The profound immunosupression seen in AIDS is due to the depletion of T4 helper lymphocytes. HIV is present at a high level in the blood immediately after exposure. It then settles down to a certain low level set-point during the incubation period that lasts from 3-8 weeks. During the incubation perid, there is a massive turnover of CD4 cells as the CD4 cells killed by HIV are replaced rapidly and efficiently. The immune system eventually succumbs and AIDS is developed when killed CD4 cells can no longer be replaced, as witnessed by high HIV-RNA, HIV-Antigen and low CD4 counts.
When HIV gets resistant to one medicine, this is changed to another medicine. So the AIDS cocktail that people with AIDS take changes over time. But after a long time, the HIV learns to be resistant to many drugs. This is called multi-drug-resistant (acronym MDR) HIV. After the HIV in a person has MDR-HIV there may be no more medicines to treat them. So scientists keep trying to find new medicines to fight HIV. The five most important HIV medicines are:
You might not know if you are infected by HIV. Within a few weeks of being infected, some people get fever, headache, sore muscles and joints, stomach ache, swollen lymph glands, or a skin rash for one or two weeks. Most people think it’s the flu. Some people have no symptoms. Fact Sheet 103 has more information on the early stage of HIV infection.
Other measures can help. For men, circumcision, an inexpensive, safe procedure, reduces the risk of becoming infected during vaginal intercourse with an infected woman by about half. Whether circumcision reduces the risk of HIV infection in other circumstances is unclear. Because circumcision provides only partial protection against HIV infection, people should also use other measures to prevent HIV infection . For example, if either partner has a sexually transmitted disease or HIV infection, it should be treated, and condoms should be used correctly and consistently.
Nef also interacts with SH3 domains. The Vpu protein (p16) influences the release of new virus particles from infected cells. The ends of each strand of HIV RNA contain an RNA sequence called the long terminal repeat (LTR). Regions in the LTR act as switches to control production of new viruses and can be triggered by proteins from either HIV or the host cell. Direct cytotoxic effects of viral replication are likely not the primary cause of CD4 T-cell loss; a significant bystander effect  is likely secondary to T-cell apoptosis as part of immune hyperactivation in response to the chronic infection.
