The first HIV vaccine efficacy study in seven years is currently underway in South Africa. The experimental vaccine is an updated version of one used in a 2009 trial that took place in Thailand. A 3.5-year follow up after vaccination showed the vaccine was 31.2 percent effective in preventing HIV infection. It’s the most successful HIV vaccine trial to date.

The new centerpiece of the American effort to cure H.I.V. is the Martin Delaney Collaboratories, funded by the N.I.H. Launched in 2011, the collaborative was formulated as a way to link clinical labs, research facilities, and pharmaceutical companies. Federal support was set at seventy million dollars for the first five years, on the premise of coöperation and open communication among all parties. The Human Immunodeficiency Virus (HIV) targets the immune system and weakens people’s defence systems against infections and some types of cancer. As the virus destroys and impairs the function of immune cells, infected individuals gradually become immunodeficient. Immune function is typically measured by CD4 cell count.

For primary prophylaxis against some fungal infections (eg, esophageal candidiasis, cryptococcal meningitis or pneumonia), oral fluconazole 100 to 200 mg once/day or 400 mg weekly is successful but is infrequently used because the cost per infection prevented is high and diagnosis and treatment of these infections are usually successful.

Immunodeficiency describes the condition in which the body’s immune response is damaged, weakened, or is not functioning properly. In AIDS, immunodeficiency results from the way that the virus binds to a protein called CD4, which is primarily found on the surface of certain subtypes of white blood cells. After the virus has attached to the cell’s CD4 receptor, the virus-CD4 complex refolds to uncover another receptor called a chemokine receptor that helps mediate entry of the virus into the cell. One chemokine receptor in particular, CCR5, has been the focus of recent research after studies showed that defects in its structure (caused by genetic mutations) result in a slowing or stopping of the progression of AIDS. Scientists hope that this discovery will lead to the development of drugs that trigger an artificial mutation of the CCR5 gene or target the CCR5 receptor.

HIV: Acronym for the Human Immunodeficiency Virus, the cause of AIDS (acquired immunodeficiency syndrome).
