Sturdevant was devastated. HIV is a virus spread through certain body fluids that attacks the body’s immune system, specifically the CD4 cells, often called T cells. Over time, HIV can destroy so many of these cells that the body can’t fight off infections and disease. These special cells help the immune system fight off infections. Untreated, HIV reduces the number of CD4 cells (T cells) in the body. This damage to the immune system makes it harder and harder for the body to fight off infections and some other diseases. Rarely, HIV has been transmitted via transplantation of organs from HIV-seropositive donors. Infection has developed in recipients of kidney, liver, heart, pancreas, bone, and skin—all of which contain blood—but screening for HIV greatly reduces risk of transmission. HIV transmission is even more unlikely from transplantation of cornea, ethanol-treated and lyophilized bone, fresh-frozen bone without marrow, lyophilized tendon or fascia, or lyophilized and irradiated dura mater.
If, on balance, a breach of confidence is deemed necessary, practitioners should work in advance to anticipate and manage potentially negative consequences (ie, reactions of intimate partners, family). As well, practitioners should consider whether the goal of maintaining patient privacy would be better served by personal communication with the individual placed at risk by the patient’s seropositivity or by notification of local public health authorities. In some areas, anonymous notification of sexual contacts is possible through local or state departments of health. As a practical matter, because disclosure is only possible when the index case freely identifies at-risk partners, superseding an individual’s refusal to disclose should be a rare occurrence.
The list of medical pros and cons regarding circumcision is long. Entry (fusion) inhibitors prevent HIV from entering cells. To enter a human cell, HIV must bind to a CD4 receptor and one other receptor, such as the CCR-5 receptor. One type of entry inhibitor, CCR-5 inhibitors, blocks the CCR-5 receptor, preventing HIV from entering human cells.
The genome of HIV mutates at a very high rate, and the virus in each infected individual is thus slightly different. The genetic mechanisms that underlie the individual variation have been investigated through approaches based on genome sequencing. The HIV-1 genome in 2009 was the first HIV genome to be sequenced in its entirety. Prior to that achievement, the ability of HIV RNA to fold into highly intricate structures had complicated attempts to elucidate the genomic sequence, and scientists could sequence only small segments of the genome. The HIV-1 genome is composed of 9,173 nucleotides of RNA (nucleotides are the building blocks of nucleic acids).

Patients beginning ART sometimes deteriorate clinically, even though HIV levels in their blood are suppressed and their CD4 count increases, because of an immune reaction to subclinical opportunistic infections or to residual microbial antigens after successful treatment of opportunistic infections. IRIS usually occurs in the first months of treatment but is occasionally delayed. IRIS can complicate virtually any opportunistic infection and even tumors (eg, Kaposi sarcoma) but is usually self-limited or responds to brief regimens of corticosteroids.

A retrovirus of the subfamily lentivirus that causes acquired immunodeficiency syndrome (AIDS). The most common type of HIV is HIV-1, identified in 1984. HIV-2, first discovered in West Africa in 1986, causes a loss of immune function and the subsequent development of opportunistic infections identical to those associated with HIV-1 infections. The two types developed from separate strains of simian immunodeficiency virus.
