“Black men are not just out here having unprotected sex willy-nilly; the science disproves that,” said Terrance Moore, deputy executive director of the National Alliance of State and Territorial AIDS Directors in Washington. He pointed to stacks of studies over the years, including a groundbreaking, exhaustive 2006 data dive led by Greg Millett that was published in The American Journal of Public Health. In this and other studies, Millett and his colleagues found that gay black men engage in risky sexual practices no more frequently, are as consistent about condom use and have fewer sex partners than their nonblack peers. “It’s that the viral load in communities of black gay men is higher, which puts them at disproportionate risk,” Moore explained. “Plus, these are the same individuals that are dealing with structural barriers around lack of employment, lack of education and opportunities, transportation and, of course, very, very overt institutional racism.”

HIV cannot survive more than a few minutes outside the body. The virus does not spread through casual contact such as preparing food, sharing towels and bedding, or via swimming pools, telephones, sneezing, or toilet seats. Transmission through kissing alone is extremely rare.
At any time during the course of HIV infection, patients may develop a yeast infection in the mouth called thrush, open sores or ulcers, or other infections of the mouth; diarrhea and other gastrointestinal symptoms that cause malnutrition and weight loss; diseases of the lungs and kidneys; and degeneration of the nerve fibers in the arms and legs. Infection with the human immunodeficiency virus (HIV) is the cause of acquired immune deficiency syndrome (AIDS). This worldwide epidemic is now spreading at an alarming rate, especially through heterosexual contact in less-developed countries. HIV is an enveloped retrovirus that replicates in cells of the immune system. Viral entry requires the presence of CD4 and a particular chemokine receptor, and the viral cycle is dependent on transcription factors found in activated T cells. Infection with HIV causes a loss of CD4 T cells and an acute viremia that rapidly subsides as cytotoxic T-cell responses develop, but HIV infection is not eliminated by this immune response. HIV establishes a state of persistent infection in which the virus is continually replicating in newly infected cells. The current treatment consists of combinations of viral protease inhibitors together with nucleoside analogues and causes a rapid decrease in virus levels and a slower increase in CD4 T-cell counts. The main effect of HIV infection is the destruction of CD4 T cells, which occurs through the direct cytopathic effects of HIV infection and through killing by CD8 cytotoxic T cells. As the CD4 T-cell counts wane, the body becomes progressively more susceptible to opportunistic infection with intracellular microbes. Eventually, most HIV-infected individuals develop AIDS and die; however a small minority (3–7%), remain healthy for many years, with no apparent ill effects of infection. We hope to be able to learn from these individuals how infection with HIV can be controlled. The existence of such people and other people who have been naturally immunized against infection gives hope that it will be possible to develop effective vaccines against HIV.

HIV infection takes different forms within different cells. As we have seen, more than 95% of the virus that can be detected in the plasma is derived from productively infected cells, which have a very short half-life of about 2 days. Productively infected CD4 lymphocytes are found in the T-cell areas of lymphoid tissue, and these are thought to succumb to infection in the course of being activated in an immune response. Latently infected memory CD4 cells that are activated in response to antigen presentation also produce virus. Such cells have a longer half-life of 2 to 3 weeks from the time that they are infected. Once activated, HIV can spread from these cells by rounds of replication in other activated CD4 T cells. In addition to the cells that are infected productively or latently, there is a further large population of cells infected by defective proviruses; such cells are not a source of infectious virus.

HIV seeks out and destroys CCR5 expressing CD4+ T cells during acute infection. A vigorous immune response eventually controls the infection and initiates the clinically latent phase. CD4+ T cells in mucosal tissues remain particularly affected. Continuous HIV replication causes a state of generalized immune activation persisting throughout the chronic phase. Immune activation, which is reflected by the increased activation state of immune cells and release of pro-inflammatory cytokines, results from the activity of several HIV gene products and the immune response to ongoing HIV replication. It is also linked to the breakdown of the immune surveillance system of the gastrointestinal mucosal barrier caused by the depletion of mucosal CD4+ T cells during the acute phase of disease.
With the numbers of those who acquired their infections heterosexually there has been an decrease in the number of women diagnosed. The male:female ratio for all new infections diagnosed in 2008 was about 1.6:1 whereas in 2012 it was 2.6:1.
HIV is not spread to a person who donates blood or organs. People who donate organs are never in direct contact with the people who receive them. Likewise, a person who donates blood is never in contact with the person receiving it. In all of these procedures, sterile needles and instruments are used.

Without treatment, human immunodeficiency virus (HIV) infection will usually result in acquired immune deficiency syndrome (AIDS). However, in Australia the HIV therapies introduced in the mid-1990s, which are available to all Australians living with HIV, have resulted in fewer AIDS related illnesses and deaths. Therefore, whilst a cure is yet to be found for HIV and it remains a lifelong infection, HIV in Australia is now considered a chronic manageable condition.

Scientists who study (look at and learn about) people who use condoms, see that if teenagers (children 13–19) learn about condoms (and other birth control) they have less unsafe sex. Scientists see that learning about these things does not make teenagers start having sex earlier. The teenagers also have safer sex. Safer sex means doing things (like wearing condoms) to try not to get pregnant or get sexually transmitted diseases (STDs or STIs) like HIV, gonorrhea, and syphilis. Using a condom works very well for keeping people from getting pregnant or getting STDs if people know how to use a condom the right way.

Restricting sexual activity to a single partner and practicing safer sex (i.e., always using a condom). Besides avoiding the risk of HIV infection, condoms are successful in reducing other sexually transmitted diseases and unwanted pregnancies. Before engaging in a sexual relationship with someone, getting tested for HIV infection is recommended.
The best combination of drugs for HIV are those that effectively suppress viral replication in the blood and also are well tolerated and simple to take so that people can take the medications consistently without missing doses.
Humoral response to HIV. The humoral immune response occurs later in infection; therefore, the level of antibodies during the acute infection is very low. Non-neutralising antibodies to structural proteins (i.e. P17 and P24) are first to appear and generally do not persist. Later neutralising antibodies specific to proteins, involved in the entry of the virus into the cells, will be generated. These antibodies are specific to: (1) the variable region of gp120 (V3); (2) CD4 binding sites and chemokine receptors (i.e., CXCR4 and CCR5); (3) the transmembrane protein gp41. Potent neutralizing antibodies have been shown to play a major role in controlling HIV infection in a few symptom-free HIV+ individuals who maintain high level of CD4+ T cells and low viral load.
