International Issues By 2003 the international AIDS problem had become a crisis in Africa and parts of Asia. The United Nations(UN) and the World Health Organization (WHO) have worked together to address the issues of prevention and treatment, but the statistics reveal grim conditions. The World Health Organization and United States recommends antiretrovirals in people of all ages including pregnant women as soon as the diagnosis is made regardless of CD4 count.[14][122][151] Once treatment is begun it is recommended that it is continued without breaks or “holidays”.[29] Many people are diagnosed only after treatment ideally should have begun.[29] The desired outcome of treatment is a long term plasma HIV-RNA count below 50 copies/mL.[29] Levels to determine if treatment is effective are initially recommended after four weeks and once levels fall below 50 copies/mL checks every three to six months are typically adequate.[29] Inadequate control is deemed to be greater than 400 copies/mL.[29] Based on these criteria treatment is effective in more than 95% of people during the first year.[29]

As of early 2009, there was no cure for AIDS and no vaccine to prevent infection. Treatment stresses aggressive combination drug therapy for those patients with access to the expensive medications and who tolerate them adequately. The use of these multi-drug therapies has significantly improved and prolonged the life of HIV/AIDS patients in the United States.

Untreated HIV destroys certain cells within the immune system (CD4+ or helper T cells) from the time infection onwards, causing more and more damage. Eventually the damage to the immune system is so great the body can no longer stop some infections or cancers it normally fights successfully. Infections not usually seen in healthy people, called opportunistic infections, and certain unusual tumours such as Kaposi’s sarcoma, may occur. Women with untreated HIV infection are at increased risk of developing cervical cancer and both men and women are at increased risk of anal cancer. Untreated HIV can cause infection in the brain, which can lead to nervous system disorders or dementia in some people with HIV infection.

Transmission by saliva or droplets produced by coughing or sneezing, although conceivable, is extremely unlikely.
