HIV has been transmitted when organs (kidneys, livers, hearts, pancreases, bone, and skin) from infected donors were unknowingly used as transplants. HIV transmission is unlikely to occur when corneas or certain specially treated tissues (such as bone) are transplanted.
ABSTRACT: Because human immunodeficiency virus (HIV) infection often is detected through prenatal and sexually transmitted disease testing, an obstetrician–gynecologist may be the first health professional to provide care for a woman infected with HIV. Universal testing with patient notification and right of refusal (“opt-out” testing) is recommended by most national organizations and federal agencies. Although opt-out and “opt-in” testing (but not mandatory testing) are both ethically acceptable, the former approach may identify more women who are eligible for therapy and may have public health advantages. It is unethical for an obstetrician–gynecologist to refuse to accept a patient or to refuse to continue providing health care for a patient solely because she is, or is thought to be, seropositive for HIV. Health care professionals who are infected with HIV should adhere to the fundamental professional obligation to avoid harm to patients. Physicians who believe that they have been at significant risk of being infected should be tested voluntarily for HIV.
There are many potential side effects associated with antiviral therapies. The most common ones for each class of drug are summarized in readily available product information. Some specific toxicities are summarized by class below.
HIV is different in structure from other retroviruses. It is roughly spherical with a diameter of about 120 nm, around 60 times smaller than a red blood cell. It is composed of two copies of positive-sense single-stranded RNA that codes for the virus’s nine genes enclosed by a conical capsid composed of 2,000 copies of the viral protein p24. The single-stranded RNA is tightly bound to nucleocapsid proteins, p7, and enzymes needed for the development of the virion such as reverse transcriptase, proteases, ribonuclease and integrase. A matrix composed of the viral protein p17 surrounds the capsid ensuring the integrity of the virion particle.

One of the problems with finding a cure is that the virus can persist in cells throughout the body and potentially hide in areas that are difficult for drugs to reach, like the brain. New research is helping us understand how to effectively treat viruses in these secluded areas of the body. In addition, those infected cells that persist in the body are being studied to determine how they can be stimulated to produce virus and/or be targeted for clearance from the body by novel therapies.
The longer diagnosis delay among non-white racial/ethnic groups might partly reflect the higher proportion of infections attributable to heterosexual contact among these groups compared with whites (14), given that heterosexual persons had longer diagnosis delays. Among all transmission categories, males with infection attributed to heterosexual contact had the longest median diagnosis delay (4.9 years). This observation was consistent with the finding that heterosexual males at increased risk for infection were less likely to report testing in the past 12 months than were heterosexual females at increased risk. Heterosexual men are less likely to visit a health care provider than are both women and MSM, leading to fewer opportunities for testing (15). Moreover, compared with other risk groups, heterosexual persons at increased risk were less likely to have been offered an HIV test even when visiting a health care provider in the past 12 months, possibly because of low perceived risk for infection (15,16). This finding highlights the importance of implementing routine screening in health care settings.

Some medicines used to treat HIV or other infections can cause a rash. It usually appears within a week or two of starting on a new medication. Sometimes the rash will clear up on its own. If it doesn’t, you may need to switch medicines.

In 1983, two separate research groups led by Robert Gallo and Luc Montagnier declared that a novel retrovirus may have been infecting people with AIDS, and published their findings in the same issue of the journal Science. Gallo claimed that a virus his group had isolated from a person with AIDS was strikingly similar in shape to other human T-lymphotropic viruses (HTLVs) his group had been the first to isolate. Gallo’s group called their newly isolated virus HTLV-III. At the same time, Montagnier’s group isolated a virus from a person presenting with swelling of the lymph nodes of the neck and physical weakness, two characteristic symptoms of AIDS. Contradicting the report from Gallo’s group, Montagnier and his colleagues showed that core proteins of this virus were immunologically different from those of HTLV-I. Montagnier’s group named their isolated virus lymphadenopathy-associated virus (LAV). As these two viruses turned out to be the same, in 1986, LAV and HTLV-III were renamed HIV.
Compliance with medications is important to provide the best outcome for mother and child. Even though a physician might highly recommend a medication regimen, the pregnant woman has a choice of whether or not to take the medicines. Studies have shown that compliance is improved when there is good communication between the woman and her doctor, with open discussions about the benefits and side effects of treatment. Compliance also is improved with better social support, including friends and relatives.
The initial infection with HIV generally occurs after transfer of body fluids from an infected person to an uninfected one. The virus is carried in infected CD4 T cells, dendritic cells, and macrophages, and as a free virus in blood, semen, vaginal fluid, or milk. It is most commonly spread by sexual intercourse, contaminated needles used for intravenous drug delivery, and the therapeutic use of infected blood or blood products, although this last route of transmission has largely been eliminated in the developed world where blood products are screened routinely for the presence of HIV. An important route of virus transmission is from an infected mother to her baby at birth or through breast milk. In Africa, the perinatal transmission rate is approximately 25%, but this can largely be prevented by treating infected pregnant women with the drug zidovudine (AZT) (see Section 11-23). Mothers who are newly infected and breastfeed their infants transmit HIV 40% of the time, showing that HIV can also be transmitted in breast milk, but this is less common after the mother produces antibodies to HIV. Because host cells do not have the ability to replicate “viral RNA” but are able to transcribe messenger RNA, RNA viruses must contain enzymes to produce genetic material for new virions. For certain viruses the RNA is replicated by a viral enzyme (transcriptase) contained in the virion, or produced by the host cell using the viral RNA as a messenger. In other viruses a reverse transcriptase contained in the virion transcribes the genetic message on the viral RNA into DNA, which is then replicated by the host cell. Reverse transcriptase is actually a combination of two enzymes: a polymerase that assembles the new DNA copy and an RNase degrades the source RNA.
Use of PEP is determined by risk of infection; guidelines recommend antiretroviral therapy with ≥ 3 antiretroviral drugs. The drugs should be carefully selected to minimize adverse effects and provide a convenient dosing schedule and thus encourage PEP completion. Preferred regimens include combination of 2 NRTIs and the addition of one or more drugs (eg, 2 NRTIs plus an integrase inhibitor, a PI, or an NNRTI); drugs are given for 28 days. Nevirapine is avoided because of the rare possibility of severe hepatitis. Although evidence is not conclusive, ZDV alone probably reduces risk of transmission after needlestick injuries by about 80%. The human immunodeficiency virus (HIV) originated in Africa in the first half of the 20th century from the cross-species infection of humans by simian immunodeficiency viruses. HIV is most often transmitted during vaginal or anal sex, through blood, or perinatally from mother to child. HIV is a retrovirus that permanently integrates into the host genome of infected cells. Without antiretroviral therapy, HIV infection causes the gradual decline of CD4 T cells, eventually leading to acquired immune deficiency syndrome (AIDS). People with AIDS are more likely to contract opportunistic infections and present with cancers caused by latent viruses. Worldwide, over 37 million people are living with HIV/AIDS, and 39 million people have died of the disease. Highly active antiretroviral therapy is effective at reducing virus replication and extending the lives of HIV-infected individuals. Despite scientific advancements and substantial efforts, no effective vaccine yet exists to prevent HIV infection.
The incidence of AIDS by date of diagnosis (assuming an almost constant population at risk) has roughly doubled every half-year since the second half of 1979 (Table 1). An average of one to two cases are now diagnosed every day. Although the overall case-mortality rate for the current total of 593 is 41%, the rate exceeds 60% for cases diagnosed over a year ago.

There is evidence that even with good peripheral control of HIV, the virus may still be detectable in the CSF and semen of some infected patients. [56, 57]
