HIV infection takes different forms within different cells. As we have seen, more than 95% of the virus that can be detected in the plasma is derived from productively infected cells, which have a very short half-life of about 2 days. Productively infected CD4 lymphocytes are found in the T-cell areas of lymphoid tissue, and these are thought to succumb to infection in the course of being activated in an immune response. Latently infected memory CD4 cells that are activated in response to antigen presentation also produce virus. Such cells have a longer half-life of 2 to 3 weeks from the time that they are infected. Once HIV can spread from these cells by rounds of replication in other activated CD4 T cells. In addition to the cells that are infected productively or latently, there is a further large population of cells infected by defective proviruses; such cells are not a source of infectious virus.

Human immunodeficiency virus infection and AIDs can cause a plethora of hematologic problems. Early on during HIV infection, immune thrombocytopenia is common as is the development of antiphospholipid antibodies. Anemia is the most common manifestation of HIV infection and is multifactorial due to both direct and indirect effects of the virus.12 Anemia is most often a hypoproliferative, low reticulocyte anemia due to anemia of chronic disease. Often, there is a blunted erythropoietin response. Coombs-positive autoimmune hemolytic anemia also occurs with increased frequency in HIV infection. Antiretroviral therapy often causes macrocytosis.

Bangui definition A points-based system used to define AIDS in countries where HIV testing is not available. It was developed by workers from the CDC and WHO at a conference held in Bangui, Central African Republic, in 1985, and gives the most points for severe weight loss, protracted asthenia, recalcitrant fever and diarrhoea. AIDS is diagnosed with scores of 12 or more.

Political attitudes toward AIDS have gone through dramatically different phases. In the early 1980s, it was dubbed the gay disease and as such was easy for lawmakers to ignore. No one hurried to fund research into a disease that seemed to be killing only members of a historically unpopular group. When it was not being ignored, some groups dismissed AIDS as a problem that homosexuals deserved, perhaps brought on them by divine intervention. Discriminatory action matched this talk as gay men lost jobs, housing, and medical care. AIDS activists complained bitterly about the failure of most U.S. citizens to be concerned. Public opinion only began to shift in the late 1980s, largely through awareness of highly publicized cases. As soon as AIDS had a familiar or more mainstream face, it became harder to ignore; when it became clear that heterosexuals were also contracting the disease, the epidemic acquired higher priority.

Several discredited conspiracy theories have held that HIV was created by scientists, either inadvertently or deliberately. Operation INFEKTION was a worldwide Soviet active measures operation to spread the claim that the United States had created HIV/AIDS. Surveys show that a significant number of people believed – and continue to believe – in such claims.[288]

Estimation of current incidence of HIV is difficult. A back-calculation analysis (a statistical method using incubation period to project future distribution of infection) suggests there has been little change in HIV incidence in MSM over recent years. If there has been a decrease in transmissibility associated with antiretroviral treatment in those diagnosed it may have been offset by an increase in risky behaviours. In 2012, there were 2,300-2,500 new infections annually and 7,200 MSM undiagnosed.[5]London has been the main focus of the HIV epidemic in the UK. Of those MSM receiving HIV care in 2012, 50% lived in London.[7]

Greg Millett, a senior scientist for the C.D.C. for 14 years and a senior policy adviser for the Obama administration’s White House Office of National AIDS Policy, put it more candidly. “During the Bush years, the administration dropped all pretense that they cared about AIDS in this country,” said Millett, who is now the vice president and director of public policy at amfAR, the Foundation for AIDS Research. “The White House said H.I.V. is only a problem in sub-Saharan Africa, and that message filtered down to the public. Though the Bush administration did wonderful work in combating H.I.V. globally, the havoc that it wreaked on the domestic epidemic has been long-lasting.”

Many people do not know that condoms and clean needles help stop HIV. They may not even know that sharing needles and sex with someone who has HIV can make them get HIV. Even if people know about condoms and clean needles, they may not have condoms and clean needles.
