public health issue by 2030.93

NNRTIs include NVP, DLV, EFV, ETR, and RPV. ETR was developed specifically to be an option for patients who have developed resistance to the earlier drugs in the class. NVP, DLV, EFV, and RPV are typically used with two NRTIs, and ETR is primarily being used as part of regimens for those with a history of different types of treatment to which they have developed resistance.

Full blood count: This is a test to check on the levels of white blood cells, red blood cells, platelets and haemoglobins in your blood. This test needs to be done before and regularly after treatment to check for anaemia (reduced blood haemoglobin) and reduction of other blood cells.

Genetic studies have led to a general classification system for HIV that is primarily based on the degree of similarity in viral gene sequence. The two major classes of HIV are HIV-1 and HIV-2. HIV-1 is divided into three groups, known as group M (main group), group O (outlier group), and group N (new group). Worldwide, HIV-1 group M causes the majority of HIV infections, and it is further subdivided into subtypes A through K, which differ in expression of viral genes, virulence, and mechanisms of transmission. In addition, some subtypes combine with one another to create recombinant subtypes. HIV-1 group M subtype B is the virus that spread from Africa to Haiti and eventually to the United States. Pandemic forms of subtype B are found in North and South America, Europe, Japan, and Australia. Subtypes A, C, and D are found in sub-Saharan Africa, although subtypes A and C are also found in Asia and some other parts of the world. Most other subtypes of group M are generally located in specific regions of Africa, South America, or Central America.

Kaposi’s sarcoma. A tumor of the blood vessel walls, this cancer is rare in people not infected with HIV, but common in HIV-positive people. It usually appears as pink, red or purple lesions on the skin and mouth. In people with darker skin, the lesions may look dark brown or black. Kaposi’s sarcoma can also affect the internal organs, including the digestive tract and lungs.

Patients with most acute opportunistic infections benefit from early ART (initiated during the management of the opportunistic infection). However, for some opportunistic infections, such as tuberculous meningitis or cryptococcal meningitis, the evidence suggests that ART should be delayed until the first phase of antimicrobial therapy for these infections is finished.

The Centers for Disease Control reported cases of Pneumocystis carinii pneumonia and Kaposi’s sarcoma in otherwise healthy young male homosexuals in 1981. Until then, pneumocystis carinii was mainly known to occur in immunodepressed patients after organ transplants or suffering from congenital immunodeficiencies. Soon thereafter, the same condition was seen in IV drug abusers, haemophilliacs and babies of IV drug abusing mothers. These patients had profound immunosuppression due to the depletion of T4 helper lymphocytes and the name ‘acquired immunodeficiency’ was coined for this syndrome. Epidemiological studies have now established that the disease is infectious and can be transmitted by sexual intercourse, blood or blood products. The lymphocytes of patients died early, creating a difficulty in isolating the virus. Montagnier and Gallo eventually isolated the virus in 1984 and HIV-2 was isolated in 1986 from West Africa. Diagnostic blood tests for AIDS are given to individuals in high-risk populations, pregnant women, health care and public service workers who have been exposed to HIV, those who have symptoms associated with AIDS, or others who fear they may have been exposed to the virus. The first blood test for AIDS was developed in 1985. Patients who are being tested for HIV infection are usually given an enzyme-linked immunosorbent assay (ELISA) test for the presence of HIV antibody in their blood. Positive ELISA results are then tested with a Western blot or immunofluorescence (IFA) assay for confirmation. The combination of the ELISA and Western blot tests is more than 99.9% accurate in detecting HIV infection within four to eight weeks following exposure. The polymerase chain reaction (PCR) test can be used to detect the presence of viral nucleic acids in the very small number of HIV patients who have false-negative results on the ELISA and Western blot tests. These tests are also used to detect viruses and bacteria other than HIV and AIDS.

Proviral DNA enters the host cell’s nucleus and is integrated into the host DNA in a process that involves integrase, another HIV enzyme. With each cell division, the integrated proviral DNA is duplicated along with the host DNA. Subsequently, the proviral HIV DNA can be transcribed to HIV RNA and translated to HIV proteins, such as the envelope glycoproteins 41 and 120. These HIV proteins are assembled into HIV virions at the host cell inner membrane and budded from the cell surface within an envelop of modified human cell membrane. Each host cell may produce thousands of virions.

HIV is transmitted in about 93% of blood transfusions using infected blood.[66] In developed countries the risk of acquiring HIV from blood transfusion is extremely low (less than one in half a million) where improved donor selection and HIV screening is performed;[12] for example, in the UK the risk is reported at one in five million[68] and in the United States it was one in 1.5 million in 2008.[69] In low income countries, only half of transfusions may be appropriately screened (as of 2008),[70] and it is estimated that up to 15% of HIV infections in these areas come from transfusion of infected blood and blood products, representing between 5% and 10% of global infections.[12][71] Although rare because of screening, it is possible to acquire HIV from organ and tissue transplantation.[72]

HIV/AIDS research includes all medical research which attempts to prevent, treat, or cure HIV/AIDS along with fundamental research about the nature of HIV as an infectious agent and AIDS as the disease caused by HIV.

The main cellular target of HIV is a special class of white blood cells critical to the immune system known as helper T lymphocytes, or helper T cells. Helper T cells are also called CD4+ T cells, because they have on their surfaces a protein called CD4. Helper T cells play a central role in normal immune responses by producing factors that activate virtually all the other immune system cells. Those include B lymphocytes, which produce antibodies needed to fight infection; cytotoxic T lymphocytes, which kill cells infected with a virus; and macrophages and other effector cells, which attack invading pathogens. AIDS results from the loss of most of the helper T cells in the body.

From the time of infection by HIV, AIDS normally develops within ten years, though there are now drugs which may be used to extend this time. The immune failure, which is characteristic of AIDS, occurs as a consequence of a gradual decline in the number of CD4 T lymphocytes. Eventually the infected person succumbs to a variety of infections by BACTERIA, FUNGI, protozoa or viruses and/or develops a cancer(s) such as Kaposi’s Sarcoma.

Nowhere are the two sides more split than on the issue of condoms. Schools in at least 23 cities sought to distribute condoms during the mid-to late-1990s. The assumption was that since students will have sex anyway—despite warnings not to—they had better be protected. Conservatives see this position as a cop-out in two ways: it sells values short and it undermines parental authority. In 1992, in Washington, D.C., critics erupted over a decision by the Public Health Commission to hand out condoms in junior and senior high schools without parental consent. William Brown, president of the D.C. Congress of Parents and Teachers, complained: “We are looking to build and reinforce and establish family values where they have been lost, and here we have an agency of our government that totally ignores those things we are working for.” Dr. Mary Ellen Bradshaw, the commission’s chief, replied: “Our whole focus is to save the lives of these children, stressing abstinence as the only sure way to avoid [AIDS] and making condoms available only after intensive education.” In other cities, upset parents simply sued. By 1992, Class Action lawsuits had been brought against school districts in New York City, Seattle, and Falmouth, Massachusetts, arguing that condom distribution violated parents’ right to privacy.

In 1999, the WHO announced that AIDS was the fourth biggest cause of death worldwide and number one killer in Africa. An estimated 33 million people were living with HIV and 14 million people had died from AIDS since the start of the epidemic.70

Some religious organizations have claimed that prayer can cure HIV/AIDS. In 2011, the BBC reported that some churches in London were claiming that prayer would cure AIDS, and the Hackney-based Centre for the Study of Sexual Health and HIV reported that several people stopped taking their medication, sometimes on the direct advice of their pastor, leading to a number of deaths.[263] The Synagogue Church Of All Nations advertised an “anointing water” to promote God’s healing, although the group denies advising people to stop taking medication.[263]

Condoms provide a way for men and women to prevent pregnancy. There are many methods of birth control; some types also protect against sexually transmitted diseases. Condoms are one type of birth control that in addition to preventing pregnancy also prevent the spread of STD’s.

By 1984 researchers working in Africa had provided clear evidence for heterosexual transmission of the causative agent, HIV. The virus had been isolated the year before by a team of French researchers led by virologist Luc Montagnier. Montagnier and his colleagues identified the virus as a new type of human retrovirus, and they suspected that it was the cause of AIDS. But more-detailed characterization was needed to confirm the connection, so Montagnier sent samples to American virologist Robert C. Gallo, who had contributed to the discovery of the first known human retrovirus (human T-lymphotropic virus) several years earlier. Gallo helped establish that HIV caused AIDS, and he contributed to the subsequent development of a blood test for its detection. Montagnier initially called the new infectious agent lymphadenopathy-associated virus (LAV), but in 1986 the International Committee on Taxonomy of Viruses renamed it HIV. Montagnier and French virologist Françoise Barré-Sinoussi were awarded the 2008 Nobel Prize for Physiology or Medicine for their discovery of HIV; despite Gallo’s role in confirming HIV as the cause of AIDS, Montagnier and colleagues were the first to isolate the virus.

HIV infects vital cells in the human immune system such as helper T cells (specifically CD4+ T cells), macrophages, and dendritic cells.[8] HIV infection leads to low levels of CD4+ T cells through a number of mechanisms, including pyroptosis of abortively infected T cells,[9] apoptosis of uninfected bystander cells,[10] direct viral killing of infected cells, and killing of infected CD4+ T cells by CD8+ cytotoxic lymphocytes that recognize infected cells.[11] When CD4+ T cell numbers decline below a critical level, cell-mediated immunity is lost, and the body becomes progressively more susceptible to opportunistic infections, leading to the development of AIDS.

Current treatments do not cure the infection. The medicines only work as long as they are taken every day. If the medicines are stopped, the viral load will go up and the CD4 count will drop. If the medicines are not taken regularly, the virus can become resistant to one or more of the drugs, and the treatment will stop working.

In April 2011, he embarked on tour of his one-man show, “My Violent Torpedo of Truth/Defeat Is Not an Option.” The first show, in Detroit, went off the rails quickly. “Early in the evening, before the crowd turned sour, there was a creepy atmosphere that suggested group indoctrination into a cult,” said a Hollywood Reporter review. And that was before the booing and shouts of “You suck” started. He changed the style to a Q&A for the second show, but the tour never really caught fire.

You might not know if you are infected by HIV. Within a few weeks of being infected, some people get fever, headache, sore muscles and joints, stomach ache, swollen lymph glands, or a skin rash for one or two weeks. Healthcare visits in the preceding year were associated with a lower rate of unawareness (37% vs 81%) but a higher rate of HIV-positivity (21% vs 12%). Because this study targeted a high-risk group and may involve participation bias, the overall rate of HIV infection (19%) cannot be easily extrapolated to the overall population. [73]

Psychological – common misconceptions about AIDS and HIV are diminishing. However, the stigma of the condition persists in many parts of the world. People who are living with HIV may feel excluded, rejected, discriminated, and isolated.

If treatment fails, drug susceptibility (resistance) assays can determine the susceptibility of the dominant HIV strain to all available drugs. Genotypic and phenotypic assays are available and can help clinicians select a new regimen that should contain at least 2 and preferably 3 drugs to which the HIV strain is more susceptible. The dominant HIV strain in the blood of patients who are taken off antiretroviral therapy may revert over months to years to the wild-type (ie, susceptible) strain because the resistant mutants replicate more slowly and are replaced by the wild type. Thus, if patients have not been treated recently, the full extent of resistance may not be apparent through resistance testing, but when treatment resumes, strains with resistance mutations often reemerge from latency and again replace the wild-type HIV strain.

Risk of infection is about 0.3% (1:300) after a typical percutaneous exposure and about 0.09% (1:1100) after mucous membrane exposure. These risks vary, reflecting the amount of HIV transferred to the person with the injury; the amount of HIV transferred is affected by multiple factors, including viral load of the source and type of needle (eg, hollow or solid). However, these factors are no longer taken into account in PEP recommendations.

The ability of HIV to enter particular types of cell, known as the cellular tropism of the virus, is determined by the expression of specific receptors for the virus on the surface of those cells. HIV enters cells by means of a complex of two noncovalently associated viral glycoproteins, gp120 and gp41, in the viral envelope. The gp120 portion of the glycoprotein complex binds with high affinity to the cell-surface molecule CD4. This glycoprotein thereby draws the virus to CD4 T cells and to dendritic cells and macrophages, which also express some CD4. Before fusion and entry of the virus, gp120 must also bind to a co-receptor in the membrane of the host cell. Several different molecules may serve as a co-receptor for HIV entry, but in each case they have been identified as chemokine receptors. The chemokine receptors (see Chapters 2 and 10) are a closely related family of G protein-coupled receptors with seven transmembrane-spanning domains. Two chemokine receptors, known as CCR5, which is predominantly expressed on dendritic cells, macrophages, and CD4 T cells, and CXCR4, expressed on activated T cells, are the major co-receptors for HIV. After binding of gp120 to the receptor and co-receptor, the gp41 then causes fusion of the viral envelope and the plasma membrane of the cell, allowing the viral genome and associated viral proteins to enter the cytoplasm.

Some HIV-infected people actively seek out other persons with HIV infection for sex under the assumption that they are not putting themselves or anyone else at an increased risk. However, it is clear that co-infections with multiple HIV strains (whether the same or different clades) can and do occur, and that such events may result in a rapid deterioration of a previously stable infection. A growing number of new infections are drug resistant upon first presentation, suggesting that these infections were transmitted from individuals receiving therapy.

Exposure to HIV does not always lead to infection, and some people who have had repeated exposures over many years remain uninfected. Moreover, many HIV-infected people remain well for more than a decade. A very few HIV-infected, untreated people have remained well for over 20 years. Why some people become ill so much sooner than others is not fully understood, but a number of genetic factors appear to influence both susceptibility to infection and progression to AIDS after infection.

In a too brightly lit wood-paneled back room, Sturdevant and the younger men set up a table, displaying brochures, condoms, lube and a few lollipops. Stevenson and Watson, both open, friendly and handsome, attracted a few guys to the table, but mainly ones who had already heard the protect-yourself-against-H.I.V. spiel. Stevenson pointed out that the crowd was sparse — maybe 50 men and a few transgender women — because so many Jackson residents were attending the annual state fair. “Anyway, it’s always hard to make contact in the club,” he said. “I prefer one on one. That way it’s not, ‘I’m trying to educate you’; we’re just talking and having fun. I tell them what I do, and they feel comfortable asking questions.”

Reactive arthritis is a chronic, systemic rheumatic disease characterized by three conditions, including conjunctivitis, joint inflammation, and genital, urinary, or gastrointestinal system inflammation. Inflammation leads to pain, swelling, warmth, redness, and stiffness of the affected joints. Non-joint areas may experience irritation and pain. Treatment for reactive arthritis depends on which area of the body is affected. Joint inflammation is treated with anti-inflammatory medications.

In 2008, 2,120,000 people were receiving treatment – 30% of the total number needing it. In 2012, this figure had risen to 7.6 million. Because the WHO expanded its criteria for people who would benefit from antiretroviral therapy, this still only equates to 25% of the population who needs it.

The chemokine ligands to these receptors were found to block virus infection. Even though CCR4, the receptor for ABCD-1, is apparently not used by human immunodeficiency virus as coreceptor for infection, N-terminally processed human ABCD-1 showed human immunodeficiency virus suppressor activity independent of the viral phenotype (Pal et al., 1997; Struyf et al., 1998).

The second problem is our uncertainty over what form protective immunity to HIV might take. It is not known whether antibodies, cytotoxic T lymphocyte responses, or both are necessary to achieve protective immunity, and which epitopes might provide the targets of protective immunity. Third, if strong cytotoxic responses are necessary to provide protection against HIV, these might be difficult to develop and sustain through vaccination. Other effective viral vaccines rely on the use of live, attenuated viruses and there are concerns over the safety of pursuing this approach for HIV. Another possible approach is the use of DNA vaccination, a technique that we discuss in Section 14-25. Both of these approaches are being tested in animal models.

These results provide a dramatic confirmation of experimental work suggesting that CCR5 is the major macrophage and T-lymphocyte co-receptor used by HIV to establish primary infection in vivo, and offers the possibility that primary infection might be blocked by therapeutic antagonists of the CCR5 receptor. Indeed, there is preliminary evidence that low molecular weight inhibitors of this receptor can block infection of macrophages by HIV in vitro. Such low molecular weight inhibitors might be the precursors of useful drugs that could be taken by mouth. Such drugs are very unlikely to provide complete protection against infection, as a very small number of individuals who are homozygous for the nonfunctional variant of CCR5 are infected with HIV. After this earliest stage of HIV infection, HIV continues to multiply but at very low levels. More severe symptoms of HIV infection, such as signs of opportunistic infections, generally don’t appear for many years. (Opportunistic infections are infections and infection-related cancers that occur more frequently or are more severe in people with weakened immune systems than in people with healthy immune systems.)

Bonhoeffer et al.[76] suggested that template switching by reverse transcriptase acts as a repair process to deal with breaks in the single-stranded RNA genome. In addition, Hu and Temin[72] suggested that recombination is an adaptation for repair of damage in the RNA genomes. Strand switching (copy-choice recombination) by reverse transcriptase could generate an undamaged copy of genomic DNA from two damaged single-stranded RNA genome copies. This view of the adaptive benefit of recombination in HIV could explain why each HIV particle contains two complete genomes, rather than one. Furthermore, the view that recombination is a repair process implies that the benefit of repair can occur at each replication cycle, and that this benefit can be realized whether or not the two genomes differ genetically. On the view that recombination in HIV is a repair process, the generation of recombinational variation would be a consequence, but not the cause of, the evolution of template switching.[76]

Peripheral neuropathy is a problem with the functioning of the nerves outside of the spinal cord. Symptoms may include numbness, weakness, burning pain (especially at night), and loss of reflexes. Possible causes may include tunnel syndrome, meralgia paresthetica, vitamin or nutritional deficiencies, and illnesses like diabetes, syphilis, AIDS, and kidney failure. Most causes of peripheral neuropathy can be successfully treated or prevented.

Another group working contemporaneously with the Montagnier and Gallo groups was that of Dr. Jay Levy at the University of California, San Francisco. He independently discovered the AIDS virus in 1983 and named it the AIDS associated retrovirus (ARV).[138] This virus was very different from the virus reported by the Montagnier and Gallo groups. The ARV strains indicated, for the first time, the heterogeneity of HIV isolates and several of these remain classic examples of the AIDS virus found in the United States.[139]

The Siliciano laboratory occupies the eighth floor of the Miller Research Building, at the Johns Hopkins School of Medicine. The twenty-six-person research team—technicians, students, fellows, and faculty—works in an airy, open space and in a smaller Biosafety Level 3 facility on the north side of the building. There they handle the specimens of their clinic’s H.I.V.-positive subjects and many more from labs like Deeks’s worldwide. Inside a room with negative air pressure, researchers retrieve blood samples from an incubator and place them in a laminar flow hood, which draws up a stream of air. Nothing leaves the facility without being double-bagged and sterilized.

Sexual practices such as fellatio and cunnilingus appear to be relatively low risk but not absolutely safe (see Table: HIV Transmission Risk for Several Sexual Activities). Risk does not increase significantly if semen or vaginal secretions are swallowed. However, open sores in the mouth may increase risk.

The profound immunosupression seen in AIDS is due to the depletion of T4 helper lymphocytes. HIV is present at a high level in the blood immediately after exposure. It then settles down to a certain low level set-point during the incubation period that lasts from 3-8 weeks. During the incubation perid, there is a massive turnover of CD4 cells as the CD4 cells killed by HIV are replaced rapidly and efficiently. The immune system eventually succumbs and AIDS is developed when killed CD4 cells can no longer be replaced, as witnessed by high HIV-RNA, HIV-Antigen and low CD4 counts.

Behavioural changes among injectors and the prompt introduction of harm reduction measures such as needle exchange programmes from the mid-1980s probably prevented many other urban areas in the UK from experiencing the localised epidemics on the scale seen in Scotland. In the UK, sharing rates remain higher than in the mid-1990s with almost one in three injectors in the Unlinked Anonymous survey of injecting drug users reporting direct sharing of needles and syringes in the previous four weeks. The continuing transmission of hepatitis B and hepatitis C in those aged under 25 shows the potential for further HIV spread among injecting drug users.

Patients beginning ART sometimes deteriorate clinically, even though HIV levels in their blood are suppressed and their CD4 count increases, because of an immune reaction to subclinical opportunistic infections or to residual microbial antigens after successful treatment of opportunistic infections. IRIS usually occurs in the first months of treatment but is occasionally delayed. IRIS can complicate virtually any opportunistic infection and even tumors (eg, Kaposi sarcoma) but is usually self-limited or responds to brief regimens of corticosteroids.

Sequencing revealed that variation occurs throughout the HIV genome but is especially pronounced in the gene encoding the gp120 protein. By constantly changing the structure of its predominant surface protein, the virus can avoid recognition by antibodies produced by the immune system. Sequencing also has provided useful insight into genetic factors that influence viral activity. Knowledge of such factors is expected to contribute to the development of new drugs for the treatment of AIDS.

If treatment fails, drug susceptibility (resistance) assays can determine the susceptibility of the dominant HIV strain to all available drugs. Genotypic and phenotypic assays are available and can help clinicians select a new regimen that should contain at least 2 and preferably 3 drugs to which the HIV strain is more susceptible. The dominant HIV strain in the blood of patients who are taken off antiretroviral therapy may revert over months to years to the wild-type (ie, susceptible) strain because the resistant mutants replicate more slowly and are replaced by the wild type. Thus, if patients have not been treated recently, the full extent of resistance may not be apparent through resistance testing, but when treatment resumes, strains with resistance mutations often reemerge from latency and again replace the wild-type HIV strain.

A person is considered to have wasting syndrome if they lose 10% or more of their body weight and have had diarrhea or weakness and fever for more than 30 days, according to the U.S. Department of Health and Human Services.

There are two goals of treatment for pregnant women with HIV infection: to treat maternal infection and to reduce the risk of HIV transmission from mother to child. Women can pass HIV to their babies during pregnancy, during delivery, or after delivery by breastfeeding. Without treatment of the mother and without breastfeeding, the risk of transmission to the baby is about 25%. With treatment of the mother before and during birth and with treatment of the baby after birth, the risk decreases to less than 2%. Because of this benefit, it is recommended that all pregnant women be routinely tested for HIV as part of their prenatal care. diagnosed, there are several options for treatment, although some antiretroviral medications cannot be used in pregnancy and others have not been studied in pregnancy. For example, the medication efavirenz (Sustiva) is usually avoided in early pregnancy or in women who are likely to become pregnant. Fortunately, there are treatment regimens that have been shown to be well-tolerated by most pregnant women, significantly improving the outcome for mother and child. The same principles of testing for drug resistance and combining antiretrovirals that are used for nonpregnant patients are used for pregnant patients. All pregnant women with HIV should be treated with ART regardless of their CD4 cell count, although the choice of drugs may differ slightly from nonpregnant women. In developed countries, women also are instructed not to breastfeed their children.

One of the proteins that enters the cell with the viral genome is the viral reverse transcriptase, which transcribes the viral RNA into a complementary DNA (cDNA) copy. The viral cDNA is then integrated into the host cell genome by the viral integrase, which also enters the cell with the viral RNA. The integrated cDNA copy is known as the provirus. The infectious cycle up to the integration of the provirus is shown in Fig. 11.23. In activated CD4 T cells, virus replication is initiated by transcription of the provirus, as we will see in the next section. However, HIV can, like other retroviruses, establish a latent infection in which the provirus remains quiescent. This seems to occur in memory CD4 T cells and in dormant macrophages, and these cells are thought to be an important reservoir of infection.

Because disease-related complications can occur in untreated patients with high CD4 counts and because less toxic drugs have been developed, treatment with ART is now recommended for nearly all patients. The benefits of ART outweigh the risks in every patient group and setting that has been carefully studied. In the Strategic Timing of AntiRetroviral Treatment (START) study, 5472 treatment-naïve patients with HIV infection and CD4 counts > 350 cells/mL were randomized to start ART immediately (immediate initiation) or to defer ART until their CD4 count decreased to < 250 cells/mL(deferred initiation). AIDS, byname of acquired immunodeficiency syndrome, transmissible disease of the immune system caused by the human immunodeficiency virus (HIV). HIV is a lentivirus (literally meaning “slow virus”; a member of the retrovirus family) that slowly attacks and destroys the immune system, the body’s against infection, leaving an individual vulnerable to a variety of other infections and certain malignancies that eventually cause death. AIDS is the final stage of HIV infection, during which time fatal infections and cancers frequently arise.

Antibody tests in children younger than 18 months are typically inaccurate due to the continued presence of maternal antibodies.[106] Thus HIV infection can only be diagnosed by PCR testing for HIV RNA or DNA, or via testing for the p24 antigen.[26] Much of the world lacks access to reliable PCR testing and many places simply wait until either symptoms develop or the child is old enough for accurate antibody testing.[106] In sub-Saharan Africa as of 2007–2009 between 30 and 70% of the population were aware of their HIV status.[107] In 2009, between 3.6 and 42% of men and women in Sub-Saharan countries were tested[107] which represented a significant increase compared to previous years.[107]

Although widely used, alternative or complementary medications, such as herbal ones, have not been proven to be effective. According to some limited studies, mineral or vitamin supplements may provide some benefits in overall health. It is important to discuss these options with a healthcare provider because some of these options, even vitamin supplements, may interact with ARVs.

Key populations are groups who are at increased risk of HIV irrespective of epidemic type or local context. They include: men who have sex with men, people who inject drugs, people in prisons and other closed settings, sex workers and their clients, and transgender people.

The major ethical principles that must be considered when formulating policies for HIV counseling and testing include respect for autonomy, confidentiality, justice, protection of vulnerable individuals, and beneficence to both the woman tested and, if she is pregnant, to her newborn as well. Individuals offering testing need to be mindful not only of the benefits of testing but also its potential risks because, if a woman’s test result is positive, she faces the possibility of being ostracized by her family, friends, and community or being subjected to intimate partner violence. In addition, although the overt stigma of HIV infection has been reduced over the past 20 years, the potential for job discrimination, loss of health insurance, and loss of housing still exists.

Humoral response to HIV. The humoral immune response occurs later in infection; therefore, the level of antibodies during the acute infection is very low. Non-neutralising antibodies to structural proteins (i.e. P17 and P24) are first to appear and generally do not persist. Later neutralising antibodies specific to proteins, involved in the entry of the virus into the cells, will be generated. These antibodies are specific to: (1) the variable region of gp120 (V3); (2) CD4 binding sites and chemokine receptors (i.e., CXCR4 and CCR5); (3) the transmembrane protein gp41. Potent neutralizing antibodies have been shown to play a major role in controlling HIV infection in a few symptom-free HIV+ individuals who maintain high level of CD4+ T cells and low viral load.

HIV stands for human immunodeficiency virus. It is the virus that can lead to acquired immunodeficiency syndrome or AIDS if not treated. Unlike some other viruses, the human body can’t get rid of HIV completely, even with treatment. So once you get HIV, you have it for life.

By January of 2000, the Centers for Disease Control reported that, for the first time since the beginning of the AIDS epidemic, the majority of new HIV/AIDS cases could be found among African American and Latino men.

The World Health Organization first proposed a definition for AIDS in 1986.[26] Since then, the WHO classification has been updated and expanded several times, with the most recent version being published in 2007.[26] The WHO system uses the following categories:

The mortality rate in some countries has greatly increased. In South Africa (a country that, despite having a relatively late-onset HIV epidemic, has developed one of the highest prevalence rates), the all-cause HIV-associated mortality rate increased by 79% between 1997 and 2004. In women aged 25-34 years, mortality rates increased by 500% during this period.

Opportunistic infections may be caused by bacteria, viruses, fungi, and parasites that are normally controlled by the immune system.[35] Which infections occur depends partly on what organisms are common in the person’s environment.[28] These infections may affect nearly every organ system.[36]

Sexual transmission — it can happen when there is contact with infected sexual fluids (rectal, genital, or oral mucous membranes). This can happen while having sex without a condom, including vaginal, oral, and anal sex, or sharing sex toys with someone who is HIV-positive.

A final prevention strategy of last resort is the use of antiretrovirals as post-exposure prophylaxis, so-called “PEP,” to prevent infection after a potential exposure to HIV-containing blood or genital secretions. Animal studies and some human experience suggest that PEP may be effective in preventing HIV transmission, and it is based upon these limited data that current recommendations have been developed for health care workers and people in the community exposed to potentially infectious material. Current guidelines suggest that those experiencing a needle stick or who are sexually exposed to genital secretions of an HIV-infected person should take antiretrovirals for four weeks. Those individuals considering this type of preventative treatment, however, must be aware that post-exposure treatment cannot be relied upon to prevent HIV infection. Moreover, such treatment is not always available at the time it is most needed and is probably best restricted to unusual and unexpected exposures, such as a broken condom during intercourse. Nowhere are the two sides more split than on the issue of condoms. Schools in at least 23 cities sought to distribute condoms during the mid-to late-1990s. The assumption was that since students will have sex anyway—despite warnings not to—they had better be protected. Conservatives see this position as a cop-out in two ways: it sells values short and it undermines parental authority. In 1992, in Washington, D.C., critics erupted over a by the Public Health Commission to hand out condoms in junior and senior high schools without parental consent. William Brown, president of the D.C. Congress of Parents and Teachers, complained: “We are looking to build and reinforce and establish family values where they have been lost, and here we have an agency of our government that totally ignores those things we are working for.” Dr. Mary Ellen Bradshaw, the commission’s chief, replied: “Our whole focus is to save the lives of these children, stressing abstinence as the only sure way to avoid [AIDS] and making condoms available only after intensive education.” In other cities, upset parents simply sued. By 1992, Class Action lawsuits had been brought against school districts in New York City, Seattle, and Falmouth, Massachusetts, arguing that condom distribution violated parents’ right to privacy.

When CD4 T-cell numbers decline below a critical level, cell-mediated immunity is lost, and infections with a variety of opportunistic microbes appear (Fig. 11.29). Typically, resistance is lost early to oral Candida species and to Mycobacterium tuberculosis, which shows as an increased prevalence of thrush (oral candidiasis) and tuberculosis. Later, patients suffer from shingles, caused by the activation of latent herpes zoster, from EBV-induced B-cell lymphomas, and from Kaposi’s sarcoma, a tumor of endothelial cells that probably represents a response both to cytokines produced in the infection and to a novel herpes virus called HHV-8 that was identified in these lesions. Pneumonia caused by the fungus Pneumocystis carinii is common and often fatal. In the final stages of AIDS, infection with cytomegalovirus or Mycobacterium avium complex is more prominent. It is important to note that not all patients with AIDS get all these infections or tumors, and there are other tumors and infections that are less prominent but still significant. Rather, this is a list of the commonest opportunistic infections and tumors, most of which are normally controlled by robust CD4 T cell-mediated immunity that wanes as the CD4 T-cell counts drop toward zero (see Fig. 11.21).

In patients with HIV infection, certain syndromes are common and may require different considerations (see Table: Common Manifestations of HIV Infection by Organ System). Some patients present with cancers (eg, Kaposi sarcoma, B-cell lymphomas) that occur more frequently, are unusually severe, or have unique features in patients with HIV infection (see Cancers Common in HIV-Infected Patients). In other patients, neurologic dysfunction may occur.

HIV infections in the United States continue to be a major public health crisis. An estimated 1.2 million Americans are living with HIV, and 1 out of 8 people with HIV do not know they have it.1 Although recent data show that annual HIV infections declined 18% in the U.S. from 2008 to 2014, HIV continues to spread.2

After you get tested, it’s important to find out the result of your test so you can talk to your health care provider about treatment options if you’re HIV-positive or learn ways to prevent getting HIV if you’re HIV-negative.

Sexually transmitted diseases, or STDs, are infections that are transmitted during any type of sexual exposure, including intercourse (vaginal or anal), oral sex, and the sharing of sexual devices, such as vibrators. Women can contract all of the STDs, but may have no symptoms, or have different symptoms than men do. Common STDs in women are:

Other drugs can prevent or treat opportunistic infections (OIs). In most cases, these drugs work very well. The newer, stronger ARVs have also helped reduce the rates of most OIs. A few OIs, however, are still very difficult to treat. See Fact Sheet 500 for more information on opportunistic infections.

Some people infected with HIV are asymptomatic at first. Most people experience symptoms in the first month or two after becoming infected. That’s because your immune system is reacting to the virus as it rapidly reproduces.

If the source’s virus is known or suspected to be resistant to≥ 1 drug, an expert in antiretroviral therapy and HIV transmission should be consulted. However, clinicians should not delay PEP pending expert consultation or drug susceptibility testing. Also, clinicians should provide immediate evaluation and face-to-face counseling and not delay follow-up care.

A 2003 analysis in the Journal of Acquired Immune Deficiency Syndromes calculated that more than $18 billion in medical costs could have been saved by the year 2010 had the CDC invested just $383 million more in prevention programming per year from 2000 to 2005, an amount that theoretically could have cut the annual HIV infection rate in half.
