According to the August 2008 report issued by the Joint United Nations Programme on HIV/AIDS (UNAIDS), as of 2007, approximately 33 million people worldwide are HIV positive. Over half of the 33 million are women and this statistic has remained stable for several years. The highest number of cases is found in sub-Saharan Africa and Southeast Asia.
It should be noted that not every child born to an HIV-infected mother will acquire the virus. Without treatment, a woman with human immunodeficiency virus (HIV) has a one in four chance of infecting her fetus. Before preventive treatments were available, the CDC estimated that 1,000 to 2,000 infants were born with HIV infection in the U.S. each year. Now, health officials say there has been a dramatic reduction in mother-to-child, or perinatal HIV transmission rates due to increased HIV testing, which makes it possible to provide antiretroviral medication treatment of the mother during pregnancy and labor and short-term treatment of the infant after birth.
Stage II (also known as clinically asymptomatic stage): This stage may last for 8-10 years with no major symptoms except for swollen glands (lymph nodes), some weight loss, mouth ulceration and mild skin and nail infections.

Ultimately, HIV causes AIDS by depleting CD4+ T cells. This weakens the immune system and allows opportunistic infections. T cells are essential to the immune response and without them, the body cannot fight infections or kill cancerous cells. The mechanism of CD4+ T cell depletion differs in the acute and chronic phases. During the acute phase, HIV-induced cell lysis and killing of infected cells by cytotoxic T cells accounts for CD4+ T cell depletion, although apoptosis may also be a factor. During the chronic phase, the consequences of generalized immune activation coupled with the gradual loss of the ability of the immune system to generate new T cells appear to account for the slow decline in CD4+ T cell numbers.
The size of the proviral reservoir correlates to the steady-state viral load and is inversely correlated to the anti-HIV CD8+ T-cell responses. Aggressive early treatment of acute infection may lower the proviral load, but generally, treatment in newly infected (but postseroconversion) patients yields no long-term benefit.

Although the tests for detecting HIV infection continue to improve, they still require that people volunteer for testing. It is estimated that approximately 15% of those infected with HIV in the United States are unaware of their infection because they have never been tested. In order to decrease the number that are unaware of their HIV infection status, in 2006, the Centers for Disease Control and Prevention recommended that all people between 13 and 64 years of age be provided HIV testing whenever they encounter the health care system for any reason.
