One way to measure the damage to your immune system is to count your CD4 cells you have. These cells, also called “T-helper” cells, are an important part of the immune system. Healthy people have between 500 and 1,500 CD4 cells in a milliliter of blood. Fact Sheet 124 has has more information on CD4 cells.

During a blood transfusion, blood or blood products are transferred from one person to another. There are two types of transfusions, autologous (your own blood), and donor blood (someone else’s blood). There are four blood types: A; B; C; and O.

HIV is probably directly responsible for a substantial loss of weight (AIDS wasting) in some people. Wasting in people with AIDS may also be caused by a series of infections or by an untreated, persistent digestive tract infection.

Background: Persons unaware of their human immunodeficiency virus (HIV) infection account for approximately 40% of ongoing transmissions in the United States. Persons are unaware of their infection because of delayed HIV diagnoses that represent substantial missed opportunities to improve health outcomes and prevent HIV transmission.

In addition to the CD4 lymphocyte count, chest X-rays, Pap smears, and other tests are useful in managing HIV disease. Gay men who engage in receptive anal sex may wish to consider anal Pap smears to detect potential cancers.

When CD4 T-cell numbers decline below a critical level, cell-mediated immunity is lost, and infections with a variety of opportunistic microbes appear (Fig. 11.29). Typically, resistance is lost early to oral Candida species and to Mycobacterium tuberculosis, which shows as an increased prevalence of thrush (oral candidiasis) and tuberculosis. Later, patients suffer from shingles, caused by the activation of latent herpes zoster, from EBV-induced B-cell lymphomas, and from Kaposi’s sarcoma, a tumor of endothelial cells that probably represents a response both to cytokines produced in the infection and to a novel herpes virus called HHV-8 that was identified in these lesions. Pneumonia caused by the fungus Pneumocystis carinii is common and often fatal. In the final stages of AIDS, infection with cytomegalovirus or Mycobacterium avium complex is more prominent. It is important to note that not all patients with AIDS get all these infections or tumors, and there are other tumors and infections that are less prominent but still significant. Rather, this is a list of the commonest opportunistic infections and tumors, most of which are normally controlled by robust CD4 T cell-mediated immunity that wanes as the CD4 T-cell counts drop toward zero (see Fig. 11.21).

In individuals not infected with HIV, the CD4 count in the blood is normally above 400 cells per mm3 of blood. People generally do not become at risk for HIV-specific complications until their CD4 cells are fewer than 200 cells per mm3. At this level of CD4 cells, the immune system does not function adequately and is considered severely suppressed. A declining number of CD4 cells means that HIV disease is advancing. Thus, a low CD4 cell count signals that the person is at risk for one of the many opportunistic infections that occur in individuals who are immunosuppressed. In addition, the actual CD4 cell count indicates which specific therapies should be initiated to prevent those infections.

Genetic studies have led to a general classification system for HIV that is primarily based on the degree of similarity in viral gene sequence. The two major classes of HIV are HIV-1 and HIV-2. HIV-1 is divided into three groups, known as group M (main group), group O (outlier group), and group N (new group). Worldwide, HIV-1 group M causes the majority of HIV infections, and it is further subdivided into subtypes A through K, which differ in expression of viral genes, virulence, and mechanisms of transmission. In addition, some subtypes combine with one another to create recombinant subtypes. HIV-1 group M subtype B is the virus that spread from Africa to Haiti and eventually to the United States. Pandemic forms of subtype B are found in North and South America, Europe, Japan, and Australia. Subtypes A, C, and D are found in sub-Saharan Africa, although subtypes A and C are also found in Asia and some other parts of the world. Most other subtypes of group M are generally located in specific regions of Africa, South America, or Central America.

51% of infections in the UK in 2012 occurred through sex between men and this group remains at greatest risk.[6]There has been no evidence in recent years of a decline in the numbers of new infections in this group and over 3,250 new diagnoses of HIV occurred in 2012.[5]

HIV transmission is also possible when sperm from an infected donor is used to inseminate a woman. In the United States, measures have been taken to reduce this risk. Fresh semen samples are no longer used. Sperm from donors is frozen for 6 months or more. Then the donors are retested for HIV infection before the sperm is used.

His was one of several cases of the same rare pneumonia seen by physicians on both coasts. Michael Gottlieb, a U.C.L.A. immunologist, studied the blood of some of these patients and made the key observation that they had lost almost all their helper T cells, which protect against infections and cancers. In June, 1981, the Centers for Disease Control published Gottlieb’s cases in its Morbidity and Mortality Weekly Report, and, in July, Dr. Alvin Friedman-Kien, of New York University, reported that twenty-six gay men in New York and California had received diagnoses of Kaposi sarcoma, a cancer of the lymphatic channels and blood vessels. This, too, was strange: Kaposi sarcoma typically affected elderly men of Eastern European Jewish and Mediterranean ancestry.

Specific adverse events are related to the antiretroviral agent taken.[160] Some relatively common adverse events include: lipodystrophy syndrome, dyslipidemia, and diabetes mellitus, especially with protease inhibitors.[2] Other common symptoms include diarrhea,[160][161] and an increased risk of cardiovascular disease.[162] Newer recommended treatments are associated with fewer adverse effects.[29] Certain medications may be associated with birth defects and therefore may be unsuitable for women hoping to have children.[29]

Black gay and bisexual men and the organizations and activists that support them have come to the painful realization that the nation and society have failed them and that they must take care of themselves and one another. Their group names and slogans reflect a kind of defiant lift-as-we-climb self-reliance: My Brother’s Keeper; Us Helping Us in Washington; the Saving Ourselves Symposium that takes place in Jackson this week; Our People, Our Problem, Our Solution, the tag line of the Black AIDS Institute. Since last October, the young men in Sturdevant’s orbit have been supported by the fragile scaffolding that “Mr. Ced” has constructed around them and with them. Jordon has gained weight and is up and walking. Marq has promised to stay on his meds and has begun calling Sturdevant “Dad.” Benjamin Jennings has a new job as a corrections officer at a prison north of Jackson. Jermerious Buckley is “mother,” as he puts it, to six gay “children” of his own.

Two points in this update deserve emphasis. First, the eventual case-mortality rate of AIDS, a few years after diagnosis, may be far greater than the 41% overall case-mortality rate noted above. Second, the reported incidence of AIDS has continued to increase rapidly. Only a small percentage of cases have none of the identified risk factors (male homosexuality, intravenous drug abuse, Haitian origin, and perhaps hemophilia A). To avoid a reporting bias, physicians should report cases regardless of the absence of these factors.

HIV can be transmitted via a variety of means, from unprotected sex (most common method of transmission) to blood transfusions to sharing of needles. Pregnant mothers may also transmit the virus to their unborn child.

People with AIDS have had their immune system damaged by HIV. They are at very high risk of getting infections that are uncommon in people with a healthy immune system. These infections are called opportunistic infections. These can be caused by bacteria, viruses, fungi, or protozoa, and can affect any part of the body. People with AIDS are also at higher risk for certain cancers, especially lymphomas and a skin cancer called Kaposi sarcoma.

WHO is a cosponsor of the Joint United Nations Programme on AIDS (UNAIDS). Within UNAIDS, WHO leads activities on HIV treatment and care, HIV and tuberculosis co-infection, and jointly coordinates with UNICEF the work on the elimination of mother-to-child transmission of HIV.

The first step in fusion involves the high-affinity attachment of the CD4 binding domains of gp120 to CD4. Once gp120 is bound with the CD4 protein, the envelope complex undergoes a structural change, exposing the chemokine receptor binding domains of gp120 and allowing them to interact with the target chemokine receptor.[55][56] This allows for a more stable two-pronged attachment, which allows the N-terminal fusion peptide gp41 to penetrate the cell membrane.[55][56] Repeat sequences in gp41, HR1, and HR2 then interact, causing the collapse of the extracellular portion of gp41 into a hairpin. This loop structure brings the virus and cell membranes close together, allowing fusion of the membranes and subsequent entry of the viral capsid.[55][56]

The only way to know if you have HIV is to take an HIV test. Most tests looks for signs of HIV in your blood. A small sample of blood is taken from your arm. The blood is sent to a lab and tested for HIV. There are other tests available that check for HIV in the urine and oral fluid. The urine test is not very sensitive. There are currently two FDA-approved oral fluid tests. They are OraSure and OraQuick Advance.

All too often, when people living with H.I.V. in Jackson lack the support of their families, community and the church, they end up in Grace House, a homeless facility on a sleepy block in the midtown section of the city. A cluster of four suburban-looking houses, Grace House originally as a hospice, where the sick came to die. Now that the infected are living longer — and the numbers of gay and bisexual men with the virus continue to creep up — more and more young men are seeking shelter.

Visible effects of HIV infection come in the form of disrupted lymph-node architecture. This disruption is temporal, and, at one point, lymph-node biopsy was considered as a form of staging the disease. [54, 55] The disruption of the follicular dendritic network in the lymph nodes and subsequent failure of normal antigen presentation are likely contributors to the disease process.

HIV/AIDS affects the economics of both individuals and countries.[211] The gross domestic product of the most affected countries has decreased due to the lack of human capital.[211][257] Without proper nutrition, health care and medicine, large numbers of people die from AIDS-related complications. They will not only be unable to work, but will also require significant medical care. It is estimated that as of 2007 there were 12 million AIDS orphans.[211] Many are cared for by elderly grandparents.[258]

Until recently, Justin Huff, a former Jackson State student, shared a room on the second floor of Grace House’s main facility. He was infected with H.I.V. a year and a half ago, when a man he met on Jack’d sexually assaulted him. He received his diagnosis just after his 21st-birthday celebration. “I was throwing up and couldn’t eat anything for a few days; I thought it was from the drinking,” Huff said. “When I went to the doctor, he was like, if I hadn’t made it in the next two days, I would’ve been dead.”

Not everyone who has HIV have AIDS. When people first get HIV, they can be healthy for years. A person is diagnosed as having AIDS when he or she gets specific types of illnesses or gets sick in certain ways due to their HIV. Once a person’s HIV progresses to (or turns into) AIDS, the person will continue to have AIDS for the rest of their life. While there are many treatments for HIV/AIDS, at this point there is no cure.
