No test is perfect. Tests may be falsely positive or falsely negative. For example, it can take some time for the immune system to produce enough antibodies for the antibody test to turn positive. This time period is commonly referred to as the “window period” and may last six weeks to three months following infection. The antigen/antibody assay is most sensitive and may be positive within two weeks after infection. If the initial antibody test is negative or unclear, a repeat test should be performed three months later.

Therapy is initiated and individualized under the supervision of a physician who is an expert in the care of HIV-infected patients. A combination of at least three ART drugs is needed to suppress the virus from replicating and boost the immune system. How these drugs are combined depends on the most current treatment guidelines, individual patient preferences, other medical conditions, past treatment history, and any resistance mutations in the individual’s virus. Resistance mutations may already be present at the time of infection, thus most clinicians will test the patient’s virus for resistance mutations prior to starting or changing a regimen.
D4T can damage nerves and cause peripheral neuropathy, a neurological condition with numbness and/or tingling of the feet and hands, and inflammation of the pancreas (pancreatitis) that causes nausea, vomiting, and mid/upper abdominal pain.
If a person gets an “AIDS-defining illness,” this is usually a sign that the person has AIDS. Healthy people do not get these illnesses, because a healthy immune system is strong enough to fight off these diseases. Because of this, getting an AIDS-defining illness is a sign that a person’s immune system is seriously damaged. In a person with HIV, getting an AIDS-defining illness signals that the HIV has damaged the immune system badly enough that the person now has AIDS.
Antiviral treatment options have primarily included combinations of two NRTIs, often referred to as “nucs,” and a third drug, typically being a boosted PI, a NNRTI, often called “non-nucs, and InSTIs such as RAL, EVG or dolutegravir (Tivicay, DTG). Many of these drugs are available in fixed-dose combinations as well as increasing numbers of drugs as single-tablet regimens.

HIV stands for human immunodeficiency virus. It harms your immune system by destroying the white blood cells that fight infection. This puts you at risk for serious infections and certain cancers. AIDS stands for acquired immunodeficiency syndrome. It is the final stage of infection with HIV. Not everyone with HIV develops AIDS.
Asymptomatic, mild-to-moderate cytopenias (eg, leukopenia, anemia, thrombocytopenia) are also common. Some patients experience progressive wasting (which may be related to anorexia and increased catabolism due to infections) and low-grade fevers or diarrhea.
There is little evidence that HIV can be transferred by casual exposure, as might occur in a household setting. For example, unless there are open sores or blood in the mouth, kissing is generally considered not to be a risk factor for transmitting HIV. This is because saliva, in contrast to genital secretions, has been shown to contain very little HIV. Still, theoretical risks are associated with the sharing of toothbrushes and shaving razors because they can cause bleeding, and blood can contain large amounts of HIV. Consequently, these items should not be shared with infected people. Similarly, without sexual exposure or direct contact with blood, there is little if any risk of HIV contagion in the workplace or classroom.
Another, less well-understood prognostic factor is the level of immune activation as determined by evaluating the expression of activation markers on CD4 and CD8 lymphocytes. Activation, which may be caused by leakage of bacteria across the HIV-damaged colonic mucosa, is a strong prognostic predictor but is not used clinically because this test is not widely available and antiretroviral therapy changes the prognosis, making this test less important.
Treating infected women with HIV drugs can dramatically reduce the risk of transmission. Infected pregnant women should be treated during the 2nd and 3rd trimesters of pregnancy, during delivery, and during breastfeeding. Doing a cesarean delivery and treating the baby for several weeks after birth also reduce the risk.

HIV-1 appears to have originated in southern Cameroon through the evolution of SIV(cpz), a simian immunodeficiency virus (SIV) that infects wild chimpanzees (HIV-1 descends from the SIV(cpz) endemic in the chimpanzee subspecies Pan troglodytes troglodytes). The closest relative of HIV-2 is SIV (smm), a virus of the sooty mangabey (Cercocebus atys atys), an Old World monkey living in littoral West Africa (from southern Senegal to western Côte d’Ivoire). New World monkeys such as the owl monkey are resistant to HIV-1 infection, possibly because of a genomic fusion of two viral resistance genes. HIV-1 is thought to have jumped the species barrier on at least three separate occasions, giving rise to the three groups of the virus, M, N, and O.
If you’re pregnant, get medical care right away. If you’re HIV-positive, you may pass the infection to your baby. But if you receive treatment during pregnancy, you can cut your baby’s risk significantly.
5DRV can be given those with a history of drug resistance at a dose of 600 mg twice daily with 100 mg RTV twice daily. For those without resistance, it can be given at a dose of 800 mg with 100 mg RTV or 150 mg COBI once daily.
Circumcision of men: In young African men, circumcision has been shown to reduce their risk of acquiring HIV infection from female partners during vaginal sex by about 50%; male circumcision is probably similarly effective elsewhere. Whether male circumcision reduces HIV transmission from HIV-positive men to women or reduces the risk of acquiring HIV from an infected male partner is unknown.
Doctors and the person who was exposed typically decide together whether to use these preventive drugs. They base the decision on the estimated risk of infection and the possible side effects of the drugs. If they do not know whether the source is infected with HIV, they consider how likely the source is to be infected. However, even when the source of the exposure is known to be infected with HIV, the risk of infection after exposure varies, depending on the type of exposure. For example, risk from a blood splash is less than that from a needlestick.
When the immune system is damaged enough that significant opportunistic infections begin to develop, the person is considered to have AIDS. For surveillance purposes in the United States, a CD4+ T-cell count less than 200/µL is also used as a measure to diagnose AIDS, although some opportunistic infections develop when CD4+ T-cell counts are higher than 200/µL, and some people with CD4 counts under 200/µL may remain relatively healthy.

It is important to remember that these symptoms appear when the body is fighting off many types of viruses, not just HIV. However, if you have several of these symptoms and believe you could have been at risk of contracting HIV in the last few weeks, you should take a test.
Initially, some researchers referred to the syndrome as gay-related immune deficiency (GRID), since it appeared to be limited to homosexuals. In the media the disease commonly was referred to as the “gay plague.” But the disease had also been detected in intravenous drug users, who became infected mainly by sharing contaminated hypodermic needles. It also had been observed in women with male sexual partners. As a result, the term acquired immunodeficiency syndrome, or AIDS, was introduced to describe the disease; the CDC published its first report using the term in 1982.
Stevens-Johnson syndrome (SJS) is a rare allergic reaction to HIV medication. Symptoms include fever and swelling of the face and tongue. Rash, which can involve the skin and mucous membranes, appears and spreads quickly.

The ability of cytotoxic T lymphocytes to destroy HIV-infected cells is demonstrated by studies of peripheral blood cells from infected individuals, in which cytotoxic T cells specific for viral peptides can be shown to kill infected cells in vitro. In vivo, cytotoxic T cells can be seen to invade sites of HIV replication and they could, in theory, be responsible for killing many productively infected cells before any infectious virus can be released, thereby containing viral load at the quasi-stable levels that are characteristic of the asymptomatic period. The best evidence for the clinical importance of the control of HIV-infected cells by CD8 cytotoxic T cells comes from studies relating the numbers and activity of CD8 T cells to viral load. An inverse correlation was found between the number of CD8 T cells carrying a receptor specific for an HLA-A2-restricted HIV peptide and plasma RNA viral load. Similarly, patients with high levels of HIV-specific CD8 T cells showed slower progression of disease than those with low levels. There is also direct evidence from experiments in macaques infected with simian immunodeficiency virus (SIV) that CD8 cytotoxic T cells control retrovirally-infected cells in vivo. Treatment of infected animals with depleting anti-CD8 monoclonal antibodies was followed by a large increase in viral load.
Keep in mind that the body hasn’t produced antibodies to HIV yet so an antibody test may not pick it up. (It can take a few weeks to a few monthsfor HIV antibodies to show in a blood test). Investigate other test options such as one that detects viral RNA, typically within nine days of infection.
Mother-to-child transmission is the most common way that children become infected with HIV. HIV medicines, given to women with HIV during pregnancy and childbirth and to their babies after birth, reduce the risk of mother-to-child transmission of HIV.
(Acquired Immune Deficiency Syndrome) An immunological disorder in which the body’s immune response system becomes defective, leaving the sufferer open to opportunistic infections and some forms of cancer, such as Kaposi’s sarcoma. It is caused by infection with the HIV virus, transmitted mainly through sexual intercourse or infected blood products.
HIV is a retrovirus. That is, it stores its genetic information as ribonucleic acid (RNA). Once inside a CD4+ lymphocyte, the virus uses an enzyme called reverse transcriptase to make a copy of its RNA, but the copy is made as deoxyribonucleic acid (DNA). HIV mutates easily at this point because reverse transcriptase is prone to making errors during the conversion of HIV RNA to DNA. These mutations make HIV more difficult to control because the many mutations increase the chance of producing HIV that can resist attacks by the person’s immune system and/or antiretroviral drugs.
