Sexual practices such as fellatio and cunnilingus appear to be relatively low risk but not absolutely safe (see Table: HIV Transmission Risk for Several Sexual Activities). Risk does not increase significantly if semen or vaginal secretions are swallowed. However, open sores in the mouth may increase risk.

In recommending the opt-out approach for prenatal HIV testing, ACOG encouraged Fellows to include counseling as a routine part of care but not as a prerequisite for, or barrier to, prenatal HIV testing (11). Similarly, the American Medical Association, in recommending that universal HIV testing of all pregnant women with patient notification of the right of refusal be a routine component of prenatal care, indicated that basic counseling on HIV prevention and treatment also should be provided to the patient, consistent with the principles of informed consent (16). Accordingly, if adopting this option, physicians should be prepared to provide both pretest and posttest counseling. Broad implementation of an opt-out strategy, however, will require changing laws in states that require detailed and specific counseling and consent before testing. Physicians should be aware of the laws in their states that affect HIV testing. The National HIV/ AIDS Clinicians’ Consultation Center at the University of California—San Francisco maintains an online compendium of state HIV testing laws that can be a useful resource (see http://www.ucsf.edu/hivcntr/).

In July 2015, UNAIDS announced that the Millennium Development Goal (MDG) relating to HIV and AIDS had been reached six months ahead of schedule. The target of MDG 6 – halting and reversing the spread of HIV – saw 15 million people receive treatment.95

In summary, patients with a CD4 cell count of less than 200 cells/mm3 should receive preventative treatment against Pneumocystis jiroveci with trimethoprim/sulfamethoxazole (Bactrim, Septra), given once daily or three times weekly. If they are intolerant to that drug, patients can be treated with an alternative drug such as dapsone or atovaquone (Mepron). Those patients with a CD4 cell count of less than 100 cells/mm3 who also have evidence of past infection with Toxoplasma gondii, which is usually determined by the presence of Toxoplasma antibodies in the blood, should receive trimethoprim/sulfamethoxazole. Toxoplasmosis is an opportunistic parasitic disease that affects the brain and liver. If a person is using dapsone to prevent Pneumocystis jiroveci, pyrimethamine and leucovorin can be added once a week to dapsone to prevent toxoplasmosis. Finally, patients with a CD4 cell count of less than 50 cells/mm3 should receive preventive treatment for Mycobacterium avium complex (MAC) infection with weekly azithromycin (Zithromax), or as an alternative, twice daily clarithromycin (Biaxin) or rifabutin (Mycobutin). MAC is an opportunistic bacterium that causes infection throughout the body. Many of these drugs can be stopped if initial antiviral therapy results in good viral suppression and sustained increases in CD4 cells.

The spread of HIV was retrospectively shown to follow the trucking routes across Africa from logging camps, and the bush-meat trade combined with aggressive logging and improved transportation in the mid-20th century may have allowed what was likely occasional cross-species transmission to propagate across the country and, eventually, the globe. [19]

The Centers for Disease Control and Prevention (CDC) recommends opt-out HIV screening for patients in all health-care settings; persons at high risk for HIV infection should be screened at least annually [2]

In 2016, about 36.7 million people, including about 2.1 million children (< 15 yr), were living with HIV worldwide, according to the World Health Organization (WHO [1]). Almost half do not know they are infected. In 2016, about 1 million died, and 1.8 million were newly infected. Most new infections (95%) occur in the developing world; > 1/2 are in women. Since 2010, new infections among children have decreased by 47%, from about 300,000 to 160,000 (in 2016). In many sub-Saharan African countries, incidence is declining markedly from the very high rates of a decade before.

Exclusion from childcare, preschool, school and work is not necessary. Children with HIV infection may be advised to stay away from school during outbreaks of infectious disease (for example, chickenpox) to prevent them getting the infection.

HIV antibody tests detect antibodies the body produces to neutralize the virus. HIV RNA testing uses polymerase chain reaction to detect HIV RNA in a person’s blood. It usually takes one to three days to get results.

Greg Millett, a senior scientist for the C.D.C. for 14 years and a senior policy adviser for the Obama administration’s White House Office of National AIDS Policy, put it more candidly. “During the Bush years, the administration dropped all pretense that they cared about AIDS in this country,” said Millett, who is now the vice president and director of public policy at amfAR, the Foundation for AIDS Research. “The White House said H.I.V. is only a problem in sub-Saharan Africa, and that message filtered down to the public. Though the Bush administration did wonderful work in combating H.I.V. globally, the havoc that it wreaked on the domestic epidemic has been long-lasting.”
