An elevated viral load in a smaller sexual network (because most people still tend to have sex with people of the same race), amplified by the structural issues that Moore pointed to, also explains why H.I.V. rates have always been disproportionately high in the black community over all. But in the first decades of the epidemic, these ideas and explanations had not been widely accepted to explain the growing body of data pointing to fast-rising numbers of H.I.V. cases among black heterosexuals — in rates unmatched by those of white Americans. In fact, the African-American community was largely in denial about the fact that H.I.V./AIDS was a black issue. Worse, most believed the was a conspiracy on the part of the federal government to kill off the race, God’s punishment for homosexuality or simply not a subject for polite conversation, because the disease was thought to be connected to promiscuity and crack and heroin use.
A subgroup of HIV-infected people (termed long-term nonprogressors) remains asymptomatic with high CD4 counts and low HIV levels in the blood without antiretroviral treatment. These people usually have vigorous cellular and humoral immune responses to their infecting HIV strain as measured by assays in vitro. The specificity of this effective response is shown by the following: When these people acquire a superinfection with a second strain of HIV to which their immune response is not as effective, they convert to a more typical pattern of progression. Thus, their unusually effective response to the first strain does not apply to the second strain. These cases provide a rationale for counseling HIV-infected people that they still need to avoid exposure to possible HIV superinfection through unsafe sex or needle sharing.

HIV is an enveloped retrovirus whose structure is shown in Fig. 11.22. HIV belongs to a group of retroviruses called the lentiviruses, from the Latin lentus, meaning slow, because of the gradual course of the diseases that they cause. These viruses persist and continue to replicate for many years before causing overt signs of disease.
Awareness of modes of transmission is very important, as the key to tackling this disease lies less in treating it than in preventing its spread.
