acquired immune deficiency syndrome of humans, caused by the lentivirus, human immunodeficiency virus 1 (HIV1), less commonly HIV2. The virus initially infects macrophages and then attacks and destroys T helper CD4 lymphocytes, thereby producing immunodeficiency and resulting in death, usually after a very prolonged incubation period followed by a very prolonged clinical course. A very similar virus SIV1 causes simian AIDS in captive macaque monkeys. A further similar virus SIV2 has been isolated from healthy green monkeys.

Although the symptoms of immune deficiency characteristic of AIDS do not appear for years after a person is infected, the bulk of CD4+ T cell loss occurs during the first weeks of infection, especially in the intestinal mucosa, which harbors the majority of the lymphocytes found in the body.[99] The reason for the preferential loss of mucosal CD4+ T cells is that the majority of mucosal CD4+ T cells express the CCR5 protein which HIV uses as a co-receptor to gain access to the cells, whereas only a small fraction of CD4+ T cells in the bloodstream do so.[100] A specific genetic change that alters the CCR5 protein when present in both chromosomes very effectively prevents HIV-1 infection.[101]

In April 1984, the National Cancer Institute announced they had found the cause of AIDS, the retrovirus HTLV-III. In a joint conference with the Pasteur Institute they announced that LAV and HTLV-III are identical and the likely cause of AIDS.22 A blood test was created to screen for the virus with the hope that a vaccine would be developed in two years.23

Infection with the human immunodeficiency virus (HIV) is the cause of acquired immune deficiency syndrome (AIDS). This worldwide epidemic is now spreading at an alarming rate, especially through heterosexual contact in less-developed countries. HIV is an enveloped retrovirus that replicates in cells of the immune system. Viral entry requires the presence of CD4 and a particular chemokine receptor, and the viral cycle is dependent on transcription factors found in activated T cells. Infection with HIV causes a loss of CD4 T cells and an acute viremia that rapidly subsides as cytotoxic T-cell responses develop, but HIV infection is not eliminated by this immune response. HIV establishes a state of persistent infection in which the virus is continually replicating in newly infected cells. The current treatment consists of combinations of viral protease inhibitors together with nucleoside analogues and causes a rapid decrease in virus levels and a slower increase in CD4 T-cell counts. The main effect of HIV infection is the destruction of CD4 T cells, which occurs through the direct cytopathic effects of HIV infection and through killing by CD8 cytotoxic T cells. As the CD4 T-cell counts wane, the body becomes progressively more susceptible to opportunistic infection with intracellular microbes. Eventually, most HIV-infected individuals develop AIDS and die; however a small minority (3–7%), remain healthy for many years, with no apparent ill effects of infection. We hope to be able to learn from these individuals how infection with HIV can be controlled. The existence of such people and other people who have been naturally immunized against infection gives hope that it will be possible to develop effective vaccines against HIV.

In the UK in 2012, 15 donors tested positive for HIV infection at screening. This represented 0.6 detected infections per 100,000 donations. These were mainly in men who probably acquired the infection via heterosexual transmission.[5]

The prevalence of women with HIV in the United States is low compared to the rate in many countries in the developing world. Worldwide about half the people living with HIV are women. According to the United Nations, in 2005 about 59% of women living in sub-Saharan Africa are infected with HIV. The vast majority of them were infected through sex with an infected male partner.

The topic of religion and AIDS has become highly controversial in the past twenty years, primarily because some religious authorities have publicly declared their opposition to the use of condoms.[261][262] The religious approach to prevent the spread of AIDS according to a report by American health expert Matthew Hanley titled The Catholic Church and the Global AIDS Crisis argues that cultural changes are needed including a re-emphasis on fidelity within marriage and sexual abstinence outside of it.[262]

The Centers for Disease Control has defined AIDS as beginning when a person with HIV infection has a CD4 cell (also called “t-cell”, a type of immune cell) count below 200. It is also defined by numerous opportunistic infections and cancers that occur in the presence of HIV infection.

  1. Since then, H.I.V. has been transformed into a treatable condition, one of the great victories of modern medicine. In 1987, the F.D.A. approved AZT, a cancer drug that had never gone to market, for use in H.I.V. patients. At first, it was extortionately priced and was prescribed in high doses, which proved toxic, provoking protest from the gay community. But AZT was able to insinuate itself into the virus’s DNA as it formed, and later it was used in lower doses. Scientists have now developed more than thirty antiretroviral medicines that stop H.I.V. from reproducing in helper T cells.

    Strategies to reduce the risk of HIV infection include not having sex, limiting your number of sexual partners, never sharing needles, and using condoms the right way every time you have sex. People who are at high risk may take HIV prevention medicines.
    Though there are two cases of people who have been cured, there is currently no safe cure for HIV (see fact sheet 485.) There is no way to “clear” HIV from the body. Antiretroviral therapy (ART, see fact sheet 403) can prevent or reverse the damage to your immune system. Most people stay healthy if they stay adherent to ART.
