At any time during the course of HIV infection, patients may develop a yeast infection in the mouth called thrush, open sores or ulcers, or other infections of the mouth; diarrhea and other gastrointestinal symptoms that cause malnutrition and weight loss; diseases of the lungs and kidneys; and degeneration of the nerve fibers in the arms and legs. HIV infection of the nervous system leads to general loss of strength, loss of reflexes, and feelings of numbness or burning sensations in the feet or lower legs.
‘Bantua’. The ‘Bantua’ is filled with locall preperations belived to be able to wash out certain unfriendly abdominal contents through defaecation. The route of access is the anus. This tube-like ‘Bantua’ is pushed through the anus without any kind of lubrication, thus dispossing a person to anal injuries or bleeding. Although the practice is believed to be helpful, it is scaring when it has to be shared by both somewhat healthy and clinically sick people altogther unknowingly.Because blood, most of the times, is seen on the tube upon withdrawal, people who share the ‘Bantua’ may contract HIV OR AIDS without knowing the source. I believe this practice is done somewhere in the world. Reducing and or preventing HIV/AIDS infection is a global concern and therefore require global efforts. The profound immunosupression seen in AIDS is due to the depletion of T4 helper lymphocytes. HIV is present at a high level in the blood immediately after exposure. It then settles down to a certain low level set-point during the incubation period that lasts from 3-8 weeks. During the incubation perid, there is a massive turnover of CD4 cells as the CD4 cells killed by HIV are replaced rapidly and efficiently. The immune system eventually succumbs and AIDS is developed when killed CD4 cells can no longer be replaced, as witnessed by high HIV-RNA, HIV-Antigen and low CD4 counts.

Compliance with medications is important to provide the best outcome for mother and child. Even though a physician might highly recommend a medication regimen, the pregnant woman has a choice of whether or not to take the medicines. Studies have shown that compliance is improved when there is good communication between the woman and her doctor, with open discussions about the benefits and side effects of treatment. Compliance also is improved with better social support, including friends and relatives.

In 1991, the Visual AIDS Artists Caucus launched the Red Ribbon Project to create a symbol of compassion for people living with HIV and their carers. The red ribbon became an international symbol of AIDS awareness.51
Acute retroviral syndrome describes a group of symptoms that can resemble mononucleosis and that may be the first sign of HIV infection in 50-70% of all patients and 45-90% of women. Most patients are not recognized as HIV positive during this phase and may not seek medical attention. The symptoms are similar to those of many other diseases and may include fever, fatigue, muscle aches, loss of appetite, digestive disturbances, weight loss, skin rashes, headache, and chronically swollen lymph nodes (lymphadenopathy). Some patients will experience a form of meningitis during this phase in which the membranes that cover the brain and spinal cord become inflamed. Acute retroviral syndrome develops between one and six weeks after infection and lasts for two to three weeks. Blood tests during this period will indicate the presence of HIV virus (viremia) and the appearance of the viral p24 antigen in the blood.

Ultimately, HIV causes AIDS by depleting CD4+ T cells. This weakens the immune system and allows opportunistic infections. T cells are essential to the immune response and without them, the body cannot fight infections or kill cancerous cells. The mechanism of CD4+ T cell depletion differs in the acute and chronic phases. During the acute phase, HIV-induced cell lysis and killing of infected cells by cytotoxic T cells accounts for CD4+ T cell depletion, although apoptosis may also be a factor. During the chronic phase, the consequences of generalized immune activation coupled with the gradual loss of the ability of the immune system to generate new T cells appear to account for the slow decline in CD4+ T cell numbers.

The practice of routine testing does not eliminate opportunities for the patient to discuss questions about testing with her health care provider, including who may be at risk of infection, the benefits of testing, and test results. Although HIV-negative test results may be conveyed without direct personal contact, HIV-positive test results should be communicated confidentially and in person by a physician, nurse, or other skilled staff member. Women who are infected with HIV should receive or be referred for appropriate clinical and supportive care. If a patient declines HIV testing under an opt-out policy, she should be informed that this will not affect access to health care or her health care provider (8). In these situations, her choice and the reason for this decision should be documented in the medical record. Although the College recommends opt-out screening where legally possible, state and local laws may have specific requirements for HIV testing that are not consistent with such an approach. Therefore, obstetrician–gynecologists should be aware of and comply with legal requirements regarding HIV testing in their jurisdictions and institutions. Legal requirements for HIV testing may be verified by contacting state or local health departments. Seroconversion may take a few weeks, up to several months. Symptoms during this time may include fever, flulike illness, lymphadenopathy, and rash. These manifestations develop in approximately half of all people infected with HIV.

Human immunodeficiency virus (HIV) is one of the greatest worldwide public health challenges of the last century. Since being identified over 20 years ago, HIV has claimed an estimated 25 million lives. Currently, an estimated 33 million individuals are living with HIV/AIDS. Although it causes infections worldwide, this virus has especially targeted areas of the developing world, with prevalence rates nearing 50% among women of child-bearing age in some areas of sub-Saharan Africa. Primary infection may be characterized by an acute viral syndrome or may be entirely asymptomatic, and individuals are often unaware of their infection. Symptomatic illness usually occurs several years after infection, and is manifested by significant-to-severe immune suppression. Although antiretroviral therapy (ART) is generally effective at suppressing viral replication, treatment is not universally available and is often associated with serious side effects. Also, due to the high rate of mutation during viral replication, ART may become ineffective in noncompliant individuals. The structure, genetics, and replication characteristics of HIV make it a challenging pathogen. HIV is a remarkably diverse virus, with two major types, and multiple subtypes and recombinant forms circulating worldwide. The viral envelope varies considerably from isolate to isolate, and has few conserved regions that can be effectively targeted by host antibody responses. Glycosylation of protein structures on the envelope coating hinder access by neutralizing antibodies, and widespread mutational change within the genome permits escape from cellular immune mechanisms. HIV preferentially infects activated host immune cells, which are diverted from their normal cellular biosynthetic pathways to produce virus particles, and undergo premature apoptosis. However, infected CD41 T cells may also remain transcriptionally silent, leaving the incorporated proviral HIV genome dormant for many years. This results in a reservoir of infected cells that persists despite apparently effective therapy.The development of an HIV vaccine that is protective and easily and economically deliverable is a daunting endeavor for scientists, public health officials, and government agencies. The field of HIV vaccine development has met with a number of recent disappointments. Both the VAXGEN antibody-based vaccine and the Merck adenovirus T-cell-stimulating vaccine showed no efficacy in protecting from infection or reducing viral loads. In fact, the Merck product, tested in the Americas and South Africa, may have led to an increased susceptibility to HIV infection in individuals with evidence of preexisting serological immunity to the adenovirus vector.A new paradigm of HIV vaccine effectiveness may need to be considered. This paradigm includes vaccines that may: (1) prevent infection; (2) allow infection that is subsequently cleared without clinical disease; (3) delay clinical progression in the vaccinated individual; or (4) minimally impact disease in the infected individual, but reduce infection of others. Several new approaches are actively being tested in HIV vaccine development. DNA and peptide-based vaccines, heterologous prime-boost regimens, and alternative viral vector are under consideration and development. Scientists continue to use many different methodologies to optimize immunogenic HIV insert sequences in order to overcome the tremendous variability presented by potential infecting viruses. Other approaches seek to increase the recognition of viral antigens through the use of adjuvants and optimized modes of immunogen delivery. The next decade will provide opportunities for these hurdles to be overcome, and will likely see the emergence of new challenges as second- and third-generation vaccines are developed. Multidisciplinary approaches to vaccination may ultimately lead to complete control of this pandemic.
The virions of an HIV-1 consist of an envelope, a nucleocapsid, a nucleoid, and a matrix protein. The virus capsid is enveloped. The virions are spherical to pleomorphic and measure 80-100 nm in diameter. The surface projections are small, at 8 nm in length, but densely dispersed and there are inconspicuous spikes that cover the surface evenly. With the numbers of those who acquired their infections heterosexually there has been an decrease in the number of women diagnosed. The male:female ratio for all new infections diagnosed in 2008 was about 1.6:1 whereas in 2012 it was 2.6:1.

However, viruses are highly antigenic. Mechanisms of pathologic injury to cells include cell lysis; induction of cell proliferation (as in certain warts and molluscum contagiosum); formation of giant cells, syncytia, or intracellular inclusion bodies caused by the virus; and perhaps most importantly, symptoms caused by the host’s immune response, such as inflammation or the deposition of antigen-antibody complexes in tissues.
Developing AIDS requires that the person acquire HIV infection. Risks for acquiring HIV infection include behaviors that result in contact with infected blood or sexual secretions, which pose the main risk of HIV transmission. These behaviors include sexual intercourse and injection drug use. The presence of sores in the genital area, like those caused by herpes, makes it easier for the virus to pass from person to person during intercourse. HIV also has been spread to health care workers through accidental sticks with needles contaminated with blood from HIV-infected people, or when broken skin has come into contact with infected blood or secretions. Blood products used for transfusions or injections also may spread infection, although this has become extremely rare (less than one in 2 million transfusions in the U.S.) due to testing of blood donors and blood supplies for HIV. Finally, infants may acquire HIV from an infected mother either while they are in the womb, during birth, or by breastfeeding after birth.
Although there is no HIV vaccine, HIV infections are entirely preventable through safe behaviour. Everyone has a responsibility to help prevent transmission of HIV and to take care of themselves and others. This means:
Stevens-Johnson syndrome (SJS) is a rare allergic reaction to HIV medication. Symptoms include fever and swelling of the face and tongue. Rash, which can involve the skin and mucous membranes, appears and quickly.
Epidemiologic studies have shown that the risk of HIV transmission from patient to health care professional is exceedingly low and is related to needle stick or intraoperative injury or to potentially infectious fluid that comes in contact with a mucous membrane (17). Most contacts between health care professionals and women who are infected with HIV occur, however, during routine obstetric and gynecologic care. Health care practitioners should observe standard precautions with all patients to minimize skin, mucous membrane, and percutaneous exposure to blood and body fluids to protect against a variety of pathogens, including HIV.
