When HIV infection is diagnosed in a routine test, as for blood donation, in pregnancy, or after counselling a person with a lifestyle that puts him or her at risk, there is not usually full AIDS but just infection with HIV. When the disease is suspected, HIV counselling must precede testing. There is a characteristic presentation of the infection that is described in the separate article Primary HIV Infection. Once the diagnosis is made, the separate article Managing HIV-positive Individuals in Primary Care becomes relevant. Despite generally high levels of of the risks for HIV acquisition, in 2012 an estimated 34% of adults were diagnosed with a CD4 cell count ≤200 per mm3 within three months of diagnosis. The percentage diagnosed with CD4 cell counts ≤350 per mm3 (the threshold at which treatment should be considered according to 2008 British HIV Association guidelines) was 34%.
In the end, the organized H.I.V. outreach and education that proved successful to black women never translated to black gay men — and the excessive focus on the down low sucked away critical time, energy and resources. Between 2005 and 2014, new H.I.V. diagnoses among African-American women plummeted 42 percent, though the number of new infections remains unconscionably high — 16 times as high as that of white women. During the same time period, the number of new H.I.V. cases among young African-American gay and bisexual men surged by 87 percent.

HIV-2 carries a slightly lower risk of transmission, and HIV-2 infection tends to progress more slowly to acquired immune deficiency syndrome (AIDS). This may be due to a less-aggressive infection rather than a specific property of the virus itself. Persons infected with HIV-2 tend to have a lower viral load than people with HIV-1, [12, 13] and a greater viral load is associated with more rapid progression to AIDS in HIV-1 infections. [14, 15]
HIV is a preventable disease. Effective HIV prevention interventions have been proven to reduce HIV transmission. People who get tested for HIV and learn that they are infected can make significant behavior changes to improve their health and reduce the risk of transmitting HIV to their sex or drug-using partners. Recent scientific advances have demonstrated that early initiation of antiretroviral therapy (ART) not only preserves the health of people living with HIV but also reduces their risk of transmitting HIV to others by 93%.3
If the source’s virus is known or suspected to be resistant to≥ 1 drug, an expert in antiretroviral therapy and HIV transmission should be consulted. However, clinicians should not delay PEP pending expert consultation or drug susceptibility testing. Also, clinicians should provide immediate evaluation and face-to-face counseling and not delay follow-up care.
HIV is a member of the genus Lentivirus, part of the family Retroviridae. Lentiviruses have many morphologies and biological properties in common. Many species are infected by lentiviruses, which are characteristically responsible for long-duration illnesses with a long incubation period. Lentiviruses are transmitted as single-stranded, positive-sense, enveloped RNA viruses. Upon entry into the target cell, the viral RNA genome is converted (reverse transcribed) into double-stranded DNA by a virally encoded enzyme, reverse transcriptase, that is transported along with the viral genome in the virus particle. The resulting viral DNA is then imported into the cell nucleus and integrated into the cellular DNA by a virally encoded enzyme, integrase, and host co-factors. Once integrated, the virus may become latent, allowing the virus and its host cell to avoid detection by the immune system, for an indiscriminate amount of time. The HIV virus can remain dormant in the human body for up to ten years after primary infection; during this period the virus does not cause symptoms. Alternatively, the integrated viral DNA may be transcribed, producing new RNA genomes and viral proteins, using host cell resources, that are packaged and released from the cell as new virus particles that will begin the replication cycle anew.

At sixty-two, lanky and circumspect, Siliciano is highly regarded in the tight-knit community of H.I.V. researchers. He met his wife and collaborator, Janet, in the nineteen-seventies, when she was a graduate student at Johns Hopkins, studying the proteins that T cells release when they encounter microbes. Now fifty-nine years old, with curly red hair and a hint of a New Jersey accent, Janet joined Bob’s lab after his paper appeared in Nature. She said that the idea was his, but Bob told me that Janet developed it over the next seven years, tracking the levels of dormant virus in patients consistently treated with HAART. Her data confirmed his thesis: the virus could survive almost indefinitely. “We calculated that it would take seventy years of continuous HAART for all the memory T cells to die,” she said.
Two types of HIV have been characterized: HIV-1 and HIV-2. HIV-1 is the virus that was initially discovered and termed both LAV (Lymphadenopathy Associated Virus) and HTLV-III (Human T cell Lymphotropic Virus III). HIV-1 is more virulent and more infective than HIV-2, and is the cause of the majority of HIV infections globally. The lower infectivity of HIV-2 compared to HIV-1 implies that fewer of those exposed to HIV-2 will be infected per exposure. Due to its relatively poor capacity for transmission, HIV-2 is largely confined to West Africa.
Some people will wish to use herbal remedies and a Cochrane review was able to find a small number of trials, some of which seemed to have adequate methodology.There was no significant clinical benefit and objective criteria such as CD4 count were unaffected. Since the review there have been a few studies in the literature suggesting some benefit from herbal remedies but larger trials are needed.[15, 16]

He told me, “I’m no longer that concerned about the virus itself.
