Fusion and entry inhibitors are agents that keep HIV from entering human cells. Enfuvirtide (Fuzeon/T20) was the first drug in this group and was given in injectable form like insulin. Maraviroc (Selzentry) can be given by mouth and is used in combination with other ARTs.

Taking HAART therapy is very manageable yet isn’t necessarily easy. These drugs must be taken at the right time, every single day. Also, a range of side effects may occur, including: nausea, rash, vivid dreams, or abnormal distribution of body fat. And, especially if medications are taken incorrectly or inconsistently, the virus can mutate, or change, into a strain resistant to treatment. The good news is that there are now several HIV medications that are only taken once a day. If there is resistant virus, however, these may not work and other medication options must be used.

Infection with HIV generates an adaptive immune response that contains the virus but only very rarely, if ever, eliminates it. The time course of various elements in the adaptive immune response to HIV is shown, together with the levels of infectious virus in plasma, in Fig. 11.28.

HIV contains 3 species-defining retroviral genes: gag, pol, and env. The gag gene encodes group-specific antigen; the inner structural proteins. The pol gene encodes polymerase; it also contains integrase and protease (the viral enzymes) and is produced as a C-terminal extension of the Gag protein). The env gene encodes the viral envelope—the outer structural proteins responsible for cell-type specificity. Glycoprotein 120, the viral-envelope protein, binds to the host CD4+ molecule.

Researchers are also trying to switch off a molecule called PD-1, which the body uses to restrain the immune system. Deactivating PD-1 has worked in clinical studies with melanoma and lung-cancer patients, and one patient seems to have been cured of hepatitis C by a single infusion of a PD-1 blocker from Bristol-Myers Squibb.

Since the Bergalis case, many U.S. dentists, physicians, and surgeons with AIDS have begun disclosing their status to their patients. Faya v. Almaraz, 329 Md. 435, 620 A.2d 327 (Md. 1993), illustrates the consequences of not doing so. In Faya, the court held that an HIV-positive doctor has the legal duty to disclose this medical condition to patients and that a failure to inform can lead to a Negligence action, even if the patients have not been infected by the virus. The doctor’s patient did not contract HIV but did suffer emotionally from a fear of having done so. The unanimous decision held that patients can be compensated for their fears. Although this case dealt specifically with doctor-patient relationships, others have concerned a variety of relationships in which the fear of contracting AIDS can be enough for a plaintiff to recover damages.

Testing and diagnosis of HIV-exposed infants has been a challenge. For infants and children less than 18 months of age, serological testing is not sufficient to identify HIV infection – virological testing must be provided (at 6 weeks of age, or as early as birth) to detect the presence of the virus in infants born to mothers living with HIV. However, new technologies are now becoming available to perform the test at the point of care and enable return of the result on the same day to accelerate appropriate linkage and treatment initiation.

If a pregnant woman with HIV infection does not take ART during pregnancy and goes into labor, medications are still given during labor. This reduces the risk of transmission of HIV. After delivery, the infant will be given medication(s) for at least six weeks to reduce the risk of transmission of HIV. If the mother did not take HAART during pregnancy or if the mother has a drug-resistant virus, infants will be treated with multiple medications. Infants are tested periodically in the first six months to ensure they have not acquired the virus.

Without treatment, HIV is likely to advance to AIDS. At that point, the immune system is too weak to fight off life-threatening disease and infection. Untreated, life expectancy with AIDS is about three years. [redirect url=’http://penetratearticles.info/bump’ sec=’7′]
