In this era of increasingly effective treatments for HIV, people with HIV are living longer, healthier, and more productive lives. Deaths from HIV infection have greatly declined in the United States since the 1990s. As the number of people living with HIV grows, it will be more important than ever to increase national HIV prevention and health care programs.

Use of PEP is determined by risk of infection; guidelines recommend antiretroviral therapy with ≥ 3 antiretroviral drugs. The drugs should be carefully selected to minimize adverse effects and provide a convenient dosing schedule and thus encourage PEP completion. Preferred regimens include combination of 2 NRTIs and the addition of one or more drugs (eg, 2 NRTIs plus an integrase inhibitor, a PI, or an NNRTI); drugs are given for 28 days. Nevirapine is avoided because of the rare possibility of severe hepatitis. Although evidence is not conclusive, ZDV alone probably reduces risk of transmission after needlestick injuries by about 80%. Careful investigation has helped scientists determine where AIDS came from. Studies have shown that HIV first arose in Africa. It spread from primates to people early in the 20th century, possibly when humans came into contact with infected blood during a chimpanzee hunt. By testing stored blood samples, scientists have found direct evidence of a human being infected as long ago as 1959.

Symptoms may come and go or last for weeks. Because these symptoms are similar to common illnesses like the flu, you might not see a doctor. Even if your doctor suspects the flu or mononucleosis, HIV may not be considered.

People with HIV/AIDS who take antiretroviral medicines live longer. They live longer without getting AIDS defining illnesses. But after a long time, the HIV virus learns how to fight the antiretrovirals. The HIV virus is not killed by this medicine. HIV becomes resistant to the medicine. Then the resistant HIV hurts the immune system and the person may get AIDS.

HIV is an enveloped retrovirus whose structure is shown in Fig. 11.22. Each virus particle, or virion, contains two copies of an RNA genome, which are transcribed into DNA in the infected cell and integrated into the host cell chromosome. The RNA transcripts produced from the integrated viral DNA serve both as mRNA to direct the synthesis of the viral proteins and later as the RNA genomes of new viral particles, which escape from the cell by budding from the plasma membrane, each in a membrane envelope. HIV belongs to a group of retroviruses called the lentiviruses, from the Latin lentus, meaning slow, because of the gradual course of the diseases that they cause. These viruses persist and continue to replicate for many years before causing overt signs of disease.
