Cure of HIV infection has not been thought possible, and thus lifelong drug treatment is considered necessary. Patients living with HIV infection should be urged to take their antiretroviral drugs consistently. An instance of a possible cure was widely reported in an infant with transient eradication of replication-competent HIV after about 15 mo of antiretroviral therapy. However, HIV replication subsequently resumed. In a large international clinical trial, risk of opportunistic infection or death from any cause, particularly from premature coronary artery disease, cerebrovascular events, or liver and kidney disorders, was significantly higher when antiretroviral therapy was taken episodically (guided by the CD4 count) than when it was taken continuously (1).

Activated cells that become infected with HIV produce virus immediately and die within one or two days. The vast majority of viruses present in the plasma can be attributed to the short-lived, activated cells. It takes approximately 1.5 days to complete a single HIV life-cycle. Resting cells that become infected produce virus only after immune stimulation and these cells have a half-life of at least 5-6 months. Some cells are infected with defective virus that cannot complete the viral cycle. Such cells survive for a long period of time and have an estimated half-life of 3-6 months. An immune deficiency disease occurs when the immune system is not working properly. If you are born with a deficiency or if there is a genetic cause, it is called primary immunodeficiency disease. There are more than 100 primary immunodeficiency disorders.
This flu-like illness may be so mild it goes unnoticed, or in some people it may be quite severe and last for a few weeks before there is a return to seemingly normal health. Either way, this illness at the beginning of the infection is so similar to many other viral infections that the diagnosis of HIV infection may not be made at this time.

Side effects associated with EFV are mostly dizziness, confusion, fatigue, and vivid dreams. These tend to be most prominent during the first weeks of therapy and then often decrease in severity. It is generally recommended that EFV be taken at bedtime so that the patient is asleep during the time dizziness and confusion may be most severe. It is also noteworthy that there may be an increased risk of depression associated with the use of this drug, and it should be used with caution in those with poorly managed depression. Rash and liver inflammation can occur with both EFV and DLV, and these drugs may also be linked to abnormalities of lipids in the blood.
Protease inhibitors. Protease inhibitors work by disabling protease, an enzyme necessary for HIV reproduction. Protease inhibitors include saquinavir (Invirase), ritonavir (Norvire), indinavir (Crixivan), nelfinavir (Viracept), amprenavir (Agenerase), kaletra, and many others.
“At this point the virus is moving into the blood stream and starting to replicate in large numbers,” says Carlos Malvestutto, MD, instructor of infectious diseases and immunology in the department of medicine at NYU School of Medicine in New York City. “As that happens, there is an inflammatory reaction by the immune system.”

Drug therapy is often recommended for patients who are committed to taking all their medications and have a CD4 count less than 500 (indicating immune system suppression) or a high viral load (amount of HIV virus in the bloodstream).

By the late 1980s, much of the harshness in public debate had diminished. Both liberals and conservatives lined up to support legislative solutions. President ronald reagan left office, recommending increases in federal funding for medical research on AIDS. Already the amount spent in this area had risen from $61 million in 1984 to nearly $1.3 billion in 1988. President george h.w. bush took a more active approach, and in 1990 signed two new bills into law. One was the Ryan White Comprehensive AIDS Resources Emergency (CARE) Act (Pub. L. No. 101-381, 104 Stat. 576), which provides much-needed money for states to spend on treatment. The other was the ground-breaking Americans with Disabilities Act (ADA) (42 U.S.C.A. §§ 12112–12117), which has proved to be the most effective weapon against the discrimination that individuals with the disease routinely suffer. Bush also hurried approval by the Food and Drug Administration for AIDS-related drugs. Though he supported Americans with the disease, Bush agreed to a controversial ban by Congress on travel and immigration to the United States for people with HIV.
The new formulation of tenofovir (TAF) is available as combination pills only, including EVG/COBI/FTC/TAF (Genvoya) (150/150/200/10 mg), FTC/TAF (200/25 mg) and TAF/FTC/RPV (25/200/25 mg). There is also single tablet boosted PI in advanced stages of development, DRV/COBI/FTC/TAF (800/150/200/10 mg). The new formulation of tenofovir results in lower plasma levels and higher intracellular concentrations of the active drug. Data to date suggests that compared to TDF-containing regimens this form is equally effective with less adverse effects on bone mineral density and possibly on the kidneys.
HIV drugs (antiretroviral drugs), usually three or more taken together, can stop HIV from reproducing, strengthen the immune system, and thus make people less susceptible to infection, but the drugs cannot, with rare exceptions, eliminate HIV, which persists in an inactive form.

HIV is a retrovirus that causes AIDS. HIV attacks the immune system. This system consists of cells and organs that protect the body against diseases like infections and cancer. HIV attacks the immune system through special types of white blood cell known as CD4 cells. CD4 cells play an important role in orchestrating and controlling the functions of the whole immune system.
HIV produces cellular immune deficiency characterized by the depletion of helper T lymphocytes (CD4+ cells). The loss of CD4+ cells results in the development of opportunistic infections and neoplastic processes.
If you have these symptoms, that doesn’t mean you have HIV. Each of these symptoms can be caused by other illnesses. But if you have these symptoms after a potential exposure to HIV, see a health care provider and tell them about your risk. The only way to determine whether you are infected is to be tested for HIV infection.
“Charlie does not have AIDS,” Huizenga said. “AIDS is a condition where the HIV virus markedly suppresses the immune system and you are susceptible to rare, difficult cancers and infections. Charlie has none of those. He is healthy; he does not have AIDS.”

Turning things around would mean expanding testing and providing affordable treatment for those who are positive — to stop sickness and dying and also to block transmission of the virus. It would also require getting information and medication, including PrEP, to those most at risk. Even more challenging would be reducing the stigma, discrimination and shame that drive gay and bisexual men to hide their sexuality and avoid the health care system — and making sure providers have adequate resources and understand how to care for H.I.V. patients.
Choose less risky sexual behaviors. Anal sex is the highest-risk sexual activity for HIV transmission, especially for the receptive partner (bottom). Oral sex is much less risky than anal or vaginal sex. Sexual activities that don’t involve contact with body fluids (semen, vaginal fluid, or blood) carry no risk of HIV transmission.
