Each virus can be contracted individually, or they can be contracted together in what is referred to as co-infection. HIV-2 seems to have lower mortality rates, less severe symptoms and slower progression to AIDS than HIV-1 alone or the co-infection. In co-infection, however, this is largely dependent on which virus was contracted first. HIV-1 tends to out compete HIV-2 for disease progression. Co-infection seems to be a growing problem globally as time progresses, with most cases being identified in West African countries, as well as some cases in the US.

Behavioural changes among injectors and the prompt introduction of harm reduction measures such as needle exchange programmes from the mid-1980s probably prevented many other urban areas in the UK from experiencing the localised epidemics on the scale seen in Scotland. In the UK, sharing rates remain higher than in the mid-1990s with almost one in three injectors in the Unlinked Anonymous survey of injecting drug users reporting direct sharing of needles and syringes in the previous four weeks. The continuing transmission of hepatitis B and hepatitis C in those aged under 25 shows the potential for further HIV spread among injecting drug users.
Fungi. The most common fungal disease associated with AIDS is Pneumocystis carinii pneumonia (PCP). PCP is the immediate cause of death in 15-20% of AIDS patients. It is an important measure of a patient’s prognosis. Other fungal infections a yeast infection of the mouth (candidiasis or thrush) and cryptococcal meningitis.

Several years ago, they began looking at “blips,” the small, sudden jumps in viral load that sometimes occur in the blood of HAART patients. Physicians had been concerned that blips might be particles of virus that had become resistant to HAART and struck out on their own. The Silicianos believed otherwise: that the viral particles were released by latently infected cells that had become activated. They analyzed the blood of patients with blips every two to three days over three to four months, and their hypothesis proved correct: the virus had not become resistant to the drugs, but had been dormant in its reservoir within memory T cells. It could be intermittently released from the reservoir, even when the patient took antiretroviral drugs.
A 2011 trial has confirmed that if an HIV-positive person adheres to an effective ART regimen, the risk of transmitting the virus to their uninfected sexual partner can be reduced by 96%. The WHO recommendation to initiate ART in all people living with HIV will contribute significantly to reducing HIV transmission.
Some religious organizations have claimed that prayer can cure HIV/AIDS. In 2011, the BBC reported that some churches in London were claiming that prayer would cure AIDS, and the Hackney-based Centre for the Study of Sexual Health and HIV reported that several people stopped taking their medication, sometimes on the direct advice of their pastor, leading to a number of deaths. The Synagogue Church Of All Nations advertised an “anointing water” to promote God’s healing, although the group denies advising people to stop taking medication.

HIV symptoms (which often appear many times months after the infection) are similar to flu symptoms, and may disappear after some time. HIV may remain dormant and asymptomatic for years until it surfaces suddenly. A common first symptom of HIV is enlarged lymph nodes for three months or more. UNAIDS announced that 18.2 million people were on ART, including 910 000 children, double the number five years earlier. However, achieving increased ART access means a greater risk of drug resistance and the WHO released a report on dealing with this growing issue.99
Two blood tests are routinely used to monitor HIV-infected people. One of these tests, which counts the number of CD4 cells, assesses the status of the immune system. The other test, which determines the so-called viral load, directly measures the amount of virus in the blood.
In people with unmasked IRIS, doctors treat the newly identified opportunistic infection with antimicrobial drugs. Occasionally, when the symptoms are severe, corticosteroids are also used. Usually, when unmasked IRIS occurs, cART is continued. An exception is when a cryptococcal infection affects the brain. Then cART is temporarily interrupted until the infection is controlled.
