The infected person frequently gets infections and even some forms of cancer which a healthy immune system would have gotten rid of quite easily. These infections are known as opportunistic infections. HIV infection, once established, cannot be eliminated by the body or by drugs.

The next year, two research teams—one led by Luc Montagnier and Françoise Barré-Sinoussi, of the Pasteur Institute, in Paris, the other by Robert Gallo, at the National Cancer Institute, in Maryland—published papers in Science that described a new retrovirus in the lymph nodes and blood cells of AIDS patients. A retrovirus has a pernicious way of reproducing: it permanently inserts a DNA copy of its genome into the nucleus of a host cell, hijacking the cell’s machinery for its own purposes. When the retrovirus mutates, which it often does, its spawn becomes difficult for the body or a vaccine to target and chase out. Retroviral diseases were widely believed to be incurable. In May of 1986, after much dispute about credit for the discovery (the French finally won the Nobel, in 2008), an international committee of scientists agreed on the name H.I.V., or human immunodeficiency virus. By the end of that year, about twenty-five thousand of the nearly twenty-nine thousand Americans with reported AIDS diagnoses had died.
But good intentions have not translated into enough funding and resources — from either the government or philanthropic organizations. Good intentions also have not counteracted the crippled medical infrastructure in states like Mississippi, which the Commonwealth Fund, an independent health-policy research foundation, ranks dead last in more than 40 measures of health-system performance. A 2014 study conducted by Dr. David Holtgrave of the Johns Hopkins Bloomberg School of Public Health found that to make any real progress in the H.I.V./AIDS crisis among black gay and bisexual men in the United States, the government would need to invest an additional $2.5 billion to address unmet testing, care, treatment and prevention needs. Despite the higher H.I.V. diagnosis and death rates in the Deep South, the region received $100 less in federal funding per person living with H.I.V. than the United States over all in 2015.
A person gets HIV when an infected person’s body fluids (blood, semen, fluids from the vagina or breast milk) enter his or her bloodstream. The virus can enter the blood through linings in the mouth, anus, or sex organs (the penis and vagina), or through broken skin.
Nathan King wants to help fight the stigma associated with PrEP. “Unlike many medical breakthroughs and preventive strategies, PrEP, and its users, faced criticism from the beginning,” he said. “People who used the medication are stigmatized and stereotyped, rather than supported for taking steps to protect the health of themselves and their communities.”

The number of new cases of AIDS acquired from heterosexual intercourse used to be greater than from men who have sex with men, but this situation was reversed in 2011. Approximately half (52%, 1,560/2,990 in 2011) of all infections among heterosexuals were probably acquired in the UK and this proportion has increased over recent years. The figure in 2002 was 27%.
As patients near the end of life, clinicians may need to prescribe drugs to relieve pain, anorexia, agitation, and other distressing symptoms. The profound weight loss in many people during the last stages of AIDS makes good skin care difficult. The comprehensive support provided by hospice programs helps many patients because hospice providers are unusually skilled at symptom management, and they support caregivers and patient autonomy.
The human immunodeficiency virus (HIV) was identified in 1983, 2 years after the first five cases of the acquired immunodeficiency syndrome (AIDS) were reported by the Centers for Disease Control and Prevention (CDC). The ensuing years witnessed rapid advances in the prevention and management of HIV/AIDS and dramatic shifts in its epidemiology. In developed countries, the availability of effective antiretroviral therapy reduced perinatal transmission to 1–3%; prolonged survival; increased resistance to 15% of circulating strains; and introduced a set of common side effects called body-fat abnormalities. In developing countries, however, less than 20% of those needing antiretroviral therapy receive it and interventions to reduce behavioral risk have had limited impact. As a result, the developing world accounts for 95% of AIDS-related deaths and new HIV infections.
A small proportion of individuals infected with HIV can survive more than 10 years without developing AIDS. It was suspected for many years that such individuals mount a more-vigorous immune response to the virus, but scientists could not explain why. Then, genetic variations known as single nucleotide polymorphisms, or were identified in different HLA (human leukocyte antigen) genes, which code for molecules that stimulate the immune response. A variation in the HLA-G gene, for example, was identified in a subset of female prostitutes who had remained HIV-negative despite having had sexual contact with more than 500 HIV-positive men. Scientists identified additional SNPs that influenced viral load and disease progression in genes that code for HLA-B, HLA-C, and HCP5 (HLA complex P5), an inactive retrovirus first incorporated into the human genome millions of years ago that shares similarities in DNA sequence with HIV and is thought to interfere with viral replication.
In addition to thrush and painful ulcers in the mouth, patients may develop a condition called hairy leukoplakia. The CDC also regards this condition as an indicator of full-blown AIDS. Hairy leukoplakia is a white area of diseased tissue on the tongue that may be flat or slightly raised. It is associated with infection by the Epstein-Barr virus.
Integrase strand transfer inhibitors (integrase inhibitors or integrases) stop HIV genes from becoming incorporated into the human cell’s DNA and are very well tolerated. Raltegravir (Isentress) was the first drug in this class. Elvitegravir is part of a fixed-dose combination (elvitegravir/cobicistat/tenofovir/emtricitabine) taken as one pill once daily, called Stribild. Dolutegravir (Tivicay) is also available in a once-daily combination pill with two NRTIs, abacavir and lamivudine, called Triumeq.

The patient with HIV may present with signs and symptoms of any of the stages of HIV infection. No physical findings are specific to HIV infection; the physical findings are those of the presenting infection or illness. Manifestations include the following:
Strategies to reduce the risk of HIV infection include not having sex, limiting your number of sexual partners, never sharing needles, and using condoms the right way every time you have sex. People who are at high risk may take HIV prevention medicines.
Major advancements in HIV prevention, treatment, and care have put an AIDS-free generation squarely within reach. HIV tests are faster and more reliable than ever before. HIV medications are safer and more effective, and there are now several ways to prevent the spread of HIV, including condoms and Pre-Exposure Prophylaxis (PrEP). PrEP is an HIV prevention strategy that currently involves taking a once daily-pill called Truvada ®. When taken as prescribed, PrEP is safe and highly effective at preventing people from becoming HIV-positive.

Dealing with the potential consequences of bias and discrimination – job loss, homelessness, lack of healthcare insurance – often results in LGBTQ people engaging in behaviors that facilitate the spread of HIV. For example, in the face of persistent employment discrimination, many transgender women are left with few other options but to engage in survival sex work in order to meet their most basic needs. According to a 2015 survey of more than 27,000 transgender people, “The rate of HIV [diagnosis] was…five times higher among those who have participated in sex work at any point in their lifetime” than among those who have not.
HIV infection takes different forms within different cells. As we have seen, more than 95% of the virus that can be detected in the plasma is derived from productively infected cells, which have a very short half-life of about 2 days. Productively infected CD4 lymphocytes are found in the T-cell areas of lymphoid tissue, and these are thought to succumb to infection in the course of being activated in an immune response. Latently infected memory CD4 cells that are activated in response to antigen presentation also produce virus. Such cells have a longer half-life of 2 to 3 weeks from the time that they are infected. Once activated, HIV can spread from these cells by rounds of replication in other activated CD4 T cells. In addition to the cells that are infected productively or latently, there is a further large population of cells infected by defective proviruses; such cells are not a source of infectious virus.
Side effects associated with EFV are mostly dizziness, confusion, fatigue, and vivid dreams. These tend to be most prominent during the first weeks of therapy and then often decrease in severity. It is generally recommended that EFV be taken at bedtime so that the patient is asleep during the time dizziness and confusion may be most severe. It is also noteworthy that there may be an increased risk of depression associated with the use of this drug, and it should be used with caution in those with poorly managed depression. Rash and liver inflammation can occur with both EFV and DLV, and these drugs may also be linked to abnormalities of lipids in the blood.
RNA testing (viral load test) detects HIV RNA in the blood. It is not commonly used for screening but can be helpful in detecting early HIV infection when a person is in the window period or if the screening tests are unclear.
During Millett’s decades in government and nonprofit organizations, he has combed through mounds of data about H.I.V./AIDS and black gay and bisexual men. Two years ago, he and his amfAR colleagues published a comprehensive report titled “H.I.V. and the Black Community: Do #Black(Gay)Lives Matter?” When the calm, usually sunny Millett, known for his bookish blue glasses and ready smile, talks about what he calls this “perfect storm,” his voice takes on a harder edge. “We are going to eventually end AIDS in the United States, but I fear it’s not going to happen for black M.S.M.,” he said, referring to men who have sex with men. “We have waited too long. With so many black gay men already infected, the horse is already out of the barn.”
When people get HIV and don’t receive treatment, they will typically progress through three stages of disease. Medicine to treat HIV, known as antiretroviral therapy (ART), helps people at all stages of the disease if taken the right way, every day. Treatment can slow or prevent progression from one stage to the next. It can also dramatically reduce the chance of transmitting HIV to someone else.
These studies show that most of the HIV present in the circulation of an infected individual is the product of rounds of replication in newly infected cells, and that virus from these productively infected cells is released into, and rapidly cleared from, the circulation at the rate of 109 to 1010 virions every day. This raises the question of what is happening to these virus particles: how are they removed so rapidly from the circulation? It seems most likely that HIV particles are opsonized by specific antibody and complement and removed by phagocytic cells of the mononuclear phagocyte system. Opsonized HIV particles can also be trapped on the surface of follicular dendritic cells, which are known to capture antigen:antibody complexes and retain them for prolonged periods (see Chapters 9 and 10).
