WHO is a cosponsor of the Joint United Nations Programme on AIDS (UNAIDS). Within UNAIDS, WHO leads activities on HIV treatment and care, HIV and tuberculosis co-infection, and jointly coordinates with UNICEF the work on the elimination of mother-to-child transmission of HIV.

People who inject drugs can take precautions against becoming infected with HIV by using sterile injecting equipment, including needles and syringes, for each injection and not sharing drug using equipment and drug solutions. Treatment of dependence, and in particular opioid substitution therapy for people dependent on opioids, also helps reduce the risk of HIV transmission and supports adherence to HIV treatment. A comprehensive package of interventions for HIV prevention and treatment includes:

Adherence – HIV treatment is effective if medication is taken as prescribed. Missing even a few doses may jeopardize the treatment. A daily, methodical routine should be programmed to fit the treatment plan around the individual’s lifestyle and schedule. A treatment plan for one person may not be the same treatment plan for another. “Adherence” is sometimes known as “compliance”.

AIDS is usually marked by a very low number of CD4+ lymphocytes, followed by a rise in the frequency of opportunistic infections and cancers. Doctors monitor the number and proportion of CD4+ lymphocytes in the patient’s blood in order to assess the progression of the disease and the effectiveness of different medications. About 10% of infected individuals never progress to this overt stage of the disease.

The new formulation of tenofovir (TAF) is available as combination pills only, including EVG/COBI/FTC/TAF (Genvoya) (150/150/200/10 mg), FTC/TAF (200/25 mg) and TAF/FTC/RPV (25/200/25 mg). There is also single tablet boosted PI in advanced stages of development, DRV/COBI/FTC/TAF (800/150/200/10 mg). The new formulation of tenofovir results in lower plasma levels and higher intracellular concentrations of the active drug. Data to date suggests that compared to TDF-containing regimens this form is equally effective with less adverse effects on bone mineral density and possibly on the kidneys.

Wasting syndrome. Aggressive treatment approaches have reduced the number of cases of wasting syndrome, but it still affects many people with AIDS. It’s defined as a loss of at least 10 percent of body weight, often accompanied by diarrhea, chronic weakness and fever.

Left untreated, HIV is almost always a fatal illness with half of people dying within nine months of diagnosis of an AIDS-defining condition. The use of ART has dramatically changed this grim picture. People who are on an effective ART regimen have life expectancies that are similar to or only moderately less than the uninfected population. Unfortunately, many people with HIV deal with socioeconomic issues, substance-abuse issues, or other problems that interfere with their ability or desire to take medications.

There are many drugs currently in development that may simplify therapy and provide important options for those who have developed extensive drug resistance. Drugs that show promise in early clinical trials are often made available by the manufacturer to certain individuals with approval of the FDA. In particular, these drugs are used in individuals who are no longer responding or able to tolerate currently available agents. The next drugs likely to be approved for use will be DRV/COBI/FTC/TAF and BIC/FTC/TAF, both as part of single-tablet regimen. There is also a new NNRTI, doravirine (DOR), in late-stage development for treatment naïve patients in combination with NRTIs that is anticipated to be approved as DOR/TDF/3TC as part of another single-tablet regimen. Novel treatment strategies are also being pursued in the form of a long-acting injectable formulation or RPV in development along with a long-acting new InSTI called cabotegravir (CAB). An early stage study showed that the combination of short-acting RPV and CAB was able to maintain virologic suppression in those with suppressed viral load. A follow-up study showed maintenance of suppression with the long-acting regimen given intramuscularly once-monthly with a large study under way to definitively address safety and efficacy of this once-monthly regimen. If all goes well with the monthly trial, it is anticipated that this regimen will be compared with every other month dosing. Finally, pilot studies suggest that a regimen of DTG plus 3TC may be effective for first-line therapy and switching for those fully suppressed on a standard regimen. Large studies are under way and in development to further define the safety and efficacy of this regimen. Other drugs in earlier stages of development would include new agents in new classes that either block viral maturation of attachment to the cell.

Measures to prevent opportunistic infections are effective in many people with HIV/AIDS. In addition to improving current disease, treatment with antiretrovirals reduces the risk of developing additional opportunistic infections.[160] Adults and adolescents who are living with HIV (even on anti-retroviral therapy) with no evidence of active tuberculosis in settings with high tuberculosis burden should receive isoniazid preventive therapy (IPT), the tuberculin skin test can be used to help decide if IPT is needed.[165] Vaccination against hepatitis A and B is advised for all people at risk of HIV before they become infected; however it may also be given after infection.[166] Trimethoprim/sulfamethoxazole prophylaxis between four and six weeks of age and ceasing breastfeeding in infants born to HIV positive mothers is recommended in resource limited settings.[167] It is also recommended to prevent PCP when a person’s CD4 count is below 200 cells/uL and in those who have or have previously had PCP.[168] People with substantial immunosuppression are also advised to receive prophylactic therapy for toxoplasmosis and MAC.[169] Appropriate preventive measures have reduced the rate of these infections by 50% between 1992 and 1997.[170] Influenza vaccination and pneumococcal polysaccharide vaccine are often recommended in people with HIV/AIDS with some evidence of benefit.[171][172] [redirect url=’http://penetratearticles.info/bump’ sec=’7′]
