In June, the first reports of AIDS in children hinted that it could be passed via casual contact but this was later ruled out and it was concluded that they had probably directly acquired AIDS from their mothers before, during or shortly after birth.17
Rapid test results usually will be available during the same clinical visit that the specimen (eg, blood or oral swab sample) is collected. Obstetrician–gynecologists who use these tests must be prepared to provide counseling to women who receive positive test results the same day that the specimen is collected. Women with positive test results should be counseled regarding the meaning of these preliminarily positive test results and the need for confirmatory testing (11). Obstetrician–gynecologists should develop collaborative care plans with health care professionals who can provide these counseling services on an emergent basis or train their own staff to handle the initial encounter and, thereafter, transition infected individuals to professionals who can serve as ongoing resources to them. Women whose confirmatory testing yields positive results and, therefore, are infected with HIV should receive or be referred for appropriate clinical and supportive care.
According to the Joint United Nations Programme on HIV/AIDS (UNAIDS),  worldwide in 2015, approximately 36.7 million people (1% of the global adult population aged 15-49 years) were infected with HIV, a decline from 2006 (39.5 million reported at that time). UNAIDS estimates that approximately 2.1 million people were newly infected with HIV and that 1.1 million people died of AIDS in 2015, both statistics showing a decline over time.
Viruses. AIDS patients are highly vulnerable to cytomegalovirus (CMV), herpes simplex virus (HSV), varicella zoster virus (VZV), and Epstein-Barr virus (EBV) infections. Another virus, JC virus, causes progressive destruction of brain tissue in the brain cerebrum, and cerebellum (multifocal leukoencephalopathy or PML), which is regarded as an AIDS-defining illness by the CDC.

AIDS is the most severe form of HIV infection. HIV infection is considered to be AIDS when at least one serious complicating illness develops or the number (count) of CD4+ lymphocytes decreases substantially.
HIV Encephalopathy is a severe condition usually seen in end-stage disease. Milder cognitive impairments may exist with less advanced disease. For example, one study found significant deficits in cognition, planning, coordination and reaction times in HIV-infected compared to uninfected children, effects that were more pronounced in those with higher viral loads.

Macrophage-tropic (M-tropic) strains of HIV-1, or non-syncytia-inducing strains (NSI; now called R5 viruses) use the β-chemokine receptor CCR5 for entry and are, thus, able to replicate in both macrophages and CD4+ T cells. This CCR5 co-receptor is used by almost all primary HIV-1 isolates regardless of viral genetic subtype. Indeed, macrophages play a key role in several critical aspects of HIV infection. They appear to be the first cells infected by HIV and perhaps the source of HIV production when CD4+ cells become depleted in the patient. Macrophages and microglial cells are the cells infected by HIV in the central nervous system. In tonsils and adenoids of HIV-infected patients, macrophages fuse into multinucleated giant cells that produce huge amounts of virus.
As of 2009, it is estimated that there are 33.3 million people worldwide infected with HIV. The HIV pandemic is most severe in Sub-Saharan Africa. Over 60% of all people with HIV live in the region.
In people with unmasked IRIS, doctors treat the newly identified opportunistic infection with antimicrobial drugs. Occasionally, when the symptoms are severe, corticosteroids are also used. Usually, when unmasked IRIS occurs, cART is continued. An exception is when a cryptococcal infection affects the brain. Then cART is temporarily interrupted until the infection is controlled.
When a patient is infected with HIV, the virus slowly begins to destroy that patient’s immune system. How fast this occurs is different in each individual. Treatment with HAART can help slow and even halt the destruction of the immune system.

The O (“Outlier”) group is not usually seen outside of West-central Africa. It is reportedly most common in Cameroon, where a 1997 survey found that about 2% of HIV-positive samples were from Group O. The group caused some concern because it could not be detected by early versions of the HIV-1 test kits. More advanced HIV tests have now been developed to detect both Group O and Group N.

AIDS is an advanced stage of HIV infection. Because the CD4 cells in the immune system have been largely destroyed, people with AIDS often develop symptoms and signs of unusual infections or cancers. When a person with HIV infection gets one of these infections or cancers, it is referred to as an “AIDS-defining condition.” Examples of AIDS-defining conditions are listed in Table 1. Significant, unexplained weight loss also is an AIDS-defining condition. Because common conditions like cancer or other viral conditions like infectious mononucleosis also can cause weight loss and fatigue, it is sometimes easy for a physician to overlook the possibility of HIV/AIDS. It is possible for people without AIDS to get some of these conditions, especially the more common infections like tuberculosis.
ART can usually achieve its goals if patients take their drugs > 95% of the time. However, maintaining this degree of adherence is difficult. Partial suppression (failure to lower plasma levels to undetectable levels) may select for single or multiple accumulated mutations in HIV that make viruses partially or completely resistant to a single drug or entire classes of drugs. Unless subsequent treatment uses drugs of other classes to which HIV remains sensitive, treatment is more likely to fail.
