For HIV treatment to be effective in reducing HIV incidence, infections need to be diagnosed as quickly as possible. This requires increasing HIV testing coverage and frequency. CDC recommends testing all persons aged 13–64 years at least once as a routine part of medical care and more frequent testing (at least annually) for persons at high risk for HIV infection (7). A large proportion (84%) of HIV sexually transmitted from MSM and heterosexual persons is transmitted by MSM (1). Some sexually active MSM might benefit from more frequent testing (e.g., every 3 to 6 months) (18). Testing according to CDC guidelines is critical to diagnosing HIV infection, so that anyone who receives a diagnosis of HIV infection can start antiretroviral treatment. Overall, prior year testing increased among groups at high risk over time. However, 29% of MSM (in 2014), 42% of persons who inject drugs (in 2015), and 59% of heterosexual persons at increased risk (in 2016) did not report testing in the past 12 months. In addition, it is important to note that these data are from persons residing in large metropolitan statistical areas in the United States. Studies have found that persons residing in rural areas are less likely to report prior HIV testing, including in the past 12 months, compared with their urban counterparts, and that persons living in rural areas are more likely to have HIV infection diagnosed at a late stage (19,20). Barriers to implementing routine testing include lack of time, competing priorities, and concerns about reimbursement on the health care provider’s part and stigma and lack of perceived risk on the client’s part (21). Lack of perceived risk was also one of the main reasons cited by MSM in NHBS for not testing in the past 12 months.
Stage II (also known as clinically asymptomatic stage): This stage may last for 8-10 years with no major symptoms except for swollen glands (lymph nodes), some weight loss, mouth ulceration and mild skin and nail infections.
Near the end of life, many people have pain and other distressing symptoms (such as agitation) and usually lose their appetite. Hospice programs are particularly equipped to deal such problems. They can provide comprehensive support and care, which focuses on managing symptoms, helping dying people maintain their independence, and supporting their caregivers.
“In the early stages of HIV infection, the most common symptoms are none,” says Michael Horberg, MD, director of HIV/AIDS for Kaiser Permanente, in Oakland, Calif. One in five people in the United States with HIV doesn’t know they have it, which is why it’s so important to get tested, especially if you have unprotected sex with more than one partner or use intravenous drugs.
Although the tests for detecting HIV infection continue to improve, they still require that people volunteer for testing. It is estimated that approximately 15% of those infected with HIV in the United States are unaware of their infection because they have never been tested. In order to decrease the number that are unaware of their HIV infection status, in 2006, the Centers for Disease Control and Prevention recommended that all people between 13 and 64 years of age be provided HIV testing whenever they encounter the health care system for any reason. In addition, resources are available to facilitate people finding local HIV testing centers (https://gettested.cdc.gov/).

These studies show that most of the HIV present in the circulation of an infected individual is the product of rounds of replication in newly infected cells, and that virus from these productively infected cells is released into, and rapidly cleared from, the circulation at the rate of 109 to 1010 virions every day. This raises the question of what is happening to these virus particles: how are they removed so rapidly from the circulation? It seems most likely that HIV particles are opsonized by specific antibody and complement and removed by phagocytic cells of the mononuclear phagocyte system. Opsonized HIV particles can also be trapped on the surface of follicular dendritic cells, which are known to capture antigen:antibody complexes and retain them for prolonged periods (see Chapters 9 and 10).

During the 2004 election, the PBS journalist Gwen Ifill brought the issue to the mainstream stage as the moderator for the vice-presidential debate. She asked the candidates Dick Cheney and John Edwards what they planned to do to end the spread of H.I.V./AIDS — “not about AIDS in China or Africa, but AIDS right here in this country” — among black women. Cheney replied that he was not aware of the numbers, while Edwards spent more than a minute discussing AIDS in Africa. In 2006, I attended the International AIDS Conference in Toronto with a delegation of black journalists, civil rights leaders, government officials, politicians and celebrities, including the singer Sheryl Lee Ralph, Representatives Maxine Waters and Barbara Lee, the Rev. Jesse Jackson and Julian Bond, chairman of the N.A.A.C.P., who famously announced, “Now is the time for us to face the fact that AIDS has become a black disease.”
