The killing stage is more challenging, because the shocked cells carry few H.I.V. antigens, the toxic flags released by pathogenic particles and recognized by the immune system prior to attack. One approach to the killing strategy comes from an unusual type of H.I.V.-positive patient who may carry the virus for decades yet seems not to be disturbed by it. Once enough of these cells have been destroyed and the person has another “opportunistic” infection like pneumonia or tuberculosis, the diagnosis moves to the final stage of the infection, called AIDS.

Following decades of inadequate funding, our nation’s public health infrastructure lacks the resources it needs to respond aggressively to the HIV and AIDS epidemic. This arrangement has been devastating for members of the LGBTQ community, since the little funding that does exist for HIV prevention, treatment, and care has not been focused on or funded in the communities most impacted by HIV. HIV disease becomes AIDS when your immune system is seriously damaged. If you have less than 200 CD4 cells or if your CD4 percentage is less than 14%, you have AIDS. In the past, people with HIV infection would start antiretroviral treatment after their CD4 count dropped or they developed HIV complications. Today, HIV treatment is recommended for all people with HIV infection, even if their CD4 count is still normal.

Several specialists at the hospital were enlisted to make sense of the infection. Other tests can detect antibodies in body fluids other than blood, such as saliva, urine, and vaginal secretions. Some of these are designed to be rapid HIV tests that produce results in approximately 20 minutes. These tests have accuracy rates similar to traditional blood tests. OraQuick is an at-home test that uses an oral swab to detect HIV antibodies in oral fluid. Clearview is another rapid HIV test that can detect HIV antibodies in blood or plasma. HIV home-testing kits are available at many local drugstores. Blood is obtained by a finger prick and blotted on a filter strip. Other test kits use saliva or urine. The filter strip is mailed in a protective envelope to a laboratory to be tested. Results are returned by mail within one to two weeks.

In July 2015, UNAIDS announced that the Millennium Development Goal (MDG) relating to HIV and AIDS had been reached six months ahead of schedule. The target of MDG 6 – halting and reversing the spread of HIV – saw 15 million people receive treatment.95

Because HIV is not transmitted through the air or by casual contact (such as touching, holding, or dry kissing), hospitals and clinics do not isolate HIV-infected people unless they have another contagious infection.

HIV infection takes different forms within different cells. As we have seen, more than 95% of the virus that can be detected in the plasma is derived from productively infected cells, which have a very short half-life of about 2 days. Productively infected CD4 lymphocytes are found in the T-cell areas of lymphoid tissue, and these are thought to succumb to infection in the course of being activated in an immune response. Latently infected memory CD4 cells that are activated in response to antigen presentation also produce virus. Such cells have a longer half-life of 2 to 3 weeks from the time that they are infected. Once activated, HIV can spread from these cells by rounds of replication in other activated CD4 T cells. In addition to the cells that are infected productively or latently, there is a further large population of cells infected by defective proviruses; such cells are not a source of infectious virus.

HIV attacks and destroys a type of white blood cell called a CD4 cell, commonly called the T-cell. This cell’s main function is to fight disease. When a person’s CD4 cell count gets low, they are more susceptible to illnesses.

Because of the inaccurate results, the FDA has not approved any of the home-use HIV tests which allow people to interpret their tests in a few minutes at home. There is however a Home Access test approved which can be found at most drugstores. In this test blood from a finger prick is placed on a card and sent to a licensed lab. Consumers are given an identification number to use when phoning for results and have the opportunity to speak with a counselor if desired.

HIV-infected mothers can pass the virus through their breast milk. However, if the mother is taking the correct medications, the risk of transmitting the virus is greatly reduced.
