HSV-2 has been identified as one of the few factors that distinguish areas of high and low HIV prevalence.51 HSV-2 seropositivity is associated with a threefold increase in the risk of HIV acquisition, and persons with both HIV and HSV-2 are more likely to transmit HIV. The proportion of HIV that is attributable to HSV-2 infection may increase over time and has been estimated to be as high as 35–48%.52,53 Efforts to reduce the risk of HIV transmission by treating HSV-2 have been disappointing.54 Given the strong epidemiologic association between HIV and HSV-2, however, further strategies to prevent HSV-2 transmission (e.g. introduction of an effective HSV-2 vaccine) should be explored.

Importantly, many researchers have consistently shown that the primary risk factor for infection affects mortality. For example, the mortality rate among intravenous drug users tends to be higher, whether related to HIV disease or non-HIV disease.

In the developed world, HIV infection is much more common in males. In 2015, males accounted for 81% of all diagnoses of HIV infection among adults and adolescents in the United States. [72] Among heterosexuals, females are more likely to acquire HIV infection from an infected male than a male is from an infected female, but a large proportion of infections in males are due to homosexual contact, with or without injection drug use. Males are also more likely to acquire HIV infection from injection drug use alone.

An updated algorithm published by the CDC in June 2014 recommends that diagnosis starts with the p24 antigen test. A negative result rules out infection, while a positive one must be followed by an HIV-1/2 antibody differentiation immunoassay. A positive differentiation test confirms diagnosis, while a negative or indeterminate result must be followed by nucleic acid test (NAT). A positive NAT result confirms HIV-1 infection whereas a negative result rules out infection (false positive p24).[111]

This stage of HIV infection generally lasts around 10 years if you’re not receiving antiretroviral therapy. But sometimes, even with this treatment, it lasts for decades. Some people develop more severe disease much sooner.

This complex scenario leads to the generation of many variants of HIV in a single infected patient in the course of one day.[87] This variability is compounded when a single cell is simultaneously infected by two or more different strains of HIV. When simultaneous infection occurs, the genome of progeny virions may be composed of RNA strands from two different strains. This hybrid virion then infects a new cell where it undergoes replication. As this happens, the reverse transcriptase, by jumping back and forth between the two different RNA templates, will generate a newly synthesized retroviral DNA sequence that is a recombinant between the two parental genomes.[87] This recombination is most obvious when it occurs between subtypes.[87]

HIV-2 carries a slightly lower risk of transmission, and HIV-2 infection tends to progress more slowly to acquired immune deficiency syndrome (AIDS). This may be due to a less-aggressive infection rather than a specific property of the virus itself. Persons infected with HIV-2 tend to have a lower viral load than people with HIV-1, [12, 13] and a greater viral load is associated with more rapid progression to AIDS in HIV-1 infections.
