HIV-1 infection causes chronic ongoing inflammation and production of reactive oxygen species. Thus, the HIV genome may be vulnerable to oxidative damages, including breaks in the single-stranded RNA. HIV, as well as for viruses generally, successful infection depends on overcoming host defensive strategies that often include production of genome-damaging reactive oxygen. Thus, Michod et al. suggested that recombination by viruses is an adaptation for repair of genome damages, and that recombinational variation is a byproduct that may provide a separate benefit.
Health care workers who are accidentally pricked with an HIV-contaminated needle have about a 1 in 300 chance of contracting HIV unless they are treated as soon as possible after exposure. Such treatment reduces the chance of infection to less than 1 in 1,500. The risk increases if the needle penetrates deeply or if the needle is hollow and contains HIV-contaminated blood (as with a needle used to draw blood or to inject street drugs) rather than simply being coated with blood (as with a needle used to stitch a cut).

In July 2015, UNAIDS announced that the Millennium Development Goal (MDG) relating to HIV and AIDS had been reached six months ahead of schedule. The target of MDG 6 – halting and reversing the spread of HIV – saw 15 million people receive treatment.95
The initial infection with HIV generally occurs after transfer of body fluids from an infected person to an uninfected one. The virus is carried in infected CD4 T cells, dendritic cells, and macrophages, and as a free virus in blood, semen, vaginal fluid, or milk. It is most commonly spread by sexual intercourse, contaminated needles used for intravenous drug delivery, and the therapeutic use of infected blood or blood products, although this last route of transmission has largely been eliminated in the developed world where blood products are screened routinely for the presence of HIV. An important route of virus transmission is from an infected mother to her baby at birth or through breast milk. In Africa, the perinatal transmission rate is approximately 25%, but this can largely be prevented by treating infected pregnant women with the drug zidovudine (AZT) (see Section 11-23). Mothers who are newly infected and breastfeed their infants transmit HIV 40% of the time, showing that HIV can also be transmitted in breast milk, but this is less common after the mother produces antibodies to HIV. Health care workers are at risk on the job and should take special precautions. Some health care workers have become infected after being stuck with needles containing HIV-infected blood or less frequently, after infected blood comes into contact with an open cut or through splashes into the worker’s eyes or inside their nose.

The use of mother-to-child transmission prevention strategies is another important strand of AIDS prevention programmes. In South Africa, for example, expansion of the strategy has resulted in the mother-to-child transmission rate falling to 3.5%.
Restricting sexual activity to a single partner and practicing safer sex (i.e., always using a condom). Besides avoiding the risk of HIV infection, condoms are successful in reducing other sexually transmitted diseases and unwanted pregnancies. Before engaging in a sexual relationship with someone, getting tested for HIV infection is recommended.
A new (fourth-generation) ELISA test can test for both HIV antibodies and the p24 antigen simultaneously. Thus, people can find out as early as 14 days after being exposed to HIV whether they are infected.
