Scientists believe the first human who got HIV was a person in Africa. This happened when Simian Immunodeficiency Virus (SIV) went from apes or chimpanzees to humans. This virus probably crossed to humans by contact with monkey blood while cutting up monkeys to eat. Research in October 2014 shows that the virus started in Kinshasa during the 1920s. It was quickly spread by sex workers, dirty needles used by doctors, and people using the railway to travel around the country. Some people described the spread of the disease as a sexidemic (widespread).
In September 2014, new UNAIDS “Fast Track” targets called for the dramatic scaling-up of HIV prevention and treatment programmes to avert 28 million new infections and end the epidemic as a public health issue by 2030.93
It is estimated that currently only 70% of people with HIV know their status. reach the target of 90%, an additional 7.5 million people need to access HIV testing services. In mid-2017, 20.9 million people living with HIV were receiving antiretroviral therapy (ART) globally.

The ability of HIV to mutate and rapidly evolve to escape immune detection by the most-prevalent HLA molecules is similar to the rapid adaptation and mutation of other infectious viruses, such as influenza. There is some evidence, however, that within populations the adaptation of HIV to protective HLA variants may reduce its replicative capacity. In Botswana, for instance, where HIV has adapted to overcome the protective effects of the HLA-B*57 variant, seroprevalence (the frequency of HIV infection) is increased but viral replication capacity is reduced. Researchers have speculated that declines in HIV replication capacity and virulence may be attributed to not only rapid adaptation to protective variants but also increasing use of antiretroviral treatments.

Urea and electrolytes: These are chemical compounds normally found in blood. Their levels are controlled by the renal system. This test is done to check on the condition of the kidneys. If the kidneys are functioning normally, then the levels of urea and creatinine will be normal. Otherwise the levels will be elevated.
The initial period following the contraction of HIV is called acute HIV, primary HIV or acute retroviral syndrome. Many individuals develop an influenza-like illness or a mononucleosis-like illness 2–4 weeks post exposure while others have no significant symptoms. Symptoms occur in 40–90% of cases and most commonly include fever, large tender lymph nodes, throat inflammation, a rash, headache, and/or sores of the mouth and genitals. The rash, which occurs in 20–50% of cases, presents itself on the trunk and is maculopapular, classically. Some people also develop opportunistic infections at this stage. Gastrointestinal symptoms, such as vomiting or diarrhea may occur. Neurological symptoms of peripheral neuropathy or Guillain–Barré syndrome also occurs. The duration of the symptoms varies, but is usually one or two weeks.

Voluntary testing with counseling is the strategy most consistent with respect for patient autonomy. Under this option, physicians provide both pretest and posttest counseling. Some physicians may perform such counseling themselves, whereas others may prefer to refer the patient for counseling and testing. (Such specialized HIV counseling was more widely available in previous years but has become less available as more health care professionals have become more comfortable treating patients with HIV and as the opt-out approach to testing—an approach that places less emphasis on pretest counseling—has become more common.) In addition to medical information, such counseling could include information regarding potential uses of test information and legal requirements pertaining to the release of information. Patients should be told what information will be communicated and to whom and the possible implications of reporting the information. This approach to testing maintains HIV’s status as being in a class by itself (sui generis), even as many ethicists have acknowledged the end to the exceptionalism that marked this disease in the early years of the epidemic (5).
Analysis of reported AIDS cases shows that 51% had Pneumocystis carinii pneumonia (PCP) without Kaposi’s sarcoma (KS) (with or without other “opportunistic” infections (OOI) predictive of cellular immunodeficiency); 30% had KS without PCP (with or without OOI); 7% had both PCP and KS (with or without OOI); and 12% had OOI with neither PCP nor KS. The overall mortality rate for cases of PCP without KS (47%) was more than twice that for cases of KS without PCP (21%), while the rate for cases of both PCP and KS (68%) was more than three times as great. The mortality rate for OOI with neither KS nor PCP was 48%.
Integrase inhibitors. Integrase inhibitors prevent the virus from inserting its own genetic material into the DNA of the infected cell. This stops the virus from replicating. Integrase was the only FDA-approved drug in this class as of early 2009. Several investigational drugs in this category were in clinical trials at that time.

Routine HIV-testing in healthcare facilities also raises legal issues. Most people who are HIV-positive want this information kept confidential. Facilities are free to use HIV testing to control the infection but in most states only with the patient’s informed consent. Some states, such as Illinois, require written consent. The level of protection for medical records varies from state to state. California, for example, has broad protections; under its statutes, no one can be compelled to provide information that would identify anyone who is the subject of an HIV test. However, every state requires that AIDS cases be reported to the CDC, which tracks statistics on the spread of HIV. Whether the name of an HIV-infected person is reported to the CDC depends on state laws and regulations.AIDS and Education Issues in the field of education include the rights of HIV-positive students to attend class and of HIV-positive teachers to teach, the confidentiality of HIV records, and how best to teach young people about AIDS. A few areas have been settled in court: for instance, the right of students to attend classes was of greater concern in the early years of the epidemic and later ceased to be a matter of dispute.
According to a report from Public Health England (PHE), there were an estimated 100,000 adults aged 15-59 living with HIV in the UK in 2012, 22% of whom were unaware of their infection. The number of deaths among HIV-infected people has continued to decline since the introduction of antiretroviral therapy and a total of 490 people infected with HIV were reported to have died in 2012. There were 6,360 new diagnoses in 2012 in the UK. New diagnoses in men who have sex with men (MSM) continue to rise. This reflects both an ongoing high level of transmission and an increase in the number of men coming forward for testing.
It is important to note that although HIV is highly virulent, transmission is greatly reduced when an HIV-infected person has a suppressed or undetectable viral load (<50 copies/ml) due to prolonged and successful anti-retroviral treatment. Hence, it can be said to be almost impossible (but still non-zero) for an HIV-infected person who has an undetectable viral load to transmit the virus, even during unprotected sexual intercourse, as there would be a negligible amount of HIV present in the seminal fluid, vaginal secretions or blood, for transmission to occur. This does not mean however, that prolonged anti-retroviral treatment will result in a suppressed viral load. An undetectable viral load, generally agreed as less than 50 copies per milliliter of blood, can only be proven by a polymerase chain reaction (PCR) test. Q. The HIV test came back POSITIVE! During the first weeks of infection when a patient may have symptoms of primary HIV infection, antibody testing may still be negative (the so-called window period). If there is suspicion of early infection based upon the types of symptoms present and a potential recent exposure, consideration should be given to having a test performed that specifically looks for the virus circulating in the blood, such as a viral load test or the use of an assay that identifies HIV p24 antigen, for example, the new fourth-generation antibody/antigen combination test. Identifying and diagnosing individuals with primary infection is important to assure early access into care and to counsel them regarding the risk of transmitting to others. The latter is particularly important since patients with primary HIV infection have very high levels of virus throughout their body and are likely to be highly infectious. There is no definitive data showing that initiation of antiretroviral therapy during this early stage of infection results in clinical benefits. Nevertheless, it is generally thought that the benefits of reducing the size of the HIV in the body, preserving select immune responses, and reducing transmissibility favors early treatment. Once the patient enters the asymptomatic phase, infected individuals will know whether or not they are infected if a test for HIV antibodies is done. It is also important to foster wider availability of comprehensive services for people living with HIV and their partners through partnerships among health departments, community-based organizations, and health care and social service providers. HIV contains 3 species-defining retroviral genes: gag, pol, and env. The gag gene encodes group-specific antigen; the inner structural proteins. The pol gene encodes polymerase; it also contains integrase and protease (the viral enzymes) and is produced as a C-terminal extension of the Gag protein). The env gene encodes the viral envelope—the outer structural proteins responsible for cell-type specificity. Glycoprotein 120, the viral-envelope protein, binds to the host CD4+ molecule. Sexual abstinence is completely effective in eliminating sexual transmission, but educational campaigns have not been successful in promoting abstinence in at-risk populations. Monogamous sexual intercourse between two uninfected partners also eliminates sexual transmission of the virus. Using barrier methods, such as condoms, during sexual intercourse markedly reduces the risk of HIV transmission. These measures have had some success in blunting the rate of new cases, especially in high-risk areas such as sub-Saharan Africa or Haiti. As discussed above, medications may be used to reduce the risk of HIV infection if used within hours of an exposure. There also is data that if uninfected people can take antiretroviral medications, in particular tenofovir disoproxil fumarate plus emtricitabine (TDF/FTC or Truvada) once daily, that it markedly reduces the risk of sexual transmission. Perhaps the most effective way to reduce HIV transmission is for the HIV-infected partner to be on ART with undetectable levels of virus in their blood. As noted above, a pregnant woman with HIV can reduce the risk of passing the infection to her baby by taking medications during pregnancy and labor and avoiding breastfeeding. In addition to the concern for new opportunistic infections, pre-existing infections can reactivate and cause significant disease in people with AIDS. The most important example on a global scale is that of tuberculosis, as reactivated tuberculosis can cause symptomatic disease with lower levels of reactivation. One way to measure the damage to your immune system is to count your CD4 cells you have. These cells, also called "T-helper" cells, are an important part of the immune system. Healthy people have between 500 and 1,500 CD4 cells in a milliliter of blood. Fact Sheet 124 has has more information on CD4 cells. When HIV infection destroys CD4+ lymphocytes, it weakens the body’s immune system, which protects against many infections and cancers. This weakening is part of the reason that the body is unable to eliminate HIV infection once it has started. However, the immune system is able to mount some response. Within a month or two after infection, the body produces lymphocytes and antibodies that help lower the amount of HIV in the blood and keep the infection under control. For this reason, untreated HIV infection may cause no symptoms or only a few mild symptoms for an average of about 10 years (ranging from 2 to more than 15 years). Once HIV is in the immune system, it multiplies inside the CD4 cells, disabling and killing them in the course of the infection, and thus interfering with their normal function.
