In retrospect, the high rate of H.I.V. infection among African-American women was a result of a complicated combination of all these factors, as well as the reality that after decades of denial and neglect, the viral load piled up in black communities, making any unprotected sexual encounter with anyone a potential “bridge to infection.” But two decades ago, in the midst of a very scary, fast-growing epidemic, the down-low brother became the AIDS boogeyman. I first heard about the “D.L.” from J.L. King, an author and self-proclaimed sex educator whom I interviewed in 2001. He had just warned a rapt audience of health care providers and H.I.V. educators at an AIDS conference in Washington: “I sleep with men, but I am not bisexual, and I am certainly not gay. I am not going to your clinics, I am not going to read your brochures, I am not going to get tested. I assure you that none of the brothers on the down low like me are paying the least bit of attention to anything you have to say.”

The virus initially infects macrophages and then attacks and destroys T helper CD4 lymphocytes, thereby producing immunodeficiency and resulting in death, usually after a very prolonged incubation period followed by a very prolonged clinical course. A very similar virus SIV1 causes simian AIDS in captive macaque monkeys. A further similar virus SIV2 has been isolated from healthy green monkeys.

Although a fever technically is any body temperature above the normal of 98.6 F (37 C), in practice, a person is usually not considered to have a significant fever until the temperature is above 100.4 F (38 C). Fever is part of the body’s own disease-fighting arsenal; rising body temperatures apparently are capable of killing off many disease-producing organisms.

HIV can infect dendritic cells (DCs) by this CD4-CCR5 route, but another route using mannose-specific C-type lectin receptors such as DC-SIGN can also be used. DCs are one of the first cells encountered by the virus during sexual transmission. They are currently thought to play an important role by transmitting HIV to T-cells when the virus is captured in the mucosa by DCs. The presence of FEZ-1, which occurs naturally in neurons, is believed to prevent the infection of cells by HIV.
The infections that occur with AIDS are called opportunistic infections because they take advantage of the opportunity to infect a weakened host. A person diagnosed with AIDS may need to be on antibiotic prophylaxis to prevent certain opportunistic infections from occurring. The infections include (but are not limited to) the following:

HIV (human immunodeficiency virus) is a virus that most likely mutated decades ago from a virus that infected chimpanzees to one that infects humans. It began to spread beyond the African continent in the late 1970s and is now endemic worldwide. HIV causes disease because it attacks critical immune defense cells and over time overwhelms the immune system.
The earliest, well-documented case of HIV in a human dates back to 1959 in the Belgian Congo. The virus may have been present in the United States as early as the mid-to-late 1950s, as a sixteen-year-old male presented with symptoms in 1966 died in 1969.
Complementary and alternative medicine, including Chinese medicine (CM), has been used to treat acquired immune deficiency syndrome (AIDS) for almost 30 years. We aimed to compare the main differences between AIDS treatment and evaluation strategies between CM and Western Medicine (WM), and analyze advantages and disadvantages. The characteristics of integrative medicine (IM), based on CM and WM, include a patient-centered mode of medicine based on evidence. IM focuses on complex intervention and management with systemic and individual treatment. The evaluation indexes of IM might consist of objective indicators and subjective indexes. IM might be a more valuable method for treating AIDS in the future instead of WM or CM alone.
The weakening of the immune system associated with HIV infection can lead to unusual cancers like Kaposi’s sarcoma. Kaposi’s sarcoma develops as raised patches on the skin which are red, brown, or purple. Kaposi’s sarcoma can spread to the mouth, intestine, or respiratory tract. AIDS also may be associated with lymphoma (a type of cancer involving white blood cells).
Although researchers were chastened by the realization that the drug regimen was not itself a cure, they recently found three unusual cases that were encouraging enough to make them keep trying. The first was that of Timothy Ray Brown.
Some viruses have only a few genes coding for capsid proteins. Other more complex ones may have a few hundred genes. But no virus has the thousands of genes required by even the simplest cells. Although in general viruses “steal” their lipid envelope from the host cell, virtually all of them produce “envelope proteins” that penetrate the envelope and serve as receptors. Some envelope proteins facilitate viral entry into the cell, and others have directly pathogenic effects.
Diagnosis is made through a blood test that screens specifically for the virus. If HIV has been found, the test result is “positive.” The blood is re-tested several times before a positive result is given.

Immunodeficiency describes the condition in which the body’s immune response is damaged, weakened, or is not functioning properly. In AIDS, immunodeficiency results from the way that the virus binds to a protein called CD4, which is primarily found on the surface of certain subtypes of white blood cells. After the virus has attached to the cell’s CD4 receptor, the virus-CD4 complex refolds to uncover another receptor called a chemokine receptor that helps mediate entry of the virus into the cell. One chemokine receptor in particular, CCR5, has been the focus of recent research after studies showed that defects in its structure (caused by genetic mutations) result in a slowing or stopping of the progression of AIDS. Scientists hope that this discovery will lead to the development of drugs that trigger an artificial mutation of the CCR5 gene or target the CCR5 receptor.
The second role of the federal government is largely symbolic but no less controversial. It is to guide school efforts through advice, sponsorship, and public speeches, and primarily involves the offices of the surgeon general and of the federal AIDS policy coordinator. Koop, who was a Reagan appointee, roused a fair degree of controversy, yet it was nothing compared to the upheaval that greeted statements by appointees of the Clinton administration. AIDS policy czar Kristine Gebbie and surgeon general M. Joycelyn Elders were forced from their posts after making statements that conservatives found appalling—Gebbie promoting attitudes toward pleasurable sex and Elders indicating a willingness to have schools talk about masturbation. Thereafter, the administration frequently stressed abstinence as its top priority for school AIDS programs.
Sequencing revealed that variation occurs throughout the HIV genome but is especially pronounced in the gene encoding the gp120 protein. By constantly changing the structure of its predominant surface protein, the virus can avoid recognition by antibodies produced by the immune system. Sequencing also has provided useful insight into genetic factors that influence viral activity.
