Immunodeficiency describes the condition in which the body’s immune response is damaged, weakened, or is not functioning properly. In AIDS, immunodeficiency results from the way that the virus binds to a protein called CD4, which is primarily found on the surface of certain subtypes of white blood cells. After the virus has attached to the cell’s CD4 receptor, the virus-CD4 complex refolds to uncover another receptor called a chemokine receptor that helps mediate entry of the virus into the cell. One chemokine receptor in particular, CCR5, has been the focus of recent research after studies showed that defects in its structure (caused by genetic mutations) result in a slowing or stopping of the progression of AIDS. Scientists hope that this discovery will lead to the development of drugs that trigger an artificial mutation of the CCR5 gene or target the CCR5 receptor.
Three groups of HIV-1 have been identified on the basis of differences in the envelope (env) region: M, N, and O. Group M is the most prevalent and is subdivided into eight subtypes (or clades), based on the whole genome, which are geographically distinct. The most prevalent are subtypes B (found mainly in North America and Europe), A and D (found mainly in Africa), and C (found mainly in Africa and Asia); these subtypes form branches in the phylogenetic tree representing the lineage of the M group of HIV-1. Co-infection with distinct subtypes gives rise to circulating recombinant forms (CRFs). In 2000, the last year in which an analysis of global subtype prevalence was made, 47.2% of infections worldwide were of subtype C, 26.7% were of subtype A/CRF02_AG, 12.3% were of subtype B, 5.3% were of subtype D, 3.2% were of CRF_AE, and the remaining 5.3% were composed of other subtypes and CRFs. Most HIV-1 research is focused on subtype B; few laboratories focus on the other subtypes. The existence of a fourth group, “P”, has been hypothesised based on a virus isolated in 2009. The strain is apparently derived from gorilla SIV (SIVgor), first isolated from western lowland gorillas in 2006.
Investigations for HIV are described in the separate article Human Immunodeficiency Virus (HIV). Further investigations for AIDS-defining conditions may be indicated. Media interventions can improve the uptake of testing but this might not be sustained.
HIV-positive women who might become pregnant should talk to their provider about the risk to their unborn child. They should also discuss methods to prevent their baby from becoming infected, such as taking antiretroviral medicines during pregnancy.
Healthcare visits in the preceding year were associated with a lower rate of unawareness (37% vs 81%) but a higher rate of HIV-positivity (21% vs 12%). Because this study targeted a high-risk group and may involve participation bias, the overall rate of HIV infection (19%) cannot be easily extrapolated to the overall population. 
After HIV infection is confirmed, your doctor will start you on a drug regimen consisting of several drugs; combinations of different types of anti-HIV drugs sometimes are called HAART, for highly-active antiretroviral therapy (HIV is a kind of virus called a retrovirus).
Infection with HIV generates an adaptive immune response that contains the virus but only very rarely, if ever, eliminates it. The time course of various elements in the adaptive immune response to HIV is shown, together with the levels of infectious virus in plasma, in Fig. 11.28.
Approximately 20% of new diagnoses are in women. In the United States, heterosexual transmission accounts for approximately one-quarter of new diagnoses, with intravenous drug use contributing to the remaining cases in the U.S.
This is, in turn, surrounded by the viral envelope, that is composed of the lipid bilayer taken from the membrane of a human host cell when the newly formed virus particle buds from the cell. The viral envelope contains proteins from the host cell and relatively few copies of the HIV Envelope protein, which consists of a cap made of three molecules known as glycoprotein (gp) 120, and a stem consisting of three gp41 molecules that anchor the structure into the viral envelope. The Envelope protein, encoded by the HIV env gene, allows the virus to attach to target cells and fuse the viral envelope with the target cell’s membrane releasing the viral contents into the cell and initiating the infectious cycle.
In addition, each person’s blood is either Rh-positive or Rh-negative. It is important to know what to expect before, during, and after a blood transfusion, and the risk factors or complications of a blood transfusion.

In the developed world, antiretroviral therapy has greatly improved prognosis and increased survival rates. Public education programs have raised awareness such that testing and prevention of infection are more common. Both of these approaches are difficult in countries with undereducated or underfunded populations.
A final prevention strategy of last resort is the use of antiretrovirals as post-exposure prophylaxis, so-called “PEP,” to prevent infection after a potential exposure to HIV-containing blood or genital secretions. Animal studies and some human experience suggest that PEP may be effective in preventing HIV transmission, and it is based upon these limited data that current recommendations have been developed for health care workers and people in the community exposed to potentially infectious material. Current guidelines suggest that those experiencing a needle stick or who are sexually exposed to genital secretions of an HIV-infected person should take antiretrovirals for four weeks. Those individuals considering this type of preventative treatment, however, must be aware that post-exposure treatment cannot be relied upon to prevent HIV infection. Moreover, such treatment is not always available at the time it is most needed and is probably best restricted to unusual and unexpected exposures, such as a broken condom during intercourse. In the UK in 2012, 15 donors tested positive for HIV infection at screening. This represented 0.6 detected infections per 100,000 donations. These were mainly in men who probably acquired the infection via heterosexual transmission.

The inflammation is exacerbated by side effects of the medicines. Early treatments caused anemia, nerve damage, and lipodystrophy—the wasting of the limbs and face, and the deposits of fat around the belly. Lipodystrophy is still a major problem. Deeks has observed many patients in the SCOPE cohort with high levels of cholesterol and triglyceride, and these can lead to organ damage. One serious consequence is heart disease, which appears to be caused by inflammation of the artery walls. Deeks has also seen lung, liver, and skin cancers in his patients. In a disturbing echo of the early days of the epidemic, he has noticed that middle-aged patients develop diseases associated with aging: kidney and bone disease and possibly neurocognitive defects. A better definition for AIDS, according to Deeks, might be “acquired-inflammatory-disease syndrome.”

As opposed to treating infected people to protect their uninfected partners, another approach is to provide antiviral treatment to uninfected individuals, so-called pre-exposure prophylaxis (PrEP). The first success in this research arena came from the CAPRISA 004 study, which showed that vaginal administration before and after intercourse of a gel containing the antiretroviral agent tenofovir the risk of transmission of both HIV and herpes simplex virus to heterosexual women. Other studies are under way to confirm the results of this study as well as to determine whether the results are any different if the agent is administered daily rather than simply around the time of intercourse. One such study was not be able to show that once-daily tenofovir vaginal gel demonstrated protection from infection compared to placebo gel. The reasons for this finding are not completely known, but it does appear that adherence with the therapy was very poor.

Effective chemoprophylaxis is available for many opportunistic infections and reduces rates of disease due to P. jirovecii, Candida, Cryptococcus, and MAC. If therapy restores CD4 counts to above threshold values for > 3 mo, chemoprophylaxis can be stopped.
In general, most antiviral regimens for HIV disease contain a backbone of at least two NRTIs. The NRTIs include zidovudine (Retrovir, ZDV), stavudine (Zerit, d4T), didanosine (Videx, ddI), zalcitabine (HIVID, ddC), lamivudine (Epivir, 3TC), emtricitabine (Emtriva, FTC), abacavir (Ziagen, ABC), tenofovir disoproxil fumarate (Viread, TDF), and tenofovir alafenamide (Descovy, TAF). The latter drug is a new formulation of tenofovir that has become available as tenofovir alafenamide (TAF) as part of multiple fixed-dose combinations. This form of tenofovir has been shown to be equally effective as TDF but with less renal and bone toxicity. The NRTIs FTC and 3TC are highly related compounds and, although data is somewhat limited, most experts agree that they probably can be used interchangeably. That said, many combinations of NRTIs can be used together, with current guidelines generally recommending the fixed-dose combination of TDF with FTC (Truvada), or TAF with FTC (Descovy), both of which are also available as part of single tablet regimens. An alternative regimen uses the fixed-dose combination of ABC/3TC (Epzicom) alone or combined as a single tablet regimen with dolutegravir (Triumeq). ABC has been associated with severe allergic reactions in approximately 5% of patients. Recent studies have shown that a blood test (HLA-B*5701) can be performed to determine who is at risk for this reaction so that the drug can be avoided in these individuals and be used in others with greater confidence that there will not be such a reaction. In fact, when available, it is now the standard of care to perform this test prior to initiation of ABC. The main side effects associated with TDF are reduced kidney function and bone density.
