HIV/AIDS has become a chronic rather than an acutely fatal disease in many areas of the world. Prognosis varies between people, and both the CD4 count and viral load are useful for predicted outcomes. Without treatment, average survival time after infection with HIV is estimated to be 9 to 11 years, depending on the HIV subtype. After the diagnosis of AIDS, if treatment is not available, survival ranges between 6 and 19 months. HAART and appropriate prevention of opportunistic infections reduces the death rate by 80%, and raises the life expectancy for a newly diagnosed young adult to 20–50 years. This is between two thirds and nearly that of the general population. If treatment is started late in the infection, prognosis is not as good: for example, if treatment is begun following the diagnosis of AIDS, life expectancy is ~10–40 years. Half of infants born with HIV die before two years of age without treatment.
The most common side effect reported with the most recently approved NNRTI, ETR, is rash and it was generally mild and rarely required that medications needed to be stopped. Side effects appear to be uncommon with RPV with some uncertainty as to whether it is associated with various neurologic symptoms.
HIV releases RNA, the genetic code of the virus, into the cell. For the virus to replicate, its RNA must be converted to DNA. The RNA is converted by an enzyme called reverse transcriptase (produced by HIV). HIV mutates easily at this point because reverse transcriptase is prone to errors during the conversion of viral RNA to DNA.
Because HIV infection often is detected through prenatal and STD screening, it is not uncommon for an obstetrician–gynecologist to be the first health professional to provide care for an infected woman. This Committee Opinion is designed to provide guidance to obstetrician–gynecologists regarding ethical issues associated with HIV testing, including the use of newly developed rapid HIV tests and disclosure of positive test results. It also outlines responsibilities related to patient care for women who are infected with HIV, access for affected couples to assisted reproductive technology, and the health care professional who is infected with HIV.
The initial infection with HIV generally occurs after transfer of body fluids from an infected person to an uninfected one. The virus is carried in infected CD4 T cells, dendritic cells, and macrophages, and as a free virus in blood, semen, vaginal fluid, or milk. It is most commonly spread by sexual intercourse, contaminated needles used for intravenous drug delivery, and the therapeutic use of infected blood or blood products, although this last route of transmission has largely been eliminated in the developed world where blood products are screened routinely for the presence of HIV. An important route of virus transmission is from an infected mother to her baby at birth or through breast milk. In Africa, the perinatal transmission rate is approximately 25%, but this can largely be prevented by treating infected pregnant women with the drug zidovudine (AZT) (see Section 11-23). Mothers who are newly infected and breastfeed their infants transmit HIV 40% of the time, showing that HIV can also be transmitted in breast milk, but this is less common after the mother produces antibodies to HIV. Gum disease is caused by plaque and may result in tooth loss without proper treatment. Symptoms and signs of gum disease (gingivitis or periodontal disease) include receding gums, bad breath and pocket formation between the teeth and gums. When HIV infection is diagnosed in a routine test, as for blood donation, in pregnancy, or after counselling a person with a lifestyle that puts him or her at risk, there is not usually full AIDS but just infection with HIV. When the disease is suspected, HIV counselling must precede testing. There is a characteristic presentation of the infection that is described in the separate article Primary HIV Infection. As the sole viral protein on the surface of the virus, the Envelope protein is a major target for HIV vaccine efforts. Over half of the mass of the trimeric envelope spike is N-linked glycans. The density is high as the glycans shield the underlying viral protein from neutralisation by antibodies. This is one of the most densely glycosylated molecules known and the density is sufficiently high to prevent the normal maturation process of glycans during biogenesis in the endoplasmic and Golgi apparatus. The majority of the glycans are therefore stalled as immature ‘high-mannose’ glycans not normally present on human glycoproteins that are secreted or present on a cell surface. The unusual processing and high density means that almost all broadly neutralising antibodies that have so far been identified (from a subset of patients that have been infected for many months to years) bind to or, are adapted to cope with, these envelope glycans.
The first HIV vaccine efficacy study in seven years is currently underway in South Africa. The experimental vaccine is an updated version of one used in a 2009 trial that took place in Thailand. A 3.5-year follow up after vaccination showed the vaccine was 31.2 percent effective in preventing HIV infection. It’s the most successful HIV vaccine trial to date.
Recently, the CDC changed testing recommendations. All adults should be screened at least once. People who are considered high risk (needle drug users, multiple sex partners, for example) should be tested more often. All pregnant women should be tested. Anyone who has sustained a needle stick or significant blood exposure from a person known to have HIV or from an unknown source should be tested, too.
One community-based study targeting areas where men who have sex with men (MSM) meet demonstrated that an average of 44% of study participants appeared unaware of their HIV-positive status. High rates of positivity and unawareness of positive status were associated with younger participants, men of black non-Hispanic race, and lower education levels.

of West Lafayette, Indiana, announced today that favorable results have been attained in a clinical study utilizing an extracorporeal (outside the body) whole body hyperthermia procedure on patients with Acquired Immune Deficiency Syndrome (AIDS) who exhibited Kaposi’s sarcoma, and AIDS-related skin cancer.
There are still tremendous hurdles. Thirty-five million people in the world are living with the virus. In sub-Saharan Africa, where most new cases are reported, sixty-three per cent of those eligible for the drug regimen do not receive it; those who do often fail to receive it in full. In the United States, a year’s worth of HAART costs many thousands of dollars per patient, and the long-term side effects can be debilitating.
Many patients develop low-grade fevers, chronic fatigue, and general weakness. HIV also may cause a combination of food malabsorption, loss of appetite, and increased metabolism that contribute to AIDS wasting syndrome.

Protease inhibitors (PIs) interrupt virus replication at a later step in the HIV life cycle, preventing cells from producing new viruses. Currently, these include ritonavir (Norvir), darunavir (Prezista), and atazanavir (Reyataz). Using PIs with NRTIs reduces the chances that the virus will become resistant to medications. Atazanavir and darunavir are available in combination with cobicistat as atazanavir/cobicistat (Evotaz) and darunavir/cobicistat (Prezcobix). Cobicistat and ritonavir inhibit the breakdown of other drugs, so they are used as boosters to reduce the number of pills needed.

HIV is probably directly responsible for a substantial loss of weight (AIDS wasting) in some people. Wasting in people with AIDS may also be caused by a series of infections or by an untreated, persistent digestive tract infection.
HIV stands for human immunodeficiency virus. It is the virus that can lead to acquired immunodeficiency syndrome or AIDS if not treated. Unlike some other viruses, the human body can’t get rid of HIV completely, even with treatment. So once you get HIV, you have it for life.
The history of the human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS) dates back to 1981, when gay men with symptoms and signs of a disease that now are considered typical of AIDS were first described in Los Angeles and New York. The men had an unusual type of lung infection (pneumonia) called Pneumocystis carinii (now known as Pneumocystis jiroveci) pneumonia (PCP) and rare skin tumors called Kaposi’s sarcomas. The patients were noted to have a severe reduction in a type of cell in the blood (CD4 cells) that is an important part of the immune system. These cells, often referred to as T cells, help the body fight infections. Shortly thereafter, this disease was recognized throughout the United States, Western Europe, and Africa. In 1983, researchers in the United States and France described the virus that causes AIDS, now known as HIV, belonging to the group of viruses called retroviruses. While HIV infection is required to develop AIDS, the actual definition of AIDS is the development of a low CD4 cell count (<200 cells/mm3) or any one of a long list of complications of HIV infection ranging from a variety of so-called "opportunistic infections," cancers, neurologic symptoms, and wasting syndromes. HIV Encephalopathy is a severe condition usually seen in end-stage disease. Milder cognitive impairments may exist with less advanced disease. For example, one study found significant deficits in cognition, planning, coordination and reaction times in HIV-infected compared to uninfected children, effects that were more pronounced in those with higher viral loads.  There is an emerging consensus that indications for assisted reproductive technology use should not vary with HIV serostatus; therefore, assisted reproductive technology should be offered to couples in which one or both partners are infected with HIV. This approach is consistent with the principles of respect for autonomy and beneficence (18, 19).
