Researchers are actively working on producing preventative and therapeutic vaccines for HIV. Preventative vaccines immunize an individual against a disease, so that he or she does not become infected. A therapeutic vaccine, also called a treatment vaccine, does not keep someone from getting a disease the way a preventative vaccine does. Instead, therapeutic vaccines are used to boost the body’s immune system in order to help control infection. The potential exists to prolong life indefinitely using these and other drug therapies to boost the immune system, keep the virus from replicating, and ward off opportunistic infections and malignancies.
Therese Frare’s photograph of gay activist David Kirby, as he lay dying from AIDS while surrounded by family, was taken in April 1990. LIFE magazine said the photo became the one image “most powerfully identified with the HIV/AIDS epidemic.” The photo was displayed in LIFE magazine, was the winner of the World Press Photo, and acquired worldwide notoriety after being used in a United Colors of Benetton advertising campaign in 1992. In 1996, Johnson Aziga, a Ugandan-born Canadian was diagnosed with HIV, but subsequently had unprotected sex with 11 women without disclosing his diagnosis. By 2003 seven had contracted HIV, and two died from complications related to AIDS. Aziga was convicted of first-degree murder and was sentenced for life.
Most individuals infected with HIV will progress to AIDS if not treated. However, there is a tiny subset of patients who develop AIDS very slowly, or never at all. These patients are called non-progressors.
A failure to inform in AIDS cases has given rise to both civil and criminal lawsuits. One such case was brought by Mark Christian, the lover of actor Rock Hudson, against Hudson’s estate. Christian won his suit on the ground that Hudson concealed his condition and continued their relationship, and the jury returned a multimillion-dollar verdict despite the fact that there was no evidence that Christian had been infected. Another case was brought in Oregon in 1991, when criminal charges were filed against Alberto Gonzalez for knowingly spreading HIV by having sex with his girlfriend. After Gonzalez pleaded no contest to third-degree assault (a felony) and to two charges of recklessly endangering others, he received an unusual sentence: the court ordered him to abstain from sex for five years and placed him under house arrest for six months. Although such convictions are increasingly common, courts have also recognized that not knowing one has HIV can be a valid defense. In C. A. U. v. L., 438 N.W.2d 441 (1989), for example, the Minnesota Court of Appeals affirmed a trial court’s finding that the plaintiff could not recover damages from her former fiancé, who had unknowingly given her the virus.

After infection with HIV, it can take from 3 weeks to 6 months for the virus to show up in testing. Re-testing may be necessary. If the moment an individual was most at risk of infection was within the last 6 months, they can have the test immediately. However, the provider will urge that another test is carried out within a few weeks.

With ‘M’ for “major”, this is by far the most common type of HIV, with more than 90% of HIV/AIDS cases deriving from infection with HIV-1 group M. The M group is subdivided further into clades, called subtypes, that are also given a letter. There are also “circulating recombinant forms” or CRFs derived from recombination between viruses of different subtypes which are each given a number. CRF12_BF, for example, is a recombination between subtypes B and F.
Beginning in the late ’90s, the United States government funneled billions of federal dollars into abstinence-until-marriage programs here and abroad. In place of effective sex education, these programs often discouraged condom use while teaching abstinence as the only way to prevent the spread of AIDS — even as well-regarded research established that this kind of sex education does not lower the risk of contracting H.I.V. and other sexually transmitted diseases.

HIV infection occurs when particular body fluids (blood, semen, vaginal fluid and breast milk) containing the virus come into contact with another person’s tissues beneath the skin (for example, though needle puncture or broken skin), or mucous membranes (the thin moist lining of many parts of the body such as the nose, mouth, throat and genitals).

The earliest unambiguously identified HIV-antibody positive serum stems from Kinhasa, Zaire dating back to 1959. HIV infection spread unrecognized in the 1960s and 1970s before it was finally recognized in 1981. The spread of the virus has been phenomenal thereafter, and close to 40 million people are estimated to be infected with the virus.
In IRIS, symptoms of various infections worsen or appear for the first time because immune responses improve (are reconstituted), increasing inflammation at sites of infection. Symptoms sometimes worsen because parts of dead viruses persist, triggering immune responses.
After the virus enters a person’s lymph nodes during the acute retroviral syndrome stage, the disease becomes latent for 10 years or more before symptoms of advanced disease develop. During latency, the virus continues to replicate in the lymph nodes, where it may cause one or more of the following conditions:
HIV can be suppressed by combination ART consisting of 3 or more ARV drugs. ART does not cure HIV infection but suppresses viral replication within a person’s body and allows an individual’s immune system to strengthen and regain the capacity to fight off infections.
Human immunodeficiency virus (HIV) has led to a worldwide pandemic that has exacted a dramatic toll on children, especially in resource-limited countries. It is estimated that there are approximately 2.1 million children younger than 14 years living with HIV, with the vast majority in sub-Saharan Africa. Worldwide, approximately 700,000 children were infected perinatally with HIV in 2005, and 570,000 children died due to HIV/AIDS (acquired immunodeficiency syndrome) in 2005 (see www.cdc.gov and www.unaids.org). As of 2003, there were more than 9000 children younger than 13 years living with AIDS in the United States. The vast majority of these children were infected by perinatal transmission. In resource-rich countries, the perinatal infection rate has dropped to less than 2%, and combination antiretroviral therapy (known as highly active antiretroviral therapy, or HAART) has diminished mortality and morbidity associated with HIV disease.1 The pediatric hospitalist must be familiar with the care of HIV-exposed newborns and HIV-infected children, because the initial diagnosis and management of complications often occur in the hospital setting.
