When HIV infection is advanced, either through treatment failure or in untreated infection, and has caused immune system destruction, secondary infections (opportunistic infections) can occur. Using other antiviral drugs and antibiotics to prevent secondary infection may prevent severe illness and premature (early) death.
In 2006, male circumcision was found to reduce the risk of female-to-male HIV transmission by 60%.81 Since then, the WHO and UNAIDS have emphasised that male circumcision should be considered in areas with high HIV and low male circumcision prevalence.82
Patients beginning ART sometimes deteriorate clinically, even though HIV levels in their blood are suppressed and their CD4 count increases, because of an immune reaction to subclinical opportunistic infections or to residual microbial antigens after successful treatment of opportunistic infections. IRIS usually occurs in the first months of treatment but is occasionally delayed. IRIS can complicate virtually any opportunistic infection and even tumors (eg, Kaposi sarcoma) but is usually self-limited or responds to brief regimens of corticosteroids.

If the CD4 count drops below 50 cells per microliter of blood, azithromycin taken weekly or clarithromycin taken daily may prevent Mycobacterium avium complex infections. If people cannot take either of these drugs, they are given rifabutin.
Since the Bergalis case, many U.S. dentists, physicians, and surgeons with AIDS have begun disclosing their status to their patients. Faya v. Almaraz, 329 Md. 435, 620 A.2d 327 (Md. 1993), illustrates the consequences of not doing so. In Faya, the court held that an HIV-positive doctor has the legal duty to disclose this medical condition to patients and that a failure to inform can lead to a Negligence action, even if the patients have not been infected by the virus. The doctor’s patient did not contract HIV but did suffer emotionally from a fear of having done so. The unanimous decision held that patients can be compensated for their fears. Although this case dealt specifically with doctor-patient relationships, others have concerned a variety of relationships in which the fear of contracting AIDS can be enough for a plaintiff to recover damages.

Drugs used to treat HIV and AIDS do not eliminate the infection. Although effective ART reduces the risk of transmitting HIV, it is important for the person to remember that he or she is still contagious even when receiving effective treatment. Intensive research efforts are being focused on developing new and better treatments. Although currently there is no promising vaccine, work continues on this front.
The main cellular target of HIV is a special class of white blood cells critical to the immune system known as helper T lymphocytes, or helper T cells. Helper T cells are also called CD4+ T cells, because they have on their surfaces a protein called CD4. Helper T cells play a central role in normal immune responses by producing factors that activate virtually all the other immune system cells. Those include B lymphocytes, which produce antibodies needed to fight infection; cytotoxic T lymphocytes, which kill cells infected with a virus; and macrophages and other effector cells, which attack invading pathogens. AIDS results from the loss of most of the helper T cells in the body.
Latent toxoplasmosis: This asymptomatic condition is indicated by serum antibodies (IgG) to Toxoplasma gondii. TMP/SMX (in doses used to prevent P. jirovecii pneumonia) is used to prevent reactivation and consequent toxoplasmic encephalitis. Latent infection is less common (about 15% of adults) in the US than in Europe and most developing countries (up to 70 to 80% of adults).
As the infection progressively weakens the immune an individual can develop other signs and symptoms, such as swollen lymph nodes, weight loss, fever, diarrhoea and cough. Without treatment, they could also develop severe illnesses such as tuberculosis, cryptococcal meningitis, severe bacterial infections and cancers such as lymphomas and Kaposi’s sarcoma, among others.
Peripheral neuropathy is a problem with the functioning of the nerves outside of the spinal cord. Symptoms may include numbness, weakness, burning pain (especially at night), and loss of reflexes. Possible causes may include carpel tunnel syndrome, meralgia paresthetica, vitamin or nutritional deficiencies, and illnesses like diabetes, syphilis, AIDS, and kidney failure. Most causes of peripheral neuropathy can be successfully treated or prevented.
The group turned toward Benjamin Jennings, who wore a serious expression, with a shock of long hair in dreadlocks flipped to the side. When he said it was his first time there, everyone clapped. “I was diagnosed July 8 of this year, and my goal is to learn everything that I can about this thing,” said Jennings, 21, talking in a tumble of words as he pulled at his cropped T-shirt. “The first person I told was my mom. Thank God — I am so lucky to have her in my life.” He paused, looking into the faces of the men around the table and speaking more slowly. “I used to keep my feelings bottled up, but then I started opening my mouth on it,” he said. “I did everything to prevent this disease, but because of one slip-up I have it. Now I want to help anyone I can in any type of way. My goal is to not to let anyone judge me or let this disease own me.”
Exposure to HIV does not always lead to infection, and some people who have had repeated exposures over many years remain uninfected. Moreover, many HIV-infected people remain well for more than a decade. A very few HIV-infected, untreated people have remained well for over 20 years. Why some people become ill so much sooner than others is not fully understood, but a number of genetic factors appear to influence both susceptibility to infection and progression to AIDS after infection.
Other important pathogens include cytomegalovirus, (which causes retinitis, pneumonitis, and colitis) and Pneumocystis jiroveci (formerly known as Pneumocystis carinii; the causative organism in Pneumocystis pneumonia). In immunocompetent hosts, these organisms are generally nonpathogenic, and asymptomatic infection is common (and in the case of cytomegalovirus infection, life-long).
There are two types of HIV viruses, both resulting in forms of AIDS which are indistinguishable from each other. In addition to the two major types, HIV-1 and HIV-2. The genetic composition of these different types distinguishes one from the other.
