Subunit vaccines, which induce immunity to only some proteins in the virus, have also been made. One such vaccine has been made from the envelope protein gp120 and has been tested on chimpanzees. This vaccine proved to be specific to the precise strain of virus used to make it, and was therefore useless in protection against natural infection. Subunit vaccines are also less efficient at inducing prolonged cytotoxic T-cell responses.

Even after starting therapy and with effective suppression of viral load, patients with persistently low CD4 counts remain at high risk for opportunistic infections. In general, all patients remain at a relatively high risk for opportunistic infections and other AIDS-related events for the first 6 months of antiretroviral therapy. [67] An observational study of 20,730 HIV patients in Uganda found that, among patients with more than six months of follow-up after the initiation of antiretroviral therapy, the pre-therapy CD4 count was still predictive of mortality. [68]

Cervical cancer is cancer of the entrance to the womb (uterus). Regular pelvic exams and Pap testing can detect precancerous changes in the cervix. Precancerous changes in the cervix may be treated with cryosurgery, cauterization, or laser surgery. The most common symptom of cancer of the cervix is abnormal bleeding.

Sexually transmitted diseases, or STDs, are infections that are transmitted during any type of sexual exposure, including intercourse (vaginal or anal), oral sex, and the sharing of sexual devices, such as vibrators. Women can contract all of the STDs, but may have no symptoms, or have different symptoms than men do. Common STDs in women are:

Most individuals develop antibodies to HIV within 28 days of infection and therefore antibodies may not be detectable early, during the so-called window period. This early period of infection represents the time of greatest infectivity; however HIV transmission can during all stages of the infection.

“Safe sex” practices, such as latex condoms, are highly effective in preventing HIV transmission. HOWEVER, there remains a risk of acquiring the infection even with the use of condoms. Abstinence is the only sure way to prevent sexual transmission of HIV.

In the years since the virus was first identified, HIV has spread to every corner of the globe and is one of the leading causes of infectious death worldwide. Statistics from the World Health Organization show that approximately 1.5 million people die each year from AIDS, and 240,000 of these are children. Worldwide, half of HIV-infected people are women. Two-thirds of current cases are in sub-Saharan Africa.

WHO is a cosponsor of the Joint United Nations Programme on AIDS (UNAIDS). Within UNAIDS, WHO leads activities on HIV treatment and care, HIV and tuberculosis co-infection, and jointly coordinates with UNICEF the work on the elimination of mother-to-child transmission of HIV.

At least once a week, I am asked by one of my HIV-infected patients whether they need to continue to practice safe sex if they are in a monogamous (one mate only) relationship with an HIV-infected partner. Put another way, since both partners already have HIV, what’s the harm of unprotected sex? Actually, this is not an easy question to answer fully.

Mother-to-child transmission is the most common way that children become infected with HIV. HIV medicines, given to women with HIV during pregnancy and childbirth and to their babies after birth, reduce the risk of mother-to-child transmission of HIV.

Two points in this update deserve emphasis. First, the eventual case-mortality rate of AIDS, a few years after diagnosis, may be far greater than the 41% overall case-mortality rate noted above. Second, the reported incidence of AIDS has continued to increase rapidly. Only a small percentage of cases have none of the identified risk factors (male homosexuality, intravenous drug abuse, Haitian origin, and perhaps hemophilia A). To avoid a reporting bias, physicians should report cases regardless of the absence of these factors.

Your doctor can monitor how well your HIV treatment is working by measuring the amount of HIV in your blood (also called the viral load.) The goal of treatment is to get the viral load undetectable on labs tests; ideally less than 20 copies. This does not mean the virus is gone or cured, it means the medication is working and must be continued.

Further evidence for the importance of chemokine receptors in HIV infection has come from studies in a small group of individuals with high-risk exposure to HIV-1 but who remain seronegative. Cultures of lymphocytes and macrophages from these people were relatively resistant to macrophage-tropic HIV infection and were found to secrete high levels of RANTES, MIP-1α and MIP-1β in response to inoculation with HIV. In other experiments, the addition of these same chemokines to lymphocytes sensitive to HIV blocked their infection because of competition between these CC chemokines and the virus for the cell-surface receptor CCR5.

The second role of the federal government is largely symbolic but no less controversial. It is to guide school efforts through advice, sponsorship, and public speeches, and primarily involves the offices of the surgeon general and of the federal AIDS policy coordinator. Koop, who was a Reagan appointee, roused a fair degree of controversy, yet it was nothing compared to the upheaval that greeted statements by appointees of the Clinton administration. AIDS policy czar Kristine Gebbie and surgeon general M. Joycelyn Elders were forced from their posts after making statements that conservatives found appalling—Gebbie promoting attitudes toward pleasurable sex and Elders indicating a willingness to have schools talk about masturbation. Thereafter, the administration frequently stressed abstinence as its top priority for school AIDS programs.

It is unethical for an obstetrician–gynecologist to refuse to accept a patient or to refuse to continue providing health care for a patient solely because she is, or is thought to be, seropositive for HIV. Refusing to provide care to women who are infected with HIV for fear of contracting HIV infection or simply as a practice preference is unreasonable, unscientific, and unethical.

Acronym for acquired immune deficiency (or immunodeficiency) syndrome; disorder of the immune system characterized by opportunistic diseases, including candidiasis, Pneumocystis jiroveci pneumonia, oral hairy leukoplakia, herpes zoster, Kaposi sarcoma, toxoplasmosis, isosporiasis, cryptococcosis, non-Hodgkin lymphoma, and tuberculosis. The syndrome is caused by the human immunodeficiency virus (HIV-1, groups M and O, and HIV-2), which is transmitted in body fluids (notably breast milk, blood, and semen) through sexual contact, sharing of contaminated needles (by IV drug abusers), accidental needle sticks, contact with contaminated blood, or transfusion of contaminated blood or blood products. Hallmark of the immunodeficiency is depletion of T4+ or CD4+ helper/inducer lymphocytes, primarily the result of selective tropism of the virus for the lymphocytes.
