Human immunodeficiency virus uses chemokine receptors, mainly CXCR4 and CCR5, in conjunction with CD4 to infect healthy cells. The chemokine ligands to these receptors were found to block virus infection. Even though CCR4, the receptor for ABCD-1, is apparently not used by human immunodeficiency virus as coreceptor for infection, N-terminally processed human ABCD-1 showed human immunodeficiency virus suppressor activity independent of the viral phenotype (Pal et al., 1997; Struyf et al., 1998).

Currently, there is no licensed vaccine for HIV or AIDS. The most effective vaccine trial to date, RV 144, was published 2009 and found a partial reduction in the risk of transmission of roughly 30%, stimulating some hope in the research community of developing a truly effective vaccine. Further trials of the RV 144 vaccine are ongoing.
DDI also causes pancreatitis and, to a lesser extent, peripheral neuropathy. Peripheral neuropathy can become permanent and painful, and pancreatitis can be life-threatening if therapy is not discontinued. The drug ddC also is associated with peripheral neuropathy, as well as oral ulcers.

Some people are resistant to certain strains of HIV. For example, people with the CCR5-Δ32 mutation are resistant to infection by the R5 virus, as the mutation leaves HIV unable to bind to this co-receptor, reducing its ability to infect target cells.

“It’s no longer a death sentence,” Boswell said of HIV. “It’s a very different time now. Most people just diagnosed with HIV will live an almost normal life span if they get an early diagnosis, appropriate care and stay on their medications.”
Medications are also used to prevent opportunistic infections (such as Pneumocystis carinii pneumonia) and can keep AIDS patients healthier for longer periods of time. Opportunistic infections are treated as they occur.
Subunit vaccines, which induce immunity to only some proteins in the virus, have also been made. One such vaccine has been made from the envelope protein gp120 and has been tested on chimpanzees. This vaccine proved to be specific to the precise strain of virus used to make it, and was therefore useless in protection against natural infection. Subunit vaccines are also less efficient at inducing prolonged cytotoxic T-cell responses.

Medications are continued throughout pregnancy, labor, and delivery. Some medicines, such as zidovudine (also known as AZT), can be given intravenously during labor, particularly for those women who do not have good viral suppression at the time of delivery. Other medications are continued orally during labor to try to reduce the risk of transmission to the baby during delivery. If the quantity of virus in the mother’s blood (viral load) is more than 1,000 copies/mL near the time of delivery, scheduled cesarean delivery is done at 38 weeks gestation to reduce the risk of transmitting the virus during vaginal delivery. Women with HIV who otherwise meet criteria for starting antiretroviral therapy, per local guidelines or the patient’s preference, should continue taking ART after delivery for their own health.
