A type of white blood cell. T-lymphocytes are part of the immune system and develop from stem cells in the bone marrow. They help protect the body from infection and may help fight cancer. Also called T cell and thymocyte.

Patients with most acute opportunistic infections benefit from early ART (initiated during the management of the opportunistic infection). However, for some opportunistic infections, such as tuberculous meningitis or cryptococcal meningitis, the evidence suggests that ART should be delayed until the first phase of antimicrobial therapy for these infections is finished.

Mitochondria (structures within cells that generate energy) can be damaged when certain nucleoside reverse transcriptase inhibitors are used. Side effects include anemia, foot pain caused by nerve damage (neuropathy), liver damage that occasionally progresses to severe liver failure, and heart damage can result in heart failure. Individual drugs differ in their tendency to cause these problems. When possible, doctors do not use the drugs with the most damaging side effects, such as stavudine and didanosine.

Correct and consistent use of male and female condoms during vaginal or anal penetration can protect against the spread of sexually transmitted infections, including HIV. Evidence shows that male latex condoms have an 85% or greater protective effect against HIV and other sexually transmitted infections (STIs).

51% of infections in the UK in 2012 occurred through sex between men and this group remains at greatest risk.[6]There has been no evidence in recent years of a decline in the numbers of new infections in this group and over 3,250 new diagnoses of HIV occurred in 2012.[5]

Unsafe medical injections play a significant role in HIV spread in sub-Saharan Africa. In 2007, between 12 and 17% of infections in this region were attributed to medical syringe use.[73] The World Health Organization estimates the risk of transmission as a result of a medical injection in Africa at 1.2%.[73] Significant risks are also associated with invasive procedures, assisted delivery, and dental care in this area of the world.[73]

Psychological – common misconceptions about AIDS and HIV are diminishing. However, the stigma of the condition persists in many parts of the world. People who are living with HIV may feel excluded, rejected, discriminated, and isolated.

Currently, there is no vaccine or cure for HIV, but treatments have evolved which are much more effective and better tolerated; they can improve patients’ general health and quality of life considerably, in as little as one pill per day.

HIV-associated neurologic syndromes can be differentiated via lumbar puncture with CSF analysis and contrast-enhanced CT or MRI (see Table: Common Manifestations of HIV Infection by Organ System and elsewhere in The Manual).

A failure to inform in AIDS cases has given rise to both civil and criminal lawsuits. One such case was brought by Mark Christian, the lover of actor Rock Hudson, against Hudson’s estate. Christian won his suit on the ground that Hudson concealed his condition and continued their relationship, and the jury returned a multimillion-dollar verdict despite the fact that there was no evidence that Christian had been infected. Another case was brought in Oregon in 1991, when criminal charges were filed against Alberto Gonzalez for knowingly spreading HIV by having sex with his girlfriend. After Gonzalez pleaded no contest to third-degree assault (a felony) and to two charges of recklessly endangering others, he received an unusual sentence: the court ordered him to abstain from sex for five years and placed him under house arrest for six months. Although such convictions are increasingly common, courts have also recognized that not knowing one has HIV can be a valid defense. In C. A. U. v. R. L., 438 N.W.2d 441 (1989), for example, the Minnesota Court of Appeals affirmed a trial court’s finding that the plaintiff could not recover damages from her former fiancé, who had unknowingly given her the virus.

Anti-LGBTQ bias further enables the spread of HIV by discouraging many in our community from getting tested or treated for HIV for fear of harassment. A 2014 Kaiser Family Foundation survey of gay and bisexual men in the U.S. found that 15% of them had received poor treatment from a medical professional as a result of their sexual orientation, and least 30% did not feel comfortable discussing their sexual behaviors with a healthcare provider. For gay and bisexual youth who are just beginning to explore their sexuality, homophobia and other forms of anti-LGBTQ bias help explain why so many young people in our community are unaware of their HIV status.

In the beginning, the CDC did not have an official name for the disease, often referring to it by way of the diseases that were associated with it, for example, lymphadenopathy, the disease after which the discoverers of HIV originally named the virus.[126][127] They also used Kaposi’s Sarcoma and Opportunistic Infections, the name by which a task force had been set up in 1981.[128] In the general press, the term GRID, which stood for gay-related immune deficiency, had been coined.[129] The CDC, in search of a name, and looking at the infected communities coined “the 4H disease”, as it seemed to single out homosexuals, heroin users, hemophiliacs, and Haitians.[130][131] However, after determining that AIDS was not isolated to the gay community,[128] it was realized that the term GRID was misleading and AIDS was introduced at a meeting in July 1982.[132] By September 1982 the CDC started using the name AIDS.[133]

HIV is a retrovirus. That is, it stores its genetic information as ribonucleic acid (RNA). Once inside a CD4+ lymphocyte, the virus uses an enzyme called reverse transcriptase to make a copy of its RNA, but the copy is made as deoxyribonucleic acid (DNA). HIV mutates easily at this point because reverse transcriptase is prone to making errors during the conversion of HIV RNA to DNA. These mutations make HIV more difficult to control because the many mutations increase the chance of producing HIV that can resist attacks by the person’s immune system and/or antiretroviral drugs.

At least once a week, I am asked by one of my HIV-infected patients whether they need to continue to practice safe sex if they are in a monogamous (one mate only) relationship with an HIV-infected partner. Put another way, since both partners already have HIV, what’s the harm of unprotected sex? Actually, this is not an easy question to answer fully.

If, on balance, a breach of confidence is deemed necessary, practitioners should work in advance to anticipate and manage potentially negative consequences (ie, reactions of intimate partners, family). As well, practitioners should consider whether the goal of maintaining patient privacy would be better served by personal communication with the individual placed at risk by the patient’s seropositivity or by notification of local public health authorities. In some areas, anonymous notification of sexual contacts is possible through local or state departments of health. As a practical matter, because disclosure is only possible when the index case freely identifies at-risk partners, superseding an individual’s refusal to disclose should be a rare occurrence.

There are an estimated 42 million people in the world living with HIV/AIDS, of which 19.2 million are women, and 3.2 million are children below 15 years of age. In 2002, there were 5 million newly infected HIV individuals and 3.1 million deaths relating to AIDS. In the United States, HIV/AIDS is more prevalent in African American and Hispanic communities than Caucasians.

In the UK in 2012, 15 donors tested positive for HIV infection at screening. This represented 0.6 detected infections per 100,000 donations. These were mainly in men who probably acquired the infection via heterosexual transmission.[5]

A severe immunological disorder caused by the retrovirus HIV, resulting in a defect in cell-mediated immune response that is manifested by increased susceptibility to opportunistic infections and to certain rare cancers, especially Kaposi’s sarcoma. It is transmitted primarily by exposure to infected body fluids, especially blood and semen.

Administration of HIV treatment to HIV-positive pregnant women during pregnancy and labour and after delivery, as well as to the newborn baby, dramatically reduces the risk of mother-to-baby transmission of HIV.
