The World Health Organization and United States recommends antiretrovirals in people of all ages including pregnant women as soon as the diagnosis is made regardless of CD4 count.[14][122][151] Once treatment is begun it is recommended that it is continued without breaks or “holidays”.[29] Many people are diagnosed only after treatment ideally should have begun.[29] The desired outcome of treatment is a long term plasma HIV-RNA count below 50 copies/mL.[29] Levels to determine if treatment is effective are initially recommended after four weeks and once levels fall below 50 copies/mL checks every three to six months are typically adequate.[29] Inadequate control is deemed to be greater than 400 copies/mL.[29] Based on these criteria treatment is effective in more than 95% of people during the first year.[29]

Talk to your partner before you have sex the first time. Find out if he or she is at risk for HIV. Get tested together. Getting tested again at 6, 12, and 24 weeks after the first test can be done to be sure neither of you is infected. Use condoms in the meantime.

I had been writing about AIDS in the black community since the mid-’80s but had never seen anything like the coordinated efforts that started in the late ’90s, when civil rights groups, politicians, clergy, fraternities and sororities and celebrities stepped up to encourage testing and distribute prevention information. All the major black publications collaborated in a highly visible campaign to spotlight the disease as a major health crisis. Black churches created AIDS ministries and offered H.I.V. testing — and the number of congregations participating in the Black Church Week of Prayer for the Healing of AIDS ballooned to more than 10,000.

Treatment with HAART is not without complications. HAART is a collection of different medications, each with its own side effect profile. Some common side effects are nausea, headache, weakness, malaise, and fat accumulation on your back and abdomen (“buffalo hump,” lipodystrophy). When used long-term, these medications may increase the risk of heart attack by affecting fat metabolism.

Sexual contact. People at greatest risk are those who do not practice safer sex by always using a condom, those who have multiple sexual partners, those who participate in anal intercourse, and those who have sex with a partner who has HIV infection and/or other sexually transmitted diseases (STDs). In the United States and Europe, most cases of sexually transmitted HIV infection result from homosexual contact, whereas in Africa, the disease is spread primarily through sexual intercourse among heterosexuals. people with AIDS in the United States are between 25 and 44 years of age.

HIV/AIDS can be diagnosed via a blood test to see the presence of antibodies to the HIV virus. Blood given for donation in many places is screened for HIV before it is administered to patients, as blood transfusion can be one mode of transmission of the HIV virus. HIV/AIDS patients face many serious health conditions. For example, they are more prone to cancers which can be aggressive and devastating. Sometimes, individuals may not be able to carry out their normal lifestyles, while in other cases, individuals may experience bouts of illness and then a calm. There are two general classes of drugs used to treat HIV/AIDS: nucleoside reverse transcriptase inhibitors and protease inhibitors. The first class works during the replication of the virus while the second influences the virus life cycle later on.

Black America, however, never got a Pepfar. Though the raw numbers were much lower than in Africa, parts of our country looked like the continent the program was created to save. Yet while buckets of money went overseas, domestic funding for H.I.V./AIDS remained flat, and efforts to fight the disease here were reduced to a poorly coordinated patchwork affair. “When we saw that the epidemic was out of proportion in the black community, we started calling for a domestic Pepfar that would bring new resources to the effort, create clear and ambitious objectives and rebuild health care infrastructure around the country,” Lee said. “But we just couldn’t get the administration to focus on a domestic plan.”

After the virus enters the body there is a period of rapid viral replication, leading to an abundance of virus in the peripheral blood. During primary infection, the level of HIV may reach several million virus particles per milliliter of blood.[95] This response is accompanied by a marked drop in the number of circulating CD4+ T cells. The acute viremia is almost invariably associated with activation of CD8+ T cells, which kill HIV-infected cells, and subsequently with antibody production, or seroconversion. The CD8+ T cell response is thought to be important in controlling virus levels, which peak and then decline, as the CD4+ T cell counts recover. A good CD8+ T cell response has been linked to slower disease progression and a better prognosis, though it does not eliminate the virus.[96]

This has been true of even the most recent advances. In 2010, the Obama administration unveiled the first National H.I.V./AIDS Strategy, an ambitious plan that prioritized government research and resources to so-called key populations, including black men and women, gay and bisexual men, transgender women and people living in the South. With a mandate to “follow the epidemic,” several pharmaceutical companies and philanthropic organizations also started projects to help gay black men, particularly in the Southern states. That same year, the Affordable Care Act and later the expansion of Medicaid in more than half of the country’s states linked significantly more H.I.V.-positive Americans to lifesaving treatment and care.

For people who are taking antiretrovirals and are rigidly compliant, this phase can be interrupted, with complete viral suppression. Effective antiretrovirals arrest on-going damage to the immune system.

HIV treatments (antiretrovirals) are available and all people with HIV infection in Australia have access to this treatment. Available HIV treatments have dramatically improved the outlook for people with HIV.

Throughout the disease, viral load steadily increases and immunodeficiency progressively worsens (due to the decreasing CD4 count), thereby causing HIV/AIDS to manifest in stages. The World Health Organization (WHO) has categorized HIV disease into 4 stages:

AIDS-related symptoms also includes serious weight loss, brain tumors, and other health problems. Without treatment, these opportunistic infections can kill you. The official (technical) CDC definition of AIDS is available at http://www.thebody.com/content/art14002.html

The largest Collaboratory, with more than twenty members, is led by David Margolis, at the University of North Carolina. Margolis, an infectious-disease expert, is targeting the reservoirs directly. The idea, which has come to be known as “shock and kill,” is to reactivate the dormant virus, unmasking the cells that carry it, so that they can be destroyed. In 2012, he published the results of a clinical trial of the drug Vorinostat, which was originally developed for blood cancers of T cells, as a shock treatment. This October, “shock and kill” was widely discussed when the Collaboratory teams convened at the N.I.H., along with hundreds of other researchers, assorted academics, and interested laypeople. Margolis and his group explored in their talk new ways to shock the virus out of dormancy.

It is important to document that an exposure has occurred or was likely. A needle stick from a person with HIV or a person likely to have HIV constitutes a significant exposure. Medications should be started immediately. If it is unknown whether the person who is the source of the potentially infected material has HIV, the source person can be tested. Medications that were started immediately in the exposed person can be discontinued if the source person does not turn out to carry HIV. Potentially infectious material splashed in the eye or mouth, or coming into contact with non-intact skin, also constitutes an exposure and should prompt immediate evaluation to determine if medications should be started.
