Before starting ART, blood tests usually are done to make sure the virus is not already resistant to the chosen medications. These resistance tests may be repeated if it appears the drug regimen is not working or stops working. Patients are taught the importance of taking all of their medications as directed and are told what side effects to watch for. Noncompliance with medications is the most common cause of treatment failure and can cause the virus to develop resistance to the medication. Because successful therapy often depends on taking several pills, it is important for the patient to understand that this is an “all or nothing” regimen. If the person cannot tolerate one of the pills, then he or she should call their physician, ideally prior to stopping any medication. Taking just one or two of the recommended medications is strongly discouraged because it allows the virus to mutate and become resistant. It is best to inform the HIV health care provider immediately about any problems so that a better-tolerated combination can be prescribed.
The later stages of HIV infection are characterized by the progressive depression of T cells and repeated infections that can even occur during a course of antibiotic therapy for another infection (superinfections). People with AIDS are particularly vulnerable to “opportunistic infections” from bacteria that other people normally fight off. Pneumocystis carinii, which causes severe inflammation of the lungs (pneumonia), is a common infection that affects people with AIDS. Cancers (malignant neoplasms), and a wide variety of neurological abnormalities, most notably the AIDS dementia complex, may also occur. These neurological symptoms when of HIV, infects the nervous system.
In September 2014, new UNAIDS “Fast Track” targets called for the dramatic scaling-up of HIV prevention and treatment programmes to avert 28 million new infections and end the epidemic as a public health issue by 2030.93
Overall, with the increasing use of antiretroviral therapy and the introduction of better antiviral regimens, survival with HIV infection has increased over time, although it is not yet equivalent to that in uninfected individuals. (See the image below.)
Routine social or community contact with an HIV infected person carries no risk of infection. There is no evidence of spread of HIV through social contact in schools, at home or in the work place. HIV has not been transmitted through:
Such attitudes are inappropriate because HIV is poorly transmissible without sexual contact or blood contact. In addition, the expected survival is long in patients with HIV infection who are receiving treatment. HIV is not transmitted during casual contact and is readily inactivated by simple detergents. Much of the concern regarding HIV infection is due to the incurability of the infection and the relentless immune decline and eventual premature death in the vast majority of infected people.
Epidemiologic studies have shown that the risk of HIV transmission from patient to health care professional is exceedingly low and is related to needle stick or intraoperative injury or to potentially infectious fluid that comes in contact with a mucous membrane (17). Most contacts between health care professionals and women who are infected with HIV occur, however, during routine obstetric and gynecologic care. Health care practitioners should observe standard precautions with all patients to minimize skin, mucous membrane, and percutaneous exposure to blood and body fluids to protect against a variety of pathogens, including HIV.

Sex with an infected partner without using a condom or other barrier protection can transmit HIV. The virus can enter the body through the lining of the vagina, vulva, penis, rectum, or mouth during sex. Anal intercourse, followed by vaginal intercourse, are the primary risk factors. Oral sex is less likely to transmit HIV, but studies have shown that it can transmit both HIV and other sexually transmitted diseases (STDs).
HIV has been found in saliva, tears, nervous system tissue, blood, semen (including pre-seminal fluid, or “pre-cum”), vaginal fluid, and breast milk. However, only blood, semen, vaginal secretions, and breast milk have been proven to transmit infection to others.
Until very recently, one of the biggest questions related to the management of HIV disease was the optimal time to start antiviral treatment. For some time, there had been very strong data demonstrating that therapy is appropriate for those with CD4 cells numbering less than 350 cells/mm3 in the blood. There have also long been strong recommendations to treat patients with select conditions regardless of their CD4 cell count, such as during pregnancy in order to prevent transmission of HIV to the baby or those who have HIV-associated renal disease or chronic hepatitis B infection where the antiviral treatment for HIV also treats the hepatitis virus. There are now several very large studies that have shifted all guidelines around the world to recommending treatment of all HIV-infected individuals at the time of diagnosis no matter what the CD4 cell count. Regardless, prior to initiating antiviral therapy, everything possible should be done to ensure that the patient is committed to the treatment, able to adhere to the regimen, and will follow with his or her health care professional to assess whether medications are tolerated and working.
In the developed world, antiretroviral therapy has greatly improved prognosis and increased survival rates. Public education programs have raised awareness such that testing and prevention of infection are more common. Both of these approaches are difficult in countries with undereducated or underfunded populations.
It should be noted that not every child born to an HIV-infected mother will acquire the virus. Without treatment, a woman with human immunodeficiency virus (HIV) has a one in four chance of infecting her fetus. Before preventive treatments were available, the CDC estimated that 1,000 to 2,000 infants were born with HIV infection in the U.S. each year. Now, health officials say there has been a dramatic reduction in mother-to-child, or perinatal HIV transmission rates due to increased HIV testing, which makes it possible to provide antiretroviral medication treatment of the mother during pregnancy and labor and short-term treatment of the infant after birth.

Blood and genital secretions from people with HIV are considered infectious and the utmost care should be taken in handling them. Fluids that are contaminated with blood also are potentially infectious. Feces, nasal secretions, saliva, sputum, sweat, tears, urine, and vomit are not considered infectious unless visibly bloody.
There are currently nine approved PIs that all have distinct toxicities. The most common side effects associated with these drugs are nausea and diarrhea, which occur more often with some PIs than others. For example, diarrhea is more common with NFV than other PIs but can occur with any and all drugs in this class. Many of the drugs in this class also increase blood lipid levels, some more than others with ATV and DRV appearing to have less effect on lipids than other drugs in the class. Other unique toxicities associated with various PIs are kidney stones, kidney damage, and increases in blood bilirubin levels and potentially jaundice with IDV and ATV. Some of these drugs also have been associated with elevations in blood sugar levels and bleeding in hemophiliacs. Finally, little is known regarding the role these drugs may play in the development of lipodystrophy. There is also some data suggesting that LPV/RTV and DRV may be associated with an increased risk of cardiovascular events.
