Analysis of reported AIDS cases shows that 51% had Pneumocystis carinii pneumonia (PCP) without Kaposi’s sarcoma (KS) (with or without other “opportunistic” infections (OOI) predictive of cellular immunodeficiency); 30% had KS without PCP (with or without OOI); 7% had both PCP and KS (with or without OOI); and 12% had OOI with neither PCP nor KS. The overall mortality rate for cases of PCP without KS (47%) was more than twice that for cases of KS without PCP (21%), while the rate for cases of both PCP and KS (68%) was more than three times as great. The mortality rate for OOI with neither KS nor PCP was 48%.

Because human immunodeficiency virus (HIV) infection is incurable, preventing HIV transmission is paramount. Exposure to HIV can occur by percutaneous, mucous membrane or non-intact skin exposure to infected blood or body fluids. It can also occur by sexual contact, trauma or needle sharing. Postexposure prophylaxis (PEP) is one method of preventing HIV transmission. PEP is the provision of antiretroviral therapy (ART) to HIV-negative persons exposed to infected materials. It should be emphasized that PEP should not replace standard infection control measures and behavioral practices that best prevent HIV exposure.

These factors include the age of the individual, the body’s ability to defend against HIV, access to healthcare, the presence of other infections, the individual’s genetic inheritance, resistance to certain strains of HIV, and more.

The CDC recommends HIV testing as a part of standard prenatal care for all pregnant women. When a pregnant woman tests positive for HIV, testing of her infant ideally begins within 48 hours of birth. Testing is repeated at between 1 and 2 months of age and again at age 3-6 months. Testing of infants uses a different technique to detect the presence of HIV virus. Infants can be diagnosed by direct culture of the HIV virus, PCR testing, and p24 antigen testing. By one month of age, results are highly accurate. Diagnostic blood testing in children older than 18 months is similar to adult testing, with ELISA screening confirmed by Western blot.

Most AIDS patients require complex long-term treatment with medications for infectious diseases. This treatment is often complicated by the development of resistance in the disease organisms. AIDS-related malignancies in the central nervous system are usually treated with radiation therapy. Cancers elsewhere in the body are treated with chemotherapy.   1. The size of the proviral reservoir correlates to the steady-state viral load and is inversely correlated to the anti-HIV CD8+ T-cell responses. Aggressive early treatment of acute infection may lower the proviral load, but generally, treatment in newly infected (but postseroconversion) patients yields no long-term benefit.
    In retrospect, the high rate of H.I.V. infection among African-American women was a result of a complicated combination of all these factors, as well as the reality that after decades of denial and neglect, the viral load piled up in black communities, making any unprotected sexual encounter with anyone a potential “bridge to infection.” But two decades ago, in the midst of a very scary, fast-growing epidemic, the down-low brother became the AIDS boogeyman. I first heard about the “D.L.” from J.L. King, an author and self-proclaimed sex educator whom I interviewed in 2001. He had just warned a rapt audience of health care providers and H.I.V. educators at an AIDS conference in Washington: “I sleep with men, but I am not bisexual, and I am certainly not gay. I am not going to your clinics, I am not going to read your brochures, I am not going to get tested. I assure you that none of the brothers on the down low like me are paying the least bit of attention to anything you have to say.”
    Most of the lock-step mobilization efforts focused on preventing the disease in black women, who, for the most part, were contracting the virus through sex with male partners. Though the C.D.C. and other agencies offered plenty of alarming statistics confirming the high and growing numbers of H.I.V. cases and deaths among black women, there was a lack of empirical evidence to clearly explain why the rates were so high. Experts in academia and government researchers tried to unravel a knotted tangle of factors: Women were contracting the virus from bisexual men; higher rates of sexually transmitted infections among black women facilitated the spread of H.I.V.; socioeconomic issues drove up the rates of all disease. The lack of research to create a coherent explanation was further confounded by a reluctance on the part of some scientists and activists to perpetuate the dangerous myth of black women as sexually promiscuous — another holdover from slavery.
    HIV is a complicated virus. It mutates rapidly and is adept at evading immune system responses. Only a small number of people infected with HIV develop broadly neutralizing antibodies, the kind of antibodies that can fight a range of strains.
