HIV needs the integrase enzyme to infect T cells. This drug prevents that step. Integrase inhibitors are often used in the first line of treatment because they are effective for many people, and cause minimal side effects. Other drugs can prevent or treat opportunistic infections (OIs). ART has also reduced the rate of most OIs. In most cases, these drugs work very well. The newer, stronger ARVs have helped reduce the rates of most OIs.

Mother-to-child transmission is the most common way that children become infected with HIV. HIV medicines, given to women with HIV during pregnancy and childbirth and to their babies after birth, reduce the risk of mother-to-child transmission of HIV.

Treatment for immunodeficiency disorders commonly includes antibiotics and immunoglobulin therapy. Other antiviral drugs, amantadine and acyclovir, or a drug called interferon are used for treatment of the viral infections caused by immunodeficiency disorders.

AIDS and Health Care Closely related to work is the issue of health care. In some cases, the two overlap: Health Insurance, Social Security, and disability benefits for people with AIDS were often hard to obtain during the 1980s. Insurance benefits can be quickly exhausted—in fact, AIDS usually bankrupts people who have the disease. The problem is compounded when employers serve as their own health insurers. In McGann v. H&H Music Co., 946, F.2d 401 (5th Cir. [1991]), a federal court ruled that such employers could legally change their policies to reduce coverage for workers who develop expensive illnesses such as AIDS.

HIV/AIDS has had a great impact on society, both as an illness and as a source of discrimination.[22] The disease also has large economic impacts.[22] There are many misconceptions about HIV/AIDS such as the belief that it can be transmitted by casual non-sexual contact.[23] The disease has become subject to many controversies involving religion including the Catholic Church’s position not to support condom use as prevention.[24] It has attracted international medical and political attention as well as large-scale funding since it was identified in the 1980s.[25]

Left untreated, HIV is almost always a fatal illness with half of people dying within nine months of diagnosis of an AIDS-defining condition. The use of ART has dramatically changed this grim picture. People who are on an effective ART regimen have life expectancies that are similar to or only moderately less than the uninfected population. Unfortunately, many people with HIV deal with socioeconomic issues, substance-abuse issues, or other problems that interfere with their ability or desire to take medications.

If treatment fails, drug susceptibility (resistance) assays can determine the susceptibility of the dominant HIV strain to all available drugs. Genotypic and phenotypic assays are available and can help clinicians select a new regimen that should contain least 2 and preferably 3 drugs to which the HIV strain is more susceptible. The dominant HIV strain in the blood of patients who are taken off antiretroviral therapy may revert over months to years to the wild-type (ie, susceptible) strain because the resistant mutants replicate more slowly and are replaced by the wild type. Thus, if patients have not been treated recently, the full extent of resistance may not be apparent through resistance testing, but when treatment resumes, strains with resistance mutations often reemerge from latency and again replace the wild-type HIV strain.

Blood contamination. HIV may also be spread through contact with infected blood. However, due to the screening of blood for evidence of HIV infection, the risk of acquiring HIV from blood transfusions is extremely low.

An elevated viral load in a smaller sexual network (because most people still tend to have sex with people of the same race), amplified by the structural issues that Moore pointed to, also explains why H.I.V. rates have always been disproportionately high in the black community over all. But in the first decades of the epidemic, these ideas and explanations had not been widely accepted to explain the growing body of data pointing to fast-rising numbers of H.I.V. cases among black heterosexuals — in rates unmatched by those of white Americans. In fact, the African-American community was largely in denial about the fact that H.I.V./AIDS was a black issue. Worse, most believed the disease was a conspiracy on the part of the federal government to kill off the race, God’s punishment for homosexuality or simply not a subject for polite conversation, because the disease was thought to be connected to promiscuity and crack and heroin use.

HIV is a member of the genus Lentivirus,[83] part of the family Retroviridae.[84] Lentiviruses share many morphological and biological characteristics. Many species of mammals are infected by lentiviruses, which are characteristically responsible for long-duration illnesses with a long incubation period.[85] Lentiviruses are transmitted as single-stranded, positive-sense, enveloped RNA viruses. Upon entry into the target cell, the viral RNA genome is converted (reverse transcribed) into double-stranded DNA by a virally encoded reverse transcriptase that is transported along with the viral genome in the virus particle. The resulting viral DNA is then imported into the cell nucleus and integrated into the cellular DNA by a virally encoded integrase and host co-factors.[86] Once integrated, the virus may become latent, allowing the virus and its host cell to avoid detection by the immune system.[87] Alternatively, the virus may be transcribed, producing new RNA genomes and viral proteins that are packaged and released from the cell as new virus particles that begin the replication cycle anew.[88]

LPV/r comes coformulated as Kaletra while all other RTV-containing regimens require taking RTV along with the other PI. In the case of TPV, RTV must be given as 200 mg with each dose of TPV twice per day. In contrast, ATV can be given without RTV at a dose of two 200 mg capsules once daily or 300 mg with 100 mg RTV once daily. The latter should always be used in PI-experienced subjects and when used in combination with TDF or NNRTIs which can reduce the drug levels of ATV. Similarly, FPV is also used differently in PI-naïve and experienced individuals. In treatment-naïve individuals, it can be given as two 700 mg tablets twice daily or two 700 mg tablets (1,400 mg total) with either 100 or 200 mg RTV, all once daily. In treatment-experienced patients, or when used with NNRTIs, it should be given as one 700 mg tablet with 100 mg RTV, both twice daily. The most recently approved of the PIs is DRV, which was initially used exclusively in treatment-experienced patients with drug-resistant virus. In this setting, it is given as 600 mg with 100 mg RTV, both given twice daily. More recently, DRV was approved for those who have never been treated before given at a dose of 800 mg once daily with 100 mg of RTV once daily.

During pregnancy or delivery or through breast-feeding. Infected mothers can pass the virus on to their babies. HIV-positive mothers who get treatment for the infection during pregnancy can significantly lower the risk to their babies.

Seroconversion may take a few weeks, up to several months. Symptoms during this time may include fever, flulike illness, lymphadenopathy, and rash. These manifestations develop in approximately half of all people infected with HIV.

Such rampant levels of anti-LGBTQ bias is particularly worrisome when so few PLWH in the U.S. seem to have the virus under control. Of the 1.2 million people living with HIV in the U.S. in 2011, only 30% of them had consistently taken their medication and were able to lower the amount of HIV in their bodies to undetectable levels. While undetectable, a person living with HIV remains in good health, and it is virtually impossible transmit the virus to a partner. Prevention options (e.g., condoms, Pre-Exposure Prophylaxis) exist for those in relationships where one partner is not yet undetectable.

Acute HIV infection progresses over a few weeks to months to become an asymptomatic HIV infection (no symptoms). This stage can last 10 years or longer. During this period, the person might have no reason to suspect they have HIV, but they can spread the virus to others.

  1. Another group working contemporaneously with the Montagnier and Gallo groups was that of Dr. Jay Levy at the University of California, San Francisco. He independently discovered the AIDS virus in 1983 and named it the AIDS associated retrovirus (ARV).[138] This virus was very different from the virus reported by the Montagnier and Gallo groups. The ARV strains indicated, for the first time, the heterogeneity of HIV isolates and several of these remain classic examples of the AIDS virus found in the United States.[139]
    Drug therapy is often recommended for patients who are committed to taking all their medications and have a CD4 count less than 500 (indicating immune system suppression) or a high viral load (amount of HIV virus in the bloodstream).
