Notable progress has been made to the extent that it could be said that the end of the AIDS epidemic is in sight. In many parts of Africa the prevalence appears to be getting stable. This means that the number of people dying from the disease is roughly equal to the number of new cases. However, whilst new HIV infections have dropped by 38% globally since 2001, 2.1 million people were newly infected in 2013. There are also 22 million people who are not accessing life-saving treatment. Access to AIDS services are still patchy due to such issues as geography, gender and socio-economic factors.

State Legislation and the Courts To stem transmission of HIV, states have adopted several legal measures. Two states attempted to head off the virus at the pass: Illinois and Louisiana at one point required HIV blood testing as a prerequisite to getting a marriage license. Both states ultimately repealed these statutes because they were difficult to enforce; couples simply crossed state lines to be married in neighboring states. Several states have taken a less stringent approach, requiring only that applicants for a marriage license must be informed of the availability—and advisability—of HIV tests. More commonly, states criminalize sexual behavior that can spread AIDS. Michigan law makes it a felony for an HIV or AIDS-infected person to engage in sex without first informing a partner of the infection. Florida law provides for the prosecution of any HIV-positive person committing prostitution, and it permits rape victims to demand that their attackers undergo testing. Indiana imposes penalties on persons who recklessly or knowingly donate blood or semen with the knowledge that they are HIV-infected.
Activated cells that become infected with HIV produce virus immediately and die within one or two days. The vast majority of viruses present in the plasma can be attributed to the short-lived, activated cells. It takes approximately 1.5 days to complete a single HIV life-cycle. Resting cells that become infected produce virus only after immune stimulation and these cells have a half-life of at least 5-6 months. HIV is a retrovirus that causes AIDS. HIV attacks the immune system. This system consists of cells and organs that protect the body against diseases like infections and cancer. HIV attacks the immune system through special types of white blood cell known as CD4 cells. CD4 cells play an important role in orchestrating and controlling the functions of the whole immune system.
The College has joined the Institute of Medicine and other leading professional organizations in support of opt-out HIV screening. Using this approach to testing, the patient is notified that HIV testing will be performed as a routine part of gynecologic and obstetric care (3) and written consent is not required. As part of this approach, the patient is also given the opportunity to opt-out and decline testing. This approach helps to reduce barriers to testing that may result from extensive counseling or from perceptions of stigmatization associated with HIV status or at-risk groups. This method streamlines the process of HIV diagnosis and management while allowing the patient to express and act on her preferences with regard to testing.
Viruses. AIDS patients are highly vulnerable to cytomegalovirus (CMV), herpes simplex virus (HSV), varicella zoster virus (VZV), and Epstein-Barr virus (EBV) infections. Another virus, JC virus, causes progressive destruction of brain tissue in the brain stem, cerebrum, and cerebellum (multifocal leukoencephalopathy or PML), which is regarded as an AIDS-defining illness by the CDC.
Protease inhibitors. Protease inhibitors work by disabling protease, an enzyme necessary for HIV reproduction. Protease inhibitors include saquinavir (Invirase), ritonavir (Norvire), indinavir (Crixivan), nelfinavir (Viracept), amprenavir (Agenerase), kaletra, and many others.

It should be noted that not every child born to an HIV-infected mother will acquire the virus. Without treatment, a woman with human immunodeficiency virus (HIV) has a one in four chance of infecting her fetus. Before preventive treatments were available, the CDC estimated that 1,000 to 2,000 infants were born with HIV infection in the U.S. each year. Now, health officials say there has been a dramatic reduction in mother-to-child, or perinatal HIV transmission rates due to increased HIV testing, which makes it possible to provide antiretroviral medication treatment of the mother during pregnancy and labor and short-term treatment of the infant after birth.
Technologies have recently become available that allow for testing with rapid results (eg, turnaround less than 1 hour). The advantage of these tools is that patients can be informed of their results at the same visit at which the testing occurs. In that manner, it is possible to lower the rate of loss to follow-up associated with the traditional two-stage testing and notification approach. Nothing about rapid testing precludes the need for a patient to opt-in or to be offered the opportunity to opt-out of testing (depending on which strategy is adopted). Rapid testing should not be implemented either as mandatory testing or testing performed without informing the patient that she will be tested.
