Therapy is initiated and individualized under the supervision of a physician who is an expert in the care of HIV-infected patients. A combination of at least three ART drugs is needed to suppress the virus from replicating and boost the immune system. How these drugs are combined depends on the most current treatment guidelines, individual patient preferences, other medical conditions, past treatment history, and any resistance mutations in the individual’s virus. Resistance mutations may already be present at the time of infection, thus most clinicians will test the patient’s virus for resistance mutations prior to starting or changing a regimen.
For primary prophylaxis against some fungal infections (eg, esophageal candidiasis, cryptococcal meningitis or pneumonia), oral fluconazole 100 to 200 mg once/day or 400 mg weekly is successful but is infrequently used because the cost per infection prevented is high and diagnosis and treatment of these infections are usually successful.
Blood and genital secretions from people with HIV are considered infectious and the utmost care should be taken in handling them. Fluids that are contaminated with blood also are potentially infectious. Feces, nasal secretions, saliva, sputum, sweat, tears, urine, and vomit are not considered infectious unless visibly bloody.
Both HIV-1 and HIV-2 are believed to have originated in non-human primates in West-central Africa and were transferred to humans in the early 20th century. HIV-1 appears to have originated in southern Cameroon through the evolution of SIV(cpz), a simian immunodeficiency virus (SIV) that infects wild chimpanzees (HIV-1 descends from the SIVcpz endemic in the chimpanzee subspecies Pan troglodytes troglodytes). The closest relative of HIV-2 is SIV(smm), a virus of the sooty mangabey (Cercocebus atys atys), an Old World monkey living in coastal West Africa (from southern Senegal to western Côte d’Ivoire). New World monkeys such as the owl monkey are resistant to HIV-1 infection, possibly because of a genomic fusion of two viral resistance genes. HIV-1 is thought to have jumped the species barrier on at least three separate occasions, giving rise to the three groups of the virus, M, N, and O.

Another group working contemporaneously with the Montagnier and Gallo groups was that of Dr. Jay Levy at the University of California, San Francisco. He independently discovered the AIDS virus in 1983 and named it the AIDS associated retrovirus (ARV). This virus was very different from the virus reported by the Montagnier and Gallo groups. The ARV strains indicated, for the first time, the heterogeneity of HIV isolates and several of these remain classic examples of the AIDS virus found in the United States.
HIV infection negatively affects the ability to diagnose TB in both adults and children. Progression to disease may occur soon after infection by M. tuberculosis or latent infection may be reactivated. Further, response to treatment is often slower and outcome is worse than in HIV-uninfected patients. Therefore, in areas with a high prevalence of HIV infection in the general population (HIV prevalence > 1%) where TB and HIV infection are likely to coexist, HIV counselling and testing is indicated for all TB patients as part of their routine management.2,27 In areas with lower prevalence rates of HIV, counselling and testing is indicated for TB patients with symptoms and/or signs of conditions and in those having a history suggestive of a high risk of exposure to HIV. A rapid test for HIV (a side room investigation) could be used as a screening test. Commercially available HIV ELISA tests are most commonly used as confirmatory tests, with HIV PCR as a confirmatory test in children less than 18 months of age.
Black America, however, never got a Pepfar. Though the raw numbers were much lower than in Africa, parts of our country looked like the continent the program was created to save. Yet while buckets of money went overseas, domestic funding for H.I.V./AIDS remained flat, and efforts to fight the disease here were reduced to a poorly coordinated patchwork affair. “When we saw that the epidemic was out of proportion in the black community, we started calling for a domestic Pepfar that would bring new resources to the effort, create clear and ambitious objectives and rebuild health care infrastructure around the country,” Lee said. “But we just couldn’t get the administration to focus on a domestic plan.”

HIV attacks and destroys a type of white blood cell called a CD4 cell, commonly called the T-cell. This cell’s main function is to fight disease. When a person’s CD4 cell count gets low, they are more susceptible to illnesses.
